CT performance phantom을 이용한 cone beam형 전산화단층영상의 특성 (Image characteristics of cone beam computed tomography using a CT performance phantom)

  • Purpose: To evaluate the characteristics of (widely used) cone beam computed tomography (CBCT) images. Materials and Methods: Images were obtained with CT performance phantoms (The American Association of Physicists in Medicine; AAPM). CT phantom as the destination by using PSR $9000N^{TM}$ dental CT system (Asahi Roentgen Ind. Co., Ltd., Japan) and i-CAT CBCT (Imaging Science International Inc., USA) that have different kinds of detectors and field of view, and compared these images with the CT number for linear attenuation, contrast resolution, and spatial resolution. Results: CT number of both PSR $9000N^{TM}$ dental CT system and i-CAT CBCT did not conform to the base value of CT performance phantom. The contrast of i-CAT CBCT is higher than that of PSR $9000N^{TM}$ dental CT system. Both contrasts were increased according to thickness of cross section. Spatial resolution and shapes of reappearance was possible up to 0.6 mm in PSR $9000N^{TM}$ dental CT system and up to 1.0 mm in i-CAT CBCT. Low contrast resolution in region of low contrast sensitivity revealed low level at PSR $9000N^{TM}$ dental CT system and i-CAT CBCT. Conclusion: CBCT images revealed higher spatial resolution, however, contrast resolution in region of low contrast sensitivity was the inferiority of image characteristics.

Isthmuses, accessory canals, and the direction of root curvature in permanent mandibular first molars: an in vivo computed tomography study

  • Objectives: This study was performed to assess the anatomy of mandibular first molars. Materials and Methods: In this in vivo study, cone-beam computed tomography (CBCT) volumes of 312 bilateral intact first mandibular molars from 156 patients (79 men and 77 women; average age, 35.6 ± 11.2 years) were investigated in terms of the direction of each canal's curvature in the buccolingual and mesiodistal dimensions (direction of the position of the apex in relation to the longitudinal axis of the root), the presence of an isthmus (a narrow, ribbon-shaped communication between 2 root canals) in 3 segments (0-2, 2-4, and 4-6 mm) from the apex), and the presence and number of accessory canals (smaller canals besides the main root canals, connecting the pulp to the periodontium). Data were analyzed statistically (α = 0.05). Results: Mesiolingual canals were mostly buccally and distally inclined, while mesiobuccal and distolingual canals were mostly distally curved. Isthmuses were more common in younger patients (χ2 test, p < 0.05). The average numbers of accessory canals in the apical, middle, and coronal segments were 9.9 ± 4.2, 6.9 ± 2.9, and 9.3 ± 3.0 canals per segment, respectively (analysis of variance, p < 0.001). Age and sex were not associated with the number of accessory canals (p > 0.05). Conclusions: The complex anatomy of these teeth deserves attention during non-surgical or surgical endodontic treatment. Around the apex, isthmuses might be more prevalent in younger and female individuals.

미만성 축삭 손상에서 전산화단층촬영과 경사에코 자기공명영상을 이용한 예후의 평가 (Prognostic Value of Computed Tomography and Gradient-echo Magnetic Resonance Imaging in Diffuse Axonal Injury)

  • Purpose: Diffuse axonal injury (DAI) is clinically defined as a coma of over six hours in a head trauma victim without a focal mass lesion. The emergency physician usually resuscitates and stabilizes a comatose head trauma victim in the emergency Department. After assessment and treatment, the prognosis is very important to both the victim and the physician. The prognosis for DAI is based on Glasgow Coma Scale (GCS) and other imaging data. We investigated the prognostic value of computed tomography (CT) and gradient-echo magnetic resonance imaging (GRI) for head trauma victims with DAI. Methods: Fifty-three(53) head trauma victims of DAI were enrolled in this study from 2007 to 2012. During the study period of six years, data on trauma victims were collected retrospectively. We analyzed the differences in the Glasgow Outcome Scale (GOS) result between the CT and the GRI modalities. Results: We classified the study group by using GOS. Between the good outcome subgroup (GOS scores of 4 and 5) and the poor outcome subgroup (GOS score of 1-3), there were no statistical difference in sex, age, initial vital signs and initial GCS score. The good outcome subgroup had non-hemorrhage on CT(52%), which was correlated with good outcome and a shorter awakening time, while a larger number and a deeper location of hemorrhagic lesions on in GRI were correlated with poor outcome in DAI. Conclusion: We conclude that the existence of hemorrhagic lesions on CT, and the number and location of those lesions on GRI had good prognostic value for head trauma victims with DAI.

