Effect of Preexisting Musculoskeletal Diseases on the 1-Year Incidence of Fall-related Injuries

  • Objectives: People who have chronic diseases, as well as gait imbalance or psychiatric drug use, may be susceptible to injuries from falls and slips. The purpose of this study was to evaluate the effect of musculoskeletal diseases on incidental fall-related injuries among adults in Korea. Methods: We analyzed data from the 4th Korea National Health and Nutrition Examination Survey (2007-2009), which are national data obtained by a rolling survey sampling method. The 1-year incidence of fall-related injuries was defined by health service utilization within the last year due to injury occurring after a slip and fall, and musculoskeletal diseases included osteoarthritis, rheumatoid arthritis, osteoporosis, and back pain. To evaluate the effects of preexisting musculoskeletal diseases, adults diagnosed before the last year were considered the exposed group, and adults who had never been diagnosed were the unexposed group. Results: The weighted lifetime prevalence of musculoskeletal disease was 32 540 per 100 000 persons. Musculoskeletal diseases were associated with a higher risk of fall-related injury after adjustment for sex, age, residence, household income, education, occupation, visual disturbance, paralysis due to stroke, and medication for depression (odds ratio [OR], 1.41; 95% confidence interval [CI], 1.03 to 1.93). As the number of comorbid musculoskeletal diseases increased, the risk of fall-induced injuries increased (p-value for trend <0.001). In particular, patients who had any musculoskeletal condition were at much higher risk of recurrent fall-related injuries (OR, 6.20; 95% CI, 1.06 to 36.08). Conclusions: One must take into account the risk of fall-related injuries and provide prevention strategies among adults who have musculoskeletal diseases.

공황장애와 도파민 D2 수용체 TaqI A 다형성의 관련 (Association between Panic Disorder and Dopamine D2 Receptor TaqI A Polymorphism)

  • Puropse : Disturbances of dopaminergic system might be related to the possible mechanism of panic disorder. This study was aimed to examine the association of DRD2 Taq 1 polymorphism and panic disorder. Methods : One hundred and fourteen patients with panic disorder (62 male (54.4%), mean age $40.96{\pm}0.11$ years) and 200 comparison subjects (114 male (57.0%), mean age $35.57{\pm}8.81$ years)were tested for DRD2 TaqI A polymorphism. We excluded panic patients with comorbid alcohol related disorders, bipolar disorders, and any kinds of psychotic disorders because there have been some reports about association of these disease and DRD2 TaqI A polymorphism. Results : There was significant difference in the frequency of the genotype in DRD2 polymorphism between patients and controls (${\chi}^2$=6.09, df=2, p=0.048). The A1+ allele (A1A1 and A1A2) frequency analysis also showed significant association (${\chi}^2$=4.08, df=1, p=0.043). In addition, we observed a more strong and specific association between panic disorder and the A1+ allele of the DRD2 TaqI polymorphism for men (${\chi}^2$=4.71, df=1, p=0.03), but not for women (${\chi}^2$=0.45, df=1, p=0.50). Conclusion : These results in our Korean sample suggest that the DRD2 TaqI A polymorphism may be associated with panic disorder. Furthermore, we found sex-specific association of DRD2 A1 allele with panic disorder.

Prevalence of unrecognized depression in patients with chronic pain without a history of psychiatric diseases

  • Background: We aimed to investigate the prevalence of unrecognized depression in patients with chronic pain, but with no history of psychiatric diseases. Methods: Patients with chronic pain who did not have a history of psychiatric disease were selected for this study. The Beck Depression Index (BDI) was used to evaluate depression. Participants' socio-demographic characteristics and pain-related characteristics were also recorded. Results: The study included 94 consecutive patients with chronic pain (28 men and 66 women). Based on the BDI scores, 33/94 (35.1%) patients with chronic pain had comorbid depression. The prevalence of depression was significantly higher in our cohort than it was in the general population (P < 0.001). The standardized incidence ratio, adjusted for age and sex, was 2.77 in men and 2.60 in women. Patients who were unmarried (odds ratio [OR] = 3.714, P = 0.044), and who had subjective sleep disturbance (OR = 8.885, P < 0.001), were more likely to have moderate to severe depression. Patients with high education levels (OR = 0.244, P = 0.016), and who were economically active (OR = 0.284, P = 0.023), were less likely to have moderate to severe depression. Conclusions: Our results indicate that unrecognized depression in patients with chronic pain is common. Therefore, pain physicians should actively seek to identify these problems rather than relying on the patient to volunteer such information.

