Linear Accelerator를 이용한 Stereotactic Radiosurgery 방법 (A Method of Stereotactic Radiosurgery Using A Linear Accelerator)

  • A modified irradiation technique utilizing a linear accelerator for radiation surgery within the brain was performed in 41 cases of patients with anteriovenous malformation(AVM), astrocytoma, meningioma. etc. The treatment planning and dosimetry of small field for stereotactic radiosurgery with 10 MV X-ray isocentically mounted linear accelerator will be presented dose with field size, the central axis persent depth dose and the combined moving beam dose distribution. The three dimensional dose planning of stereotactic focusing irradiation on small size tumor region was perfomed with dose planning computer system(Therac 2300) and was verified with film dosimetry. The more the number of strip and the wider the angle of arc rotation, the larger were the dose delivered on tumor and the less the dose to surrounding the normal tissues. In this study, the using machine and method was as fellowing. 1) Apparatus : NELAC-1018 10MV X-ray 2) Strip No. : Select the 5-7 strips 3) Cone and field size are from $1{\times}1cm^2$ to $3.5{\times}3.5cm^2$, and special circular cone designed for the purpose of minimized the risk to normal tissue and those size are $0.7{\~}3.6cm{\phi}$.

5 축 동시 구동을 통한 볼바 측정용 원추형 경로 생성 방법 (Conical Path Generation Technique for Ball Bar Measurement Using Simultaneous 5-Axis Motion Control)

  • 본 연구는 원추대 가공과 등가인 동시 5 축 구동을 통한 볼바 측정용 경로 생성 기법을 소개하며 시스템 구조 형태(Structural configuration)에 관계없이 모든 5 축 공작기계에 대해 적용 가능한 일반화된 방법을 제시한다. 5 축 공작기계 시스템 정보와 원추형 정보, 그리고 볼바 측정 정보 등을 입력 받아 NC 코드 생성, 다양한 오차 평가 시뮬레이션 및 측정 시험을 쉽게 수행할 수 있도록 원호 경로 생성에 필요한 수학적 기계 입력 모델을 제시한다. 또한 제시된 원추형 경로 모델을 토대로 5 축 동시 구동 시 회전축의 이송 범위를 수학적으로 검토하며 반꼭지각 및 기울기각의 크기 차이, 볼바 틸팅 허용각 및 워크피스 볼의 오프셋 위치 등 여러 가지 조건이 이송 범위에 미치는 영향에 대해 분석한다.

Assessment of buccal bone thickness of aesthetic maxillary region: a cone-beam computed tomography study

  • Purpose: The aim of this study was to analyze the anatomical dimensions of the buccal bone walls of the aesthetic maxillary region for immediate implant placement, based upon cone-beam computed tomography (CBCT) scans in a sample of adult patients. Methods: Two calibrated examiners analyzed a sample of 50 CBCT scans, performing morphometric analyses of both incisors and canines on the left and right sides. Subsequently, in the sagittal view, a line was traced through the major axis of the selected tooth. Then, a second line (E) was traced from the buccal to the palatal wall at the level of the observed bone ridges. The heights of the buccal and palatal bone ridges were determined at the major axis of the tooth. The buccal bone thickness was measured across five lines. The first was at the level of line E. The second was at the most apical point of the tooth, and the other three lines were equidistant between the apical and the cervical lines, and parallel to them. Statistical analysis was performed with a significance level of $P{\leq}0.05$ for the bone thickness means and standard deviations per tooth and patient for the five lines at varying depths. Results: The means of the buccal wall thicknesses in the central incisors, lateral incisors and canines were $1.14{\pm}0.65mm$, $0.95{\pm}0.67mm$ and $1.15{\pm}0.68mm$, respectively. Additionally, only on the left side were significant differences in some measurements of buccal bone thickness observed according to age and gender. However, age and gender did not show significant differences in heights between the palatal and buccal plates. In a few cases, the buccal wall had a greater height than the palatal wall. Conclusions: Less than 10% of sites showed more than a 2-mm thickness of the buccal bone wall, with the exception of the central incisor region, wherein 14.4% of cases were ${\geq}2mm$.