MDCT 관상동맥 조영 검사에서 전향적 동조화 및 후향적 동조화 기법의 화질과 선량 관계 (An Estimate of Image Quality and Radiation Doses of Coronary Artery in MDCT Using Prospective and Retrospective ECG Gating Scan Mode)

  • 본 연구는 검사 기법에 따른 영상의 질과 방사선 피폭의 정도를 제시하여 검사자가 받는 방사선 피폭을 저감시키는 데 목적이 있다. 관상동맥 전산화 단층 혈관조영(Coronary CTA, coronary computed tomography angiography) 검사자를 대상으로 SnapShot Pulse의 전향적 동조화 기법과 SnapShot Segment의 후향적 동조화 기법으로 coronary CTA 검사에서 검사자가 받는 단면 피폭선량(CTDIvol, volume computed tomography dose index), 총 피폭선량(dose-length product; DLP)을 각각 측정하였다. 또한, Coronary CTA 촬영조건을 동일하게 팬텀을 이용하여 CT감약계수, 노이즈 및 균일도, 공간분해능을 측정하였다. 연구 결과 두기법에서 CT감약계수, 노이즈 및 균일도, 공간분해능의 질적 수준이 비슷하게 나타났으며, CTDIvol, DLP는 SnapShot Segment의 후향적 동조화 기법에 비해 피폭선량이 약 37.5%, 40.3% 감소되었다. 임상에서 coronary CTA 검사 시 SnapShot Pulse의 전향적 동조화 기법을 적극 채택하여 검사자가 받는 방사선 피폭을 감소시키기 위한 노력이 절실히 요구된다.

Do Blebs or Bullae on High-Resolution Computed Tomography Predict Ipsilateral Recurrence in Young Patients at the First Episode of Primary Spontaneous Pneumothorax?

  • Background: The relationship between the size of bullae and pneumothorax recurrence is controversial. The aim of this study was to retrospectively evaluate the role of blebs or bullae in predicting ipsilateral recurrence in young patients experiencing their first episode of primary spontaneous pneumothorax (PSP) who underwent conservative treatment. Methods: A total of 299 cases of first-episode PSP were analyzed. The status of blebs or bullae was reviewed on high-resolution computed tomography (HRCT). The dystrophic severity score (DSS; range, 0 to 6 points) was calculated based on HRCT. Results: The 5-year recurrence rate was 38.2%. In univariate analysis, age (<20 years), body mass index (<$20kg/m^2$), a unilateral lesion, and intermediate risk (DSS 4 and 5) were associated with recurrence. Sex; smoking history; and the presence, number, and maximal size of blebs or bullae were not related to recurrence. In Cox regression, age and intermediate risk were independent risk factors for recurrence. High risk (DDS 6) was not an independent risk factor. Conclusion: The presence, number, and size of blebs or bullae did not affect ipsilateral recurrence. DSS failed to show a positive correlation between severity and recurrence. The decision to perform surgery in patients experiencing their first episode of PSP should not be determined by the severity of blebs and bullae.

