Interventions for anesthetic success in symptomatic irreversible pulpitis: A network meta-analysis of randomized controlled trials

  • Background: Local anesthetics alone or in combination with adjuncts, such as oral medications, have routinely been used for pain control during endodontic treatment. The best clinical choice amongst the vast numbers of agents and techniques available for pain control for irreversible pulpitis is unclear. This network meta-analysis combined the available evidence on agents and techniques for pulpal anesthesia in the maxilla and mandible, in order to identify the best amongst these approaches statistically, as a basis for future clinical trials. Methods: Randomized trials in MEDLINE, DARE, and COCHRANE databases were screened based on inclusion criteria and data were extracted. Heterogeneity was assessed and odds ratios were used to estimate effects. Inconsistencies between direct and indirect pooled estimates were evaluated by H-statistics. The Grading of Recommendation, Assessment, Development, and Evaluation working group approach was used to assess evidence quality. Results: Sixty-two studies (nine studies in the maxilla and 53 studies in the mandible) were included in the meta-analysis. Increased mandibular pulpal anesthesia success was observed on premedication with aceclofenac + paracetamol or supplemental 4% articaine buccal infiltration or ibuprofen+paracetamol premedication, all the above mentioned with 2% lignocaine inferior alveolar nerve block (IANB). No significant difference was noted for any of the agents investigated in terms of the success rate of maxillary pulpal anesthesia. Conclusion: Direct and indirect comparisons indicated that some combinations of IANB with premedication and/or supplemental infiltration had a greater chance of producing successful mandibular pulpal anesthesia. No ideal technique for maxillary anesthesia emerged. Randomized clinical trials with increased sample size may be needed to provide more conclusive data. Our findings suggest that further high-quality studies are required in order to provide definitive direction to clinicians regarding the best agents and techniques to use for mandibular and maxillary anesthesia for irreversible pulpitis.

Is meconium exposure associated with autism spectrum disorders in children?

  • Background: The results differ among published studies regarding exposure to meconium and the risk of developing autism spectrum disorders (ASDs). Purpose: The present study pooled all of the epidemiologic studies retrieved from broader databases on the association between meconium exposure and risk of developing ASD in children. Methods: The Web of Science, PubMed, Scopus, and Google Scholar databases were searched without language restrictions for articles published between their inception to February 20, 2020, using relevant keywords. The pooled odds ratios (ORs) and their 95% confidence intervals (CIs) were calculated as random-effect estimates of the associations among studies. A subgroup analysis was conducted to explore any potential sources of heterogeneity among studies. Results: The pooled estimate of OR reported a weakly significant association between meconium exposure and ASD development in children (OR, 1.13; 95% CI, 1.03-1.24). There was low heterogeneity among the articles reporting risk for ASD among children (I2=19.3%; P=0.259). The results of subgroup analysis based on meconium exposure showed a significant association between a meconium-stained neonate and ASD development (OR, 1.18; 95% CI, 1.11-1.24). Couclusion: Meconium exposure was weakly associated with an increased risk of ASD. However, more evidence based on large prospective cohort studies is required to provide conclusive evidence about whether meconium exposure is associated with an increased risk of ASD development.

The Effects of Non-pharmacological Interventions on Sleep among Older Adults in Korean Long-term Care Facilities: A Systematic Review and Meta-analysis

  • Purpose: This study aimed to examine the effects of non-pharmacological sleep intervention programs in improving sleep quality among older adults in long-term care facilities. Methods: A literature search and selection was performed on nine different databases using the guidelines of PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). Overall, 14 studies met the inclusion criteria and were systematically reviewed. For the meta-analysis, the effect size was estimated using the random-effects model in Review Manager (RevMan) desktop version 5.4 of the Cochrane Library. Results: The meta-analysis of overall non-pharmacological interventions obtained a total effect size of 1.0 (standardized mean difference [SMD]=1.0, 95% confidence interval [CI]: 0.64~1.35), which was statistically significant (Z=5.55, p<.001). The most frequently studied non-pharmacological intervention was aroma therapy, with an effect size of 0.61 (SMD=0.61, 95% CI: 0.14~1.08), which was statistically significant (Z=2.55, p=.010). In the subgroup analysis, group-based interventions, interventions for >4 weeks, and untreated control studies were more effective. Conclusion: This study confirms that non-pharmacological interventions are effective in improving sleep quality among older adults in long-term care facilities. However, the sample size was small and the risk of bias in assessing the interventions of individual studies was unclear or high, thereby limiting the generalizability of the results. Further reviews that evaluate randomized control trials, evidence-based interventions that consider older adult participants' physical activity levels, different intervention methods and durations, and different control group intervention types are needed to obtain more conclusive evidence.