  • Method : Chronic motor tic or vocal tic disorders are characterized which have one of the two kinds of tics for more than a year. It is characterized chronic motor tic or vocal tic disorders that have one of the two kinds of tics for more than a year. But Tourette's disorder is a disease which has one or more motor tics and vocal tics for more than a year. Contemporary empirical studies have suggested the rate of comorbid attention-deficit hyperactivity disorder(ADHD) or obsessive compulsive disorder(OCD) in children with tic disorders be high. Objects : As society is confusing, tic disorders are increasing. Therefore, determining the treatment strategy in children with tic disorders is very important. So we studied the literatures of traditional chinese medicine about tic disorders and reported the results. Results : The principals for medical treatment were So-gan(疏肝), I-gi(理氣), Sik-pung(熄風), Gin-kyung(鎭痙), Chung-oel(淸熱) etc. Medicines treatment is used more than acupuncture treatment. The herbs are used Uncariae ramulus et Uncus(釣鉤藤), Paeoniae Radix Alba(白芍藥), Poria(白茯岺), Glycyrrhizae Radix(甘草), Bupleuri Radi.x(柴胡), Fossilia ossis Mastodi(龍骨) etc. in the order named. Acupuncture points were used Naegwan(PC6), Shinmuin(HT7), P'ungnyur4(ST40), Paek'oe. (GV20), Chung-wan(CV12), Samumgye(SP6), Kongson(SP4), T'aech'ung(LR3) and so on.

Severe periodontitis with tooth loss as a modifiable risk factor for the development of Alzheimer, vascular, and mixed dementia: National Health Insurance Service-National Health Screening Retrospective Cohort 2002-2015

  • Purpose: The purpose of this study was to evaluate severe periodontitis with tooth loss as a modifiable risk factor for Alzheimer dementia (AD), vascular dementia (VaD), and mixed dementia (MD) using the National Health Insurance Service-National Health Screening Retrospective Cohort database with long-term follow-up over 14 years. Methods: Multivariate Cox hazards regression analysis was applied to a longitudinal retrospective database, which was updated in 2018, to evaluate the association between severe periodontitis with few remaining teeth and dementia after adjusting for potential risk factors, including sociodemographic factors and comorbid diseases. Results: Among 514,866 individuals in South Korea, 237,940 (46.2%) participants satisfying the inclusion criteria were selected. A total of 10,115 age- and sex-matched participants with severe periodontitis and 10,115 periodontally healthy participants were randomly selected and evenly assigned. The results showed that the risks of AD (hazard ratio [HR], 1.08), VaD (HR, 1.24), and MD (HR, 1.16) were significantly higher in patients with severe periodontitis with 1-9 remaining teeth after adjustment for sociodemographic factors, anthropomorphic measurements, lifestyle factors, and comorbidities. Conclusions: Severe periodontitis with few remaining teeth (1-9) may be considered a modifiable risk factor for the development of AD, VaD, and MD in Korean adults.

