A Case Report of Congestive Heart Failure Treated with Acupotomy Therapy (심부전으로 인한 호흡곤란 환자에게 시행한 침도침 시술 1례 보고)

  • Objectives : The purpose of this report is to report the effect of acupotomy for patients with congestive heart failure. Methods: We treated 1 patient who has dyspnea due to congestive heart failure with acupotomy. To check the effect and satisfaction of acupotomy we used New York Heart Association class(NYHA class), Modified Borg Scalw Dyspnea Index(Borg Index), Baseline Dyspnea Index(BDI), Five-point Likert scale. Results : After 1 month of treatment, the patient felt much better in breathing, and had better score in NYHA class, Borg Index, BDI. Conclusions : This report demonstrates that acupotomy therapy has useful effect on congestive heart failure, but the more cases and researches are needed.

  • A clinical case study of a shaoyangin patient diagnosed as congestive heart failure Congestive heart failure is dysfunction of heart that can't supply sufficient mount of blood required of tissue metabolism as a result of myocardial hyposystole. We treated 80 year-old female congestive heart failure patient with edema, dyspnea, chest discomfort, and dysuria. We have diagnosed her as shaoyangin(少陽人) pyohan(表寒) disease, infirmity, edema and have prescribed hyungbangjihwangtang(荊防地黃湯). In the result we had the improvement of the symptoms of CHF and general depressed condition. This report described the process and contents about the way the patient was cured.

Association of Congestive Heart Failure and Death with Ankylosing Spondylitis : A Nationwide Longitudinal Cohort Study in Korea

  • Objective : We attempted to discover that Ankylosing spondylitis (AS) has a comprehensive relationship with congestive heart failure and death. Methods : We used a nationwide database managed by the Korean National Health Insurance Service from 2010 to 2014. Twelve thousand nine hundred eighty-eight patients with a diagnosis of AS and 64940 age- and sex- stratified matching subjects without AS were enrolled in the AS and control groups. Incidence probabilities of 6 years congestive heart failure and death in each group were calculated. The Cox proportional hazard regression analysis was used to estimate the hazard ratio. We divided the AS and control groups into subgroups according to sex, age, income, and comorbidities. Results : During the follow-up period, 102 patients (0.79%) in the AS group and 201 patients (0.32%) in the control group developed congestive heart failure (p<0.0001). In addition, 211 (1.62%) subjects in the AS group died during the follow-up period compared to 639 (0.98%) subjects in the control group (p<0.0001). The adjusted hazard ratio of congestive heart failure and death in the AS group was 2.28 (95% confidence interval [CI], 1.80-2.89) and 1.66 (95% CI, 1.42-1.95), respectively. The hazard ratios of congestive heart failure and death were significantly increased in all of the subgroups. Conclusion : The incidence rates of congestive heart failure and death were increased in AS patients.

  • A 3-year-old girl presented with polydipsia, polyuria, hyponatremia, hypertension and congestive heart failure. Her polyuria was unresponsive to water restriction and vasopressin challenge tests, and her blood pressure was not effectively controlled by antihypertensive drugs. Radiologic examinations revealed a Wilms' tumor in the right kidney. Her plasma renin activity and aldosterone concentration were greatly increased. After surgical removal of the tumor, the congestive heart failure disappeared. Congestive heart failure due to Wilms' tumor is very rare and we report here on such a case, with a brief review of the literature.

Case Study of Patient with Pleural Effusion Due to Congestive Heart Failure (울혈성 심부전으로 인한 흉막삼출에 대한 한방치험 1례)

  • The purpose of this case was to report the effect of Korean medical treatment for patient with pleural effusion due to congestive heart failure. The patient was treated with herbal medicine(Cheongsingeonbi-tang) and acupuncture. The effect of treatment was evaluated by chest X-ray, New York Heart Association(NYHA) functional classification, and Hugh-Jones classification. After 3 weeks of treatment, the amount of pleural effusion was decreased and NYHA class, Hugh-Jones grade were improved. NYHA functional classification improved class III to II and Hugh-Jones classification changed grade IV to II. This result suggests that herbal medicine(Cheongsingeonbi-tang) and acupuncture treatment might have an effect on patient with pleural effusion due to congestive heart failure.

  • A 67-year-old woman with severe congestive heart failure (New York Heart Association, NYHA class IV) was set to receive general anesthesia for cardiac surgery. For several months, she had been in a constant sitting position from which the slightest change evoked dyspnea. A patient in such a condition is rarely considered a candidate for general anesthesia, because such patients are never eligible for any type of surgery other than that used to fix the heart problem itself. We report this case to explain how anesthesia was induced with the patient sitting in a crouching position and discuss other methods of induction that can probably be used in similar situations.

