Antibody Responses to Cryptosporidium Antigen in HIV-positive Patients in the Republic of Korea

  • The diagnosis of cryptosporidiosis has been carried out using coprologic techniques in the Republic of Korea. However, antibody responses to Cryptosporidium have rarely been studied. Serum antibodies from HIV-positive/oocyst-positive Korean patients recognized significantly 31 and 27 kDa antigens, and HIV-negative/oocyst-positive individuals clearly reacted to 15/17 kDa antigens. Compared with oocyst-positive cases, 18.7% and 75.8% of sera from HIV-positive patients reacted to 31 and 27 kDa antigens. Only 11.1% of HIV-negative individuals reacted to 15/17 kDa. Based on these findings, serum antibody responses were different between HIV-positive and HIV-negative individuals infected with Cryptosporidium, and it is suggested that HIV-positive patients are more frequently exposed to C. parvum compared to HIV-negative individuals.

Detection of Cryptosporidium spp. in Diarrheic Immunocompetent Patients in Beni-Suef, Egypt: Insight into Epidemiology and Diagnosis

  • Cryptosporidium species is an important cause of gastrointestinal infections globally. This study aimed to shed light on its role in diarrheic immunocompetent patients in Beni-Suef, Egypt and to compare three diagnostic methods. Two hundred diarrheic patients, $37{\pm}16.8year$ old, were enrolled. Stool samples were examined by light microscopy, using modified Ziehl-Neelsen stain (MZN) for Cryptosporidium spp. oocysts. Coproantigens were detected by sandwich ELISA. DNA molecular diagnosis was done by nested PCR. PCR yielded the highest detection rates (21.0%), compared to ELISA (12.5%) and MZN staining method (9.5%). The higher infection rates were in 20-40 year-old group, followed by 40-60 year-old. Association between epidemiologic factors was statistically not significant; positivity and gender, clinical manifestations, residence, source or water, or contact with animals. Cryptosporidiosis is an important enteric parasitic infection in Beni-Suef and PCR remains the gold standard for diagnosis.

Prevalence of diarrhea caused by Cryptosporidium parvum in non-HIV patients in Jeollanam-do, Korea

  • The present study investigated the prevalence rate of Cryptosporidium parvum as a cause of diarrhea. We examined 942 stools of unidentified reasons occurring in patients in whom no immunosuppression had been detected. We examined the stools for Cryptosporidium parvum via modified acid-fast staining. The clinical records of all of the positive patients were then analyzed. Nine ($1\%$) of the stools among the 942 diarrheal patients were positive for C. parvum. The positive rate in the males was $1.1\%$ (6/522) and the positive rate of the females was $0.7\%$ (3/420). Age distribution revealed that the highest positive rates were in patients in their sixties, with a positive rate of $2.5\%$ (4/158). In the clinical tests, levels of c-reactive protein, erythrocyte sedimentation rates, and neutrophil proportions were normally increased in the peripheral blood, whereas the lymphocyte proportion exhibited a tendency towards decrease. The pathological findings were compatible with an inflammatory reaction in the host.

  • From July to December 2022, we investigated the incidence of enteric pathogens causing diarrhea in calves under 100 days old at 22 farms across Korea. A total of 354 calves (including 247 dairy calves and 107 Korean native calves) were included. We used commercially available rapid diagnostic kits to identify five enteric pathogens, including bovine rotavirus (BRV), bovine coronavirus (BCoV), Cryptosporidium. Escherichia coli (E. coli), and Giardia. This study investigated how factors such as age, season, and birth month, which can influence the incidence of intestinal pathogens, affect the incidence of these pathogens in diarrheic dairy calves and Korean native calves. Our study found that the incidence of diarrhea-causing pathogens differed between dairy and Korean native calves. In diarrheic dairy calves, bovine rotavirus (BRV) incidence rates, correlated with season and birth month. Similarly, Cryptosporidium incidence C were associated with age and season, while Giardia incidence rates varied by season and birth month. Korean native calves, on the other hand, exhibited seasonal and birth month-related variations in BRV incidence rates, with Cryptosporidium incidence rates influenced by birth month.

A Waterborne Outbreak and Detection of Cryptosporidium Oocysts in Drinking Water of an Older High-Rise Apartment Complex in Seoul

  • From May to June 2012, a waterborne outbreak of 124 cases of cryptosporidiosis occurred in the plumbing system of an older high-rise apartment complex in Seoul, Republic of Korea. The residents of this apartment complex had symptoms of watery diarrhea and vomiting. Tap water samples in the apartment complex and its adjacent buildings were collected and tested for 57 parameters under the Korean Drinking Water Standards and for additional 11 microbiological parameters. The microbiological parameters included total colony counts, Clostridium perfringens, Enterococcus, fecal streptococcus, Salmonella, Shigella, Pseudomonas aeruginosa, Cryptosporidium oocysts, Giardia cysts, total culturable virus, and Norovirus. While the tap water samples of the adjacent buildings complied with the Korean Drinking Water Standards for all parameters, fecal bacteria and Cryptosporidium oocysts were detected in the tap water samples of the outbreak apartment complex. It turned out that the agent of the disease was Cryptosporidium parvum. The drinking water was polluted with sewage from a septic tank in the apartment complex. To remove C. parvum oocysts, we conducted physical processes of cleaning the water storage tanks, flushing the indoor pipes, and replacing old pipes with new ones. Finally we restored the clean drinking water to the apartment complex after identification of no oocysts.

