의식하 진정에 사용된 약물에 관한 임상적 연구 (CONSCIOUS SEDATIVE DRUGS: A CLINICAL STUDY)

  • The purpose of this retrospective study is to investigate the effects of conscious sedation in an outpatient setting. Data from 292 patients (154 males, 138 females), sedated for minor oral surgery, were studied using various parameters including age and sex distribution, as well as according to procedures and each drug group. Results show that this intravenous conscious sedation technique is a safe and effective method of controlling behaviour, but one has to be prepared to deal with any emergencies or complications. The specific results are as follows: 1. Most procedures (265/292) were used to Valium mixed with Demerol and Midazolam mixed with Demerol. 2. Valium revealed a high incidence of pain upon injection and symptoms limited to daily activity and thrombophlebitis. 3. Nausea and vomiting manifested in the procedure using Demerol. 4. The patient manifested comfort similar in response to the same drugs used in another treatment.

치과 치료를 위한 외래 정주진정 법에 대한 다기관 후향적 임상연구 (A MULTICENTER RETROSPECTIVE STUDY OF OUTPATIENT INTRAVENOUS SEDATION FOR DENTAL TREATMENTS)

  • Purpose : On outpatient facilities, our sedation protocol focuses on the intermittent bolus injections of midazolam intravenously, according to patient's and operator's needs during the dental treatment. This multicenter retrospective study was aimed to prove the efficiency and safety of our sedation protocol. Patients and Methods : In three centers using the same outpatient sedation protocol for dental treatment (Division of Oral and Maxillofacial Surgery/Department of Dentistry in Hanyang University Medical Center, S-plant Dental Hospital. and Grand Oral and Maxillofacial Surgery), total 937 patients had various dental treatments under intravenous conscious sedations with independent patient monitoring from March 2006 to March 2009. By reviewing charts, we analyzed the results of sedation and dental treatment, retrospectively. Results : Our sedation protocol had no severe postoperative complications requiring admission. while showing good compatibility with almost all dental treatments, with acceptable satisfaction of both patients and operators. Conclusion : We assure that our sedation protocol can be used efficiently and safely on routine outpatient basis. We also hope that this study will provide the concrete concepts to common dental practitioners, who desire to perform sedation for dental treatment.

Safety of chloral hydrate sedation in dental practice for children: an overview

  • Chloral hydrate is the oldest and most common sedative drug used in moderate sedation for pediatric dental patients. Hence, the purpose of this article is to review the safety and possible adverse events of this drug when used for pediatric dental treatment. A bibliographic search in PubMed, MEDLINE, Cochrane Library and KMbase, KISS, DBpia, KoreaMed, and RISS databases was performed. Using the keywords "dental sedation," "chloral hydrate," and "children or adolescent," 512 scientific articles were found. Subsequently, 183 studies were individually assessed for their suitability for inclusion in this literature review. Altogether, 24 studies were selected. They included 12 cases of death before, during, or after chloral hydrate sedation for dental treatment, majorly due to dosing error and use of multiple sedatives. Additionally, intraoperative adverse events were mostly respiratory problems such as hypoxia and apnea, but most events were temporary. After treatment, prolonged sedation, including excessive sleep and less activity were the most common postoperative adverse events, and even death cases were reported. Despite the wide acceptance of chloral hydrate as a sedative-hypnotic agent, the risk of adverse events and adequate dose should be of great concern when using it for pediatric dental sedation.

소아 치과진정법 시행 시 진정심도 감시를 위한 Cerebral State Monitor의 임상적 유용성 (Clinical Usefulness of Cerebral State Monitor for Monitoring Sedation Depth during Dental Sedation in Children)

  • Background: It is imperative that adequate sedation level should be maintained for safe and effective dental sedation. Cerebral state index (CSI) is a empirically derived parameter calculated from the processed electroencephalography (EEG). We investigated whether CSI can be used as a sedation depth indicator. Methods: We continuously recorded CSI and bispectral index (BIS) values from 10 healthy children aged 3.6 yr undergoing dental sedation. We also evaluated sedation level using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S). The correlation between CSI and BIS, or CSI and sedation score were sought a regression analysis. Results: There were good linearity between CSI values and sedation score. (r = 0.916, P < 0.001) The paired CSI and BIS values showed a significant correlation between the two values. (r = 0.895, P < 0.001) The difference between CSI and BIS values was not statistically significant at deep and conscious sedative levels. Conclusions: The CSI monitor can be easy to monitor sedation depth during dental sedation for children.

