Influencing Factors and Consequences of Near Miss Experience in Nurses' Medication Error (간호사의 투약 근접오류경험의 영향 요인과 결과)

  • Purpose: This study aimed to predict the influencing factors and the consequences of near miss in nurses' medication error based upon Salazar & Primomo's ecological system theory. Methods: A convenience sample of 198 nurses was recruited for the cross-sectional survey design. Data were collected from July to September 2016. Using the collected data, the developed model was verified by structural equation modeling analysis using SPSS and AMOS program. Results: For the fitness of the hypothetical model, the results showed that $x^2$ ($x^2=258.50$, p<.001) was not fit, but standardized $x^2$ ($x^2/df=2.35$) was a good fit for this model. Additionally, absolute fit index RMR=.06, RMSEA=.08, GFI=.86, AGFI=.81 reached the recommended level, but the Incremental fit index TLI=.82, CFI=.85 was not enough to reach to the recommended level. With the path diagram of the hypothetical model, caution (${\beta}=-.29$ p<.001), patient safety culture (${\beta}=-.20$, p=.041), and work load (${\beta}=.18$, p=.037) had a significant effect on the near miss experiences in nurses' medication error, while fatigue (${\beta}=-.06$, p=.575) did not affect it. Moreover, the near miss experience had a significant effect on work productivity (${\beta}=-.25$, p=.001). Conclusion: These results have shown that to decrease the near miss experience by nurses and increase their work productivity in hospital environments would require both personal and organizational effort.

  • 최근 과학 기술의 발달과 정보화로 인해 다양한 분야의 방대한 정보들이 디지털화되어 저장되고, 이를 효율적으로 저장하고 관리하는 기술에 대한 수요가 증대하고 있다. 특히, 수자원 분야에서도 지점 관측 형태의 직접 계측방식이 아닌 선, 면 형태의 간접 계측방식의 기술이 개발됨에 따라 짧은 시간 동안 많은 수의 양질의 자료를 취득할 수 있게 되었다. 이에 따라 과거에 비해 방대하고 연속적인 하천공간정보가 축척되었으며, 이를 저장하고 관리하고자 하는 필요성이 대두되고 있다. 하천공간정보 중 하천단면자료는 하천기본계획수립 시 핵심이 되는 자료중 하나일 뿐만 아니라 하천의 흐름 특성을 해석하기 위한 여러 물리 모형의 입력 자료로서, 하천의 물이 흐르는 하도공간을 흐름방향의 횡방향으로 절단하였을 때 생기는 면을 기록한 자료이다. 국가에서는 하천단면자료 계측을 위해 많은 예산을 투입하고 있으며, 실제로 많은 수의 하천에서 하천단면자료를 계측하고 있다. 하지만 계측된 하천단면자료는 컴퓨터를 이용하여 제도하는 응용프로그램용 파일(CAD)로 저장되어, 하천단면자료를 지속적으로 저장 관리 모니터링하기에는 어려운 실정이다. 본 연구에서는 이러한 하천단면자료를 지속적으로 저장 관리 모니터링 할 수 있는 시스템 개발을 목적으로 현재 개발 중인 하천공간정보 표준 자료모형과 하천공간정보를 전송할 수 있는 표준자료 전송 언어인 RiverML을 활용하였다. 본 연구에서 도입한 하천공간정보 표준 자료모형은 하천망을 기반으로 한 상호연계를 기본으로 하고 있어, 하천망을 구성하는 각 하천과 해당 하천의 단면자료 간의 관계를 규명하기 쉬우며, RiverML은 이와 유사하게 하천망을 기반으로 다양한 정보를 전달하기 쉬운 전송언어이다. 또한 국가에서 개발한 공간정보 오픈 플랫폼인 브이월드(Vworld)의 API를 활용하여 저장된 하천단면자료를 표출할 수 있게 하였다. 본 연구의 결과는 하천단면자료 저장 관리 모니터링에 유용하게 사용될 수 있을 뿐만 아니라, 지속적인 개발을 통해 하천에서 계측되는 공간정보뿐만 아니라 하천의 시설물, 생태환경, 문화 등의 하천공간정보과 연계하여 효율적인 시스템을 개발할 수 있을 것으로 사료된다.

