복합만성질환을 가진 자녀를 둔 부모의 소아청소년 완화의료 요구도 (Needs for Pediatric Palliative Care among Parents of Children with Complex Chronic Conditions)

  • Purpose: The purpose of this study was to investigate the needs for pediatric palliative care (PPC) among parents of children with complex chronic conditions (CCCs) and to investigate differences in the needs for PPC according to their general characteristics. Methods: A cross-sectional survey was conducted between March 28 and May 18, 2018. Parents (N=96) who had a child under 18 years with a CCC were recruited. Data were analyzed using descriptive statistics, independent t-test, and one-way ANOVA. Results: The overall average need for PPC was $3.58{\pm}0.33$ out of 4.00. In terms of care for the subjects' children, the highest need was physical care, followed by psychosocial and spiritual care. In the sub-dimensions, preservation of physical function received the highest score. Of the items, the highest need was for seizure control. In terms of care for the subjects themselves, the highest need was for psychosocial care, followed by bereavement and spiritual care. In the sub-dimensions, communication received the highest score. Of the items, the highest need was for smooth communication with medical staff. Differences in needs for PPC according to participants' general characteristics were not statistically significant. Conclusion: Medical staff should provide PPC according to the priorities of parents' perceived needs.

유아교육기관에서의 급식관리 실태에 대한 교사 및 학부모의 인식 연구 (The Different View Point of Child Education Center Food Service Program between the Parents and the Teachers)

  • To survey the different view points about food service programs among parents and teachers, 2 types of questionnaires, which consisted of attitude, perception, satisfaction and demand of the food service program in child education centers, were used. The data was collected from 2450 parents and 450 teachers who attended a child education center in 16 provinces, nationwide. SPSS was used for descriptive analysis and ANOVA test and $X^2-test$. The frinding results were as follows. 1. The average serving size of meal (lunch) were 80 meals per day and 167 meals per day at large institutions. Mean cost of snacks was 14,709 won per month and mean costs of lunch were 29,319 won per month. The mean price was not significantly different according to the scale of institution. The numbers of servings of lunch, morning snack and afternoon snack were 5, 3.4 and 3.5 times per week each. $56.4\%$ of the institutions served meals to children in classrooms, but the national/public institutions, which were attending elementary school, served meals in a dining place in the elementary school. 2. Teacher controlled serving portion size of snacks $(79.6\%)$ and lunch $(88.8\%)\;and\;30.1\%$ of teacher did not allow leaving lunch food. The ratio of knowing about preserved meals of the teacher who worked at a small institution was significantly higher than the teacher who worked at large institutions (p<0.01). 3. Between parents and teachers, several different view points about school lunch programs were detected. Most parents and teachers wanted that the school lunch to be fully cooked and served at the child education institution itself, but $12.2\%$ of parents and $14.4\%$ of teachers wanted a catering service. The teachers group preferred 'lunch box from home' and 'home partially prepared lunch' as an ideal meal serving type than the parent groups (p<0.01). And there were significantly different view points about price factors in school meals, teachers group highly answered that operating expenses must be added in meal prices. 4. The teacher groups' priorities of education activities during meal time were a significantly lower score than parents group in overall education activities. Teacher and parent groups pointed out that individual sanitation activities were most important of the education activities during meal time, but promoting good eating habits was the lowest score in both groups. 5. 'Improving taste and food quality' was most urgent in food service at child education centers, but there were significantly different view points between parent groups $(64.5\%)$ and teacher groups $(43.8\%)\;(p<0.05)$. They answered at a lower percent in 'employee qualified person' and 'cost control' point to improve food service, but there were also different opinions between the two groups (p<0.01). 6. As to the matter of the advantages and disadvantages of catering services, two group answered that the advantages of a catering service were 'convenience' and 'to solve facilities and labor problems', disadvantages were 'lower in food freshness' and 'sanitation problems'. There were also several different view pionts in catering services, the parents groups were more anxious about food sanitation than teachers. This study found several different view points about school food services among parents and teachers. To improve food services at child education institutions, there is a need to adjust the differences between the two groups through interactive communication channels and education and to employ dietitians as taking charge of adjusting roles between the two groups.

