Objectives: This paper discusses the current contexts of the collaboration between public health centers(PHCs) and community partners for health promotion. Then it suggests directions for the development and enhancement of the collaboration. Methods: The discussion in this paper is based on key literature on community health promotion, including literature reviews and case reports. Results: PHCs are mostly engaged in networking and cooperation rather than collaboration with the community. A typical pattern of cooperation is where PHCs provide healthy-setting types of programs to the community in single-partnered relationships. Current cooperation rarely involves co-planning by a multi-partnered partnership, and is greatly influenced by the interest of PHC directors and PHC performance evaluation indicators. Systems change is recommended to foster collaboration for community health promotion. Such change involves: shared understanding of health promotion and collaboration, inclusion of collaboration mechanism in public health governance, leadership development, capacity enhancement of all partners. role definition of PHCs for community collaboration, and development of collaborative system, at the least. Conclusions: At this point where collaboration should be more than rhetoric, multi-faceted, intersectoral, and concurrent approaches are required to create discourses, to develop cases, and to share experience for actual realization of collaboration for community health promotion.
The paradigm of health promotion requests community participation and its active problem-solving. Community is conceptualized as a resource pool to be organized. Such resource is called community capacity. Community participation is a process of capacity building. Community voluntary associations are considered as valuable resource to be used for health promotion. This paper tried to identify the network structure among community voluntary associations and to infer the possibility to make such network of organizations participate in health promotion programs. Two survey data were used for this research: 1) Measurements and Evaluations of Community Capacity on Dobong-gu (N=94) 2) A development plan of health medicine service to be Healthy Gangdong-gu (N=69). The questionnaire included such variables measuring community capacity as leadership, membership, organizational resources, and inter-organizational network, etc. Both regions had the following common characteristics: 1) There were positive correlations between the organization's budget and membership. 2) Organizational types were associated with their founded years. Two regions showed the following differences: Dobong displayed the high density of community organizations, but Gangdong showed the low density. Dobong community organizations were able to be classified into three network clusters such as women & environments, youth & adolescent, and sports organizations. Each cluster of organizations favored the different type of health promotion programs. Gangdong community organizations were less developed, and not possible to be clustered. Depending upon the level of community capacity or community organizations' differentiation, the strategy of community participation could be settle down in different ways. Particularly the health agency had to pay more attention to support the growth of civil organizations.

The author suggested that an health department should follow several steps to have an effective health promotion programs in their community. First step would be community mobilization that encourage key persons and major organizations and agencies to participate in the program. The second step is collecting demographic and vital statistics in the community or for a target audience as well as social, psychological and behavioral data. Based on the data analysis, the next step is to choose a target audience and health problem(s) for the target audience in question. The fourth step is the development of health education strategy for the target audience and the health problem. The fourth step also includes selecting a proper communication channel and educational materials as well as pre- and post-testing. The final step is implementing health education programs and evaluating the process, outcome and impact of the program. Korean Institute for Health and Social Affairs(KIHASA) has developed a model for health education programs used in local health department. KIHASA can provide technical assistance and health education materials to assist local health departments in Korea.

Purpose: The purpose of this study is to investigate factors affecting social problem-solving ability of alcohol-dependent patients with a focus on gender differences. Methods: Participants were 250 alcohol-dependent people(men 140, women 110) who were living in B, G and Y cities. Data were collected from January 10 to March 31, 2017 using self-report questionnaires. Abstinence self-efficacy, alcohol insight, unconditional self-acceptance, and social problem-solving ability were investigated. For data analysis, t-test, one-way ANOVA, Pearson correlation coefficients and multiple regression were employed. Results: Factors influencing social problem-solving ability for men were unconditional self-acceptance and age. The explanatory power was 28%. Factors influencing social problem-solving ability for women were unconditional self-acceptance, stress, religiousness, age, occupation and abstinence self-efficacy and the explanatory power was 72%. Unconditional self-acceptance and age were significant variables of social problem-solving ability in both men and women. Stress, occupation, religiousness and abstinence self-efficacy were significantly associated with social problem-solving ability in women but not in men. Conclusion: The results suggest that it is necessary to consider gender characteristics in order to develop effective management programs for social problem-solving ability in alcohol-dependent people.
Purpose: This study was to investigate adequate strategies of PBL in community health nursing for learning in the real community situation. Method: Data were collected in a PBL class of 14 third-year students who solved problems and assessed PBL in community health nursing related to visiting nursing and chronic diseases. Results: The students guessed situations diversely, chose learning issues widely and mapped the learning concepts specifically. In the assessment of the presentation, the peers of the same group gave the highest score $29.00{\pm}3.36$, the tutor lowest score $22.83{\pm}5.15$. In 5-point Likerts scale, the group dynamic was highest ($4.18{\pm}.61$) and the presentation was lowest ($3.59{\pm}.84$). Conclusion: The group needs to include students who have experiences in the practice at the health centers. The PBL class should be managed by the block system along with the conventional learning. Students needs to practice the self-directed learning and the presentation in a first semester and then PBL. The introduction of community health nursing begins with the conventional lecture and the programs on life circle and health centers through PBL in the comprehensive curriculums.
Due to the existence of asymmetry of information between doctor and patient, it has been believed that doctor might affect patient's decision making process of purchasing medical care. Based on this notion, doctor's reimbursement method has been suggested as an effective policy device of improving efficiency of patient's medical care use by way of its affecting doctor's practice pattern. By using the Community Tracking Study (CTS) household and physician data set, which includes not only various information on patient's medical care use, but doctor's practice arrangements and sources of practice revenue, this paper investigates the effect of community doctor's characteristics of reimbursement method on community patient's medical care use under the control of patient's socio-demographic characteristics and community doctor's practice type. In the process of estimating econometric model, the endogeneity problem of individual health insurance purchase was corrected by using 2818. And due to the existence of sample selection problem, Heckman's two-step estimation method was used for strengthen the robustness of estimation which was adversely affected by sample selection problem The empirical results show that as the average value of community doctor's portion of practice revenue determined by prospective method out of total revenue increases, the community patient's total out-of-pocket medical cost decreases. This results suggest, as doctor's practice revenues are mainly determined by prospective method, such as capitation, doctors would be more conscious about practice cost, which might affect doctor's practice pattern and by which his/her patient's use of medical care would decrease.
Purpose: The purpose of this study was to explore practice-based evidence for health promotion in vulnerable populations with hypertension in primary health care settings. Methods: Two methodological procedures were adopted for this triangulation study. In the first phase, the sample was obtained from the computerized clinical data repository of a community nursing center. A total of 286 clients were assessed for hypertension as an actual circulation problem as coded in the Omaha System. In the second phase, a qualitative focus group was surveyed through semi-structured interviews conducted by nine advanced practice nurses who had been serving the hypertensive patients. Results: The community nurses provided essential primary healthcare services including health teaching guidance and counseling, and surveillance to vulnerable populations living in medically underserved community. There was a significant positive correlation between knowledge and behavior (r=.53, p<.01), between knowledge and health status (r=.40, p<.05), and between behavior and health status (r=.48, p<.01). Conclusion: This triangulation study encompassed not only quantitative findings from the computerized records of clients but also other information acquired from advanced practice nurses. This study contributes to understanding the importance of health promotion nursing interventions even with populations already diagnosed with chronic diseases such as hypertension.
