PARALLEL COMMUNICATING AUTOMATA SYSTEMS -A SURVEY

  • A parallel communicating automata system is an accepting device based on the communications between more automata working in parallel. It consists of several automata corking independently but communicating with each other by request. We survey several variants of parallel communicating automata systems with respect to their computational power. Other aspects like decidability and complexity matters are also briefly discussed. Some open problem sand directions for future research are finally pointed out.

  • The anterior choroidal artery has several kinds of variations. Among them, the transposition of anterior choroidal artery and posterior communicating artery origins has been extremely rare. We report a case with cerebral aneurysm arising from posterior communicating artery which origin was distal to the anterior choroidal artery and review the relevant literature.

고위의 전교통동맥 동맥류에서의 Pterional Approach와 Interhemispheric Approach의 수술적 비교 분석 (Surgical Comparison of Pterional Approach with Interhemispheric Approach for High Positioned Anterior Communicating Artery Aneurysms)

  • Objective : The purpose of this study is to evaluate and compare surgical results of pterional(fronto-temporal) approach and interhemispheric approach for the high positioned anterior communicating artery aneurysm with our surgical experience. Methods : During the period between May 1990 and May 2001, 263 anterior communicating aneurysms were treated at the department of neurosurgery of Dong-A university hospital. Among them, 175 patients were operated by same operator. Thirty six out of the 175 cases were high positioned anterior communicating artery aneurysms located more than 15mm above the anterior clinoid process. Results : Pterional approaches were applied in 32 cases and interhemispheric approaches were applied in 4 cases of total 36 cases of the high positioned anterior communicating artery aneurysms. The 32 cases of pterional approach resulted in Good Recovery 20/32(63%), Moderate Disability 6/32(19%), Severe Disability 4/32(12%) and Dead 2/32(6%), and 4 cases of interhemispheric approach resulted in GR 3/4(75%) and MD 1/4(25%). Relatively, pterional approach showed poorer result on high positioned anterior communicating artery aneurysm located more than 19mm above the anterior clinoid process with GR 5/13(39%), MD 3/13(23%), SD 3/13(23%) and Dead 2/13(15%). Conclusion : Interhemispheric approach is preferable to pterional approach for certain cases of high positioned anterior communicating artery aneurysm located more than 19 mm above the anterior clinoid process because it provides adequate orientation to the regional anatomy, less retraction of frontal lobe and preservation of the olfactory tract and gyrus rectus without any surgical complications.

Isolated, Contralateral Trochlear Nerve Palsy Associated with a Ruptured Right Posterior Communicating Artery Aneurysm

  • Trochlear nerve palsy associated with spontaneous subarachnoid hemorrhage (SAH) is known to be a rare malady. We report here on a patient who suffered with left trochlear nerve palsy following rupture of a right posterior communicating artery aneurysm. A 56-year-woman visited our emergency department with stuporous mental change. Her Hunt-and-Hess grade was 3 and the Fisher grade was 4. Cerebral angiography revealed a ruptured aneurysm of the right posterior communicating artery. The aneurysm was clipped via a right pterional approach on the day of admission. The patient complained of diplopia when she gazed to the left side, and the ophthalmologist found limited left inferolateral side gazing due to left superior oblique muscle palsy on day 3. Elevated intracranial pressure, intraventricular hemorrhage or a dense clot in the basal cisterns might have caused this trochlear nerve palsy.

  • Communicating bronchopulmonary foregut malformation (CBPFM)은 매우 드문 선천성 기형으로 격리된 호흡계 조직과 식도 또는 위장 사이의 누관이 특징적이다. 원시위장관에서 기원한 부속의 폐조직이 모든 종류의 bronchopulmonary furegut malformation 발생의 중요한 요인이 된다. 반복되는 폐렴이 있는 환자의 chest X-ray에서 낭성 병변이 보인다면 CBPFM을 생각해봐야 하며, 진단적 방법으로 식도조영술, 기관조영술, 컴퓨터 단층촬영, 자기공명영상 그리고 혈관조영술 등이 도움이 된다. 치료는 수술적 제거가 권장되며 예후도 좋다. 좌측 완전 심낭결손증과 동반되었으며, 이중식도에 연결된 외엽성 폐격리증과 기관지성 낭종을 보인 CBPFM 1예를 보고한다.