Public Perception on Coinsurance Rate of the National Health Insurance in Korea (건강보험 본인일부부담률 적정성에 대한 대중의 인식)

  • Background: Although Korean government have been adopting several policies to expand coverage of National Health Insurance (NHI) program, the coverage rate is still below average across Organization for Economic Cooperation and Development countries. This study was to identify public perception on appropriateness of coinsurance rate and factors associated with coinsurance rate in National Health Insurance. Methods: For this cross-sectional study, 507 participants over aged 20 years were recruited from telephone surveys. Respondents experienced at least one and more visiting medical facilities due to medical problems in last 12 months. Demographic factors, socioeconomic positions, and experiences on medical utilization were measured in order to identify factors associated with perception appropriateness of coinsurance rate. Results: The 49.9% (n=209) of the public responded that the coinsurance rate of NHI program was appropriate. There were no differences in positive perception according to socio-demographic factors and experiences on medical utilization except for gender, residential area, and felt expensive when using medical services. People who felt burden of medical expenditure were more likely to perceive coinsurance rate inappropriate (odds ratio, 2.33; 95% confidence interval, 1.52-3.58) Conclusion: In spite of the relatively low coverage rate of NHI, this study identified that 49.9% of the public perceived the current coinsurance rate was adequate. However, people who felt the burden of medical expenditure were still had a negative perception of the coinsurance rate needed to decrease the coinsurance rate.

Factors influencing Preferences for Care near the End-of-life among Undergraduate Nursing Students (간호대학생 임종치료선호도에 영향을 미치는 요인)

  • This study aimed to identify factors influencing the preferences for end-of-life (EOL) care among undergraduate nursing students. In this cross-sectional study, data were collected from December 2017 to February 2018. This study included 217 undergraduate nursing students. Factors influencing the preference for 'autonomous physiological decision-making' were the following: education level(by grade), having biomedical education, attitude towards death, and attitude towards life-sustaining treatments (LSTs). Preference for 'decision-making by healthcare professionals' was related to having a religion. Factors influencing the preference for 'spirituality' were education level, having a religion, and academic major satisfaction. Preference for 'pain control' was associated with education level, experience with dying patients, bad self-rated health, attitude towards death, and attitude towards LSTs. The study findings suggest that education regarding LSTs, EOL care, and EOL decision-making in nursing curricula is essential.

Screening Sarcopenia in Rural Community-Dwelling Older Adults in Korea

  • Purpose: Several screening tools have been developed to identify sarcopenia in rural community-dwelling older adults. We aimed to compare the diagnostic accuracy of two such tools, namely the SARC-F and SARC-CalF assessments. Methods: This cross-sectional study on 388 community-dwelling older adults comprised 254 women and 134 men with a mean age of 77.8 ± 6.26 year in Korea. We assessed muscle mass, muscle strength, and physical performance using a bioimpedance analysis device, hydraulic hand dynamometer, and 4 m gait speed test, respectively. Three widely-used diagnostic criteria [the Asian Working Group for Sarcopenia (AWGS), European Working Group on Sarcopenia in Older People, and the International Working Group on Sarcopenia] were applied. Sensitivity and specificity analyses were performed on the SARC-CalF and SARC-F tests. We used receiver-operating characteristic curves and the area under the curves (AUCs) to compare the diagnostic accuracy of the assessments with regard to sarcopenia. Results: An analysis using four sets of diagnostic criteria showed that the prevalence of sarcopenia was 27.6% to 41.0%. Using the AWGS 2019 criteria as a reference standard, the SARC-CalF had a sensitivity of 83.02% and a specificity of 53.71% in the entire study population, whereas the SARC-F had a sensitivity of 79.87% and a specificity of 41.92%. The AUCs for the SARC-CalF and SARC-F tests were 0.725 (95% confidence interval 0.678-0.769) and 0.645 (95% confidence interval 0.595-0.693), respectively (p<001). In the analyses using the other three diagnostic criteria, similarity was also confirmed. Conclusion: SARC-CalF showed better sensitivity than did SARC-F when diagnosing sarcopenia in rural community-dwelling older adults. Further studies are needed to verify this finding in different populations.

