한국인의 Straight Wire Bracket의 형태에 관한 연구 (A MORPHOLOGIC STUDY ON STRAIGHT WIRE BRACKET FOR KOREAN)

  • The purpose of this study was to obtain the norm of the crown shape (tip, torque, in/out) and arch form, and to provide basic data for fabricating straight wire bracket and ideal arch wire for Korean. 100 subjects aged from 17 to 26 (50 females, 50 males) were selected with a normal occlusion. By measuring the size, angulation, inclination, arch width, facial prominance of the teeth and the molar offset, the following results were obtained. 1. Average, standard deviation, minimum, maximum of each measuring item for each teeth were obtained. 2. Intermolar width (${\underline{6}}$ to ${\underline{6}}$) of upper arch before occlusal surface cutting and intermolar width of upper arch (${\underline{6}}$ to ${\underline{6}}$, ${\underline{7}}$ to ${\underline{7}}$) after occlusal surface cutting showed statistical difference. There was no difference between sexes in any other measuring items. 3. Arch form and specification of straight wire bracket for Korean who have normal occlusion was obtained.

유한요소법을 이용한 환자별 교정시스템 구축의 기초 알고리즘 개발과 적용 (Finite Element Method Modeling for Individual Malocclusions: Development and Application of the Basic Algorithm)

  • 본 연구는 최근 많은 연구가 행해지고 있는 유한요소를 이용한 교정시스템 구축에 있어서 정상교합자가 아닌 부정교합 환자별 유한요소 모델 구축을 위한 기초 알고리즘 개발에 그 목표가 있다. 기존에는 유한요소 분석을 위해서 한 환자의 치열을 유한요소 모델로 재현하고자 할 때, 전처리 과정에 대부분의 시간을 소요해야만 했다. 본 연구에서는 이 전처리과정을 자동화할 수 있는 기초 알고리즘을 마련하여, 향후 이 방면의 연구를 용이하도록 하였다. 이를 위하여 임의로 선택된 부정교합 환자의 상하악 모형을 이용하여 치아의 삼차원적인 위치를 계측할 수 있는 표준화된 방법을 제시하였으며, 또한 본 연구를 통하여 개발된 프로그램을 이용하여 유한요소의 전처리 과정을 자동화할 수 있는 기초를 마련하였다. 본 연구는 아래와 같은 단계를 거쳐 수행하였다. 1. 상,하악 중절치에서 제2대구치까지 14개의 치아 형상을 상,하악 각각 구축하여 개별 파일로 저장한다. 2. 이 치아에 standard bracket을 치관의 FA point와 bracket slot의 중앙이 일치되도록 부착한다. 3. 대상 환자의 석고 모형을 제작한다. 4. 석고 모형에서 본 연구를 위해 제작된 기구들로 치아의 crown inclination, angulation, 그리고 교합면에서 치관첨까지의 수직거리를 계측한다. 5. 표준화하여 촬영한 석고 모형의 사진을 이용하여 화상처리기법으로 치아의 치열궁 형태를 파악한다. 또한 사진상에서 치아의 수평적 위치 및 회전량을 측정한다. 6. 계측된 crown inclination, angulation, 수직거리, 그리고 치열궁의 형태 및 치아의 회전정 도 등을 회전행열을 이용하여 만든 프로그램에 입력한다. 이 프로그램은 유한요소 전처리 과정에 필요한 치아의 배열상태를 담고 있는 데이터를 결과 파일로 제공하는데, 이 결과 파일은 일반적인 상용 유한요소 프로그램에서도 사용 가능하다. 7. 개개의 치아 파일은 이 결과 파일에 따라 삼차원적인 위치로 배열되어 선택한 특정 환자의 유한요소 모델을 완성하게 된다. 상기와 같은 여러 단계를 거친 후 임의로 선택한 부정교합자의 상,하악 유한요소 모델 구축을 위한 기초 자료를 구축할 수가 있었으며, 개개인 환자의 모형에서 얻은 정보로 유한요소 모델로 재현하기 위한 전처리과정의 기초 알고리즘을 마련하였다.

