영상의학과 전산화단층촬영 검사 대기일 단축을 위한 6-시그마 적용사례 연구 (A Case Study on the Six Sigma Application to Reduce Waiting Day for Computed Tomography in the Radiology Department)

  • The purpose of the study was to apply and to expand the six sigma to reduce waiting times for computed tomography (CT) examination which manipulated by the department of radiology. It was preceded by DMAIC (Define, Measure, Analyze, Improve, and Control). In the stage of definition, it wereselected for total 5 critical to quality (CTQ), which were the kindness, the waiting time, the examination explanation, the waiting day and the waiting stand environment, that increased the reserved time of CT examination. In the stage of measurement, the number of examinations and of reservation waiting days performed and resulted in final CTQ(Y) which measured each 1.68 and 1.85 sigma. In the stage of analysis, the examination concentrated on morning time, non-scheduled examination of the day, the delayed time of booking, frequent telephone contacting and equipment malfunction were determined as variable key causes. In the stage of improvement, it were performed with expansion of the examination in the morning time, integration of laboratories that used to in each steps, developing the ability of simultaneous booking schedule for the multiple examinations, developing program of examination request, and the customer management team operations. For the control, the number of examinations and reserved waiting days were measured each 3.14 and 1.13 sigma.

  • 컴퓨터단층촬영장치(computed tomography, CT)을 이용한 검사는 영상의학과에서 인체 내 정보를 파악하기 위한 가장 유용한 진단장비 중 하나로 신뢰도가 매우 높다. CT검사에 대한 신뢰성이 높아 최근에는 CT를 이용한 검사 건수도 매년 증가하고 있다. 검사 건수 증가는 CT장치의 노화를 촉진하게 되고, 이로 인해 CT장치에 대한 품질관리(quality management, QM)의 중요성이 대두된다. 특히, CT검사에서 품질관리란 임상영상에서 영상의 질(image quality, IQ) 저하로 진단영역 축소를 초래할 수 있는 발생가능한 모든 문제점을 사전에 파악하여 교정함으로써 항상 일정 수준의 영상의 질을 유지하고 영상을 획득할 수 있게 하는 행위를 의미한다. 이에 본 연구진은 CT검사의 품질관리에 대한 일반적인 내용을 요약하여 보고 한다.

Maxillary antroliths detected by cone-beam computed tomography in an adult dental population

  • Purpose: This study evaluated the prevalence and characteristics of maxillary antroliths using cone-beam computed tomography (CBCT) scans performed for maxillofacial diagnostic purposes. Materials and Methods: CBCT scans of 13,946 patients over the age of 20 were reviewed for maxillary antroliths, and prevalence according to sex, age, and the side of the jaw was calculated. The relationships of single or multiple antroliths with sex, side, and the degree of sinus inflammation were evaluated. The shape and dimension of antroliths were also assessed. The data were analyzed using descriptive statistics, the chi-square or Fisher exact tests, and Kendall's tau-b. Results: A total of 138 (0.99%) of the 13,946 patients showed an antrolith in at least 1 sinus. Only 18 patients presented a bilateral manifestation, which brought the total number of sinuses containing an antrolith to 156 (0.56%). Multiple antroliths were observed in 36 sinuses, and the total number of antroliths was 207: 110 punctate, 65 linear, and 32 amorphous. The antrolith dimensions varied from $1mm^2$ to $91mm^2$ (average, $10.2{\pm}15.5mm^2$). No statistically significant differences were found according to sex, side, and age group (P>0.05). However, there was a statistically significant difference between the multiplicity of antrolith and the degree of sinus inflammation (P<0.05). Conclusion: Cone-beam computed tomography is an effective modality for the detection of incidental antroliths. Maxillary antroliths were found to be very rare and were usually asymptomatic. Dentists should have a comprehensive understanding of their diagnosis and treatment in light of possible associated dental problems.