Cone-beam computed tomographic evaluation of the condylar remodeling occurring after mandibular set-back by bilateral sagittal split ramus osteotomy and rigid fixation

  • Objective: To evaluate condylar head remodeling after mandibular set-back sagittal split ramus osteotomy (SSRO) with rigid fixation in skeletal class III deformities. The correlation between condylar head remodeling and condylar axis changes was determined using cone-beam computed tomography (CBCT) superimposition. Methods: The CBCT data of 22 subjects (9 men and 13 women) who had undergone mandibular set-back SSRO with rigid fixation were analyzed. Changes in the condylar head measurements and the distribution of the signs of condylar head remodeling were evaluated by CBCT superimposition. Results: The subjects showed inward rotation of the axial condylar angle; reduced condylar heights on the sagittal and coronal planes; and resorptive remodeling in the anterior and superior areas on the sagittal plane, superior and lateral areas on the coronal plane, and anterior-middle and anterior-lateral areas on the axial plane (p < 0.05). Conclusions: The CBCT superimposition method showed condylar head remodeling after mandibular set-back SSRO with rigid fixation. In skeletal class III patients, SSRO with rigid fixation resulted in rotation, diminution, and remodeling of the condylar head. However, these changes did not produce clinical signs or symptoms of temporomandibular disorders.

Construction reproducibility of a composite tooth model composed of an intraoral-scanned crown and a cone-beam computed tomography-scanned root

  • Objective: To evaluate the construction reproducibility of a composite tooth model (CTM) composed of an intraoral-scanned crown and a cone-beam computed tomography (CBCT)-scanned root. Methods: The study assessed 240 teeth (30 central incisors, 30 canines, 30 second premolars, and 30 first molars in the maxillary and mandibular arches) from 15 young adult patients whose pre-treatment intraoral scan and CBCT were available. Examiner-Reference (3 years' experience in CTM construction) and Examiners-A and Examiner-B (no experience) constructed the individual CTMs independently by performing the following steps: image acquisition and processing into a three-dimensional model, integration of intraoral-scanned crowns and CBCT-scanned teeth, and replacement of the CBCT-scanned crown with the intraoral-scanned crown. The tooth axis angle in terms of mesiodistal angulation and buccolingual inclination of the CTMs constructed by the three examiners were measured. To assess the construction reproducibility of CTMs, intraclass correlation coefficient (ICC) assessments were performed. Results: The ICC values of mesiodistal angulation and buccolingual inclination among the 3 examiners showed excellent agreement (0.950-0.992 and 0.965-0.993; 0.976-0.994 and 0.973-0.995 in the maxillary and mandibular arches, respectively). Conclusions: The CTM showed excellent construction reproducibility in mesiodistal angulation and buccolingual inclination regardless of the construction skill and experience levels of the examiners.

Optimization of exposure parameters and relationship between subjective and technical image quality in cone-beam computed tomography

  • Purpose: This study was performed to investigate the effect of exposure parameters on image quality obtained using a cone-beam computed tomography (CBCT) scanner and the relationship between physical factors and clinical image quality depending on the diagnostic task. Materials and Methods: CBCT images of a SedentexCT IQ phantom and a real skull phantom were obtained under different combinations of tube voltage and tube current (Alphard 3030 CBCT scanner, 78-90 kVp and 2-8 mA). The images obtained using a SedentexCT IQ phantom were analyzed technically, and the physical factors of image noise, contrast resolution, spatial resolution, and metal artifacts were measured. The images obtained using a real skull phantom were evaluated for each diagnostic task by 6 oral and maxillofacial radiologists, and each setting was classified as acceptable or unacceptable based on those evaluations. A statistical analysis of the relationships of exposure parameters and physical factors with observer scores was conducted. Results: For periapical diagnosis and implant planning, the tube current of the acceptable images was significantly higher than that of the unacceptable images. Image noise, the contrast-to-noise ratio (CNR), the line pair chart on the Z axis, and modulation transfer function (MTF) values showed statistically significant differences between the acceptable and unacceptable image groups. The cut-off values obtained using receiver operating characteristic curves for CNR and MTF 10 were useful for determining acceptability. Conclusion: Tube current had a major influence on clinical image quality. CNR and MTF 10 were useful physical factors that showed significantly associations with clinical image quality.

