상수도수불화사업의 경제적 편익에 대한 조사연구 (Study on Influence of Water Fluoridation Program on the Economic Benefit)

  • 이 연구는 상수도수불화사업의 경제적 편익에 미치는 영향을 파악하기 위하여 2003년 1월 1일부터 6월말까지 상수도수불화지역인 청주시 상당구, 상수도수비불화지역인 안양시 만안구를 대상지역으로하여, 조사대상지역내의 치과의원에서 진료를 받은 진료지표를 비교 분석한 결과는 다음과 같다. 1. 상수도수비불화사업지역인 안양에서 상수도수불화사업을 실시할 경우 2003년 상반기의 6세에서 18세까지 연령에서 기대되는 치아우식증과 치수 및 치근단 주위조직 질환으로 인한 감소기대진료건수는 1,272건, 저추정시 636건, 고추정시 1,908건이 감소될 것으로 기대된다. 그리고 감소기대진료비는 40,888천원, 저추정시 20,444천원, 고추정시 61,333천원이 감소될 것으로 기대된다. 2. 상수도수불화사업의 전국 확대를 가정한 연간 기대건수는 2,492,018건, 저추정시 1,246,009건, 고추정시 3,738,027건이 감소가 기대되며, 감소기대진료비는 80,105,075천원, 저추정시 40,052,537천원, 고추정시 120,157,612천원이 각각 감소가 기대된다. 3. 급수인원이 48,925명인 영운정수장의 경우 편익/비용 비율이 2.9배이며, 급수인원이 239,751명인 지북정수장의 경우 편익/비용 비율이 9.0배이다. 상수도수불화사업은 급수인원이 증가함에 따라 비용의 증가는 미미하고 규모에 따른 경제 효과가 매우 크게 나타났다. 국민구강건강증진은 아울러 구강건강보험재정의 비용절감과 연관이 있다고 사료되며, 이를 위하여 전국 모든 지역에 상수도수사용을 확대 실시하여, 상수도수와 불소화에 대한 적극적인 홍보와 교육을 통한 신뢰 구축이 추가적으로 필요하다고 생각된다.

울산광역시 수돗물불소농도조정사업의 치아우식예방 효과평가 (Evaluation of community water fluoridation program on dental caries prevention in Ulsan)

  • Objectives: The aim of this study was to investigate the effect of water fluoridation program(WFP) on dental caries prevention in Ulsan, Korea. Methods: The data of control group were extracted from 2012 Korean National Oral Health Survey(KNOHS). Since 1998, WFP was implemented in Ulsan. The subjects were 670 subjects including 10 to 12-years old children in 2009. The control subjects were 4,871 people in non-fluoridated metropolitan cities comparable to Ulsan. The control subjects were selected from 2012 KNOHS. A self-reported questionnaire was completed by the variables including oral health behaviors on daily frequency of toothbrushing, eating snacks and beverages. Oral health examination was carried out by two dentists who received a training in KNOHS with an inter-examiner-agreement. Caries preventive fraction was calculated by the difference of DMFT and DMFS index. The preventive effects of the WFP program were focused on gender, number of fissure sealed teeth, daily frequency of toothbrushing, eating snacks and beverages. Data were analyzed using SPSS $19.0^{(R)}$(SPSS Inc., Chicago, IL, USA) statistical package. Results: DMFT indices of 12-year-old subjects(n=670) in WFP and control population(n=4,871) were 2.11 and 1.23, respectively with an estimated prevention effect of 41.7%. Conclusions: WFP in Ulsan reduced the prevalence of dental caries. WFP should be recommended for the public oral health program in other metropolitan cities.