Cone beam형 전산화 단층촬영장치를 이용한 설공의 해부학적 구조 (Anatomical structure of lingual foramen in cone beam computed tomography)

  • Purpose: To evaluate whether cone beam computed tomography can depict the distribution, position, frequency, relative vertical dimension, and the diameter of the lingual foramen and direction of lingual bone canal. Materials and Methods : Cone beam computed tomography of mandible was performed on 25 males and 25 females with no history of any orthodontic treatments or any other dental surgeries. A statistical comparison was done on the mean values of males and females. Results: In the location and distribution of lingual foramina, median lingual foramen was found in all subjects and lateral lingual foramen in 58%. In the lateral lingual foramen, bilateral type was found in 28% and unilateral type in 30%. In the number of lingual foramina, median lingual foramen had two foramina and lateral lingual foramen had one foramen, mostly. In the relative mean vertical dimension of lingual foramina, median lingual foramen was 0.03±0.08, and both lateral lingual foramina was 0.20±0.04. The mean diameter of lingual foramina, median lingual foramen was 0.9mm±0.28, right lateral lingual foramen was 0.92mm±0.23, and left lateral lingual foramen was 0.88mm±0.27. The most frequent direction of the lingual bone canals, median lingual bone canal proceeded in anteroinferior direction and lateral lingual bone canal in anterosuperolateral direction. Conclusion : Cone beam computed tomography can be helpful for surgery and implantation on the mandibular area. Radiologist should be aware of this anatomical feature and its possible implications.

  • 1. CT에 의한 매복치의 위치결정은 적절한 치료계획의 수립을 가능하게 하여 시술시간의 단축과 인접치, 인접구조물의 손상을 줄여 시술후의 부작용을 줄일 수 있었다. 2. 매복된 영구치의 견인시 CR를 활용한 결과 개창부위의 정확한 위치결정과 예후에 대한 예측이 가능하였다. 3. 평면방사선사진의 판독후 악골내 병소가 의심되어 CT를 활용한 결과 병소의 확진이 가능하였다. 4. 소아의 CT촬영은 평면 방사선 촬영에 비해 증가된 방사선 피폭량, 높은 수가 때문에 적절한 증례의 선택이 중요하다.

전산화단층사진을 이용한 타액선의 정량분석에 관한 연구 (Studies on Quantitative Analysis of Salivary Gland using Computed Tomography)

  • Purpose: The purpose of this study was to calculate the size and CT number of both normal parotid and submandibular gland. and evaluate their relation to sex, age and obesity using computed tomography. Materials and Methods: The computed tomography was performed parallel to the Frankfurt plane in 46 subjects with healthy salivary gland. The subjects were divided into the three groups (young, middle. old) according to their ages. The size of salivary gland was determined as maximum cross-sectional area and the CT number of salivary gland was determined as the mean CT number of three ROI's. The body mass index was calculated from weight and height. Results: The mean maximum cross-sectional area was 7.79(±1.25)cm² on parotid gland and 4.12(±0.83) cm² on submandibular gland. The mean CT number was -4.43(±23.87) HU on parotid gland and 50.01(±15.63) HU on submandibular gland. There was decreasing pattern of the maximum cross-sectional area of submandibular gland and the CT number of both parotid and submandibular gland according to age(p<0.05). As the body mass index increased. the maximum cross-sectional area of parotid gland increased and CT number of both parotid and submandibular gland decreased(p<0.05). The maximum cross-sectional area of submandibular gland in male was larger than that in female(p<0.05). As the maximum cross-sectional area and CT number of left salivary gland increased. those of right gland increased(p<0.05). Conclusion : Intra-individual differences in salivary gland size and CT number is considered in the age and individual obesity.

요오드 조영제를 이용한 전산화단층촬영장치의 자가진단 직선성 정도관리 기술 개발 (Development of Self-Diagnosis Linearity Quality Assurance Technique in Computed Tomography by Using Iodic Contrast Media)

  • 본 연구는 요오드 함량이 다른 조영제를 증류수로 희석한 비율과 관전압의 조건별로 CT number를 측정하여 전산화단층촬영장치의 자가진단 직선성 정도관리 기술을 개발하고자 하였다. 사용된 장비는 4-채널 MDCT를 이용하였고, 요오드 조영제는 300 mgI/ml, 350 mgI/ml, 370 mgI/ml, 400 mgI/ml를 사용했다. 증류수에 대한 조영제 희석비율은 최대 농도값이 측정될 때까지 5%씩 증가하였다. 관전압은 90 kVp, 120 kVp, 140 kVp를 적용하였다. 그 결과, 90 kVp에서 400 mgI/ml 조영제가 5% ~ 25%에서 희석된 비율 구간과 CT number들 간의 상관계수가 0.993으로 가장 근사한 직선성을 구하였다. 본 연구에서 제시한 요오드 조영제를 이용한 자가진단 직선성 평가는 AAPM CT 성능평가용 팬톰을 대체할 수 있는 정도관리 방법으로 이용할 수 있다.

