  • Rapid palatal expansion(RPE) with the tooth-born appliance is not sufficient to apply to the patients with periodontal problem or insufficient tooth anchorage, and it leads to tipping of the anchorage teeth and increasing teeth mobility and root resorption. To avoid these disadvantages, we present the case using palatal screws and custommade palatal expander. A 23-year-old patient underwent surgically assisted rapid maxillary expansion with the Hyrax expansion using 4 tent screws. The study models were used to measure the pre-/-post surgical width of the anterior and posterior dental arches with a digital sliding caliper. In the result, the custom-made palatal expander with 4 tent screws is suitable for delivering a force to the mid-palatal suture expansion. And it is low cost, small sized and simply applied. The results indicated that maxillary expansion with the custom-made palatal anchorage device is predictable and stable technique without significant complications in patients.

  • Background: After the resection at the mandibular site involving oral cancer, free vascularized fibular graft, a type of vascularized autograft, is often used for the mandibular reconstruction. Titanium mesh (T-mesh) and particulate cancellous bone and marrow (PCBM), however, a type of non-vascularized autograft, can also be used for the reconstruction. With the T-mesh applied even in the chin and angle areas, an aesthetic contour with adequate strength and stable fixation can be achieved, and the pores of the mesh will allow the rapid revascularization of the bone graft site. Especially, this technique does not require microvascular training; as such, the surgery time can be shortened. This advantage allows older patients to undergo the reconstructive surgery. Case presentation: Reported in this article are two cases of mandibular reconstruction using the ready-made type and custom-made type T-mesh, respectively, after mandibular resection. We had operated double blind peer-review process. A 79-year-old female patient visited the authors' clinic with gingival swelling and pain on the left mandibular region. After wide excision and segmental mandibulectomy, a pectoralis major myocutaneous flap was used to cover the intraoral defect. Fourteen months postoperatively, reconstruction using a ready-made type T-mesh (Striker-Leibinger, Freibrug, Germany) and iliac PCBM was done to repair the mandible left body defect. Another 62-year-old female patient visited the authors' clinic with pain on the right mandibular region. After wide excision and segmental mandibulectomy on the mandibular squamous cell carcinoma (SCC), reconstruction was done with a reconstruction plate and a right fibula free flap. Sixteen months postoperatively, reconstruction using a custom-made type T-mesh and iliac PCBM was done to repair the mandibular defect after the failure of the fibula free flap. The CAD-CAM T-mesh was made prior to the operation. Conclusions: In both cases, sufficient new-bone formation was observed in terms of volume and strength. In the CAD-CAM custom-made type T-mesh case, especially, it was much easier to fix screws onto the adjacent mandible, and after the removal of the mesh, the appearance of both patients improved, and the neo-mandibular body showed adequate bony volume for implant or prosthetic restoration.

  • Custom-tailored products always vary their sizes and shapes to satisfy the customers' tastes and requirements but they have to be fabricated as fast as possible when ordered. One way to solve this problem is to prepare several representative products in advance and each custom-tailored product is generated by machining the closest representative product. To realize this approach, it would be necessary to be able to group the products into several groups each of which has a representative product. Once the similar products are identified to be grouped, the representative shape can be generated such that all the products in the group can be made by machining the representative product. The custom-tailored products considered in this work have similar shapes but different sizes. Since these products have free surfaces, that are hard to be compared, their convex hulls are used for the grouping. Among all the products to be grouped, one product is chosen as a base shape. The shape and overall similarity values between the base shape and the remaining shapes are calculated as their convex hulls are rotated virtually. By calculating these similarity values at each rotation, the optimal alignment of the reference shape with respect to the base shape is determined. Overall similarity value at this optimal alignment is used as a measure for grouping. A prototype system based on the proposed methodology has been implemented and used to group the shoe-lasts for custom-tailored shoes.

  • Designing a morphologically well-fitted hip implant to a patient anatomy is desirable to improve surgical outcomes since a commercial ready-made hip implant may not well conform to the patient joint. In this study, biomechanical stability of patient-specific hip implants with two different stem lengths was compared and discussed using a 3D finite element analysis (FEA). The FEA results in this study showed that an increase in stem length brings about more the peaked von-Mises stress (PVMS) in the prosthesis and less in the femur. However the decrease in von-Mises stress in the femur causes stress shielding phenomenon that usually leads to considerable bone resorption. Although, in biomechanical stability point of view, this work recommends the use of smaller stems, the length of stem must be determined by considering both the von-Mises stress and the stress-shielding phenomenon.

  • The restoration of extensive zygomatic complex defects is a surgical challenge owing to the difficulty of accurately restoring the normal anatomy, symmetry, proper facial projection and facial width. In the present study, an extensive post-traumatic zygomatic bone defect was reconstructed using a custom-made implant that was made with a selective laser melting (SLM) technique. The computer-designed implant had the proper geometry and fit perfectly into the defect without requiring any intraoperative adjustments. A one-year follow-up revealed a stable outcome with no complications.

  • The purpose of this research was to offer basic data for study of men's suits by investigating the buying conditions of men's suits. For data collection, a questionnaire was composed of a purchasing price, a purchasing place, the source of the information, the reason of the suit buying, a standard for choice, recognition of the ready-made suit's size and a number of suits they buy etc. The subjects of this research were male who aged 20∼59 and resided in Seoul and the capital region. The results of the research were summarized as the following : 1. Most of the men purchased a ready-made suit rather than a custom suit and a easy-order suit because of the convenience for buying. But the elder was tend to purchase a custom suit than the younger. And the reason of custom suit or easy-order suit buying was size for fitness. 2. It was usually done in a department store and an agency as a purchasing place. The men who resided in Seoul used a department store more than ones who resided in the capital region. Most of the men used bargain sale. The source of the information of suits came principally himself and a wife. Most of the men purchased the suits with their wives of themselves. The elder depended upon their wives for buying suit than the younger. The standards for buying the ready-made suit were color, style and price. The number of suit they buy in a year was 0.9 suits for S/F, 0.7 suits for winter, 0.4 suits for summer and 0.2 suits for combination. The main purchasing price was 150,000∼250,000 won. 3. Most of the consummer was to be satisfied a sense of wearing, color, design etc, but they ware not to be satisfied price, A/S, size etc for ready-made suits. And they usually didn't know the size of ready-made suits.

  • Purpose: The purpose of this study was to evaluate pressure distribution on the foot with the use of custom made foot orthosis and evaluate influential factors in young people using custom orthosis. Materials and Methods: 22 individuals comprised of young males and females were evaluated by radiograph, pedobarograph, and satisfaction rate and VAS score. The data was analyzed statistically to find influential factors for satisfaction after wearing the foot orthosis. Results: Around 50% of participants were satisfied in wearing the custom made foot orthosis. Initial VAS score of satisfaction of $36.2{\pm}19.7$ improved to $73.1{\pm}15.6$ after application of foot orthosis. There was a statistically significant difference. Talo-second metatarsal angle on AP radiograph after orthosis application was significantly related to satisfaction. On analysis of pedobarograph data, total contact area was increased and weight distribution was transferred medially on ambulation with the orthosis applied. Conclusion: Before designing the foot orthosis, individual foot factors such as foot anatomy and foot pressure distribution should be evaluated for foot comfort and better patient satisfaction.
