Objectives : Despite advance in the surgical treatment of the intracranial aneurysm, we have to be surgical complication. The aim of this report is to evaluate the complication and its management in intracranial aneurysm operation. Methods : We reviewed our exprience with interesting cases of surgical complication of intracranial aneurysm : 1) rebleeding, 2) intra-operative premature rupture, 3) missed aneurysm in angiography, 4) vasospasm. Results : The risk of rebleeding was not related to the patients' initial comdition, but all other intracranial complications occurred significantly more often in patients graded poor compared with patients in good clinical condition. Rebleeding before early surgery remains as major cause of unfavorable outcome. The causes of intraoperative premature ruptures were as follows : 1) dural opening and arachnoid opening(8.3%), 2) hematoma removal(12.5%), 3) brain retraction(16.7%) 4) aneurysm dissection(62.5%). The double suction technique and primary hemostasis using a small piece of cotton or temporary clip resulted in good outcome even in cases with premature rupture. The incidence of missed aneurysm in angiography occurred in 10%. The causes were as thrombosed aneurysm, vasospasm on feeder artery. The most common missed aneurysm is also the most common aneurysm(anterior communicating artery aneurysm). The repeated angiography were documented in missed aneurysm. Balloon angioplasty is superior topapaverine for treatment of proximal vessel vasospasm by viture of a more sustained effect on the vessel. Papaverine can be useful as an adjunct to ballon angioplasty and also for the treatment of distal vessels that are not accessible for ballon angioplasty. Conclusion : The minimization of the complications and active treatment can reduced the mortality and morbidity of ruptured aneurysm patients.
무선 통신 기술의 발달로 인해 이동통신의 사용이 전보다 훨씬 편리해 졌으며 최근 이동 통신 사용자들은 언제, 어디서나 누구와도 통신이 가능하게 되었다. 하지만 이동 통신은 무선채널을 사용함으로서 통신 당사자는 심각한 보안 위협에 노출된다. 안전한 통신채널을 제공하기 위한 방법은 이동통신망의 발전에 필수적이라 하겠다. 본 논문에서는 GSM망으로의 로밍 서비스를 제공하기 위한 인증 및 메시지 암호 알고리즘에 대하여 논한다. GSM 암호 시스템에 적용 가능한 인증 및 암호 알고리즘을 제안하기 위해 GSM망의 보안구조에 관하여 간략히 소개한다. 사용자 인증 알고리즘은 새로운 해쉬함수를 그리고 메시지 암호화를 위한 스트림 암호기는 LFSR(Linear Feedback Shift Resgister)을 사용한다. 각 알고리즘은 C언어로 구현하였으며 IBM PC 상에서 시뮬레이션 하였다. 또한 통계적 분석 기법을 사용하여 개발된 알고리즘의 출력 특성을 분석한다.
Several recent aircraft accidents occurred due to goal conflicts between human and machine actors. To facilitate the management of the cockpit activities considering these observations. a computational aid. the Agenda Manager (AM) has been developed for use in simulated cockpit environments. It is important to know pilot intentions performing cockpit operations accurately to improve AM performance. Without accurate knowledge of pilot goals or intentions, the information from AM may lead to the wrong direction to the pilot who is using the information. To provide a reliable flight simulation environment regarding goal conflicts. a pilot goal communication method (GCM) was developed to facilitate accurate recognition of pilot goals. Embedded within AM, the GCM was used to recognize pilot goals and to declare them to the AM. Two approaches to the recognition of pilots goals were considered: (1) The use of an Automatic Speech Recognition (ASR) system to recognize overtly or explicitly declared pilot goals. and (2) inference of covertly or implicitly declared pilot goals via the use of an intent inferencing mechanism. The integrated mode of these two methods could overcome the covert goal mis-understanding by use of overt GCM. And also could it overcome workload concern with overt mode by the use of covert GCM. Through simulated flight environment experimentation with real pilot subjects, the proposed GCM has demonstrated its capability to recognize pilot intentions with a certain degree of accuracy and to handle incorrectly declared goals. and was validated in terms of subjective workload and pilot flight control performance. The GCM communicating pilot goals were implemented within the AM to provide a rich environment for the study of human-machine interactions in the supervisory control of complex dynamic systems.