X-ray Computed Tomography on Larger Diameter Timber than Digital Detector

  • X-ray computed tomography is a very powerful nondestructive technique in safety inspection of historic timber building. But, in field, various testing condition makes it difficult to carry out X-ray CT testing. Limited size in X-ray digital detector is one of the problems. In this study, a pitch pine disk with two holes was used to know how imperfection in X-ray projection affects CT image resolution. Using various number of projections, CT image was reconstructed by filtered back projection method, and then it was investigated how many projection is required to identify the holes in different location. Two artificial holes could be differently detected according to their location in cross section of specimen. One hole in center part of specimen was identified using more than 9 radiographs, but the other one which located in outer part of cross section could not be detected until more than 36 projections were used. Even though there is data missing in outer part of cross section due to limited size of detector, the center part of CT image could be reconstructed well and the resolution of outer part became higher with increase of the number of projections. For field application, the number of projections for CT image reconstruction needs to be decided with consideration of another nondestructive testing and the location of interest.

비수도권(非首都圈) 지역(地域)에서의 전신용(全身用) X선(線) CT의 이용(利用) (Utilization of Body Computed Tomography Scanners in Non-Metropolitan Area)

  • Computed Tomography Scanner (CT) is highly expensive in operation as well as purchasing. That reason may cause not only to increase the burden of patients but also to waste the capital resources leading to financial difficulties. However the numbers of CT installed throughout the country is increasing, because of efficiency in medical care, patient's concern, competitions among the hospitals within the same area. In the non-Metropolitan area the scanners were expected to be less utilized and less profitable. Nine hospitals equipped with the CT were studied on the utilization of that equipment during the period from November 1984 to February 1985 in non-Metropolitan area and break-even point in one hospital was analyzed for estimating profitabilities. The results were as follows ; 1. Among those nine hospitals, four hospitals had less than 400 beds, which is one of the restrictive minimum standards on the installation of Whole-body Computed Tomography Scanner. 2. The operating time during the normal operation period was longer than those of any other studies, but the accumulated down time was also longer than those of any other studies. The average number of scanning per week for each CT was 45, while the estimated number of for the break-even point was 56.7 cases. 3. When the downtime was excluded in calculating the average operation would be much closer to the cases for the break-even point. Therefore the break-down of the equipment was to be a main cause of the low profitability. 4. The average scanning rate for head area was 33.6%, however three of the nine hospitals showed about 20%. 5. If scanning ratio for the body parts excepting head was increased, the number of scanning for the break-even point would be diminished. 6. The small size hospital especially located near the Metropolitan area showed largest loss in the CT operation. In purchasing the highly expensive equipments in hospitals, demand should be taken into account and planning is recommended.

Feasibility of Computed Tomography Colonography as a Diagnostic Procedure in Colon Cancer Screening in India

  • Computed Tomography Colonography (CTC) is a medical imaging technology used in identifying polyps and colon cancer masses in the large intestine. The technique has evolved a great deal since its invention and has become a routine diagnostic procedure in Western countries due to its non invasiveness and ease of use. The objective of our study was to explore the possibility of CTC application in Indian hospitals. This paper gives an overview of the procedure and its commercial viability. The explanation begins with the domain aspects from gastroenterologist perspective, the new way of thinking in polyp classification, the technical components of CTC procedure, and how engineering solutions have helped clinicians in solving the complexities involved in colon diagnosis. The colon cancer statistics in India and the results of single institution study we carried out with retrospective data is explained. By considering the increasing number of patients developing colon malignancies, the practicality of CTC in Indian hospitals is discussed. This paper does not reveal any technical aspects (algorithms) of engineering solutions implemented in CTC.