이마고 부부치료 모델 내용의 타당성과 적용 효과성 연구 (Validity of Imago Couple Therapy's Contents and Effects of Application of the Imago Couple Therapy)

  • This study focused on the validity of Imago couple therapy's contents and the effects of application of the Imago couple therapy model. The participated subjects were 38 graduate students and their spouses. The major findings of this study were as follows. First, the types of couple relations that formed and sustained marital relations after growth could be predicted by the early childhood relationships between the subjects when they were children and their parents. Second, the marital conflicts and frustrations could be healed by the 'parent-child dialogue' and 'guided imagery'. Third, Imago couple therapy was effective for couple counseling to improve the areas of marital relationships, especially marital intimacy, marital communication, communication skills for marital conflicts, mutual respect, gender role, sexual satisfaction, time together, and decision-making. Fourth, Couple's Dialogue was a powerful tool for couple and parent-child relationships as well as general counseling practices.

이혼 가정 자녀의 긍정적 변화에 영향을 미치는 변인 분석: 가구 소득의 조절효과 (Predictors of Positive Changes in Children with Divorced Parents: Focused on Moderating Effects of Family Incomes)

  • 이 연구의 목적은 이혼 가정 자녀의 긍정적 변화에 영향을 미치는 변인을 확인하는 것이다. 이를 위해 본 연구에서는 이혼 가정 한부모의 지각된 사회적 차별 경험, 양육 어려움, 자녀와의 대화시간, 자녀의 인터넷 사용문제, 이혼 후 자녀의 긍정적 변화 간의 구조적 관련성을 확인하고, 가구 소득에 따른 구조적 관계의 차이를 확인하였다. 분석 자료는 2015년 한부모가족 실태조사 자료의 한부모 1,114명의 데이터를 활용하였으며, 연구문제를 검증하기 위해 구조방정식모형 분석과 다집단 분석을 실시하였다. 분석 결과, 첫째, 한부모가 지각한 차별 경험은 양육어려움(${\beta}=.354$), 인터넷 사용문제(${\beta}=.234$)에 통계적으로 유의한 정적인 영향을 미쳤다. 한부모가 지각한 양육 어려움은 자녀와의 대화시간(${\beta}=-.192$), 자녀의 긍정적 변화(${\beta}=-.218$)에 통계적으로 유의한 부적인 영향, 자녀의 인터넷 사용문제(${\beta}=.440$)에 통계적으로 유의한 정적인 영향을 미쳤다. 자녀의 인터넷 사용문제는 부모의 이혼 후 자녀의 긍정적 변화(${\beta}=-.199$)에 통계적으로 유의한 부적인 영향을 미쳤다. 둘째, 한부모가 지각한 차별 경험은 자녀와의 대화시간, 양육 어려움, 인터넷 사용문제를 매개로 자녀의 긍정적 변화에 통계적으로 유의한 부적인 영향을 미치는 간접효과(${\beta}=-.167$)가 확인되었다. 셋째, 다집단 분석 결과, 가구 소득에 따른 통계적으로 유의한 경로계수의 크기 차이는 없는 것으로 나타났다. 본 연구 결과는 이혼 후 자녀의 긍정적 변화를 이끌기 위해서는 한부모가 차별을 느끼지 않을 수 있는 사회적 문화 형성과 한부모의 양육 과정에 대한 공식적, 비공식적 지지가 중요하며, 한부모 가정 자녀의 인터넷 과다 사용을 예방하고 줄이기 위한 실천적 노력이 필요함을 시사한다.