Cone-beam computed tomography-guided three-dimensional evaluation of treatment effectiveness of the Frog appliance

  • Objective: To evaluate the effectiveness of the Frog appliance in three dimensions by using cone-beam computed tomography (CBCT) images. Methods: Forty patients (21 boys and 19 girls), averaged 11.7 years old, with an Angle Class II division 1 malocclusion were included in our study. They had either late mixed dentition or early permanent dentition, and the maxillary second molars had not yet erupted. All patients underwent CBCT before and after the treatment for measuring changes in the maxillary first molars, second premolars, central incisors, and profile. Paired-samples t-test was used to compare the mean difference in each variable before treatment and after the first phase of treatment. Results: The maxillary first molars were effectively distalized by 4.25 mm (p < 0.001) and 3.53 mm (p < 0.05) in the dental crown and root apex, respectively. The tipping increased by $2.25^{\circ}$, but the difference was not significant. Moreover the teeth moved buccally by 0.84 mm (p < 0.05) and 2.87 mm (p < 0.01) in the mesiobuccal and distobuccal cusps, respectively, whereas no significant changes occurred in the root apex. Regarding the anchorage parts, the angle of the maxillary central incisor's long axis to the sella-nasion plane increased by $2.76^{\circ}$ (p < 0.05) and the distance from the upper lip to the esthetic plane decreased by 0.52 mm (p = 0.01). Conclusions: The Frog appliance effectively distalized the maxillary molars with an acceptable degree of tipping, distobuccal rotation, and buccal crown torque, with only slight anchorage loss. Furthermore, CBCT image demonstrated that it is a simple and reliable method for three-dimensional analysis.

THE FORMATION OF THE DOUBLE GAUSSIAN LINE PROFILES OF THE SYMBIOTIC STAR AG PEGASI

  • We analyze high dispersion emission lines of the symbiotic nova AG Pegasi, observed in 1998, 2001, and 2002. The Hα and Hβ lines show three components, two narrow and one underlying broad line components, but most other lines, such as HI, HeI, and HeII lines, show two blue- and red-shifted components only. A recent study by Lee & Hyung (2018) suggested that the double Gaussian lines emitted from a bipolar conical shell are likely to form Raman scattering lines observed in 1998. In this study, we show that the bipolar cone with an opening angle of 74°, which expands at a velocity of 70 km s-1 along the polar axis of the white dwarf, can accommodate the observed double line profiles in 1998, 2001, and 2002. We conclude that the emission zone of the bipolar conical shell, which formed along the bipolar axis of the white dwarf due to the collimation by the accretion disk, is responsible for the double Gaussian profiles.

방광암 환자의 영상유도 방사선치료에 관한 고찰 (The Investigation Image-guided Radiation Therapy of Bladder Cancer Patients)

  • 목 적: 현재 본원에서 방광암 환자의 영상유도 방사선치료는 재현성을 높이기 위하여 환자의 상태에 따라 알맞은 양의 생리식염수를 주입하고 영상유도 시스템(On-Board Imager system, OBI, VARIAN, USA)의 Cone.Beam CT (CBCT)로 3차원 정합(3D-3D matching)을 하여 치료를 한다. 본 연구에서는 방광암 환자의 치료 시 획득한 CBCT 영상의 분석을 통해 뼈를 기준으로 한 정합과 방광을 기준으로 한 정합의 차이를 알아보고, 생리 식염수를 주입한 방광의 체적 변화를 알아보고 방광암 환자의 치료 시 더욱 적절한 영상정합방법을 평가하고 고찰하고자 한다. 대상 및 방법: 본원에서 2009년 1월에서 2010년 4월까지 방사선치료를 위해 내원한 방광암 환자 7명을 대상으로 Folly catheter를 이용하여 방광 내 잔류 소변을 제거한 뒤 환자 개개인에 맞게 정해진 양 만큼의 생리식염수를 주입하고 CT-Sim 후 치료계획을 설계하였다. 그 뒤 OBI system을 이용하여 치료 전 자세 확인을 위해 CBCT를 찍었고, 담당 주치의가 모든 대상 환자의 영상 정합을 진행하였다. 총 45개 CBCT 영상을 이용하여 뼈를 기준으로 한 영상정합과 방광을 기준으로 한 영상정합의 차이를 분석하였다. 또, 방광의 체적 변화를 Eclipse (version 8.0, VARIAN, USA)를 통해 얻어냈다. 결 과: 뼈를 기준으로 한 영상정합을 한 후 다시 방광을 기준으로 한 정합의 차이는 X축으로 평균 $3{\pm}2mm$, Y축으로 $1.8{\pm}1.3mm$, Z축으로 $2.3{\pm}1.7mm$이고 전체 이동거리는 $4.8{\pm}2.0mm$로 나타났다. 또 방광의 체적은 기준 대비 $4.03{\pm}3.97%$의 차이를 나타냈다. 결 론: 방광의 특성상 해부학적 위치 및 내부의 움직임으로 인해 뼈를 이용한 영상정합 후에도 방광의 위치 차이가 발생하였다. 또, 생리식염수를 채운 방광의 체적은 4.03%의 차이를 나타냈으나 영상 정합 시 모두 계획한 볼륨 안에 포함되는 것을 확인 할 수 있었다. 따라서 생리식염수를 주입한 뒤 방광을 기준으로 영상 정합을 실시함으로써 더욱 정확한 치료를 실시 할 수 있을 것으로 사료된다.

