A Case Report of MELAS syndrome Improved by Oriental Medicine Treatment (한방치료 후 호전된 MELAS 증후군 증례 1례)

  • Objective : Mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke like episodes (MELAS) syndrome is a progressive neurodegenerative disorder. The typical presentation of patients with MELAS syndrome includes features such as mitochondrial encephalomyopathy, lactic acidosis, and stroke like episodes. Other features, such as seizures, diabetes mellitus, hearing loss, cardiac disease, short stature, endocrinopathies, exercise intolerance, and neuropsychiatric dysfunction are clearly part of the disorder. Approximately 80% of patients with the clinical characteristics of MELAS syndrome have a genetic mutation. This progressive disorder is reported to have a high morbidity and mortality. This case report is intended to estimate clinical effects of oriental meedicinal treatment of MELAS syndrome. Methods : A 44 year old female patient diagnosed as MELSA syndrome was treated with general oriental medicinal therapy including acupuncture, moxibustion, cupping, pharmacupunture and herbal medication in 20XX in Wonkwang Medical Center Gwangju. And the changes in symptoms and signs were evaluated as time dependently. Results : Although there is currently no curable treatment and MEALS syndrome is tend to progress, our treatment showed improvement in general weakness, gait disturbance and pain in the patient. Conclusion : Our case report suggests that various oriental medicinal treatment could be effective for improvement of MELAS syndrome and may represent a new potential therapeutic approach to control the disease. It could be applied to improve general condition, prevent relapse, enhance the quality of life and reduce complaints in the patient.

Utilizing Patterns and Attitude on Collaborating Care of Korean Traditional Medicine and Western Medicine among Cerebral Apoplexy Patients (양.한방 협진병원 뇌졸중 입원환자 진료이용실태와 협진에 대한 태도에 관한 연구)

  • The primary purpose of this study was to provide the basic information for improving collaborating care of Korean traditional medicine and western medicine by surveying utilization and attitude on it among cerebral apoplexy(CA) patients hospitalized at a general hospital with both the western and Korean traditional medical department in Busan metropolitan city, Korea. The survey was conducted on 170 patients, 80 from Korean traditional medical department, and 90 from western medical department. The major results of this study were as below: First, CA patient's medical utilization patterns including selecting medical institution, term of treatment and type of medical institution at first-aid were significantly variated by their socio-demographic characteristics such as religion and job. Second, the perceptions of collaborating care, such as effectiveness and reduction of treatment period, were better at respondents who were hospitalized at oriental medical department and had been experienced with collaborating care. Third, the major contents of collaborating care which utilized by respondents in side of western medicine were physical therapy, x-ray, pathologic diagnosis, and medication, and in side of Korean traditional medicine were acupuncture, herbal medication, moxa cautery, cupping a boil therapy. Fourth, overall satisfaction on collaborating care was good(3.5 of 5.0) and was significantly variated by age and religion. Fifth, respondents perceived that collaborating care was most helpful for rehabilitation and the major problem of current duplicate medical system was increasement of medical expenditures, and the major obstacle of collaborating care was prejudice against each other medicine. The results of this study imply that effective marketing for collaborative care suitable for age and religion of customers and patient satisfaction strategy is needed to activate collaborating care.

A Study of Bi-Jeung in the Mid-Chosun Dynasty: Based on the Seungjeongwon Ilgi (조선 중기의 비증(痺症)에 대한 연구: 승정원일기(承政院日記)를 중심으로)