  • A new method for electric resistivity tomography(ERT) is developed for geophysical inverse problems by adapting the sensitivity analysis. The outputs of the potential electrodes are computed using two dimensional finite element method in the wave number space by Fourier transforming the governing equations. The resistance distribution in the region of interests, which makes the computed potential distribution coincide with the measured potential, is found by minimizing the objective function using an optimization method. In this process the sensitivity analysis is introduced in order to compute the derivatives of the objective function. And an adjoint variable method is used to save the computational efforts for sensitivity coefficients.

Cleidocranial dysplasia: a case report

  • Cleidocranial dysplasia is a rare, autosomal dominant congenital disorder. A 12-year old female visited with chief complaint of unerupted permanent teeth. Also her father showed severe class III malocclusion. The extraoral radiography and computed tomography showed delayed closure of the cranial sutures and underdevelopment of maxilla, maxillary sinuses, and frontal sinus. Both clavicles were underdeveloped and thoracic rib cage was bell­shaped. Both zygomatic process appeared as hypoplastic feature. There were many unerupted permanent and supernumerary teeth in the maxilla and mandible. We examined location and number of the unerupted teeth using 3D CT. Finally we could conclude this case was cleidocranial dysplasia based on the clinico-radiologic findings.

Assessment of Right Ventricular Function in Pulmonary Hypertension with Multimodality Imaging

  • Pulmonary hypertension (PH) is defined as resting mean pulmonary artery pressure ${\geq}25mmHg$ and is caused by multiple etiologies including heart, lung or other systemic diseases. Evaluation of right ventricular (RV) function in PH is very important to plan treatment and determine prognosis. However, quantification of volume and function of the RV remains difficult due to complicated RV geometry. A number of imaging tools has been utilized to diagnose PH and assess RV function. Each imaging technique including conventional echocardiography, three-dimensional echocardiography, strain echocardiography, computed tomography and cardiac magnetic resonance imaging has-advantages and limitations and can provide unique information. In this article, we provide a comprehensive review of the utility, advantages and shortcomings of the multimodality imaging used to evaluate patients with PH.

Pancreatic Paraganglioma: a Case Report and Literature Review

  • Paraganglioma is a rare tumor of paraganglia, derived from neural crest cells in sympathetic or parasympathetic ganglions. It can be widely distributed from the skull base to the bottom of the pelvis. The pancreas, however, is a rare location of this neoplasm, and only a limited number of cases have been reported in the English literature, especially with gadoxetic-acid-enhanced magnetic resonance imaging (MRI) and diffusion-weighted images (DWI). We herein report a case of pathologically proven paraganglioma in the pancreas head with a literature review on endoscopic ultrasonography (EUS), computed tomography (CT), gadoxetic-acid-enhanced MRI, and DWI sequence.

Three-dimensional microstructure of human alveolar trabecular bone: a micro-computed tomography study

  • Purpose: The microstructural characteristics of trabecular bone were identified using micro-computed tomography (micro-CT), in order to develop a potential strategy for implant surface improvement to facilitate osseointegration. Methods: Alveolar bone specimens from the cadavers of 30 humans were scanned by high-resolution micro-CT and reconstructed. Volumes of interest chosen within the jaw were classified according to Hounsfield units into 4 bone quality categories. Several structural parameters were measured and statistically analyzed. Results: Alveolar bone specimens with D1 bone quality had significantly higher values for all structural parameters than the other bone quality categories, except for trabecular thickness (Tb.Th). The percentage of bone volume, trabecular separation (Tb.Sp), and trabecular number (Tb.N) varied significantly among bone quality categories. Tb.Sp varied markedly across the bone quality categories (D1: $0.59{\pm}0.22mm$, D4: $1.20{\pm}0.48mm$), whereas Tb.Th had similar values (D1: $0.30{\pm}0.08mm$, D4: $0.22{\pm}0.05mm$). Conclusions: Bone quality depended on Tb.Sp and number-that is, endosteal space architecture-rather than bone surface and Tb.Th. Regardless of bone quality, Tb.Th showed little variation. These factors should be taken into account when developing individualized implant surface topographies.
