Incomplete glottic closure of vocal cord atrophy is the common cause of dysphonia. Patients with vocal cord atrophy have complaints such as dysphonia, vocal fatigue, abnormal sensation in the throat, laryngeal pain, cough or sputum like functional voice disorders. Many investigators could not confirm the pathologic laryngeal structure because of their minute pathology. But recent advancements of laryngeal examinations made the many clinicians to detect minimal laryngeal pathology and to have mind the treatment for the vocal cord atrophy. But the results were less effective than their thoughts, the reasons of ineffectiveness were not known well. Authors have found the Hyperfunctional movement of the supraglottis during phonation before and after thyroplasty type I for vocal cord atrophy. Then we have applied the combined modality treatment with thyroplaty type I and voice therapy for relieve of hypefunctional movement of the supraglottis. These options have had more imporved results.
In this study, we evaluated the muscle tension in hemiplegia patients using a real-time monitoring system combined with high-intensity laser treatment (HILT) device. Five hemiplegia patients were treated with HILT in the left forearm muscle for 8 minutes, and the muscle was relaxed for 8 minutes following the treatment. Both the gradient of the force-displacement curves and the muscle hardness decreased during the therapy, and the effects were maintained following the treatment. The results show that muscle tension decreased rapidly during laser irradiation, and the efficacy of the treatment depended on the duration of the illness. Consequently, we conclude that this combined HILT and real-time muscle tension monitoring system is useful for evaluating the therapeutic procedure and for treatment of muscle pain.
Objectives : To evaluate the evidence supporting the effectiveness of Chuna manual therapy for functional dyspepsia. Methods : We conducted search across the 3 electronic databases (Pubmed, CAJ and Oasis) to find all of randomized controlled clinical trials(RCTs) that used Chuna manual therapy as a treatment for functional dyspepsia. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : Thirteen RCTs met our inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy combined with medication treatments in terms of the efficacy rate when compared to medication treatments alone. Positive results were also obtained, in terms of the efficacy rate, when comparing Chuna manual therapy combined with medication plus specific electromagnetic therapy to medication treatments alone. Conclusions : Our systematic review found encouraging but limited evidence of Chuna manual therapy for functional dyspepsia. However, to obtain stronger evidence without the drawbacks of trial design and the quality of studies, we recommend sham-controlled RCTs or comparative effectiveness research to test the effectiveness of Chuna manual therapy.

Results : 1. As a result of evaluation by using visual analog scale(VAS) and ankle-hindfoot scale(AHS), treatment score at final was marked more higher than score before treatment on each groups. 2. treatment at final, acupuncture and manipulation therapy group had significant result on visual analog scale(VAS) and ankle-hindfoot scale(AHS) compared with acupuncture therapy group. Conclusions : Manipulation therapy can be used with acupuncture therapy for highly effective treatment for ankle sprain with ankle pain.

Conclusions The review found encouraging but limited evidence of Chuna manual therapy for primary insomnia. We recommend clinical trials which compare the effectiveness of Chuna manual therapy with usual care to obtain stronger evidence without the demerits of trial design.

