Evaluation of reverse torque value of abutment screws on CAD/CAM custom-made implant abutments (CAD/CAM을 이용한 맞춤형 임플란트 지대주의 나사 풀림 토크 평가)

  • Purpose: The purpose of this study was to compare the screw joint stability between the CADCAM custom-made implant abutment and the prefabricated implant abutment by measuring the reverse torque value after cyclic loading. Materials and methods: Twelve screw type implants (Implantium, Dentium Co., Seoul, Korea) were embedded in aluminum cylinder with acrylic resin. The implant specimens were equally divided into 3 groups, and connected to the prefabricated titanium abutments (Implantium, Dentium Co., Seoul, Korea), CADCAM custom-made titanium abutments (Myplant, Raphabio Co., Seoul, Korea) and CADCAM custom-made zirconia abutments (Zirconia Myplant, Raphabio Co., Seoul, Korea). The CAD-CAM milled titanium crown (Raphabio Co., Seoul, Korea) was cemented on each implant abutment by resin cement. Before cyclic loading, each abutment screw was tightened to 30 Ncm and the reverse torque value was measured about 30 minutes later. After the crown specimen was subjected to the sinusoidal cyclic loading (30 to 120 N, 500,000 cycles, 2 Hz), postloading reverse torque value was measured and the reverse torque loss ratio was calculated. Kruskal-Wallis test was used for statistical analysis of the reverse torque loss ratio. Results: The CADCAM custom-made titanium abutments presented higher values in reverse torque loss ratio without statistically significant differences than the prefabricated titanium abutments ($P$>.05). Reverse torque loss ratio of the custom-made zirconia abutments was significantly higher compared to that of the prefabricated titanium abutments ($P$=.014). Conclusion: Within the limitation of the present $in-vitro$ study, it was concluded that there was no significant difference in screw joint stability between the CADCAM custom-made titanium abutments and the prefabricated titanium abutments. On the other hand, the CADCAM custom-made zirconia abutments showed lower screw joint stability than prefabricated titanium abutments.

A simple technique for impression taking of teeth and functionally generated paths

  • The objective of this case report is to introduce a simple technique for simultaneously taking a closed-mouth impression and functionally generated path (FGP) for a full coverage crown restoration. A monolithic zirconia crown was the restoration of choice. An alginate impression of the abutment tooth was taken to fabricate a custom-made closed-mouth impression tray covering the abutment tooth and the adjacent teeth. The tray had an FGP table and an abutment tray in cameo and intaglio surfaces, respectively. The impression was taken with silicone impression material after adjusting the abutment tray and inscribing the FGP using self-curing acrylic resins. Plaster casts were made from the impression, and a zirconia crown was fabricated. The crown was cemented to the abutment tooth with minimal adjustments. This simple technique resulted in a well-fitting crown that accounted for mandibular movements. Using the custom closed-mouth impression tray incorporating an FGP table simultaneously aids in fabricating an accurately fitting restoration that incorporates harmonious mandibular movements using a single impression capture.

A Finite element stress analysis of abutment screw according to the implant abutment material (임플란트 지대주 재질에 따른 지대주나사의 유한요소 응력 분석)

  • Purpose: The present study was to determine the stress distribution of an abutment screw according to implant abutment material. Methods: This study was a tightening torque 10 Ncm, 20 Ncm, set to 30 Ncm, and a titanium alloy (Ti-6Al-4V), PEEK (polyetheretherketone), Endoligns (60% Carbon Fiber Reinforced PEEK) material of the custom abutment titanium alloy (Ti-6Al-4V) the stress distribution in the material of the abutment screw will be evaluated by the finite element analysis. Results: Abutment screw most stress has been concentrated on the interface between the fixture and the abutment was also part of the interface that the threads are started. Depending on the abutment of the abutment screw Material von Mises stress values are shown differently. 10Ncm T10 under the tightening torque of 294.2 MPa, P10 is 562.8 MPa, appeared to E10 is 295.8 MPa, 20Ncm tightening torque under T20 is 581.1 MPa, P20 is 1125 MPa, E20 was shown to 585.1 MPa, 30Ncm tightening torque under T30 is 918.2 MPa, P30 is 1795 MPa, E30 has appeared 925.1 MPa. Conclusion: If the abutment is used as Endoligns, it was confirmed that the abutment screw exhibits of von Mises stress value is similar to the titanium alloy abutment.

  • The mandibular 4 anterior incisor restoration has small teeth and a small space, so the choice of abutment is always a concern. The mandibular 4 anterior incisor extraction is caused by periodontal disease, interdental embrasure greatly open is advantageous of oral hygiene management. Try to make it small diameter with custom abutment, it is limited to the space for the screw. Rather than setting the post site of one body implant to a horizontal cross-section, it is advantageous to set it to a longitudinal cross-section, for interdental embrasure formation. When using an internal bone level implant, rather than using a two-piece abutment, using a one-piece abutment can more effectively secure space for interdental embrasure.

Observation of the Shrinkage Characteristics of the Machined Zirconia Abutment by the full sintering process (완전 소결에 의한 지르코니아 어뷰트먼트의 수축 등방성 평가)

  • The full sintering process of zirconia has some extent of shrinkage and the custom abutment design should consider the pre-compensation volume of the shape. In this paper, we concentrated to reveal the asymmetric sintering characteristics. The circular profile of all six samples were measured using the three dimensional coordinate machine. After the full sintering process, the profiles were measured again. The roundness were compared to those of the original shapes and the percent of shrinkage were calculated. On the other hand, surface roughness was also investigated after the full sintering process. The final average surface roughness was improved by the sintering of the machined zirconia surface.
