Increased incidence of myocardial infarction, arteriosclerosis and hypertension in oral contraceptive steroids users has been recognized as serious problem. Evidence have been accumulated that oral contraceptive steroids cause an increase in serum lipid levels, mainly triglyceride as well as an increase in serum cholesterol concentration in some women. Effect of oral contraceptives on serum lipids have been predominantly confirmed in Europian and American women. It should be emphasized that effects of contraceptives may he influenced by differences in nutrition, climate, socioeconomic status and race. The present study was, therefore, attempted to determine the effect of oral contraceptive steroids on serum lipid levels in Korean women and also to demonstrate whether the duration of pill treatment may affect serum lipid levels. Ten women, who had never taken contraceptive steroids, served as a control. One hundred women on combined oral contraceptive steroids were grouped according to the duration of pill use: duration of less than one year(14 women), 1-2 years(12 women), 2-5 years(29 women), 5-10 years(33 women) and more than 10 years(13 women). Serum levels of cholesterol, phosholipid and triglyceride were measured by enzymatic method, Fiske-SubbaRow method and Sugiura method respectively. The results obtained are as follows; 1) Oral contraceptive steroids increased serum cholesterol concentration by 4.8% as compared with cholesterol value of control group. But this increase was not statistically significant. 2) As compared with control value, concentration of serum phospholipid in pill-treated women decreased by 14.3%, which was statistically significant (p<0.05). 3) Concentration of serum triglyceride in pill-treated women increased by 10.6% over control value, but this increase was not statistically significant. 4) Duration of pill-treatment did not affect serum concentrations of cholesterol, phospholipid and triglyceride. From the results mentioned above, it may be concluded that combined oral contraceptive steroids, at least used for subjects involved in this study, decreased serum phospholipid significantly, thereby influencing serum lipid concentration.
Objective: Combined oral contraceptives (COCs) have some adverse effects on the serum lipid profile. Because hyperlipidemia is one of the risk factors in cardiovascular diseases, lipid abnormalities should be evaluated in women consuming COCs. Vitamins E and C are known to have beneficial effects on serum lipid profiles. Therefore, in this study, we evaluated the effects of vitamins E and C on serum lipids in women using COCs. Methods: The study compared changes in lipid parameters with and without vitamin therapy in women consuming COCs compared to those of a control group (40 non-contraceptive users or NCU) for 4 weeks. Total cholesterol and triglyceride, low-density lipoprotein (LDL), and high-density lipoprotein (HDL) levels along with HDL/LDL ratios were measured for all participants. Results: COC users experienced significantly higher increases in the levels of triglycerides and LDL than non-users (p<0.05). However, no significant differences were noted in the total cholesterol and HDL levels. In the treated COC group receiving vitamins E and C, the HDL level and the HDL/LDL ratio increased and the LDL and triglycerides levels decreased significantly compared with those of the other groups. Conclusion: The results of our study indicate that supplementation with antioxidant vitamins E and C restores a normal lipid profile in COC users.
The purpose of this prospective study was to investigate whether office hysteroscopy (OH) can be used to assess the mechanisms of action of progestogen-only contraceptives (POCs), diagnose possible local causes of abnormal uterine bleeding (AUB), and support the treatment plan of symptomatic patients using POCs compared with those who do not use hormones. The study included 140 women who were divided into two groups. Group A consisted of 70 women who used POCs, whereas group B consisted of 70 women who did not use hormones. They were successively examined using transvaginal ultrasonography (TVS), OH, and endometrial sampling. The TVS results were consistent with those of OH and histopathology. The changes in endometrial thickness and vasculature, as well as fallopian tube (FT) functions, were significantly more pronounced in POC users than in non-POC users. There was a significant reduction in the peristalsis of the proximal part of the FT, as well as a reduction in the bubble flow test in group A compared with group B. In addition, the combination of peristalsis and the bubble flow test (Darwishscope test) was significantly lower in group A. It was concluded that using OH as a simple diagnostic tool in women with POCs would contribute to a better understanding of the mechanisms of endometrial and FT effects and explain some local endometrial causes of AUB. This ensures that the combination of TVS and OH would limit routine endometrial sampling in POCs users.
The paper provides a brief account of 11 plant species used by the tribal people of the district Purulia, West Bengal, India. Most of the plant species are common in this district and some have not been reported earlier for abortive purposes, pharmacology for preparation of medicines for antifertility. All these data were obtained from the tribal medicine men (ojha). At least 10 interview reports of various tribal medicine men were recorded. Parts of various medicinal plants were observed personally, collected and preserved as herbarium specimens for proper identification. The reports of various indigenous methods may help to give some clue in searching the potent contraceptives.