  • The purpose of this study was to compare and evaluate the stress distribution and displacement developed in the abutment teeth and residual ridge area by madibular unilateral distal extension removable partial denture with 2 different retainer designs. The retainers on right and left canine and right 2nd molar were Alters clasp in one model and telescopic crown in the other model. The stress distribution of abutment teeth and residual ridge area on two model were compared and analyzed with 3-dimensional finite element method. 150N and 400N forces were applied vertically, 30 degree and horizontally on the central fossa area of left 1st molar of the removable partial denture, and then stress distribution patterns were analyzed and compared. The results were as follows 1. As the magnitude and angulation of applied force were increased, the magnitude of stress on the right and central residual ridge area and the right canine of the telescopic type increased and comparing to those of the Alters clasp type. 2. As the magnitude and angulation of applied force were increased, the mesial direction of displacement on the right residual ridge area and the right tooth of the telescopic type increased and the distal direction of displacement on left residual ridge area and the left canine increased comparing to those of Akers clasp type. 3. As the vertical force was applied, the distal direction of the displacement of the right tooth were greater and that of the left canine was smaller and the upward displacement of the right canine was greater in telescopic partial denture than those of Akers clasp type. 4. As the 30 degree force was applied, the mesial direction of the displacement of the right tooth were greater and the distal direction of the displacement of the left canine was smaller and the upward displacement of the right canine was greater in telescopic partial denture than those of Akers clasp type. In the horizontal force the results were same in right area tooth but the distal direction of displacement was greater in left canine. 5. In both removable partial dentures, as the magnitude and degree of force were increased, the stress and displacement were increased. The compressive force was dominative than the ten sile force. 6. In both removable partial dentures, the magnitude of stress was greater on mucosal tissue area than that of the alveolar bone area on distal extension residual ridge area but the result was reversed on anterior residual ridge area. The displacement was always greater on mucosal tissue area than that of alveolar bone area.

세라믹 브라켓의 종류 및 브라켓 슬롯과 와이어 각도에 따른 마찰 저항 차이 (Frictional resistance of different ceramic brackets and their relationship to the second order angulation between bracket slot and wire)

  • 사회 문화적 발달과 함께 심미적 요구가 증가되면서 교정 환자 수의 증가와 함께 교정 치료 중의 심미성에 대한 요구도 늘어나고 있다. 치료 중 심미성을 증진시키기 위한 목적으로 세라믹 브라켓이 개발되어 널리 사용되고 있으나 세라믹 자체의 취성(brittleness), 대합치의 마모뿐만 아니라, 임상적으로 높은 마찰 저항이 그 문제점으로 지적되고 있다. 본 연구는 세라믹 브라켓의 마찰 저항을 알아보고 그 개선방안을 찾는데 지침이 되고자 하는 목적으로 시행하였다. 연구 재료로는 마찰 저항을 줄이기 위해 금속 슬롯이 삽입된 다결정 세라믹 브라켓과 금속 슬롯이 삽우되지 않은 단결정 세라믹 브라켓과 금속 브라켓을 이용하였으며, $.019{\times}.025$ 스테인리스 와이어를 브라켓 슬롯에 대해 $0^{\circ},\;10^{\circ}$의 각도를 부여하여 만능시험기상에서 이동시켜 그때의 정적, 동적 마찰력을 측정하였다. 연구 결과, 전체적인 평균 마찰력은 금속 브라켓, 금속 슬롯이 삽입된 다결정 세라믹 브라켓, 단결정 세라믹 브라켓 순으로 증가하였다. 브라켓 슬롯과 와이어 사이의 각도가 $0^{\circ}$일 때가 $10^{\circ}$일 때에 비해서 낮은 마찰 저항을 보였으며, 브라켓 슬롯과 와이어 사이의 각도가 $10^{\circ}$일 때 단결정 세라믹 브라켓의 동적, 정적 마찰 저항은 금속 브라켓이나 금속 슬롯이 삽입된 다결정 세라믹 브라켓에 비해 유의성 있게 크게 나타났다 (p<0.05). 본 연구를 통해서 금속 브라켓에 비해 세라믹 브라켓, 특히 금속 슬롯이 삽입되지 않은 단결정 세라믹 브라켓의 마찰 저항이 높은 것을 확인할 수 있었으며, 브라켓 슬롯과 와이어간의 각도가 증가하면 마찰 저항도 커지는 것을 확인할 수 있었다. 따라서 마찰력을 줄이기 위한 세라믹 브라켓의 개발과 함께 교정 치료 시 과도한 치관 경사를 막음으로써 마찰 저항을 줄이고 심미성을 유지하면서 치료의 효율을 높이도록 해야할 것이다.

