One hundred and forty-eight cases of tennis elbow were treated by conservative managements firstly such as rest, medication, immobilization, physical therapy and local steroid injection, from Jan. 1985 to Jun. 1994 at Department of Orthopaedic Surgery, National Medical Center. Among the 148 cases, 16 cases who failed conservative managements, were treated with Nirschl & Pettrone operation. The results were summarized as follows, 1. Among the 148 cases, 110 cases$(74\%)$ were female, and 61 cases$(41.2\%)$ were in the age group 41 to 50, and mean age was 42.3 2. Ninety-nine cases were housewives and only 15 cases were related to sports. 3. The results 116cases$(78\%)$, of conservative treatment were excellent and good 10 fair and failure in 32 cases$(22\%)$, by Nirschl and Peettrone assessment. 4. Among the 16 cases who were treated with Nirschl and Pettrone operation, excellent result was obtained 10 7cases, good in 3 cases, fair in 4cases and failure in 2 cases. 5. Recurrence was developed in 18 cases$(12.2\%)$, and we obtained excellent & good result in 17 cases with both conservative and operative treatment. Above results suggest that the term, tennis elbow is a misnomer because it occurs more commonly in non-athletes such as housewives than in tennis players. So it seems to be a occupational disease rather than sports injury. Now, we propose eliminate of the term, elbow and substitute with terminology that described the true understanding and therapeutic orientation of it.
Back pain has plagued humans for many thousands of years. The treatment of back pain is divided into operative treatment and conservative treatment. It is reported that cure rate of conservative treatment is 80~90 percent. Generally, the treatment of oriental medicine is mostly conservative treatment. Persistence or recurrence of major symptoms for at least one year despite activity modification and physical therapy. 2. Tight hamstrings, persistently abnormal gait, or postural deformities unrelieved by physical therapy. 3. Sciatic scoliosis. 4. Progressive neurologic deficit. 5. Progressive slipping beyond 25 or 50 percent, even when asymptomatic. 6. A high slip angle (40 to 50 degrees) in a growing child, since it is likely to be associated with further progression and deformity. 7. Psychologic problems attributed to shortness of trunk, abnormal gait, and postural deformities characteristic of more severe slips.
In this study, a comparison between the structural design criteria for a catenary system with the maximum design speed of 100km/h and that of 300km/h was performed in order to establish that of 200km/h. According to the result, catenary design criteria for 300km/h operation is more conservative than that for 100km/h operation. This result shows that higher wind pressure and safety factor are adopted to catenary design for 300km/h operation. So, for the purpose of the economic structural design for catenary system, it is necessary to review the adoption possibility of catenary system for 100km/h operation first. In order to review the adoption possibility for catenary system for 100km/h operation, design criteria for 300km/h operation should be chosen for safer catenary structure.
Objective: This study was performed to evaluate the usefulness of early operation in children with traumatic subdural hygroma. Methods: The subjects were nine patients (Glasgow coma scale (GCS) score was below 10 and age was below 10 years old) who developed subdural hygroma after trauma between January 2000 to December 2002. Subduroperitoneal shunt was performed in one group and not performed in the other group. We analyzed the GCS score on admission and at 1 year after operation. Overall clinical results were evaluated retrospectively. Results: Patients who underwent operation exhibited higher GCS scores at 1 year after trauma compared to those in the patients who were treated by conservative therapy(p<0.05). Conclusion: The early operation could be an effective treatment to children with subdural hygroma who showed delayed improvement of consciousness and to patients with hygroma that didn't decrease or was above moderate amount.
Purpose: As patients who take immediate breast reconstructions with TRAM flap have increased, concomitant or delayed other elective intra-abdominal operations in these patients also have increased. There are few reports of concomitant or delayed intra-abdominal operation in TRAM flap patients. We report our experiences and outcomes of these operations which is safe and feasible. Methods: We reviewed the charts and postoperative follow-up results of 11 patients among 471 consecutive patients who took immediate breast reconstruction with TRAM flap from December of 2002 to September of 2006. Four patients took concomitant intra-abdominal operation and 7 patients took delayed intra-abdominal operation between 1 to 52 months after TRAM flap Results: There were no significant postoperative abdominal and systemic complications. One patient who took concomitant intra-abdominal operation presented partial skin necrosis of abdomen, but recovered completely with conservative treatments. Two patients took transfusion in peri-operative periods. Conclusion: Concomitant or delayed intra-abdominal operation in immediate breast reconstruction with TRAM flap could be performed safely and feasibly when it is necessary. Furthermore, it could be helpful to patients and surgeons.