저소득 가정의 조기개입 프로그램 참여유무에 따른 영유아 발달 및 부모 양육스트레스의 차이 (Effect of Early Intervention Program for Child Development and Parenting Stress from Low Income Families)

  • 본 연구는 저소득 가정의 조기개입 프로그램 참여 유무에 따른 영유아의 발달과 양육자인 부모의 양육스트레스의 차이를 분석하고자 하였다. 연구 대상은 저소득 가정의 만 0세부터 만 5세까지의 영유아와 그 부모이며 현재 조기개입 프로그램인 드림스타트 프로그램에 참여하고 있는 영유아와 참여하지 않는 영유아 그리고 그 부모를 대상으로 하였다. 연구도구로 영유아의 발달을 측정하기 위하여 K-ASQ 검사를 실시하였고, 양육자의 양육스트레스 차이를 측정하기 위하여 Parenting Stress Index/Short Form을 사용하였다. 연구결과는 첫째, 조기개입 프로그램의 참여유무에 따라 저소득가정 영유아의 발달에 차이가 있었다. 특히 조기개입 프로그램에 참여한 영아가 참여하지 않은 영아보다 대근육 운동, 소근육 운동, 문제해결, 의사소통, 개인-사회성 등에서 발달수준이 더 높게 나타났다. 둘째, 조기개입 프로그램에 참여한 저소득 가정의 부모가 참여하지 않은 부모보다 양육스트레스 정도가 더 낮게 나타났다. 이는 저소득 가정의 아동과 부모를 위한 조기개입 프로그램이 영아의 발달을 촉진하고 부모의 양육스트레스를 감소시키는 긍정적인 역할을 하고 있음을 보여주는 결과이다. 결과적으로 조기개입 프로그램은 저소득 가정의 아동 뿐 아니라 그 부모를 대상으로 함께 개입하고 지원해야 하며, 보다 많은 저소득 가정의 영유아 및 가족들이 참여할 수 있도록 대상자 발굴이 지속적으로 이루어져야 한다.

  • The current Sudanese preschool system depends on limited methods of education, children's education needs to be equipped to keep pace with technological development, also, the large gap that exists between the families and the Kindergartens, where many parents have no idea on how their child progresses in the KG context. The aim of this research is to integrate ICT in the preschool education to enhance and improve the preschool education, by building an Integrated Educational Application (Computerized Application for Preschool Education CAPE) which will help to improve the learning outcomes. The researchers used the Experimental Research Methodology, the characteristic of CAPE application is; suitable for children's age, the application style is more attractive to the children and contains a different way to help children get learning. Alawaeel and the Smart Child Kindergartens in Republic of Sudan were selected as a sample of the study, with sample size specifically, 50 children's. Also, the Central Bank of Sudan Kindergarten was selected as one of the institutional Kindergartens for easy communication with parents of children with a sample size 21 children. The study found that; using CAPE application in KG enables children to increase general learning effects and developing child's cognitive skills. Also, the children who were allowed to use CAPE by their parents are performed better in the overall evaluation of KG lessons. Also, using the CAPE in the Pre-School education helps the parents following their children's progress better and more reliable. The researcher recommends that to apply the computerized application and includes the second level. Also, converting the computerized program into an application to be used by children by their self, without the intervention of parents.

Changes in Children with Autism Spectrum Disorder after Theraplay Application

  • Objectives: This study identified changes in social communication and interaction among children with autism spectrum disorder (ASD) through Theraplay. Methods: This study included 20 children who were diagnosed with ASD by a child and adolescent psychiatrist and were recommended to undergo Theraplay in 2012-2017. The Marschak interaction method was used to identify and analyze the interactions between parents and their children. The Korean version of the Social Communication Questionnaire (K-SCQ) was used to evaluate symptoms of ASD. The Wilcoxon signed-rank test was used to compare the differences before and after Theraplay. Results: Upon comparing the differences in total K-SCQ scores, there were statistically significant changes (z=-3.269, p=0.001) in children with ASD. Changes were also confirmed in each category, namely social interaction, communication, and limited and repetitive behavior patterns (p=0.011, p=0.025, p=0.039, respectively). Conclusion: This study found that Theraplay may cause significant changes in social communication in children with ASD. Further studies are needed to establish the effectiveness of Theraplay in children with ASD.