Automatic Detection of Congestive Heart Failure and Atrial Fibrillation with Short RR Interval Time Series

  • Atrial fibrillation (AF) and Congestive heart failure (CHF) are increasingly widespread, costly, deadly diseases and are associated with significant morbidity and mortality. In this study, we analyzed three statistical methods for automatic detection of AF and CHF based on the randomness, variability and complexity of the heart beat interval, which is RRI time series. Specifically, we used short RRI time series with 16 beats and employed the normalized root mean square of successive RR differences (RMSSD), the sample entropy and the Shannon entropy. The detection performance was analyzed using four large well documented databases, namely the MIT-BIH Atrial fibrillation (n=23), the MIT-BIH Normal Sinus Rhythm (n=18), the BIDMC Congestive Heart Failure (n=13) and the Congestive Heart Failure RRI databases (n=25). Using thresholds by Receiver Operating Characteristic (ROC) curves, we found that the normalized RMSSD provided the highest accuracy. The overall sensitivity, specificity and accuracy for AF and CHF were 0.8649, 0.9331 and 0.9104, respectively. Regarding CHF detection, the detection rate of CHF (NYHA III-IV) was 0.9113 while CHF (NYHA I-II) was 0.7312, which shows that the detection rate of CHF with higher severity is higher than that of CHF with lower severity. For the clinical 24 hour data (n=42), the overall sensitivity, specificity and accuracy for AF and CHF were 0.8809, 0.9406 and 0.9108, respectively, using normalized RMSSD.

Self Care Compliance, Family Support, and Depression in Patients with Congestive Heart Failure (울혈성 심부전 환자의 자가관리 이행, 가족 지지 및 우울)

  • Purpose: The purpose of this study was to examine self care compliance, family support, and depression in patients with congestive heart failure. Method: The participants were 105 outpatients with congestive heart failure. Data were collected using a questionnaire survey on self care compliance, family support, and depression. The collected data were analyzed using the SPSS Win 10.0 program. Result: 1. The total score for Self care compliance was 78.34 out of a possible 120 and was significantly high for people in ages between 70-79 years and people who were not employed. Exercise compliance was significantly high in men and classifications I & II of the NYHA class. Compliance with smoking and alcohol cessation were significantly high in women. Medication compliance was significantly high in the group which had experience with hospitalization. Family support was 31.75 out of a possible 40 and showed significant differences depending on spouse status and religion. Depression was 30.18 out of a possible 64 and was significantly high in the group whose educational level was above college graduation or who were under 65 years of age. Conclusion: The results suggest that nurses have to emphasize smoking and alcohol cessation for men, exercise for women and total self care compliance for patients under 65 years of age.

The medical management of mitral stenosis in a Bull Terrier

  • An eight-year-old, 28-kg male bull terrier who showed signs of lethargy and cough was referred for further evaluation of congestive heart failure. On presentation, physical examination revealed a systolic murmur at the left apex of the heart. Moreover, chest radiograph evaluation confirmed the mild alveolar and interstitial patterns in the caudal lung lobes and a grossly enlarged left atrium and left ventricle. Electrocardiography showed atrial fibrillation with a wide QRS complex, and transthoracic echocardiography revealed marked enlargement of the left atrium with abnormal morphology of the mitral valve. The thickened, hammer-like appearance and abnormal diastolic motion of the mitral valve leaflets were characterized by decreased leaflet separation and doming of the valve. The diagnosis was mitral stenosis with congestive heart failure and atrial fibrillation. The owner declined interventional valvuloplasty. Medical treatment included furosemide, pimobendan and diltiazem. Regular health check-ups have shown that vitality and clinical signs have improved considerably, and the dog have remained stable for 6 months after the presentation.

A Case Report on Cardiac Asthma due to Congestive Heart Failure (울혈성 심부전으로 인한 심장성 천식환자 치험 1례)

  • Congestive heart failure is a cardiac dysfunction that can't supply sufficient amount of blood. The disorder usually causes hypertension, ischemic cardiac disease and give rise to pulmonary congestion and pulmonary epileptic edema. The symptoms of the disease are dyspnea, palpitation, edema, etc. We treated a 82 year-old female patient who had severe dyspnea, nausea, and dizziness. At the point of differentiation of syndromes(辨證), the case was diagnosed as Su-Cheon(水喘) induced by Yang-deficiency of Both Spleen and Kidney(脾腎陽虛) and was administered Yerunpang(胃?湯). After two weeks of the treatment, we witnessed improvement in the symptoms of CHF and general depressed condition. This report describes the process and contents about the way the patient was treated.