임플란트 수술 시 의식하진정법이 환자의 통증과 불안에 미치는 영향 (The Effect of Conscious Sedation on Pain and Anxiety of Implant Surgery)

  • 본 연구는 임플란트 수술 시 의식하진정법이 환자의 통증과 불안에 미치는 영향을 평가하고자 2013년 6월 1일부터 8월 30일까지 천안시 일개 치과병원에 내원한 임플란트 수술환자 95명을 대상으로 조사하였다. 임플란트 수술 전과 후의 불안 및 통증의 정도를 파악하고 치과불안과 통증에 영향을 미치는 요인을 규명하기 위하여 수술 전, 후 통증(VAS)과 수술 중 불안(PQ)을 측정하고 수술 후 통증(SF-MPQ)을 측정하였다. 연구결과를 요약하면 다음과 같다. 임플란트 수술 형태의 특성에 따라 분석한 결과 수면군과 비수면군간의 통계적으로 유의한 차이를 보인 특성은 수술부위, 수술치아개수, 수술시간, 부가적인 수술형태 등이었다(p<0.001). 수술 전, 후 통증의 차이를 살펴본 결과 수술 전 예상되는 통증과 수술 후 경험했던 통증의 차이는 수면군(2.30)이 비수면군(0.98)이 비해 통계적으로 유의한 차이가 있었다(p<0.05). 임플란트 수술환자의 통증 및 불안에 영향을 미치는 요인은 성별과 연령, 임플란트 수술경험및 수술시간, PCS였으며 공통적으로 관련된 요인은 PCS였다(p<0.05). 따라서, 치과 임상에서는 임플란트 수술 전, 후 심리적인 안정을 제공할 수 있도록 여성이거나 나이가 적을수록 수술치아개수와 수술시간이 길어질 것으로 예상될 때 의식 하진정법의 추천과 정확한 정보를 제공함으로써 편안한 환경에서 치료할 수 있도록 해야 한다. 또한, 의식 하 진정법시 PCS를 통해 보다 체계적이고 객관적인 행동 전략이 필요하며, 환자관리를 위한 끊임없는 연구 및 교육프로그램 개발이 진행되어야 할 것이다.

소아치과 환자에서의 세보플루란을 이용한 흡입 심진정법 (Inhalational Deep Sedation Using Sevoflurane in Pediatric Dental Patients)

  • Background: Sevoflurane, a rotatively new inhalational anesthetic. has non-pungent odor and is less reluctant to pediatric patients. The purpose of this study is to examine the feasibility of sevoflurane in inhalational sedation instead of the nitrous oxide for short and simple dental treatments in pediatric patients. Patients and Methods: Fifteen healthy children, whose dental treatment was abandoned due to their little or no cooperation, were selected with their caregivers' written permission. Deep sedation was induced and maintained with oxygen and 1-5% sevoflufane via specially designed nasal mask. Blood pressure, heart rate, oxygen saturation, and electrocardiogram were monitored at 3-nin interval. A dental anesthesiologist, who was independent of dental treatments, was wholly responsible for the sedation procedure. Post-sedation complications and operator's and caregiver's acceptability of this type of inhalational sedation were also investigated. Results: The systolic and diastolic blood pressure, heart rate, and oxygen saturation was significantly depressed during the deep sedation using sevoflurane (P < 0.05). No severe post-sedation complications were found, however, bradycardia was reported in 3 patients. Almost all the operators and caregivers answered that they would adapt this sedation procedure again if possible. Conclusion: In this study, inhalational deep sedation using sevoflurane for dental treatments was found to be very useful. Furthermore, the application of sevoflurane to conscious sedation for pediatric and adult dental patients should be added.

소아청소년 진정내시경: 대한소아소화기영양학회 학술위원회 조사 보고 (Pediatric Endoscopic Sedation in Korea: A Survey of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition)

