Costs involved in compliance with new endoscope reprocessing guidelines

  • Background/Aims: In March 2022, the Association for the Advancement of Medical Instrumentation (AAMI) released the American National Standards Institute (ANSI)/AAMI ST91:2021, their latest update on comprehensive, flexible, and semirigid endoscope reprocessing. These updated standards recommend the sterilization of high-risk endoscopes when possible and provide new recommendations for the precleaning, leak testing, manual cleaning, visual inspection, automated reprocessing, drying, storage, and transport of endoscopes. Methods: ANSI/AAMI ST91:2021 was compared with ANSI/AAMI ST91:2015 for major reprocessing differences that result in either time and/or cost increases. Time estimates were captured by explicit recommendation inclusion or taken from the literature. All the costs were estimated using publicly available resources. Results: The updated standards represent a potential 24.3-minute and 52.35 to 67.57 United States dollars increase per procedure in terms of reprocessing time and spending, respectively, not including capital investments. Capital costs per procedure were highly dependent on the procedure volume of the facility. Conclusions: The new AAMI standards recommend several major changes, such as sterilization, for facilities to reprocess and manage endoscopes between uses. As more facilities increase their reprocessing methods to reflect the updated standards, they do so at a cost and introduce several delays. As the reprocessing landscape evolves, facilities should consider their true costs and alternative solutions, such as single-use endoscopes.

Benchmarking of US General Contractor's Pre-construction Services for a CM at Risk Project to Improve Contractor's Competitiveness (책임형 CM사 경쟁력 확보 및 선진화를 위한 미국 건설사의 시공이전단계 서비스 벤치마킹 연구)

  • Construction Management at Risk (CMAR) is a project delivery method that enables CM companies to deliver projects at a Guaranteed Maximum Price (GMP). General contractors can apply CMAR from the initial design phase right through the construction phase to reduce risks and improve project performance. One of the major advantages CMAR offers is that it permits a general contractor to provide a comprehensive suite of preconstruction services, including estimating, a constructability review, value engineering, drawings and a specification review, green building, and Building Information Modeling(BIM), among others. However, general contractors in South Korea currently provide only limited preconstruction services using CMAR because few CMAR projects have yet been implemented in Korea and their experience using the method is therefore limited. This benchmark study of how foreign general contractors utilize CMAR in their projects, particularly during the preconstruction process, its purpose, and the roles and responsibilities of each of the different participants in successful implementations thus provides invaluable information and will serve as a useful guide for Korean contractors seeking to incorporate CMAR preconstruction services in their projects and thus improve the competitiveness of their construction businesses.

A Study on the Selection and Modification of Ground Motion Based on Site Response Analysis (부지응답해석에 기반한 지반운동 선정 및 보정에 관한 고찰)

  • In the recent seismic design code KDS 41 17 00, selection and modification procedures of ground motions which are used for nonlinear dynamic analyses were adopted. However, its practical applications are still limited due to the lack of literatures. This paper introduces case studies which used site-response analyses to select and modify ground motions for nonlinear dynamic analyses. Based on the case studies, design criterion for site-response analyses were reviewed thoroughly in the viewpoint of practical applications. It was found that design requirements related with bedrock motions are too conservative that ground motions are selected and modified in the excessive manner. It is especially true for low-rise building structures with period ranges including acceleration-sensitive regions. Even though surface motions have shown appropriate responses, such building structures have to re-select and re-modify ground motions based on pre-analysis procedures rather than post-ones according to the current seismic design code. Also, it was observed that building structures with soft soils under strong ground motions need more comprehensive investigations on soil properties and efficient analysis methods in order to perform site-response analyses. This is due to the fact that lack of reliabilities on soil properties and analysis methods could result in unstable site-responses.

Crafting a Quality Performance Evaluation Model Leveraging Unstructured Data (비정형데이터를 활용한 건축현장 품질성과 평가 모델 개발)

  • The frequent occurrence of structural failures at building construction sites in Korea has underscored the critical role of rigorous oversight in the inspection and management of construction projects. As mandated by prevailing regulations and standards, onsite supervision by designated supervisors encompasses thorough documentation of construction quality, material standards, and the history of any reconstructions, among other factors. These reports, predominantly consisting of unstructured data, constitute approximately 80% of the data amassed at construction sites and serve as a comprehensive repository of quality-related information. This research introduces the SL-QPA model, which employs text mining techniques to preprocess supervision reports and establish a sentiment dictionary, thereby enabling the quantification of quality performance. The study's findings, demonstrating a statistically significant Pearson correlation between the quality performance scores derived from the SL-QPA model and various legally defined indicators, were substantiated through a one-way analysis of variance of the correlation coefficients. The SL-QPA model, as developed in this study, offers a supplementary approach to evaluating the quality performance of building construction projects. It holds the promise of enhancing quality inspection and management practices by harnessing the wealth of unstructured data generated throughout the lifecycle of construction projects.