수돗물불소농도조정사업 지역과 비사업지역 아동 간의 치아우식 발생 격차 관련요인 (Factors related to the difference in the incidence of caries between children in fluoridated and non-fluoridated areas)

  • Objectives: The aim of this study is to analyze the differences in factors related to the incidence of dental caries between children in fluoridated and non-fluoridated areas and compared the DMFT and DMFS scores to confirm the effect of water fluoridation program (WFP) in Geoje and Changwon. Methods: The oral health examination and questionnaire survey were conducted in fluoridated and non-fluoridated areas. The number of surveyed children aged 8, 10, and 12 years in the fluoridated and non-fluoridated area of two cities was 1,524 and 1,383, respectively. Self-recorded questionnaires included self-perception of their own dental health, daily toothbrushing frequency, intake frequency of cariogenic sweet snacks and beverages, experience of gingival bleeding, experience of unmet dental treatment, and use of oral hygiene device except for toothbrush and toothpaste. The prevalence of caries and fissure-sealant status were surveyed. The logistic regression analysis was used to analyze the difference in children's self-rated oral health status between the fluoridated and non-fluoridated area. The mean number of sealed teeth and surfaces, marginal means of the DMFT and DMFS scores adjusted for the difference in the samples' sex distribution, and region were compared between the fluoridated and non-fluoridated areas. Results: In the fluoridated area, the experience rate of unmet dental treatments was higher among children aged 10 years, intake frequency of cariogenic sweet snacks and beverages were higher among the whole sample, and experience rate of professional fluoride application were lower than in the non-fluoridated area. The DMFT score for permanent dentition adjusted for differences in sex, region, and mean number of fissure-sealed teeth in the fluoridated area was significantly lower among whole samples and 8-, 10-, and 12-year-olds alone than in the non-fluoridated area. Conclusions: WFP can alleviate oral health inequality because it is effective in reducing the incidence of caries among children is disadvantaged demographic, socioeconomic, and cultural contexts.

김해시 수돗물불소농도조정사업의 영구치 우식예방효과 (Caries Prevention Effect of Water Fluoridation in Gimhae, Korea)

  • 본 연구에서는 경상남도 김해시 아동들을 대상으로 치면열구전색사업의 효과를 보정한 수불사업의 치아우식병 예방효과를 평가하기 위해 1999년부터 수불사업을 시행하고 있는 김해시 삼계정수장 급수구역 초 중학교 학생들(8세, 10세, 12세)을 대상으로 2009년 구강검사를 시행한 972명과 2010년 국민구강건강실태조사에서 비수불 중소도시 35개 지역 4,397명(8세, 10세, 12세)의 치아우식병 실태조사 자료를 비교하고, 성별인구와 치면열구전색치아수 및 치면수를 보정하여 분석한 결과, 다음과 같은 결과를 얻었다. DMF rate는 8세에서 가장 높았으며 김해시와 비수불 중소도시(대조군)에서 각각 18.0%와 29.1%로 우식예방효과는 38.14%로 추정되었다. 12세 DMF rate는 김해시에서 50.8%로 대조군의 60.9%보다 낮으며 우식예방효과는 16.58%로 추정되었다. 12세 DMFT index는 김해시에서 1.60개, 대조군에서 2.12개였으며 우식예방효과는 24.76%였다. 12세 DMFS index는 김해시에서 2.45면, 대조군에서 3.33면이었으며 우식예방효과는 26.45%였다. 12세 평활면 DMFS index는 김해시에서 0.68면, 대조군에서는 0.90면으로 우식예방효과는 24.03%로 추정되었다. 김해시의 수불사업은 대조군인 비수불 중소도시들과 비교한 결과 치아우식병 예방에 우수하여 효과적인 지역구강보건사업으로 평가되었으며, 본 연구의 결과가 지역구강보건 사업개발에서 우선순위를 제시하는 데에 근거자료로 활용할 수 있을 것으로 검토되었다.