The Effect of Unemployment in Depression by Age Group: Using 12 States' Data from the Behavioral Risk Factor Surveillance System

  • Purpose: This study aims to explore the association between unemployment and depression in people from different age groups ranging from 18 to 65 years old. Methods: This study used a cross-sectional design. We performed bivariate analysis and multivariable logistic regression on the 2010 Behavioral Risk Factor Surveillance System (BRFSS) data from 12 states in the United States. Results: On a sample comprised of n=53,406 individuals, of whom 2,546 (7.8%) were identified as being depressed and 3,448 (10.6%) as unemployed, we found that individuals aged 61~65 years have a lower depression risk compared to those aged 18-25 after adjusting for other variables including employment status. However, people from 61~65 have higher increased risk of depression when unemployed compared to other age groups in all three models tested (3.95 times higher in unemployed people in model 1, and 2.81 times higher in model 2 and model 3). Conclusion: Our findings indicate that there may need to be more focus on older adults who are unemployed, with associated support services for their mental health. The results of our study indicate that although older adults are less likely to be unemployed, there are more likely to experience depression if they are unemployed (once other confounding factors are taken into account) than younger adults. Policies and interventions can be developed to address not only the physical difficulties but also the mental challenges with which older adults can be at risk facing in case of unemployment.

What influences aromatase inhibitor continuation intention among breast cancer survivors?

  • Purpose: Aromatase inhibitors (AIs) are widely prescribed for postmenopausal women with breast cancer and are known to cause musculoskeletal pain. This study aimed to identify factors associated with AI continuation intention among breast cancer survivors (BCS). Methods: A cross-sectional survey was conducted on 123 BCS (stages I-III), who had been taking AIs for at least 6 weeks. Participants were recruited from a cancer center in Goyang, Korea, from September to November 2019. Descriptive statistics, Welch analysis of variance, Pearson correlation coefficients, and simple linear regression were used for the analysis. Results: Beliefs about endocrine therapy was a significant predictor of AI continuation intention (β=.66, p<.001). The majority of participants (87.0%) reported experiencing musculoskeletal pain since taking AIs and the score for the worst pain severity within 24 hours was 5.08±2.80 out of 10. Musculoskeletal pain, however, was not associated with AI continuation intention. Fear of cancer recurrence (FCR) was clinically significant (≥13) for 74.0% of the respondents (mean, 17.62±7.14). Musculoskeletal pain severity and pain interference were significantly associated with FCR (r=.21, p<.05; r=.35, p<.01, respectively). Pain interference was significantly associated with beliefs about endocrine therapy (r=-.18, p<.05). Conclusion: AI continuation intention can be modified by reinforcing patients' beliefs about endocrine therapy. Musculoskeletal pain may have a negative effect on beliefs about endocrine therapy and increase FCR among BCS. Thus, awareness of musculoskeletal pain during AI therapy should be raised and further research is required to develop multidisciplinary pain management strategies and clinical guidelines to reinforce beliefs about endocrine therapy.