A Development of Representative Condition Evaluation Standard for LNG Storage Tank Structures (LNG 저장탱크 구조물의 종합적 상태평가기준 개발)

  • As the LNG storage tank is aged, if there is a crack in the outer wall concrete or corrosion of the reinforcing steel, there is a risk of a major accident such as collapse of the structure depending on the type and degree of damage. Since 2014, LNG storage tanks have undergone precise safety diagnosis and safety inspection has been carried out. The condition evaluation criteria for each component have been revised and applied in January 2016. The condition evaluation standard is to evaluate the status of storage tanks based on the appearance survey and material test results of LNG storage tanks and it is important for maintenance. In addition, the representative condition evaluation standard that shows the comprehensive state of each LNG storage tank is important in maintenance, but the related standard for LNG storage tank outer concrete is not available in Korea and abroad, and development of the condition evaluation standard is necessary. In this paper, we examined the structural characteristics of LNG storage tanks, analyzed the status of the condition evaluation criteria for each member, and developed a comprehensive status rating system by weighting the members. We used the AHP(Analytic Hierarchy Process) technique and developed a representative conditon evaluation criteria through surveys of professional organizations.

Design of Comprehensive Security Vulnerability Analysis System through Efficient Inspection Method according to Necessity of Upgrading System Vulnerability (시스템 취약점 개선의 필요성에 따른 효율적인 점검 방법을 통한 종합 보안 취약성 분석 시스템 설계)

  • As the IT environment becomes more sophisticated, various threats and their associated serious risks are increasing. Threats such as DDoS attacks, malware, worms, and APT attacks can be a very serious risk to enterprises and must be efficiently managed in a timely manner. Therefore, the government has designated the important system as the main information communication infrastructure in consideration of the impact on the national security and the economic society according to the 'Information and Communication Infrastructure Protection Act', which, in particular, protects the main information communication infrastructure from cyber infringement. In addition, it conducts management supervision such as analysis and evaluation of vulnerability, establishment of protection measures, implementation of protection measures, and distribution of technology guides. Even now, security consulting is proceeding on the basis of 'Guidance for Evaluation of Technical Vulnerability Analysis of Major IT Infrastructure Facilities'. There are neglected inspection items in the applied items, and the vulnerability of APT attack, malicious code, and risk are present issues that are neglected. In order to eliminate the actual security risk, the security manager has arranged the inspection and ordered the special company. In other words, it is difficult to check against current hacking or vulnerability through current system vulnerability checking method. In this paper, we propose an efficient method for extracting diagnostic data regarding the necessity of upgrading system vulnerability check, a check item that does not reflect recent trends, a technical check case for latest intrusion technique, a related study on security threats and requirements. Based on this, we investigate the security vulnerability management system and vulnerability list of domestic and foreign countries, propose effective security vulnerability management system, and propose further study to improve overseas vulnerability diagnosis items so that they can be related to domestic vulnerability items.

Accuracy Analysis of Network RTK Surveying for Cadastral Re-survey Project (지적재조사사업에서 Network RTK 측량의 적용 정확도 분석)

  • The purpose of this research is to suggest the reasonable method of Network RTK surveying in future cadastral re-survey project through the accuracy analysis about Network RTK surveying achievement and the conventional TS-based confirmation surveying. To achieve it, we selected the experiment places and succeeded in achieving the result by Network RTK surveying about total of 307 parcel boundary point. We compared it with the result of confirmation surveying for cadastral, and it was shown that total connection errors of RMSE was ${\pm}0.1028m$ and total 48 places exceeded in the cadastral re-survey allowable error tolerance. The research suggested the practical alternatives in cadastral re-survey project after the comprehensive evaluation of those analysis results. Therefore, the author suggested development and adoptation of integrated electronic plane table surveying method. Moreover, we suggested unifying the first parcel boundary point method into the total station surveying and adopt the Network RTK surveying on the cadastral surveying inspection.