  • The purpose of this study is to try to date the letters of Lady Susan and to find out which year's calender Jane Austen used to write the novel. The novel in forty-one letters with a brief narrated conclusion was transcribed as a fair copy by Jane Austen around 1805. However, no conclusive evidence for a precise date of composition exists, as only the fair copy survives. There is a watermark of 1805 on two leaves of the surviving manuscript of the transcribed copy. This indicates the period of the work's transcription but it does not indicate when it was first composed beyond suggesting that it was completed sometime before 1805. In the novel itself Jane Austen marked time sufficiently and frequently enough to allow us to follow it precisely. This strongly suggests that Jane Austen used a calender to write letters of the novel, even though dates and days were not given to the letters. To determine the most probable date of the calender is significant because it can be a clue to clarify when the novel was composed originally. Many words and phrases pointing to specific days or periods are mentioned in almost all the letters of the novel. From the words and phrases, a potential date to each letter can be successfully deduced and a potential possible calender for the novel can be made. In the end, all attempts to determine the authentic calender used for the novel fail. The main reason for the failure is that discovering clues to a specific authentic date to each letter proves internally impossible. Composition date of the novel still remains as a challenge requiring further study.

Determination of Age and Metallicity of Early-Type Galaxies hosting Type Ia Supernovae

  • Type Ia supernovae (SNe) are providing the most conclusive evidence for accelerating universe with dark energy in observational cosmology. In these investigations, look-back time evolution of SNe luminosity is regarded as negligible on the basic assumption. However, several recent works present some systematic differences among hosts which have different characteristics of stellar population. For more direct investigation, we are proceeding with our YONSEI (YOnsei Nearby Supernovae Evolution Investigation) project. Only early-type hosts in our catalogue were chosen in order to estimate the luminosity-weighted mean age and metallicity directly using Single Stellar Population (SSP) models and ignore the effect from the dust extinction. Observations using low-resolution spectrographs are still in progress at Las Campanas Observatory with 2.5m telescope and at McDonald Observatory with 2.7m telescope. We have thus far obtained spectra for 30 early-type hosts. After weak emission line correction, Lick/IDS absorption-line indices are measured and YEPS spectroscopic evolution model was applied to determine mean population ages and metallicities. Our preliminary results show that SNe Ia hosted in older galaxies seem to be brighter at 1.4 - 3 sigma levels, however, more observations and analyses are still needed to confirm this correlation.

경남 양산 및 밀양지역 납석광상의 지구화학적 연구 (Geochemistry of Pyrophyllite Deposits in Yangsan-Milyang Areas in Korea)

  • Mineralogy and geochemistry of five pyrophyllite deposits in Yangsan-Milyang area such as Cheonbulsan, Dumyong, Dongrae, Youkwang, and Sungjin mines were investigated. Pyrophyllite ores consist mainly of pyrophyllite, sericite, and quartz with some amounts of kaolinite and pyrite. Polytype of pyrophyllite is 2M. Sericite has two polytypes of 1M and 2M1. The ${\delta}^{18}O$ values of pyrophyllite from the Cheonbulsan and the Dumyong mines were measured as 0.23-0.60‰ and 3.40‰, respectively, and those of montmorillonite and kaolinite from the Dumyong mine were in the range of 11.90-12.06‰. This low oxygen isotope composition provides conclusive evidence for hydrothermal activity in the studied area. Contents of major elements are more useful than those of trace elements to discriminate altered zones such as pyrophyllite, sericite, argillic, and andalusite zones from the surrounding rocks. Particularly, contents of $K_2O$, $Na_2O$ and CaO are helpful to identify alteration zones from the discriminant and the cluster analysis of multi-element data.