In fact. to increase the strength of perineal muscles, voluntary muscle contraction is more effective than electrical contraction. Electrical stimulation or bio feedback therapy is safe and effective therapy for Patients who have complex urinary incontinence. because these therapies can solve the problems of the voluntary perineal muscle contraction these therapies can help women to know to contract the perineal muscles effectively. The combined therapy ie. Electrical stimulation and bio feedback therapy with pelvic muscle training program or bladder drill can be considered as good treatment method. Pelvic floor muscle exercise is importance to make patient itself participate by making to be interested about exercise and by tacking motivations at therapy to themselves.
목 적: 70세 이상 고령의 비소세포성폐암 환자에서 단독방사선치료 및 화학방사선병행요법에 대한 독성과 생존율을 비교하였다. 대상 및 방법: 1998년에서 2002년까지 만 70세 이상의 III기 비소세포성폐암으로 방사선치료를 시행한 57명에 대해 후향적 분석을 시행하였다. 중앙추적조사기간은 9개월($1{\sim}53.4$개월)이었다. 단독방사선치료(갑군)를 시행한 환자가 33명, 동시화학방사선치료(을군)를 시행한 환자가 16명, 화학요법 후 방사선치료(병군)를 시행한 환자가 8명이었다. 남자, 여자가 각각 51, 6명, 중앙연령은 74 ($70{\sim}85$)세였다. IIIa와 IIIb기가 각각 23, 34명이었다. 갑군과 을군의 비교 시 종양의 크기가 갑군이 더 큰 것 외에는 의미 있는 분포의 차이는 없었다. 방사선치료의 분할선량은 동시화학방사선치료 시에는 1.8 Gy로, 그 외의 경우는 방사선치료범위를 고려하여 $1.8{\sim}3\;Gy$로 하였고 분할선량에 따라 $51{\sim}63\;Gy$를 조사하고자 하였다. 계획된 방사선량을 모두 조사받은 경우를 완전종결로 하였고 그렇지 못한 경우를 불완전종결로 하였다. 결 과: 57명 중 52명의 환자가 추적조사기간 중 사망하였다. 갑, 을, 병군에서 완전종결 환자는 28, 10, 7명이었고 중앙방사선치료기간은 각각 35, 60.5, 35일이었다. 전체 환자의 중앙생존기간은 10.1개월이었고 1년, 2년 생존율은 39.8, 17.6%였다. 갑, 을, 병군의 중앙생존기간은 각각 8.9, 8.2, 11.7개월이었고 1년 생존율은 38.4, 37.5, 50%였다. 방사선치료가 불완전 종결된 갑, 을, 병군은 각각 5, 6, 1명으로 N병기(N3)(p=0.081), 치료방법의 차이(p=0.079)가 불완전 종결에 영향을 줄 가능성이 높았다. 을군에서 병용 화학요법제를 시행한 8명 중 4명은 부작용으로 치료를 중단하였다. 예후인자의 평가에서 T병기(T3 이상), 종양의 크기(5 cm 이상), KPS (70 이하), 불완전 종결이 단일변량분석과 다변량분석에서 유의하였다. 결 론: 고령의 비소세포성폐암 환자에서 단독방사선치료는 타 치료와 비교 시 대등한 생존기간을 나타냈으며 동시 화학방사선치료에 비해 부작용으로 치료를 중단하는 경우가 적어 대체로 고령의 환자가 치료에 잘 견디는 치료였다. 고령의 환자에서 동시화학방사선치료를 고려할 시에는 치료의 견딤 정도를 고려하여 환자의 선택 및 약제의 선택에 주의하여야 할 것이다.

Purpose: This study aimed to demonstrate reduction in stroke symptoms by analyzing the changes in respiratory function and activities of daily living (ADL) after respiratory muscle strengthening exercise in patients who had a stroke and thereby, propose an efficient exercise method. Methods: Twenty patients with hemiplegic stroke were divided into two groups, with 10 patients in each. The control group (CG) received the traditional exercise therapy, and the experimental group (EG) received the traditional exercise therapy combined with expiratory muscle strengthening training. The training continued for 6 weeks, 5 days a week. Forced vital capacity (FVC) and forced expiratory volume at 1 second (FEV1) were measured with a spirometer, $SpO_2$ was measured with a pulse oximeter, and ADL were assessed by using the modified Barthel index (MBI). A paired t test was applied to compare the differences before and after the intervention, and an independent t test was used to compare the differences between the groups. The level of statistical significance was set as ${\alpha}=0.05$. Results: The changes in the FVC and FEV1 values within the group showed significant differences only in the EG (p<0.01). The between-group difference was statistically significant only for FVC and FEV1 in the EG (p<0.01). The changes in $SpO_2$ and MBI within the group showed significant differences only in the EG and CG (p<0.01). Between-group differences were statistically significant only for $SpO_2$ and MBI in the EG (p<0.05). Conclusion: The interventions with active patient involvement and combined breathing exercises had a positive impact on all the functions investigated in this study.