  • Objectives Through the clinical records of Seungjeongwon Ilgi, we reviewed the usage of Bi-Jeung to know the concept of the word and studied therapeutic strategies for managing Bi-Jeung. Methods We investigated the clinical records of the mid-Chosun dynasty containing the key word "Bi" from electronic database (Seungjeongwon Ilgi). Results Of 4,039 records, 249 articles thought to have medicinal value were lastly selected. We subdivided the cases into 13 categories according to time, the connection of contents and the change of associated symptoms. "Bi" was not used alone but used in combination with body parts or other symptoms. Etiological causes of "Bi" involved dampness, phlegm, fire, heat and qi disorders. We suggested that "Bi" of the mid-Chosun dynasty meant a symptom group mainly of sensory impairment and additionally pain or motor disturbance. Among the 22 herbal medicine formulas used, 15 were based on internal medical pattern identifications and 7 were symptomatic treatments. Acupuncture and moxibustion therapy were primarily applied to adjacent acupoints. In addition, External therapies were used together, such as washing therapy, plaster therapy, cupping therapy and thermotherapy. Conclusions "Bi" principally indicated sensory impairment on limbs and the main etiological cause was considered to be dampness. Herbal medicine, acupuncture, moxibustion and external therapies were used to cure Bi-Jeung.

An Analysis of Domestic Researches on the Prescriptions of Korean Herbal Medicine Used during Postpartum Period (산후기에 활용된 한약처방에 대한 국내 연구 분석)

  • Objectives: This study was performed to analyze the prescriptions of Korean herbal medicine which had been commonly used during postpartum period. Methods: We searched literature on postpartum cares with Korean Herbal medicine in 4 domestic search engines. After that, we conducted eligibility screening based on inclusion and exclusion criteria. Results: 1. We selected total 48 studies. There were 40 clinical studies (25 case reports and 15 case series) and 8 animal experimental studies. 2. Of the 40 clinical studies, other treatments were used together with herbal medicine : acupuncture (28), chuna manipulation (3), and pharmacopuncture (5). 3. Of the 25 case reports, the most common symptoms were musculoskeletal symptoms (8), followed by mental disorders including postpartum depression (5). Various prescriptions of herbal medicine were used depending on the diseases or symptoms. 4. In 15 case series, 665 subjects in 11 case series had taken Saenghwa-tang-gagam within 1~2 weeks after childbirth, and the average duration of Saenghwa-tang-gagam administration were $9{\pm}4.97$ days. 498 subjects in 9 studies were breast-feeding, and there were no side effects reported. 5. All experimental studies were about the recovery, the immunity and the breast-feeding associated puerperal characteristics. Also, 3 studies used Saenghwa-tang as an intervention. Conclusions: Saenghwa-tang can be commonly used for the purpose of prevention and care of the postpartum symptoms within the first 1~2 weeks after childbirth for 10 days. Also, various prescriptions of herbal medicine may be used depending on the diseases or symptoms, and can be conducted with other treatments like acupuncture, cupping, moxibustion, pharmacopuncture and chuna manipulation.

A Case Report of Hunter Syndrome (Hunter 증후군 환아 증례 1례 보고)

  • Objective : The purpose of this study is to report a case that has an important meaning as a result of treating Hunter syndrome patient with oriental medicine for elevation of quality of life and continuous health care. Methods : The patient complained of abdominal dropsy, abdominal pain, constipation, frequent gases and the common cold. So we treated him with herbal medicine, acupuncture, infra red, laser, CEP nebulizer, aroma massage and cupping. Results : After treatment, the girth of abdomen didn't increse any more and abdominal pain, constipation, frequent gases and common cold were improved. Conclusion : This study shows that oriental medicine can elevate the Hunter syndrome patient's quality of life with continuous heath care and treatment for chief complaint. The further study is needed with more cases.

  • The number of allergic contact dermatitis patients is steadily increasing but a proper reliability treatment guide is not prepared in Oriental Medicine. Also, the number of clinical cases on allergic contact dermatitis is not sufficient in Oriental medicine. As such, the facial flushing of allergic contact dermatitis which has been improved by infra-red therapy is significant in its treatment purpose. We reported on 43-year old male patient with allergic contact dermatitis. We treated him with herbal concoction, acupuncture, infra-red therapy and cupping therapy. We have received a good result in this case by using the combination of infra-red therapy. We reached a conclusion that using infra-red therapy can improve the facial flushing of patient with allergic contact dermatitis. However, further research on the effect of infra-red therapy for these symptoms is desired.