본 연구는 학교도서관의 SNS 마케팅 활용사례를 분석하여, 학교도서관 현장에 SNS 마케팅 활성화 방안을 제안하는 데 목적이 있다. 이를 위해 SNS를 학교도서관 마케팅에 활용하는 사서교사 10명과 심층면담을 실시하였고, 학교도서관 SNS 마케팅 운영 현황, 계기 및 목적, 전략, 성과를 분석하였다. 연구 결과, 사서교사들은 페이스북, 인스타그램, 카카오 플러스 친구 등의 SNS 서비스를 이용하는 것으로 조사되었다. 대부분 학교도서관 활성화를 고민하는 과정에서 SNS를 활용하게 된 것으로 나타났으며, 방문자 증가, 이용자의 요구 파악 등 학교도서관 활용 증대 및 이용자와 소통을 목적으로 SNS를 이용하는 것으로 확인되었다. SNS를 통해 제공되는 메시지는 학교도서관 이용안내, 독서 프로그램 홍보 등이며, 마케팅의 대상은 주로 학생으로 파악되었다. 이상의 연구 결과를 토대로 마케팅 협의체의 필요성, SNS 마케팅 대상의 확장, SNS 활용에 대한 전문성 신장 등 학교도서관 SNS 마케팅 활성화 방안을 제안하였다.
그룹 키 교환 프로토콜은 일련의 그룹을 형성하는 다수의 통신 참여자들이 공개된 통신망을 통해 그룹의 공통 비밀키를 설정하기 위한 목적으로 설계된다. 그룹 지향적인 응용분야들이 인터넷상에서 인기를 더해감에 따라서 이들 응용분야에 안전한 멀티캐스트 채널을 제공하기 위해 많은 그룹 키 교환 프로토콜이 제안되었다. 그동안 제안된 그룹 키 교환 프로토콜 중에 최근 Yi 등이 발표한 패스워드 기반 프로토콜이 있다. 이 프로토콜에서는 각 프로토콜 참가자가 자신의 패스워드를 가지고 있으며 이 패스워드는 신뢰할 수 있는 서버에 등록되어 있다고 가정된다. 패스워드 기반 키 교환에서 가장 근본적인 보안 요구사항은 오프라인 사전 공격에 대한 안전성이라 할 수 있다. 그러나 Yi 등이 제안한 프로토콜은 패스워드에 기반한 프로토콜임에도 불구하고 이 요구사항을 만족하지 못하는 것으로 드러났다. 본 논문에서는 Yi 등의 프로토콜에서 발견되는 이러한 보안 문제점을 지적하고 그 해결책을 제시한다.
Objective : Aneurysmal rebleeding is a major cause of death and disability. The aim of this study is to investigate the incidence of rebleeding, and the factors related with patient's outcome. Methods : During a period of 12 years, from September 1995 to August 2007, 492 consecutive patients with aneurysmal subarachnoid hemorrhage (SAH) underwent surgery at our institution. We reviewed the patient's clinical records, radiologic findings, and possible factors inducing rebleeding. Also, we statistically analyzed various factors between favorable outcome group (FG) and unfavorable outcome group (UG) in the rebleeding patients. Results : Rebleeding occurred in 38 (7.7%) of 492 patients. Male gender, location of aneurysm (anterior communicating artery) were statistically significant between rebleeding group and non-rebleeding group (p=0.01 and p=0.04, respectively). Rebleeding occurred in 26 patients (74.3%) within 2 hours from initial attack. There were no statistically significant factors between FG and UG. However, time interval between initial SAH to rebleeding was shorter in the UG compared to FG (FG=28.71 hrs, UG=2.9 hrs). Conclusion : Rebleeding occurs more frequently in the earlier period after initial SAH. Thus, careful management in the earlier period after SAH and early obliteration of aneurysm will be necessary.
EBLIP는 '근거'를 중심으로 실무에서 문제해결 및 의사결정을 정확히 수행하고자 하는 실천 운동이다. 본 연구에서는 이러한 실천 운동의 국내 적용을 실무자들의 근거 출판 활성화를 통해 수행하고자 하였다. 국내 5종의 학술지에 수록된 논문의 내용분석을 통해 국내 실무자들의 근거 생산 현황을 파악하고 실무자들의 표적집단면담으로 근거 생산 장애 요인과 사서들의 인식을 조사하였다. 근거생산에 실무자가 저자로 포함된 경우는 26%를 차지하나, 실무자 단독으로 출판을 한 경우는 9.2%이며 단독 저자인 실무자들의 소속 기관은 대학도서관이 36.4%로 가장 높게 나타났다. 표적집단면담을 통해서 나타난 실무자의 출판 장애요인은 조직적인 차원에서 가장 많이 나타났으며 근거출판 활성화를 위해서 조직의 적극적인 지원을 요구하고 있었다. 본 연구는 이러한 조사결과를 바탕으로 실무자들의 근거 생산 활성화 방안을 조직차원, 연구결과 소통의 차원, 교육차원에서 제안하였다. 조직차원에서는 업무 중 연구 활동의 보장, 연구결과에 대한 보상, 연구 소통의 차원에서는 실무자들이 연구 결과를 용이하게 접하고 활용할 수 있는 새로운 성격의 저널 창간과 구조적 초록과 같은 방안을 제안하였다. 끝으로 실무현장에서 연구방법론 교육의 강화와 통합 연구의 한 방안인 체계적 고찰에 대한 교육을 강조하였다.