  • 목 적: 한국 소아청소년에서 시행되는 의식하 진정내시경 시행 실태와 부작용, 그리고 새로운 약제 등의 사용 여부 등을 다기관 연구를 통해 파악하여 적절한 지침을 세우는데 도움을 주고자 하였다. 방 법: 대한소아소화기영양학회 회원 소속 병원 중 2차 병원 이상의 병원을 대상으로 2006년 6월부터 2006년 7월까지 한 달의 기간 동안 시행된 내시경 검사를 대상으로 하였으며, 16개의 문항으로 설문지를 만들어 배포 후 수거된 결과를 조사 분석하였다. 결 과: 전체 내시경 시술 횟수 465예 중 의식하 진정 내시경은 89.0% (414예), 전신 마취하에 시술한 예는 0.9% (4예)였으며, 의식하 진정내시경의 비율은 상부소화관의 경우 89.1% (362예), 하부소화관의 경우는 88.1%(52예)였다. 의사 한 명 당 한달 평균 16.0건의 내시경을 시술하고 있었으며, 내시경을 시행하는 의사 수는 병원당 평균 1.16명, 보조자는 평균 2.58명이었다. 또한 마취과 의사의 도움을 받는 경우는 3개(12%) 병원이었다. 사용한 약물은 전체 의식하 진정내시경 414예 중 미다졸람(midazolam)만 사용한 경우가 전체 84.5% (350예), 프로포폴(propofol)을 사용한 경우는 상부소화관 내시경을 시행한 2예(0.5%)였고, 2가지 이상의 복합제재를 사용한 경우는 하부소화관검사의 경우 44예(84.6%), 역행성담관조영술(ERCP)의 경우가 2예(100%)였다. 진정내시경 시행 중 사용한 모니터링은 심전도 감시와 함께 맥박산소측정기를 사용한 경우가 91.4% (320예), 자동혈압측정기를 같이 사용한 경우 5.1% (18예)였다. 부작용으로는 일시적 산소포화도의 저하로 산소를 투여한 경우로 4.6% (16예)였다. 또한 길항제로 flumazenil을 투여한 경우는 모든 환자에서 투여한다고 답한 경우를 제외하면 2.71% (9예)였다. 진정내시경 시행 여부의 결정은 환자 혹은 환자의 보호자와 상의하여 결정하는 경우가 84% (21개 병원)였다. 의사가 느끼는 만족도는 68%에서 만족하는 것으로 나타났으며, 의사가 느끼는 환자 혹은 환자 보호자의 만족도는 만족이 84% (21개 병원)였다. 결 론: 우리나라 소아의 의식하 진정내시경 시 사용한 약물은 대부분 미다졸람 단독 투여만으로 시행하며, 하부소화관 검사 시에는 복합 약물을 투여한 경우가 많았다. 또한 환자와 의사 대부분 만족하는 것으로 나타났으며, 부작용은 거의 나타나지 않았다.

Propofol with and without Midazolam for Diagnostic Upper Gastrointestinal Endoscopies in Children

  • Purpose: Various publications on the use of sedation and anesthesia for diagnostic procedures in children have demonstrated that no ideal agent is available. Although propofol has been widely used for sedation during esophagogastroduodenoscopy in children, adverse events including hypoxia and hypotension, are concerns in propofol-based sedation. Propofol is used in combination with other sedatives in order to reduce potential complications. We aimed to analyze whether the administration of midazolam would improve the safety and efficacy of propofol-based sedation in diagnostic esophagogastroduodenoscopies in children. Methods: We retrospectively reviewed the hospital records of children who underwent diagnostic esophagogastroduodenoscopies during a 30-month period. Demographic characteristics, vital signs, medication dosages, induction times, sedation times, recovery times, and any complications observed, were examined. Results: Baseline characteristics did not differ between the midazolam-propofol and propofol alone groups. No differences were observed between the two groups in terms of induction times, sedation times, recovery times, or the proportion of satisfactory endoscopist responses. No major procedural complications, such as cardiac arrest, apnea, or laryngospasm, occurred in any case. However, minor complications developed in 22 patients (10.7%), 17 (16.2%) in the midazolam-propofol group and five (5.0%) in the propofol alone group (p=0.010). Conclusion: The sedation protocol with propofol was safe and efficient. The administration of midazolam provided no additional benefit in propofol-based sedation.

An 18-year-old patient with Prader-Willi syndrome: a case report on dental management under sedation and general anesthesia

  • Prader-Willi syndrome (PWS) is a rare genetic disorder reported rarely in dentistry. Dental practitioners should know the features of PWS because affected patients have a variety of dental symptoms. The current report describes a case of PWS. An 18-year-old male patient presented with traumatic injuries. Initial emergency treatments were performed under sedation, and further treatments were conducted under general anesthesia. After adequate healing, periodic follow-up and dietary management according to the patient's age and nutritional phase were recommended. Dental management of PWS patients consists of active preventive measures in addition to dietary consultation according to age and nutritional phase.

Complications caused by nitrous oxide in dental sedation

  • The first clinical application of nitrous oxide ($N_2O$) was in 1844, by an American dentist named Horace Wells who used it to control pain during tooth extraction. Since then, $N_2O$ has shared a 170-year history with modern dental anesthesia. $N_2O$, an odorless and colorless gas, is very appealing as a sedative owing to its anxiolytic, analgesic, and amnestic properties, rapid onset and recovery, and, in particular, needle-free application. Numerous studies have reported that $N_2O$ can be used safely and effectively as a procedural sedation and analgesia (PSA) agent. However, $N_2O$ can lead to the irreversible inactivation of vitamin B12, which is essential for humans; although rare, this can be fatal in some patients.