Comparison of quality of life and related factors according to hearing impairment in elders using the Korea National Health and Nutrition Examination Survey (2016-2018) (청력저하 여부에 따른 노인의 삶의 질 관련 요인: 국민건강영양조사 자료(2016-2018년) 분석)

  • The Purpose of this paper was to explore the effect of hearing impairment on HRQOL in Korean elders. We carry out a cross-sectional analysis using nationally representative data from the KNHANES, 2016-2018. The survey was conducted on 4,754 elders who responded to questions about hearing impairment. Quality of life was compared between hearing impaired elders and elders with no hearing impairment using the t-test and chi-square test, and factors related to quality of life were analyzed by multivariate logistic regression using SPSS version 22.0. There was significant difference in quality of life between hearing impaired elders and elders with no hearing impairment. Walking exercise were identified as factors related to quality of life in elders with hearing impairment, while marriage status, walking exercise and limited movement were found to be related to quality of life among elders with no hearing impairment. In order to improve the HRQOL of elders with hearing impairment, multidisciplinary efforts and development of educational programs are required.

Factors influencing dementia prevention behaviors in older Koreans enrolled in senior welfare centers (노인복지관 이용 노인의 치매예방행위에 미치는 영향요인)

  • Purpose: This study aimed to explore factors influencing on the preventive behaviors of dementia in older Koreans enrolled in senior welfare centers to provide basic information on dementia prevention. Methods: A total of 182 older Koreans participated in this study, which employed a cross-sectional design. Data were collected using questionnaires with items related to general characteristics, depression, self-esteem, knowledge and attitudes about dementia, preventive behaviors of dementia, dementia management self-efficacy, and anxiety about dementia. Using SPSS WIN 23.0, data were analyzed by descriptive statistics, t-test, ANOVA, Scheffé test, Pearson's correlation coefficient, and multiple regression. Results: Living with a spouse (p=.013), a higher monthly income (p=.018), better nutrition (p=.010), not having a sleep disorder (p<.001), and higher number of participating programs (p=.001) were associated with better dementia prevention behaviors. Attitudes about dementia (��=.18, p=.006) and the number of programs older Koreans were participating in (��=.17, p=.006) influenced positively on the preventive behaviors of dementia, while depression (��=-.41, p<.001) and sleep disorders (��=-.17, p=.006) influenced negatively on dementia preventive behaviors. Overall, approximately 35% of the variability in dementia preventive behaviors was explained by these four variables in this study (F=24.97, p<.001). Conclusion: The findings of this study suggest that depression reduction, good attitudes about dementia, program participation, and good sleep should be encouraged to promote dementia prevention. In addition, effective and structured educational programs should be developed for older Koreans to encourage dementia preventive behaviors to reduce the risk of dementia.

Maternal correlates of vegetable preference and consumption in preschool-aged children

  • Purpose: Considering the various health benefits of vegetables, it is necessary to identify maternal correlates of vegetable preference and consumption in children for shaping desirable vegetable-related eating habits. This study aimed to investigate the maternal factors related to vegetable preferences and consumption in preschool-aged children. Methods: This is a cross-sectional study that surveyed 303 mother-child pairs (146 boys, 157 girls) to explore general characteristics, vegetable preferences, and cooked vegetable consumption in mothers and children using a questionnaire method. Maternal correlates of the child's vegetable preference and consumption were tested using a generalized linear model using SAS program. Results: Mothers' vegetable preferences and consumption were significantly higher than those in their children (p < 0.001). Mothers' vegetable preferences showed a significant positive correlation with the vegetable preferences of their children (mother-son, p < 0.001; mother-daughter, p < 0.001). Additionally, mothers' cooked vegetable consumption showed a significant positive correlation with the cooked vegetable consumption of children (mother-son, p < 0.001; mother-daughter, p < 0.001). Mothers' vegetable preferences and consumption respectively increased those of their children, in both boys and girls. Conclusion: Findings that mothers' vegetable preference and consumption correlates of children's vegetable preferences and consumption, indicate the importance of the mothers' role in increasing vegetable consumption in children. Mothers should be aware of the effects of their vegetable eating habits on their children's vegetable consumption and try to develop healthy eating habits. We suggest that the government or local communities provide nutrition education for mothers to adopt healthy eating habits and present information to educate their children on food and healthy dietary habits.