  • Objectives : The purpose of this study is to report the effect of Korean medicine treatment on limitation of eye movement in Miller-Fisher Syndrome. Methods : A patient was treated with herbal medicines, acupuncture(including electropuncture, phamacopuncture), Moxibustion and cupping therapy. To evaluate the result of this treatment, we used photographs of eye movement, Scott and Kraft score of both eye, length of eyeball movement and visual analogue scale(VAS) for subjective symptoms. Results : After treatment, the limitation of eye movement and diplopia were remarkably improved. Also, other symptoms such as dizziness, gait disturbance, facial nerve disorder were disappeared. Conclusions : This study suggests that Korean medicine treatments are effective for patient with limitation of eye movement in Miller-Fisher Syndrome.

  • Objective : Ossification of ligaments within spinal canal, i.e., OPLL and OLF, is uncommon clinical entity as a cause of the progressive compression myelopathy or radiculopathy. More and more cases being reported in the field of occidental medicine, but very few cases have been reported in the field of oriental medicine. The purpose of this study is to report on oriental medical approaches to OPLL and OLF. Methods : Subjects of this study are 3 cases of OPLL and 3 cases of OLF who visited Won-kwang oriental medical hospital(Dept. of acupuncture and Moxibustion) from May, 2002 to October, 2003. These patients undergo oriental medical treatment such as acupuncture, cupping, Bee-Venom therapy and herbal medication and so on. We made a comparison JOA scores between before treatment and after treatment and we evaluated results of treatment. Results : The results of treatment in these six cases are as follows ; One case was evaluated 'Excellent', one case was evaluated 'Good', two cases were evaluated 'Fair' and two cases were evaluated 'Failure'. One of these cases had a surgical operation after discharge from this hospital. Conclusions : After oriental medical care for these cases, there are some improvements such as decrease of pains, relief of myelopathy etc. But, it had little effect on some cases, therefore we considered that more special study to find various and effective methods of oriental medical treatment for these diseases should be made.

Integrative Oriental Medical Treatments after Hip Fracture Surgery: Two Case Report (고관절 골절 수술 후 한방 통합 치료를 시행한 2례 보고)

  • Objectives : The purpose of this study is to show the clinical effects of integrative oriental medical treatments for femur pain and disfunction after hip fracture surgery. Methods : Two patients were treated using acupuncture, pharmacopuncture, herbal medication and other oriental medical treatments include moxibustion, cupping therapy, physical rehabilitation for 2~3 months. And the effects of these integrative treatments were measured with numeric rating scale(NRS) and functional recovery score(FRS). Results : NRS of femur pain was decreased from 8 to 4~5 in case 1 and from 6 to 4 in case 2. FRS was improved in both cases. Conclusions : Integrative oriental medical treatments maybe effective to the patients complaining pain and functional limitations after hip fracture surgery.

The Clinical Analysis of Traffic Accident Patients Treated by Oriental Medicine (교통사고 후유증 환자의 임상적 분석과 한방치료효과)

  • Objectives : This study was designed to find out the characteristic of patients who had admitted due to traffic accident and to promote the superiority of oriental medical treatment on the sequelae of traffic accident. Methods : We analyzed 332 traffic accident patients statistically according to medical charts, who had been hospitalized from January 1,20000 to October 30, 2004. Results and Conclusions : The analysis showed that there was no significant difference in sex(p<0.05), but the older group and the shorter group of interval time from onset to addmission had the better remedial result(p<0.05). The most common diagnosis that had the better remedial result was lumbar sprain, followed by multiple contusion, cervical sprain, herniated lumbar disc, herniated cervical disc(p<0.05), and the most effective oriental medical treatment was moxibusition, followed by blood cupping therapy, taping therapy, chuna therapy except acupuncture, herbal medication, oriental physical therapy(p<0.05). In using of TENS, ICT, U/S, complex therapy had better remedial result than single therapy(p<0.05). The longer period of treatment group had better remedial result generally(p<0.05).