Objective : Intraoperative rupture of an intracranial aneurysm can interrupt a microsurgical procedure and jeopardize the patient's chance to favorable outcome. The purpose of this study was to analyse and evaluate intraoperative aneurysmal rupture and render ideal prevention and management to intraoperative rupture. Patients and Methods : The authors retrospectively analysed the results of 609 patients who underwent cerebral aneurysm surgery from January 1991 to December 2000. Results : 1) Intraoperative aneurysmal rupture occurred in 73 of 609 consecutive aneurysm surgery, so the incidence was about 12.0% and it was relatively lower than other reports. 2) Aneurysms arising from anterior communicating artery appeared more prone to intraoperative rupture. 3) The size of aneurysm and timing of operation didn't influence intraoperative aneurysmal rupture and temporary clipping didn't reduce the incidence of intraoperative aneurysmal rupture. 4) Intraoperative aneurysmal rupture occured during three specific periods : (1) dissection stage in 61%, (2) clip application stage in 29 %, (3) predissection stage in 10%. 5) In the patients with intraoperative aneurysmal rupture, surgical outcome was relatively good and there was no significant difference in outcome compared with unruptured group. Conclusion : Our suggestion for prevention methods of intraoperative aneurysmal rupture are as follows : 1) minimal brain retraction, 2) sharp and careful aneurysmal neck dissection, 3) gentle clipping with proper clip selection etc. Management methods after intraoperative aneurysmal rupture are as follows : 1) strong aspiration of bleeding point, 2) rapid application of temporary and/or tentative clip, 3) following rapid dissection of neck and proper clip application, 4) use of encircling clip etc.
Objective : The aneurysm remnants rate was evaluated via three-dimensional digital subtraction angiography (3D-DSA) in the postoperative evaluation of clipped aneurysms. Methods : Data on surgically clipped aneurysms of anterior circulation obtained via postoperative 3D-DSA from February 2007 to March 2009 were gathered. The results of the postoperative 3D-DSA and of two-dimensional digital subtraction angiography (2D-DSA) for the detection of aneurysm remnants were compared, and an investigation was performed as to why 2D-DSA had missed some aneurysm remnants that were detected in the 3D-DSA. Various surgical factors that revealed aneurysm remnants in the 3D-DSA were also evaluated. Results : A total of 39 neck remnants of 202 clipped aneurysms (19.3%) were confirmed in 3D-DSA, and these were classified according to Sindou's classification of aneurysm remnants. Patients with only a neck remnant found in the 3D-DSA represented 17.3% (35/202 aneurysms) of the whole series, and those with a residuum of neck plus sac found in the 3D-DSA represented 1.9% (4/202 aneurysms). The causes of aneurysm remnants were no full visualization (14/39, 35.9%), parent and perforator artery protection (10/39, 25.6%), clip design problems (8/39, 20.5%), and broadnecked aneurysm (7/39, 17.9%). Conclusion : Patients with ${\leq}2$mm aneurysm remnants showed an increased risk of undetectable aneurysm remnants in the 2D-DSA. The most frequent location of the missed aneurysm in 2D-DSA was the anterior communicating artery. 3D-DSA showed more aneurysm remnants than what is indicated in the existing literature, the 2D-DSA.

본 논문에서는 STT(Speech-to-Text), TTS(Text-to-Speech) API와 가속도 센서 기반의 청각 장애인의 의사소통을 도와주는 안드로이드 모바일 애플리케이션을 설계하고 구현한다. 이 애플리케이션은 청각 장애인의 대화 상대가 말하는 것을 마이크로 녹음하고 STT API를 이용하여 텍스트로 변환하여 청각 장애인에게 보여주는 기능을 제공한다. 또한, TTS API를 이용하여 청각 장애인이 문자를 입력하면 음성으로 변환하여 대화 상대에게 들려준다. 청각 장애인이 스마트폰을 흔들면 이 애플리케이션이 실행하도록 가속도 센서 기반의 백그라운드 서비스 기능을 제공한다. 본 논문에서 구현한 애플리케이션은 청각 장애인들이 다른 사람과 의사소통을 할 때 영상통화로 수화를 이용하지 않고 쉽게 대화할 수 있는 기능을 제공한다.