The effect of calcium gluconate on estrogen (estradiol) serum level as well as follicle stimulating hormone level was studied. Our results revealed that oral administration of calcium gluconate (100 mg/kg body wight) to adult non-pregnant female rabbits caused a significant increase of serum levels of estradiol and follicle stimulating hormone. On the other hand, oral contraceptive (Norminest tablets) decreased significantly follicle stimulating hormone serum level, while combined administration of calcium gluconate and oral contraceptive caused significant increase of serum level of follicle stimulating hormone compared with control values. Also, concurrent administration of calcium gluconate and Norminest tablet increased significantly the rate of conception compared with group recieved Norminest tablets only. These results indicated that combined administration of calcium and oral contraceptives must be cautiously.
When Korean family planning services began as a part of the National Policy in 1962, the annual population growth was 3.0%. This growth rate has been decreased to 2.0% during last ten year period. And it seems imperative that all hospitals, as well as related organizations, should participate in family planning in order to contribute to achieving the National goal of 1.5% population growth by 1976, the end of the Third Five Year Economic Development Plan. Nurses should be considered the most important human resources in charge of the core of family planning services in any setting. For the family planning services in the general hospital setting, nurses as a core members contribute much as change agent, motivators, counsellors, educators etc. A nurse can work with patients and their relatives when she is equipped with relevant knowledge and skills. Fur the more family planning cannot be ignored even in hospital setting where more comprehensive nursing care is needed Thus, the general objective of this study is to provide baseline data for better programming of In-service education in family planning so that effective hospital family planning nursing services can be made a part of comprehensive nursing care contributing to the national population program and human welfare. In order to meet the general objective, this study has the following specific objectives : 1. To find out the general characteristics of the clinical nurses working in Y Hospital 2. To evaluate their attitudes and practices of family planning 3. To assess their knowledge, attitudes and practices of population and family planning as professional nurses. 4. To examine and compare data collecting methods for the planning of an In-service Educational Program 5. To explore the contents to be included in this In-service Education Program. The study population randomly selected one hundred nurses working in Y Hospital A cross-sectional survey with questionnaires developed for this study was chosen for the study method. To collect reliable data, the questionnaires were distributed to and answered by the study population in a controlled situation. X²test and t-test was employed in analyzing the data. The findings of this study are as follows: 1. Y Hospital nurses had a lower ideal number of children (X=2.02) and showed no strong preference for male children, and 74% of them expressed the desire to use permanent methods of birth control 2. of this thirty Y Hospital nurses who were married 66.7% stated they were already practicing contraceptive methods. Most of them preferred male methods of contraception. 3. According to objective evaluation about knowledge of various aspects of population and family planning, respondents from collegiate programs significantly knew better the subjects on the average than did respondents from diploma programs of nursing. 4. There was a marked difference in the results of self-evaluation and objective evaluation in their family planning knowledge. It was found that the self-evaluation family planning knowledge seemed to be unreliable. Accordingly, the objective test methods appeared to be more reliable in the evaluation of knowledge levels. 5. The subject areas needed to be included in In-service education for the Hospital family planning services in Y Hospital are 1) rhythm methods, 2) tubal-legation, 3) family planning effects of contraceptives, 4) population growth, 5) demographic traction, 6) population structure and 7) infant mortality facts. In addition, 1) various oral contraceptives, 2) basal temperature method, 3) laparoscopic female sterilization, 4) interfering factors of family planning, 5) anatomy and physiology of the female reproductive organs were additional areas to be taught to respondents from 3-year diploma schools of nursing. Demographic transition was one subject area in which the four-year graduates need further study. 6. Population problems guidance and counselling in family planning instruction in the theory and practice of contraceptives should be included in future In-service Education Programs in order to provide more effective hospital Family Planning Services, stated 77.0% of the respondents.