  • More than one cerebral aneurysm on the same artery is uncommon. Furthermore multiple aneurysms on the anterior communicating artery(ACoA) is very rare. We surgically experienced one case of two separate aneurysms on the ACoA ; one developed at the junction of the ACoA with the right second branch of anterior cerebral artery, and the other on inferior wall of the ACoA itself. The former, confirmed as ruptured on operation, projected superiorly and the latter directed inferiorly toward the prechiasmatic cistern. Radiographic and operative findings are presented with review of literature.

기관지 식도루를 동반한 내엽형 폐격리증 -1예 보고- (Intralobar pulmonary sequestration communicating with the esophagus -1 cases report-)

  • We have experienced a case of intralobar pulmonary sequestration communicating with the esophagus. A 24 year old female patient was admitted of chronic cough related to food, purrlent sputum and left lower chest pain. Esophagography and bronchography revealed bronchoesophageal fistula and bronchiectasis with cystic lesion of the left lower lobe. There was multiple anomalous feeding vessels arinig from the intercostal arteries and no draining systemic vein on aortography. division of the bronchoesophageal fistula and left lower lobectomy was performed. Communication with the esophagus in rarely associated with intralobar pulmonary sequestration and esophagogram is useful method of diagnosis for this communication.

  • This study researches the effects of oriental treatments for brain lesions and communicating hydrocephalus of patients with mental retardations. We treated the patients with acupuncture and herbal medication, and then followed up the results 3 times. After treatment, K-MBI is significantly increased but the MMSE-K is not. Although results indicate its significance to brain lesions, the oriental treatment is insufficient to cure mental retardation.

Long-Term Outcomes of Placement of a Single Transverse Stent through the Anterior Communicating Artery via the Nondominant A1 in Coil Embolization of Wide-Necked Anterior Communicating Artery Aneurysms

  • Objective : Placement of a single transverse stent via the nondominant A1 across the anterior communicating artery (AComA) into the contralateral A2 can provide sufficient neck coverage for wide-necked bifurcation AComA aneurysms. The authors described the feasibility, safety and long-term outcomes of this technique. Methods : Between January 2015 and February 2018, placement of a single transverse stent via the nondominant A1 was attempted in 17 wide-necked bifurcation AComA aneurysms. The authors reviewed the medical records and radiological studies. Results : The technical success rate was 94.1% (16/17). Periprocedural thromboembolic complications occurred in one patient (6.3%) without permanent neurological deficits. The mean clinical follow-up duration was 39.9±9.8 months. No deaths or delayed thromboembolic complications occurred. The mean angiographic follow-up duration was 38.9±9.8 months. The immediate and final follow-up complete occlusion rates were 87.4 and 93.7%, respectively. There was no recanalization during the follow-up period. Conclusion : Placement of a single transverse stent via the nondominant A1 across the AComA into the contralateral A2 is a feasible and relatively safe endovascular technique for the treatment of wide-necked bifurcation AComA aneurysms, with good long-term occlusion rates and a reasonable complication rate, if only the nondominant A1 is applicable.

An Aneurysm Developing on the Infundibulum of Posterior Communicating Artery : Case Report and Literature Review

  • Infundibular dilatation is funnel-shaped symmetrical enlargement that occurs at the origin of cerebral arteries and which is apparent on 7 to 25% of normal angiograms. Infundibular dilatation is frequently considered a normal anatomic variation of no pathologic significance. The authors report a case in which an aneurysm developed on an infundibular dilatation of the posterior communicating artery [PComA]. A 72-year-old woman presented with severe headache, nausea, and vomiting. Digital subtraction angiography showed a saccular aneurysm arising from the origin of the left PComA. Operative findings revealed the aneurysm and infundibular widening of the right PComA. The aneurysm was successfully obliterated. Whether infundibular dilatation is a pre-aneurysmal state or a benign dilatation is controversial. However, we believe infundibular dilatation of the PComA in this case may have served as a pre-aneurysmal lesion.