  • Diagnosis of cryptosporidiosis is currently confirmed by the detection of the oocysts or endogenous stages in fecal or tissue samples. Various conventional staining methods and serodiagnostic techniques have been reported, but the latter has far been limited to a few laboratories. Cryptosporidium has recently been reported in mice and chiekens in Korea, but there has been no report on staining methods to the oocysts. The present study was performed by light and scanning electron microscopic observations, and discussed with staining properties of four conventional methods such as dichromate solution floatation method, Carbol fuchsin stain, Auramine-O stain and Giemsa stain method. Cryptosporidial oocysts were isolated from the laboratory mouse. In tissue sections of duodenum, jejunum, ileum, cecum and upper colon, numerous very small, basophilic bodies were observed on the border of mucosal epithelial cells. In scanning electron microscopic observations, a few of developmental stages of Cryptosporidium were seen. Two types of thick and thin-walled oocysts were recognized in the intestinal contents. Mean size of its were $5.19{\pm}0.23{\times}4.31{\pm}0.32{\mu}m$ and $5.14{\pm}0.25{\times}4.27{\pm}0.4{\mu}m$, respectively. Carbol fuchsin and Auramine-O stain methods are recommended as the satisfactory ones for the identification of Cryptosporidium oocysts. Giemsa stain was also recommended as available in the laboratory, because a few of developmental stage fo Cryptosporidium could be seen by it.

An epidemiological survey on Cryptosporidium parvum infection of inhabitants in Chorwon-gun, Kangwon-do

  • The present study was undertaken to know the infection status of Cryptosporidium parvum among the residents of Chorwon-gun, Kangwon-do in 1993. Total 461 fecal samples were collected from the inhabitants residing in Chorwon-gun during the period of August 12 to September 14, 1993. Fecal smears were prepared by formalin-ether sedimentation, and examined after modified acid fast staining. Of the 461 fecal samples,9 (1.9%) were positive for C. parvum oocysts. The positive cases were limited to thirties (4) patients, forties (3), and sixties (2) , and no oocyst was detected in other age groups. The oocyst positive rate for male was 1.4% and that of female was 2.6%.

  • A Cryptosporidium parvum sporozoite and oocyst λgt11 cDNA library was screened with a hyperimmune rabbit serum that was developed against insoluble fragments of ultrasonicated oocysts. A clone named Cp22.4.1 encoding a protein of 231 amino acids with 4 zinc-finger domains characterized by a Cys-X2-Cys-X4-His-X4-Cys motif was isolated and characterized. There was a complete match between the sequencing data of the coding region of Cp22.4.1 and the corresponding gene at chromosomal level. Cloning in a pBAD-TOPO-TA expression vector permitted to evaluate the antigenicty of the recombinant His-tagged antigen. This antigen was recognized by 2 out of 5 sera from Cruptosporidium immune calves and not by sera from parasite naive animals.

  • Ozone process is currently applied in 24 water treatment plants in Korea to control micropollutants and taste & odor compounds. However, one of the chlorine resistant protozoa, cryptosporidium, is not being considered as ozone disinfection performance whereas U.S. is already regulate Cryptosporidium by ozone disinfection. two ozone plants(PH, UH WTP) operation conditions are investigated for disinfection performance comparing Korea disinfection regulation and U.S. regulation. The ozone plants are unable to get Cryptosporidium inactivation credits by Korea disinfection regulation. However, the inactivation credit for Cryptosporidum was increased when the U.S. disinfection regulation was applied. The Korea disinfection credit regulation need revision to practical aspects.

Infection status of pigs with Cryptosporidium parvum

  • To investigate the infection status of pigs with Cryptosporidium parvum, 589 fecal samples were collected from pigs raised at farm in Chungcheongbuk-do and Chungcheongnam-do. Of the 589 pig fecal samples, 62 (10.5%) were positive for C. parvum. The area showing the highest positive rate was Dangjin-gun, Chungcheongnam-do (14.0%), and the lowest (0%) Salmi-myon, Chungcheongbuk-do. The positive rate of C. parvum in Judok-eup increased from 12.7% in the winter to 22.1 % in the summer. The results of this study suggest that the pigs may be a source of human C. parvum infection.