성남시 보건소 유아구강보건사업 개발을 위한 구강건강실태에 관한 조사연구 (A epidemiological study on the oral health in preschool children for the development of community based oral health program in Sungnam city)

  • Dental caries causes the majority of tooth loss among Koreans. Korea is experiencing an increase in dental caries. And it shows a high prevalence of caries in preschool children, which contrasted with the much-improved situation in developed countries. So, it is important to control caries from preschool periods in Korea. Recently, inspections of the teeth of preschool children for evidence of dental disease became a legislative duty in Korea following Oral Health Act(2000). But, standardized oral health programs in preschool children is not yet proposed. Therefore, it is necessary to develop an community based oral health program in preschool children. The purpose of this study was to obtain basic data for the development of community based oral health program in preschool children of Sungnam city. Oral health status in primary dentition were surveyed in 862 2-6 year-old preschool children of Sungnam city by WHO criteria. From the data, df rate, percentage of preschool child ren with active caries, dft index, dt rate, percentage of preschool children with pit and fissure sealant of each age were calculated, and discussed. The obtained results were as follows : 1. The experience of dental caries in primary dentition showed a tendency to increase quickly between 2 and 3 year old children. So, the oral health program in preschool children should be developed to prevent dental caries and control their oral health under three years of age. 2. In 5 year-old children, the percentage with active caries was 60.8% and dft index was 5.06. The dt rate showed a tendency to decrease as proportion to age, but even though 6 year-old children, the dt rate was 47.7%. Therefore, the annual screening dental examination and oral health education programs at the institution for preschool children should be developed to detect and treat dental caries in primary dentition at early stage. 3. The percentage of preschool children with pit and fissure sealant at six years of age was 10.2%. So, pit and fissure sealant and professional fluoride application programs in Public Health Center should be developed to prevent dental caries. 4. Also, it is necessary to establish oral health goals in preschool children and develop water fluoridation program 10 improve and preserve oral health of preschool children in Seongnam city, effectively.

보건소 내원 환자들의 구강보건 인식도에 관한 조사 (A study on the patient's awareness of oral health in public health center)

  • There are two purposes in this study. The first one is to collect some oral health information through the investigations upon the oral health care of Korean adults. Referring to the result of the investigation. I wanted to furnish fundamental data with oral health education for each age and with a publicity booklet editing. That is the second purpose of this research. Among the people who visited Seo-gu Public Health Center in Kwang-ju, 207 people answered the questionnaires. There were 86 men and 121 women from the 20-aged to the 59- aged. The questionnaires covers knowledge aquisition path for dental caries prevention, the cognition degrees for the causes of dental caries and its prevention, the cognition degrees for the causes of periodontal disease and its prevention, the degrees for oral health methods, and the importance of oral health. The conclusions are as follows; 1. The cognition degree for dental caries prevention: The cognition for pit and fissure sealant was appeared most highly in thirties by 85.3%. but 62.1 % in fifties was answered, "Never heard". The cognition degree for fluoride application was appeared most highly in thirties by 73.5%. and the cognition degree for water fluoridation was most highly in forties by 54.2%. 2. The knowledge for pit and fissure sealant was acquired mostly through dental hospital in every age by 54.2%. 3. The knowledge for fluoride application was learned mostly through dental hospitals in twenties and thirties by 32.7%. and mostly through TV or radio health programs in forties and fiftieseach by 35.7% and 50.0%. 4. The knowledge for water fluoridation was acquired through TV or radio programs in all ages such as twenties. thirties. forties and fifties. Its rate was 57.8%. 5. The cognition degree of the cause of dental caries: 53.1 % of all ages think that dental caries can most frequently be caused by being lack of toothbrushing. 6. The cognition degree of the cause of periodontal disease: 58.5% people of all ages think that both dental plaque and calculus might be the main cause of periodontal disease. 7. The cognition degree of dental caries prevention: 72.8% people think that dental caries can be prevented by right tooth brushing method. and 8.7% people think that they can be prevented by scaling. However, 10.7% people of them were not interested in dental caries prevention. 8. The importance of oral health: 35.3% people think that teeth health is the most important, and 63.8% people think oral health is one of the most important health problems. Forties answered that tooth health was the most important thing, and fifties. thirties and twenties followed in the order. 9. The cognition for oral health maintenance: all ages(twenties. thirties. forties. and fifties) answered that right toothbrushing method was the best way to keep oral health by 69.1%.