Rapid socio-cultural and economic changes in the country has brought with it changes in the society's value system. For a traditional society that is increasingly being exposed to modernization but where sex norms are still very restrictive, the adolescent sexual mores takes on added significance. Adolescents are caught between two opposing forces, the changing environment that allows for freer and liberal mores and the traditional society that cannot keep pace with the changing environment and therefore demands resistance to changes. This paper focuses on problems of adolescent sexuality in this country and considers the countermeasures for the existing problems. Amongst the problems are: (a) increasingly younger age of the adolescents who start sexual intercourse (b) non-use of contraception, (c) unwanted pregnancies, (d) increase in the number of induced abortion and (e) increase in the number of unwanted children and unmarried mothers. The Korean adolescent's sexual behavior seems to follow that of the developed countries. In other words, many western modes of life and sexual values seem to bave been copied in Korea and yet Korean adolescents lack in their knowledge of sex related matters such as reproductive physiology and contraception. Among middle and high school students, female students are reported to have less knowledge on sex than male students according to a 1988 survey by KIPH. Even among the unmarried famale factory workers, only 42.5 percent replied they know of the condom, and 25.1 percent and 23.1 percent said they had knowledge of spermicide and menstrual regulation respectively. However, 14.9 percent and 13.9 percent reported that they had a knowledge of the loop and female sterilization respectively according to the 1984 study by KIPH. Among the middle school students 0.8 percent said they had experience in sexual intercourse, while 7.3 percent of the high school students reported having had sexual intercourse. The sexual intercourse experience rate among the unmarried female factory workers is 37.8 percent. Among those female factory workers with sexual experience, 46.7 percent had more than one sex partners. Only 39.1 percent of male students and 18.9 percent of female students among those with sexual intercourse experience have used contraceptives. mostly condoms and oral pills 45.1 percent of female factory workers with sexual intercourse experience used contraceptives such as pills, condoms and rhythm methods. The pregnancy experience rate among the female factory workers who had experience in sexual intercourse is 29.5 percent, which is 11.1 percent among the total respondents. Out of the 102 pregnant female workers, 98 workers(96.1 percent) terminated their pregnancy by induced abortion and 2 workders(2 percent) in natural abortion, while 1 worker(1 percent) was in pregnancy and another 1 worker had normal birth that was subsequently sent to orphanage. In order to cope with the problem of adolescent sexuality, a drastic and strong policy measures should be taken by the government. The most effective countermeasure to the adolescent sexual problems appears to the education. The sex and population education in the school is very much in need. In addition, sex education program through mass media and at the job sit-should be promoted for a healthy development of adolescents' sexual behavior. Also, the existing national family planning program, which has focused on the married couples, should be extended to the unmarried people in its scope and contents of the program.
1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. This family record form was designed to get information about demography, family planning, environmental sanitation and vital statistics. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. The team consisted of a public health nurse, four nurse-aides, a sanitarian and four health extension workers who are working in our project, The survey was carried out during the period November 1971 to March 1972. 3. This paper is aimed to analyse the recent changes in family planning as a step in an evaluation of the results of the population control policy which has been strenuously pursued by the government since December 1981. The data used in this analysis comes from the 1985 national fertility and family planning survey conducted by the Korea Institute for Population and Health (KIPH) in May 1985. According to the 1985 survey data, there have been great changes in both the contraceptive practice and fertility rates since the strengthening of the government's population control policy in 1 981. The contraceptive practice rate for married women aged 15-44 has increased from 57.7% in 1982 to 70.3% in 1985, an increase of 12.6% points in the short span of only 3 years. During the same period, the total fertility rate has declined from 2.7 in 1982 to the population replacement level which was planned to achieve by 1 988. More than 80% of women aged over 30 or with two children or more are practicing contraception, while the practice rates of women with 0-1 children are 14.0% and 45.1% respectively. The survey has revealed that the increase of contraceptive practice rate during 1982-1985 has mainly attributed to the high acceptance of sterilization procedures which practice rate of the married women has increased from 28.1% to 40.3% for the period. Also, the survey data shows that 24.7% of those women with only one child is practicing contraception for thepurpose of fertility termination. The government, taking into account of rapid changes in contraceptive practice and fertility rates, is formulating a population plan during the sixth fiveyear economic and social development plan (1987-1991) to achieve 1% ofpopulation growth rate by 1993, which was planned to realize by 2000. In order to meet this demographic goal, the existing population control policy measures should be improved to be suited to the recent contraceptive use and fertility changes. From this standpoint, the following considerations should be put forth; 1) improvement of the current program management systems including target allocation and evaluation schems for recruiting new acceptors in the young 20s groups to use contraceptives for birth spacing and to increase high continuation rates through the strengthening of follow-up services for the acceptors, 2) increase of self-supporting contraceptive users by promoting commercial advertisements on contraceptives through mass media including T.V. and radio, 3) development of social support policies including incentive schems, and strengthening of IE & C activities for increasing the proportion of the one-child family, 4) strengthening of population and family planning education in and out school youth, and 5) strengthening of management capabilities at the provincial and local program managers.