Origin of Proteinuria as Observed from Qualitative and Quantitative Analysis of Serum and Urinary Proteins

  • It is well known that proteins present in the primary urine are reabsorbed in the renal proximal tubules, and that this reabsorption is mediated via the megalin-cubilin complex and the neonatal $Fc{\gamma}$ receptor. However, the reabsorption is also thought to be influenced by an electrostatic interaction between protein molecules and the microvilli of the renal proximal tubules. By analyzing the charge diversity of urinary IgG, we showed that this reabsorption process occurs in a cationic charge-preferential manner. The charge-selective molecular sieving function of the glomerular capillary walls has long been a target of research since Brenner et al. demonstrated the existence of this function by a differential clearance study by using the anionic dextran sulfate polymer. However, conclusive evidence was not obtained when the study was performed using differential clearance of serum proteins. We noted that immunoglobulin (Ig) A and IgG have similar molecular sizes but distinct molecular isoelectric points. Therefore, we studied the differential clearance of these serum proteins (clearance IgA/clearance IgG) in podocyte diseases and glomerulonephritis. In addition, we studied this differential clearance in patients with Dent disease rather than in normal subjects because the glomerular sieving function is considered to be normal in subjects with Dent disease. Our results clearly showed that the charge-selective barrier is operational in Dent disease, impaired in podocyte disease, and lacking in glomerulonephritis.

족관절 급성 염좌의 진단과 치료 (Diagnosis and Treatment of Acute Ankle Sprain)

  • Acute sprain of the ankle requires comprehensive history taking and physical examination in diagnosing the type of severity and deciding on the plan of treatment. Literature supports functional treatment as the treatment of choice for grade I and II injuries. During the acute phase, the goal of treatment focuses on controlling pain and swelling. PRICE (protection, rest, ice, compression, and elevation) is a well-established protocol at this phase. There is some evidence that application of ice and use of nonsteroidal anti-inflammatory drugs improves healing and speeds recovery. Then the functional treatment (motion restoration and strengthening exercises) is administered to progress the rehabilitation appropriately in order to facilitate healing and restore the mechanical strength and proprioception. Early mobilization has been shown to result in more rapid return to work and daily activities than immobilization. Grade III injuries still generate controversy in terms of the best management available, and more studies on early mobilization, cast immobilization, or surgery are needed. Even the Cochrane reviews published to date are not conclusive.

A Conclusive Review on Amyloid Beta Peptide Induced Cerebrovascular Degeneration and the Mechanism in Mitochondria

  • Promising evidence suggests that amyloid beta peptide ($A{\beta}$), a key mediator in age-dependent neuronal and cerebrovascular degeneration, activates death signalling processes leading to neuronal as well as non-neuronal cell death in the central nervous system. A major cellular event in $A{\beta}$-induced apoptosis of non-neuronal cells, including cerebral endothelial cells, astrocytes and oligodendrocytes, is mitochondrial dysfunction. The apoptosis signalling cascade upstream of mitochondria entails $A{\beta}$ activation of neutral sphingomyelinase, resulting in the release of ceramide from membrane sphingomyelin. Ceramide then activates protein phosphatase 2A (PP2A), a member in the ceramide-activated protein phosphatase (CAPP) family. PP2A dephosphorylation of Akt and FKHRL1 plays a pivotal role in $A{\beta}$-induced Bad translocation to mitochondria and transactivation of Bim. Bad and Bim are pro-apoptotic proteins that cause mitochondrial dysfunction characterized by excessive ROS formation, mitochondrial DNA (mtDNA) damage, and release of mitochondrial apoptotic proteins including cytochrome c, apoptosis inducing factor (AIF), endonuclease G and Smac. The cellular events activated by $A{\beta}$ to induce death of non-neuronal cells are complex. Understanding these apoptosis signalling processes will aid in the development of more effective strategies to slow down age-dependent cerebrovascular degeneration caused by progressive cerebrovascular $A{\beta}$ deposition.

An Experimental Study of Accelerating Phase Change Heat Transfer

  • The present paper investigated the effect of ultrasonic vibrations on the melting process of a phase-change material (PCM). Furthermore, the present study considered constant heat flux boundary conditions unlike many of the previous researches adopted constant wall temperature conditions. Therefore, in the present study, modified dimensionless parameters such as Ste* and Ra* were used. Also, general relationships between melting with ultrasonic vibrations and melting without ultrasonic vibrations were established during the melting of PCM. Experimental observations show that the effect of ultrasonic vibrations on heat transfer is very important throughout the melting process. The results of the present study reveal that ultrasonic vibrations accompany the effects like agitation, acoustic streaming, cavitation, and oscillating fluid motion. Such effects are a prime mechanism in the overall melting process when ultrasonic vibrations are applied. They enhance the melting process as much as 2.5 tildes, compared with the result of natural melting. Also, energy can be saved by applying ultrasonic vibrations to the natural melting. In addition, various time-wise dimensionless numbers provide conclusive evidence of the important role of ultrasonic vibrations on the melting phenomena.