Purpose: Even though the breast is not removed, women with a breast conservative operation(BCO) are suffering from the psychosocial distress comparable with mastectomies. The purpose of this study was to identify how clinical nurses view essential nursing problems and nursing approaches to post operative breast cancer experiences. Method: To identify the clinical nurses's interpretation, they were asked to respond to a case involving a woman struggling with breast cancer. The nurses' responses were analyzed using a manifest content analysis strategy to determine themes in problem identification and nursing care strategies. Result: The clinical nurses recognized that most of the patient's experiences originated from a psycho-social problem. Fifteen nurses(83%) interpreted the breast cancer woman's experience derived from her psychosocial problem. They identified the most effective nursing approaches for breast cancer women following surgery as counseling and providing information. Conclusion: In conclusion, the results revealed that a psychosocial skill including an empathic communication skill is essential for excellent nursing in post operative patient care.
Purpose: The aim of this study was to describe our treatment experiences with patients who had acute abdomen (AA) with common bile duct (CBD) dilatation. Methods: The treatment outcomes in children with AA and CBD dilatation were retrospectively reviewed. According to the shape of the intrahepatic bile ducts on ultrasonography (US), the origin of the pain was estimated as choledochal cyst (CC) complication or choledocholithiasis in normal CBD. Patients with complicated CC underwent surgery, and patients with choledocholithiasis in a normal appearing CBD underwent symptomatic treatment initially. Results: Of the 34 patients, 30 (88.2%) were female. The mean age of the patients was $6.4{\pm}4.9$ (range, 0.8-17) years. Seventeen (50.0%) patients had CBD stones and 17 (50.0%) did not. Surgical treatment was performed in 20 (58.8%) patients, 2 of whom underwent preoperative stone removal with endoscopic retrograde cholangiopancreatography and an operation. Conservative treatment was applied in 12 (35.3%) patients (8 with and 4 without stones), 1 of whom developed symptom relapse and underwent an operation. Among the 8 patients with CBD stones, 4 (4/17, 23.5%) had complete resolution of the stones and recovery of the CBD diameter after conservative treatment. US findings of patients with stone showed a fusiform or cylindrical shape of the CBD in 14 (82.4%) patients. Conclusion: The presence of stones in the distal CBD and the US features of CBD dilatation may be helpful to diagnose and treat the causes of biliary dilatation. Conservative treatment can be considered as initial therapy in patients with uncomplicated CBD dilatation with stone.
The purpose of this study was to evaluate the systemic and local production of immunoglobulins and their levels in patients with periapical cysts using Enzyme - Linked Immunosorbent Assay. Streptococcus sanguis, Bacteroides gingivalis, and Bacteroides intermedius were grown for use as antigen and they were harvested by centrifugation. The patients were divided into two groups: patients of periapical cysts and normal control. 5 patients of each group were selected and their blood were obtained via intravenous puncture prior to surgical operation. Sera were prepared by centrifugation of each blood samples. Cyst fluid were aspirated from cystic cavity and cyst wall were excised at operation. Control tissue were also excised at extraction site of impacted wisdom teeth from normal control. Each tissue was prepared by homogenization and centrifugation. Then antibodies of each sample were measured by modified ELISA. The following results were obtained: 1. Serum IgG and IgM levels were not significantly different between patients with periapical cyst and normal control. 2. IgG and IgM levels of cyst fluid to Bacteroides gingivalis and Bacteroides intermedius were significantly higher than those of serum of patients with periapical cyst, but there was no significant difference to Streptococcus sanguis. 3. IgG and IgM levels of cyst wall to Bacteroides gingivalis and Bacteroides intermedius were significantly higher than those of control tissue, but there was no significant difference to Streptococcus sanguis. 4. IgG and IgM levels in cyst fluid and IgG levels in cyst wall were highest to Bacteroides gingivalis, and IgM levels in cyst wall were highest to Bacteroides intermedius.