Associations between Hashimoto Thyroiditis and Clinical Outcomes of Papillary Thyroid Cancer: A Meta-Analysis of Observational Studies

  • Background: Epidemiological studies have suggested an association between Hashimoto thyroiditis (HT) and papillary thyroid cancer (PTC) development. Other studies, however, have reported a protective role of HT against PTC progression. Through this updated meta-analysis, we aimed to clarify the effects of HT on the progression of PTC. Methods: We searched citation databases, including PubMed and Embase, for relevant studies from inception to September 2017. From these studies, we calculated the pooled odds ratios (ORs) of clinicopathologic features and the relative risk (RR) of PTC recurrence with 95% confidence intervals (CIs) using the Mantel-Haenszel method. Additionally, the Higgins $I^2$ statistic was used to test for heterogeneity. Results: The meta-analysis included 71 published studies with 44,034 participants, among whom 11,132 had HT. We observed negative associations between PTC with comorbid HT and extrathyroidal extension (OR, 0.74; 95% CI, 0.68 to 0.81), lymph node metastasis (OR, 0.82; 95% CI, 0.72 to 0.94), distant metastasis (OR, 0.49; 95% CI, 0.32 to 0.76), and recurrence (RR, 0.50; 95% CI, 0.41 to 0.61). Conclusion: In this meta-analysis, PTC patients with HT appeared to exhibit more favorable clinicopathologic characteristics and a better prognosis than those without HT.

급성 허혈성 뇌졸중 환자의 퇴원시 기능 상태와 관련된 다차원적 요인 분석 - 강원도 소재 일개 병원의 환자를 대상으로 (Multi-dimensional Factors related to the Functional state of Acute Ischemic Stroke Patients - For Patients Visiting a Hospital in Gangwon-do)

  • Objectives : The purpose of this study was to analyze factors related to the functional state of stroke patients after discharge from hospital. Methods : The data was provided from a hospital in Wonju, Gangwon-do. The subjects of the analysis were those who were admitted to the emergency room due to stroke from July to December 2016. The dependent variable was the patient's functional status as measured by the modified Rankin Scale(mRS). Independent variables were demographic factors (age, sex, and marriage status), transportation and distance factors (transportation, travel distance), inpatient factors (lengths of stay, Charlson Comorbidity Index (CCI), Tissue plasminogen activator, National Institute of Health Stroke Scale (NIHSS). Hierarchial regression analysis was applied for the analysis. Results : In the hierarchical regression analysis, Model 3, including socio-demographic factors, transportation, distance factors, and inpatient factors, was the best fitted model. It showed that functional status of stroke patients was positively associated with age, length of stay, CCI, NIHSS, and negatively associated with unmarried status. Conclusions : Results indicated that management of stroke requires care from the pre-disease stage, and a customized education program policy is needed for high-risk stroke patients who are older and have comorbid illness.

Factors Associated with Worsening Oxygenation in Patients with Non-severe COVID-19 Pneumonia

  • Background: This study aimed to determine the parameters for worsening oxygenation in non-severe coronavirus disease 2019 (COVID-19) pneumonia. Methods: This retrospective cohort study included cases of confirmed COVID-19 pneumonia in a public hospital in South Korea. The worsening oxygenation group was defined as that with SpO2 ≤94% or received oxygen or mechanical ventilation (MV) throughout the clinical course versus the non-worsening oxygenation group that did not experience any respiratory event. Parameters were compared, and the extent of viral pneumonia from an initial chest computed tomography (CT) was calculated using artificial intelligence (AI) and measured visually by a radiologist. Results: We included 136 patients, with 32 (23.5%) patients in the worsening oxygenation group; of whom, two needed MV and one died. Initial vital signs and duration of symptoms showed no difference between the two groups; however, univariate logistic regression analysis revealed that a variety of parameters on admission were associated with an increased risk of a desaturation event. A subset of patients was studied to eliminate potential bias, that ferritin ≥280 ㎍/L (p=0.029), lactate dehydrogenase ≥240 U/L (p=0.029), pneumonia volume (p=0.021), and extent (p=0.030) by AI, and visual severity scores (p=0.042) were the predictive parameters for worsening oxygenation in a sex-, age-, and comorbid illness-matched case-control study using propensity score (n=52). Conclusion: Our study suggests that initial CT evaluated by AI or visual severity scoring as well as serum markers of inflammation on admission are significantly associated with worsening oxygenation in this COVID-19 pneumonia cohort.