신생아 중환자실에 입원한 환아 어머니의 스트레스 (A Study on the Perceived Stress of Mothers in Neonatal Intensive Care Unit)

  • The parents have much expectation upon the pregnancy and child birth, and in most cases, they expect the healthy parturient child. However, we can be placed on the high-risk conditions which have the physical, social and immature infant, due to the unexpected results, among the new-born. Accordingly, these high-risk newborn and premature infants will be mostly in NICU, which the concentrated medical treatment can be given, upon their conditions. After their birth and during these periods, they will be divided from the parents, and the nurse will accomplish the bringing-up activities which they can take care of the infant, expected by the parents after their birth. The hospitalization of high-risk newborn including these premature infants is the shocking experience to the parents of family, and thus they can feel the fear and uneasiness, and these reactions of parents are troubled in the behavior at the usual days, and cause the disorder and spiritless status, and these results break the supporting ability of parents, and cause the obstruction. Also, the unavoidable division between the parents and the children as like hospitalization of children can make the parents to feel the alienation emotionally, and this causes the results which the pride on the bringing-up ability of baby gets to be lost. These problems can cause the difficulties on the bonding or the parenting in the further days, and can be related to the neglect and abuse of children. Also, it is gradually increased to study and report which the emotional division by the physical division between the mother and the baby obstructs the normal affection course between the parent and the infant. The stress caused by the birth and the hospitalization of high-risk newborn, as like this, is important in the points which it can uncertainly affect the potential energy for the relationship of parent-child who are finally healthy. Accordingly, the significance and purpose of this study are to understand the contents and degree of stress which the parents of high-risk newborn including the immature child can be experienced from the hospitalization of ICU for their new borns, and thus to offer the basic program to the nursing intervention program for these. The subject of this study is the mother of newborn in NICU of 10 General Hospitals located at the 3one of Pusan, Korea from September 1997 to October 1997, and thus makes the subject of 95 person of parents who agreed to take part in the study and it is descriptive study related to the stress of mother having the newborn in NICU. The method is based on the preceding study related to the stress of mother having the experience of child hospitalization and chronic disease child, and then acquires the advice of specialists group as like 5 nursing professors, and then is amended and supplemented. Total number of questions is 43 items and consists of 5 factors as like medical treatment &nursing procedures, disease status & prognosis, role of parents, communication & inter-personal relationships, hospital environment, and is 5 point Likert Scale. The reliability of this study method is very highly shown to be Cronbach α=0.95. The collected data is analysed as Average, Frequency, Standard Deviation, T-test, ANOVA, Pearson Correlation Coefficient, Duncan multifulrange test by use of SPSS /PC (V7.5). The results of this study is summarized as under. 1. Every characteristics of subject is which the party of mother is 28.70age(±7.48) in the average ages, 51% in the high-school graduate, 38.5% in the christianity, total monthly income is 212.55 thousand won(±1.971), 74.5% in the housewife, 72.9% in the parents and children together living and the number of children to be 1.48person(± 0.6) in average, the recognition on the prognosis of baby is 74.0% in 'Don't know', the relationship with the husband after the hospitalization of babyis 37.3% in 'More Intimate', the relationship with the family of husband to be 48% in 'No-change', and the degree which is consulted with the husband about the baby is 55% in 'very frequently' and the visiting number per week is 4.59(±1.63) in average and the accompanying person in the time of visiting is which the number of husband is 56.3% and thus is the highest. The characteristics of baby is which the age is 21.88days(±16.47) after the birth in average, the sex to be 50 person in the female 52.1% and the order of birth to be 54.2% in the first chid, and the weight in the birth to be 2770gm(±610) and the height in the birth to be 46.26cm(±7.62) in aver age. The medical diagnosis is 37.5% in the premature infant, the career of hospitalization is 96.9% in 'None', and the operation plan is 90.6% in 'None' and the execution of operation is 88% in 'None' and the nursing of incubator is 55.2% in 'Yes', and the method of feeding is 50.5% in 'Oral' and the contents of feeding is 46.9% in the 'Milk'. 2. The total stress degree of subject is almost highly shown to be as 3.36(±0.86). If it is compared upon each cause, 'stress on disease status & prognosis' is highest 3.79(±1.28), and it is in the order of 'stress on medical treatment & nursing procedures' 3.70(±0.93), 'stress on hospital environment' 3.14(±0.86), 'stress on role of parents' 3.18(±0.92) and 'stress on communication & inter personal relationship' 2.62(± 0.77) 3. As the results of checking the notworthiness of stress degree upon each variable of subject, the variable showing the noted difference was the birth weight(γ=-0.16, P=0.04), birth height(γ=-0.23, P=0.03), nursing in the incubator(F=8.93, P=0.04), feed method(F=2.94, P=0.04). That is to say, it is shown which the smaller the birth weight is, the higher the stress degree of mother is noteworthily. Also, the smaller the birth height baby is, the higher the stress of mother is. In the incubator, it os shown which the mother whose baby is nursing in the incubator is higher in the stress degree than other mothers. Upon the feeding method of baby, that is to say, TPNis the highest, and it is shown in the order of NPO, Tube feeding, and P.O. feeding. When we review the above-mentioned results, as the status is serious, it is thought which we include the supporting nursing for coping with the stress of parents in the setting-up od nursing plan for the baby in the NICU.