Cone-beam CT를 이용한 안면비대칭자에서 이부편위에 따른 치성보상의 양상분석 (Correlation between menton deviation and dental compensation in facial asymmetry using cone-beam CT)

  • 본 연구는 Cone-beam CT의 MPR이미지상에서 하악골 평면에 대한 하악 제1대구치와 견치의 편위측과 비편위측의 높이와 각도차이 그리고 FH 평면에 대한 상악 제1대구치와 견치의 편위측과 비편위측의 높이와 각도차이를 계측함으로써 안면비대칭자에서 이부편위에 따른 편위측과 비편위측의 제1대구치와 견치의 높이와 협설측 경사도를 CBCT를 이용하여 분석하고 그 관련성을 알아보는 데 있다. 연구대상은 하악골의 이부편위를 동반한 안면비대칭 환자 50명(여자 29명, 남자 21명, 평균나이 24세 4개월)으로 하였다. 대조군으로 평균나이 25세 7개월인 20명(남자 11명, 여자 9명)을 분석하였다. 결과는 이부편위에 대해 FH 평면에 대한 상악 제1대구치의 편위측과 비편위측의 길이 및 각도 차이(편위측 - 비편위측)는 대조군에 대해 통계적으로 유의한 차이가 존재하였다 (p < 0.01). 비대칭군에서 상악 제1대구치 각도는 편위측이 비편위측보다 평균 $8.62\;{\pm}\;5.95^{\circ}$ 컸다 (p < 0.01). 상악 견치의 경우에도 편위측과 비편위측의 각도차이가 평균 $4.48\;{\pm}\;5.05^{\circ}$로 통계적으로 유의한 차이가 존재하였다 (p < 0.01). 하악골 평면에 대한 하악 제1대구치의 수직 길이와 각도는 편위측과 비편위측에서 통계적으로 유의할 만한 차이가 존재하였다 (p < 0.01). 각도의 차이는 평균 $-11.92\;{\pm}\;5.79^{\circ}$로 나타났다. 이부편위에 대한 상하악 제1대구치와 견치의 수직거리 및 각도의 Pearson 상관분석 및 회귀분석 결과, FH 평면에 대한 상악 제1대구치의 편위측과 비편위측의 각도차이(${\Delta\angle}UM6$-FH plane (dev.-ndev.))는 통계적으로 유의한 양의 상관관계를 나타내었고, 하악골 평면에 대한 하악 제1대구치의 편위측과 비편위측의 각도차이(${\Delta\angle}LM6$-Mn plane (dev.-ndev.))는 통계적으로 유의한 음의 상관관계를 나타내었다. 이상의 결과에서 이부편위를 동반한 하악골 비대칭이 존재하는 경우, 이부가 편위되는 방향으로 상하악 제1대구치의 치아장축의 각도가 변화되는 것으로 나타났다.