Evaluation of the accuracy of linear and angular measurements on panoramic radiographs taken at different positions

  • Purpose: This study assessed the accuracy of linear and angular measurements on panoramic radiographs taken at different positions in vitro. Materials and Methods: Two acrylic models were fabricated from a cast with normal occlusion. Straight and $75^{\circ}$ mesially and lingually angulated pins were placed, and standardized panoramic radiographs were taken at standard position, at an $8^{\circ}$ downward tilt of the occlusal plane compared to the standard position, at an $8^{\circ}$ upward tilt of the anterior occlusal plane, and at a $10^{\circ}$ downward tilt of the right and left sides of the model. On the radiographs, the length of the pins above (crown) and below (root) the occlusal plane, total pin length, crown-to-root ratio, and angulation of pins relative to the occlusal plane were calculated. The data were subjected to repeated measures ANOVA and LSD multiple comparisons tests. Results: Significant differences were noted between the radiographic measurements and true values in different positions on both models with linear (P<0.001) and those with angulated pins (P<0.005). No statistically significant differences were observed between the angular measurements and baselines of the natural head posture at different positions for the linear and angulated pins. Conclusion: Angular measurements on panoramic radiographs were sufficiently accurate and changes in the position of the occlusal plane equal to or less than $10^{\circ}$ had no significant effect on them. Some variations could exist in the pin positioning (head positioning), and they were tolerable while taking panoramic radiographs. Linear measurements showed the least errors in the standard position and $8^{\circ}$ upward tilt of the anterior part of the occlusal plane compared to other positions.

심미적 영역에서 즉시 임플란트 식립 및 임시 수복을 통한 단일치의 수복에 대한 증례 보고 (Immediate implant placement and provisionalization of single implant in the esthetic region : Cases report)

  • In the anterior maxillary area, dental implants for tooth replacement are challenging due to the need to satisfy high esthetic level as well as functionality. Immediate implant placement and provisionalization can dramatically reduce the edentulous period, and then fulfill patient's demand for esthetics. The aim of present case report is to demonstrate two cases that successfully restored single tooth with immediate implant placement and provisionalization in the anterior maxillary area. A 47 years old female was scheduled to replace her maxillary right central incisor due to crown-root fracture by trauma. Another 54-year-old female was planned to place dental implant following tooth extraction of maxillary right lateral incisor owing to continuous pus discharge despite repetitive treatments including apicoectomy. In these two cases, surgical and prosthetic procedures progressed in a similar way. After minimal flap elevation, atraumatic tooth extraction was performed. Implant was placed in proper 3-dimensional position and angulation with primary stability. Bone graft or guided bone regeneration for peri-implant bone defect was conducted simultaneously. Provisionalization without occlusal loading was carried out at the same day. Each definitive crown was delivered at 7 and 5 months after the surgery. Two cases have been followed uneventfully for 2 to 5 years of loading time. In conclusion, Immediate implant placement and provisionalization could lead to esthetic outcome for single tooth replacement with dental implant under proper case selection.

Consideration of root position in virtual tooth setup for extraction treatment: A comparative study of simulated and actual treatment results

  • Objective: The purpose of the present study was to compare the root positions in virtual tooth setups using only crowns in a simulated treatment with those achieved in the actual treatment. Methods: Pre- and post-treatment intraoral and corresponding cone beam computed tomography (CBCT) scans were obtained from 15 patients who underwent orthodontic treatment with premolar extraction. A conventional virtual tooth setup was used for the treatment simulation. Pre- and post-treatment three-dimensional digital tooth models were fabricated by integrating the patients' intraoral and CBCT scans. The simulated root positions in the virtual setup were obtained by merging the crown in the virtual setup and root in the pre-treatment tooth model. The root positions of the simulated and actual post-treatment tooth models were compared. Results: Differences in root positions between the simulated and actual models were > 1 mm in all teeth, and statistically significant differences were observed (p < 0.05), except for the maxillary lateral incisors. The differences in the inter-root angulation were > 1° in all teeth, and statistically significant differences were observed in the maxillary and mandibular canines. Conclusions: The virtual tooth setup using only crown data showed errors over the clinical limits. The clinical application of a virtual setup using crowns and roots is necessary for accurate and precise treatment simulation, particularly in extraction treatment.