부모-자녀 의사소통이 청소년의 자아개념, 문제행동에 미치는 영향 (The Effects of Parent-Adolescent Communication on Self-Concept and Problem Behavior)

  • Purpose: The purpose of this study was to examine the effect of parent- adolescent communication on self-concept and problem behavior of adolescents. Method: The participants in this study were 480 students from middle schools in Daejeon City. Data were collected from May 19 to 30, 2008 and analyzed using SPSS WIN 14.0 version. Result: The mean scores for communication with fathers and mothers were 2.98 and 3.35 respectively and for self-concept, 2.80. The highest reported percentage for problem behavior over the past year were going to the noraebang (Karoki) or videobang (PC Cafe) (78.6%). Significant differences were found for the influence of parent-adolescent communication on self-concept and problem behavior. Communication with fathers and with mothers was positive correlated (r=.365, p<.001). The difference in self-concept for communication with fathers was F=60.40, p<.001, and with mothers, F=51.74, p<.001 and for problem behavior, for communication with fathers, F=3.33, p<.05, and with mothers, F=10.05, p<.001. For self-concept and communication with fathers the correlation was r=.512, p<.001, with mothers, r=.453, p<.001 There was a negative correlation between self-concept and problem behavior(r=-.248, p<.001). Conclusion: This results suggest that practical parent-adolescent communication programs should be developed and applied to bolster communication of adolescents with their parents.

어린이집 교사의 소통능력이 집단응집력, 공감능력과 소통만족에 미치는 영향 : 어린이집 운영주체 별 차이를 중심으로 (The Effects of Child Daycare Center Teacher's Communications Skills on Group Cohesiveness, Empathy, and Communications Satisfaction : Centered on Difference between Daycare Centers)

  • 이 연구는 어린이집 교사의 소통능력이 집단응집력, 공감능력과 소통만족에 미치는 영향에 관하여 살펴보았다. 이 연구를 위해 198명의 어린이집 보육교사를 대상으로 소통능력, 집단응집력, 공감능력 및 소통만족에 대한 설문조사를 실시하였다. 어린이집 운영 주체에 따라 국공립, 민간, 가정 등 3개 집단으로 조사 대상의 소속을 구분하였다. 연구조사 결과, 첫째, 국공립, 민간, 가정 등 세 집단 모두에서 어린이집 교사의 소통능력이 집단응집력에 영향을 미치는 것으로 나타났다. 어린이집 교사의 소통능력은 집단응집력에 긍정적 영향을 주어서 조직의 목표를 원활하게 수행하는 데 도움을 주는 것으로 보인다. 둘째, 국공립, 민간, 가정 등 모든 집단에서 어린이집 보육교사의 소통능력이 역시 공감능력에 영향을 미치는 것으로 나타났다. 어린이집 교사의 소통능력이 높으면 영·유아와 학부모, 그리고 타 교직원을 공감하고 이해하는 능력도 높을 것이다. 셋째, 조사한 세 집단 모두에서 어린이집 보육교사의 소통능력이 소통만족에 영향을 미치는 것으로 나타났다. 보육교사의 소통능력이 높을 때 소통의 대상자인 영·유아, 학부모, 타 교직원들이 보육교사와의 소통에 만족하는 경향이 높음을 알 수 있다.