A study on the actual conditions of trade secrets in the MSDS and the improvement of the relevant system (사업장 MSDS 영업비밀 적용실태 및 제도 개선방안에 관한 연구)

  • Objectives: It has been pointed out that there are problems in handling and managing chemicals due to trade secrets of Material Safety Data Sheet(MSDS). To date, some company intentionally leave out of several parts and use an inaccurate expression in MSDS. In particular, with the result of the inspection of the government and labour community, it is required that the 2nd section in MSDS included the composition and information on ingredients has to be provided with comprehensive expression to secure more reliability. Methods: Therefore, this study is aimed: 1) to recognize the current status of trade secrets of MSDS in workplaces with both domestic manufacturers of chemical products and multinational corporations; 2) to make contributions to prevention of the industrial accidents by providing the accurate information of MSDS; 3) to improve the risk communication system related with chemicals; 4) to impress workers on the importance of right of known for MSDS. With the result we analyzed the status of trade secrets in MSDS in 73 companies, such as petrochemistry production, paint production, metal processing oil production, detergent production, and international company related with chemicals, we have found that 38,150 (45.5%) have the trade secrets parts in the total number of 83,832 in MSDS. Also, based on the 288 MSDS gathered by the Occupational Safety and Health Research Institute (OSHRI) from 2005 to 2009, 41.7% of the MSDS are classified into the trade secrets. Conclusions: Therefore, to procure an assurance system of MSDS, we suggest that a MSDS picking up and checking system be legislated in the Occupational Safety and Health Act to protect workers from the unidentified chemical hazards due to the secret trade of MSDS.

Suggestions on Efficient O&M Plan for Improving Users' Satisfaction on the University Dorm Facilities (대학 기숙사 시설의 사용자 만족도 향상을 위한 효율적인 유지보수관리(O&M) 방안 제안)

  • An university dorm has significant implications in terms of providing residential, living, and learning spaces. With its supportive function, a dorm enables each university to provide higher level of education. The operation & maintenance(O&M) condition of the dorm has a decisive effect on the students' satisfaction. Accordingly, high levels of O&M services should be performed for students. However, Korean dorms are being operated and maintained by their own O&M guidelines without the consideration of spatial characteristics of dorm facilities and the comprehensive and systematic understanding on effective O&M processes. Given the fact that dorm facility can be a crucial factor in determining the entire quality of university and its O&M condition is closely related to the satisfaction of students, it is imperative that we need to pay more attention to the O&M condition and services. Therefore, the main objective of this research is to improve dorm students' satisfaction levels by applying different O&M method, preventive maintenance rather than reactive maintenance which has been performed so far. In ordering for doing it, 'Facility Management(FM) Standard' from KS, 'Facility Performance Indicator(FPI)' from APPA: Leadership in educational facilities and 'Building O&M Inspection Manual' from Korean Ministry of Land, Infrastructure and Transport were analyzed to come up with 15 significant O&M factors. After extracting O&M factors, the survey was conducted to determine importance rate and performance rate of each O&M factor. Using the Important-Performance Analysis(IPA), the priority of 15 O&M factors was established. The result of this research will be helpful for the efficient dorm facility O&M services and for facility managers to appropriately allocate the limited resources and human power.

A Study on the Analysis of School Health Program by the Chronological Events in Korea (우리나라 학교보건사업(學校保健事業) 변천에 관(關)한 연구)

  • The purpose of this study was to identify and name clusters of school health program, and to describe some of the characteristics of administratie supports. The literature, materials and public documents were analysed by the chronological events from 1945 to 1989. The result of this study is as follows : 1. A brief summary of the history of school health program was included as an introduction to the analysis of the current programs of school health. Five current school health-program clusters were identified from findings of a study of programs : 1) physical assessment, laboratory examination and health services for the students, 2) health instruction 3) healthful living condition (environmental health), 4) health clinic management, 5) administrative supports. 2. The earliest school- based efforts focused on communicable disease pevention by the ministry of health and social affairs. Annual medical inspection (health assessment) for school children for eyes, ears, nose, and throat were mandated nation-wide in 1951 by physical assessment Act. 3. In 1979, the health instruction of schools to improve the health status of students was improved by health department in the Ministry of Education 4. Experiences in healthful environment were basic components of the school health program. However, without careful planning and supervision these experiences were not contributed to the goal of school health. The formal program of school health environment were initiated in 1979 5. In 1980, the guidelines of school health clinic management were prepared by Ministry of Education such as guidance of essential degrees and facilities in school health clinic. 6. Two patterns of administration of school health programs existed in Korea. In one the school health department operated its own health program and in the other the physical education department operated the health program within the school system. The school health department was established in Ministry of Education from 1979 to 1982. Improved school health programs will be a key element in the comprehensive national child health policy which I will ask the Ministry of Education to develop for the Department.