우울증과 관상동맥 질환(I) : 병태생리적 기전 (Depression and Coronary Artery Disease(I) : Pathophysiologic Mechanisms)

  • 본 논문에서는 우울증과 관상동맥 질환의 연관성에 대해 병태생리학적 기전을 중심으로 고찰하였다. 현재까지 선행 연구들을 통해 제시된 주요 병태생리학적 기전으로는 우울증에서의 증가된 혈액 응고 경향, HPA 축 기능 및 ANS 조절 이상, 상승된 염증 반응 상태 등과 더불어 관상동맥 질환과 우울증이 공통된 유전적 위험 인자를 공유하고 있을 가능성 등이 있다. 혈액 응고와 관련되어서는 혈소판의 활성 및 반응성 증가, 혈관 내피 기능 이상, 혈액 응고 인자의 증가 등이 구체적인 병리 기전으로 제시 되었으며, HPA 축 및 ANS 기능 이상과 관련되어서는 혈장내 카테콜라민의 증가와 이와 연관된 심박수 증가 및 심박 변이성의 감소, 기타 혈역학적 스트레스의 증가 등으로 인한 부정맥 및 죽상경화증의 촉진이 주요한 기전으로 제시되었다. 또한 CRP 등과 같은 염증 표지자 연구를 통해 우울증에서의 증가된 염증 반응이 죽상경화증이나 혈전 형성과 관계 되어있을 가능성 역시 제시되었다. 하지만 역으로 관상동맥 질환과 우울증의 동반 이환율이 높다는 사실은 잘 알려져 있음에도 불구하고 관상동맥 질환에서의 어떤 병태생리적 변화가 우울증의 발병 및 경과와 관련되어 있는지에 대해서는 체계적인 연구가 매우 적은 것이 사실이다. 이 이외에도 우울증과 관상동맥 질환 외의 제3의 요인 즉, 유전적 위험 인자가 두 질환에 선행함을 제시하여 우울증과 관상동맥 질환이 공통된 유전적 취약성을 공유하고 있을 가능성을 제기한 연구들도 있다. 그 밖에도 우울증과 대사 증후군과의 연관성에 기반한 연구나 ${\Omega}$-3 지방산, homocystein 수준에 주목한 일부 연구들이 존재하며, 관상동맥 질환을 가진 환자는 뇌혈관에도 죽상경화증이 있을 가능성이 높고 이로 인해 뇌의 특정 부위에 혈류장애가 있을 가능성이 높아서 이것이 혈관성 우울증을 야기할 가능성이 있다는 보고를 한 연구들도 있다. 우울증이 관상동맥 질환의 발생 및 경과에 부정적인 영향을 끼친다는 많은 연구 결과들은 우울증과 관상동맥 질환의 원인적 연관성에 대한 연구를 지속적으로 촉진시키고 있다. 하지만 우울증이라는 대상 질환 자체의 이질성과 관상동맥 질환을 가지고 있는 환자들은 대부분 항응고제 등의 약물을 투여하고 있다는 사실 등이 병태생리 규명 연구에 어려움을 주고 있는 것이 사실이다. 우울증과 관상동맥 질환의 병태생리적 연관성에 대한 지속적이고 다각적인 연구는 향후 관상동맥 질환 환자에서의 우울증의 중요성을 치료자에게 인식시키는 계기가 될 수 있으며 더 나아가 관상동맥 질환의 치료 예후의 개선으로 이어질 수 있는 통로를 제공할 것이다.

비장티푸스성 살모넬라 감염으로 발생한 농흉과 종격동염 1예 (A Case of Empyema and Mediastinitis by Non-typhi Salmonella)

  • NTS의 호흡기계 감염은 매우 드문 질환으로 보통 후천성 면역 결핍증, 악성 종양, 교원성 혈관 질환, 스테로이드의 장기간 사용, 겸상적혈구 빈혈증, 당뇨 환자에서 나타난다. 하지만 저자들은 특별한 면역성 질환을 갖고 있지 않은 26세 남자에서 농흉과 종격동염을 진단 후, 환자의 흉수에서 비장티푸스성 살모넬라균이 동정된 1예를 경험하였기에 문헌 고찰과 함께 보고한다.