  • The movement of teeth and initial stress associated with the treatment of orthodontics have been successfully studied using the finite element method. To reduce the effort in preprocessing of finite element analysis, we developed two types of three-dimensional finite element models based on the standard teeth model. To reduce the effort in preprocessing of finite element analysis, we developed two types of three-dimensional finite element models based on the standard teeth model. Individual malocclusions were incorporated in the finite element models by considering the measuring factors such as angulation, crown inclination, rotation and translations. The finite element analysis for the wire activation with a T-loop arch wire was carried out. Mechanical behavior on the movement and the initial stress for the malocclusion finite element model was shown to agree with the objectives of the actual treatment. Finite element models and procedures of analysis developed in this study would be suitably utilized for the design of initial shape of the wire and determination of activation displacements.

  • 본 연구는 상악 제1대구치에 발치공간 폐쇄를 위한 근심력 적용시, 대구치의 근원심 경사도와 회전정도의 변화가 치근막의 응력분포와 치근막 내에서의 초기 치아이동에 어떤 변화를 야기하는지를 살펴보기 위하여 시행되었다. 상악 제1대구치와 치근막의 3차원 유한요소모델을 제작하여 $20^{\circ}$의 근심경사와 원심경사를 설정하고. $20^{\circ}$의 근심회전과 원심회전을 설정하였다 각각의 상황에서 치체이동을 유도하는 근심력을 가하기 위해 협면에 rigid element를 부착하고 200g의 근심력과 9: 1의 countertipping 모멘트 5:1의 counterrotation모멘트를 부여하였다. 각각의 응력분포 및 초기이동 양상을 비교하여 다음과 같은 결론을 얻었다. 상악 제1대구치의 경사도 변화나 회전 변화를 통해 근심력 적응시 구치의 근심이동에 대한 저항의 증가를 얻을 수 없었다. $20^{\circ}$의 근심경사와 $20^{\circ}$의 원심회전이 매우 유사한 응력분포 및 변위를 나타내었다 $20^{\circ}$의 원심경사와 $20^{\circ}$의 근심회전도 마찬가지로 유사성을 보였다. $20^{\circ}$ 근심경사 및 $20^{\circ}$ 원심회전된 경우 치근이 협측 이동하고 치관이 설측 이동하는 관상면 상에서의 회전경향이 발생하였다. $20^{\circ}$ 원심경사 및 $20^{\circ}$ 근심회전된 경우 치근이 설측 이동하고, 치관이 협측 이동하는 관상면 상에서의 회전경향이 발생하였다. 경사되거나 회전된 치아에 근심력을 가해야 할 경우 관상면상에서의 회전을 방지하기 위한 부가적인 토크 조절이 필요하다.

Cone-beam computed tomographic evaluation of the temporomandibular joint and dental characteristics of patients with Class II subdivision malocclusion and asymmetry

  • Objective: Treating Class II subdivision malocclusion with asymmetry has been a challenge for orthodontists because of the complicated characteristics of asymmetry. This study aimed to explore the characteristics of dental and skeletal asymmetry in Class II subdivision malocclusion, and to assess the relationship between the condyle-glenoid fossa and first molar. Methods: Cone-beam computed tomographic images of 32 patients with Class II subdivision malocclusion were three-dimensionally reconstructed using the Mimics software. Forty-five anatomic landmarks on the reconstructed structures were selected and 27 linear and angular measurements were performed. Paired-samples t-tests were used to compare the average differences between the Class I and Class II sides; Pearson correlation coefficient (r) was used for analyzing the linear association. Results: The faciolingual crown angulation of the mandibular first molar (p < 0.05), sagittal position of the maxillary and mandibular first molars (p < 0.01), condylar head height (p < 0.01), condylar process height (p < 0.05), and angle of the posterior wall of the articular tubercle and coronal position of the glenoid fossa (p < 0.01) were significantly different between the two sides. The morphology and position of the condyle-glenoid fossa significantly correlated with the three-dimensional changes in the first molar. Conclusions: Asymmetry in the sagittal position of the maxillary and mandibular first molars between the two sides and significant lingual inclination of the mandibular first molar on the Class II side were the dental characteristics of Class II subdivision malocclusion. Condylar morphology and glenoid fossa position asymmetries were the major components of skeletal asymmetry and were well correlated with the three-dimensional position of the first molar.
