Research on Quality Components for Service Design of Health Screening : Focus on IT Services

  • This research proposes how to enhance low customer satisfaction with health screening services caused by procedural complexity and limits of health screening. The purpose of this study is to identify sub-components of the service quality provided by general health examination centers. This is a qualitative analysis of in-depth interviews of providers and consumers of medical services. The data were primarily analyzed by affinity diagram, and the data were sorted and analyzed according to the criteria suggested by Donabedian's four components. Four types of quality factors and the health screening service quality components of 39 subordinate items were assessed. Components related to the use of IT facilities comprise a significant amount of the physical factors, and there are high demands for IT facilities among customers.

민간병의원-공공기관 협력을 통한 지역사회 치매환자 관리사업 (The Development of Community Health Service for the Demented Elderly in Cooperation Between Private Medical Facilities and Public Health Centers)

  • Purpose: This research was carried out to present an ideal cooperative model between the public and private health sectors for the management of community dementia patients. Method: In this study a public-private cooperative council was formed, basic data for dementia patients and their families were collected, and a dementia service program was carried out in cooperation between the two sectors. Results: The survey data shows the majority of the registered patients were undergoing a chronic diseases which would make the dementia health service inefficient. The cooperative public-private council adopted the reinforcement of medical service to the public enrolled dementia patients. The intensive medical service program showed effects on the health status of the dementia patients. Conclusion: The results of this study pointed out that change of the health insurance program supportive to the private sectors to be made; a referral system for the public health sector to the private sector should be established; and expanding the capacity of the visiting health program in the public health sector is needed.

한국 저소득층 주민의 보건의료서비스 이용행태와 그 영향요인에 관한 연구 (A Study on Health Service Utilization and it's Determinants in the Low Income Family in Korea)

  • Objectives: The purpose of this study was to analyze health service utilization, and its related factors in low income families who earned half of the average Korean household income. Methods: This was a cross-sectional descriptive survey study in which a nationwide randomization sampling technique was used. The data were collected from July 12 to August 7, 1999, and the total sample size was 5,819 individuals, belonging to 1.753 households. Results: 1) In the utilization of health services for the last 3months, the pharmacy was the type of service that was the most utilized (32.0%), and the health center was the one that was the least utilized (10.3%). About 29% (29.2%) of the respondents could not utilize the health service at all. and 19.8% of the respondents terminated their medical treatments half way to completion because of financial difficulty (89.4%). 2) Analysis of the data using logistic regression showed that living with spouse, level of education, occupation, and income had statistically significant effects on health service utilization. Conclusion: The parameters of health care policies are equity and efficacy for health status, and the health service utilization by low income families. The conclusive resolution for these is the improvement of public health centers for an increased utilization rate of their services.

우리나라 대학(大學)에 있어서의 학생보건관리(學生保健管理) 및 의료보험제도(醫療保險制度)에 관(關)한 실태(實態) 조사(調査) 연구(硏究) (A Study on Health Administration Status and Medicare Insurance Program in Universities and Colleges in Korea)

  • The status of health administration and medicare insurance program of 58 universities and colleges of 4-year course was studyed in 1971 and the following results were obtained; 1. The average number of students of 30 universities was $4,800{\pm}2,600$ and that of 36 colleges was $780{\pm}620$. 2. The types of health service facilities for the students varied widely according to the institutions, from an elaborate one, university health center, to a poor one, first aid room. 3. Thirty-six out of 58 institutions had some sort of health service facilities, either health center or health service room. And 14 out of 36 institutions had elaborate health service facilities such as university health center or student health center 4. The number of full time staff of the health center and that of the health service room were 2 to more than 10 and 0 to 3 respectively. 5. The range of student health service fee varied widely according to the institutions from \50 to \550 per student, per semester. The average cost of student health service fee at the institutions with health centers was $\300{\pm}150$ and that with health service rooms was $\200{\pm}150$ per student, per semester. 6. Utility rate of the student health service facilities at the institutions with health centers and with health service rooms were 1,200 to 1,400 and 3,900 to 4,100 per 1,000 students per year. 7. There was an obvious increasing tendency of tuberculosis prevalence rate in the students. 8. The institutions which had appointed hospitals for student medicare were 24; where the reduction rate of medical expenses for students varied from 10 to 50 per cent. 9. Students medicare insurance program was adopted by six universities which accomodated more than 2,000 students. 10. The range of student medicare insurance fee varied widely according to the institutions from \140 to \800 per student per year. Each of the six universities which had adopted the insurance program had each own's special regulations to apply for pay claims.

서울시 보건소 맞춤형 방문건강관리 간호사의 직무스트레스 관련 요인 (Factors Related to the Job Stress of Home Health Care Nurses at Health Centers in Seoul Korea)

  • Purpose: To identify factors that affect the job stress of visiting nurses at health centers in Seoul, Korea, who perform home care services. Methods: Data were collected in the period from November 20 to December 10, 2008, from 206 visiting nurses working at 25 health centers in Seoul metropolitan city. Self-report questionnaires of the Organization-Based Self-Esteem (OBSE) and the Korean Occupational Stress Scale (KOSS) were used. Data were analyzed by the SPSS win 15.0. Results: Several findings were obtained from the study. First, the mean of job stress was 52.4 points, which was higher than that of Korean women. Second, it was shown that by the sub-domain of job stress, the mean value belonged to the top 50% in the four sub-domains of physical environment, job demand, interpersonal conflict, and organizational system. Additionally, it belonged to the top 25% in the sub-domain of job insecurity, wherein the job stress was the highest. Third, the self-esteem of the nurses had the most significant effect on their job stress. Conclusion: Stress related to job insecurity was the highest among the visiting nurses at health centers in Seoul who perform the custom home care service thus, institutional support is urgently needed to alleviate such stress and secure their employment.

영아전담보육시설 보육교사의 교육요구 (Educational Demands among Teachers at Infant Daycare Centers)

  • Purpose: This study was performed to investigate educational demands of teachers in the daycare centers for infant healthcare. Methods: Sixty-nine teachers who in charge of children under 1 year old at infant daycare centers participated from January 2 to March 31, 2012. The data were analyzed using frequency, percentage, average, and standard deviation using SPSS 21.0. Results: Education demands among teachers of infant daycare centers were high in health and safety domain. Teachers prefer 6 month as the term of education, at the beginning of the semester. Also, three hours or less of session under the auspices of health and safety organizations were preferred. Mostly demanded education contents were first-aid skills depending on diverse types of accidents. In addition, teachers required repeat of practically available first-aid education programs with improved educational conditions. Conclusion: This study offered crucial examination of current status and key problems of infant healthcare at infant daycare centers in Seoul and Gyeonggi areas, and identified educational demands for infant healthcare among infant teachers.

여성의 건강을 위한 간호전달체계 모형개발 - 조산원 중심으로 - (Model Development of Nursing Care System for Women's Health : Based on Nurse-Midwifery Clinic)

  • The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.

The Assessment of Patients Satisfaction with Healthcare Services Provided in the Hawtat Bani Tamim During the COVID-19 Pandemic

  • This study aims to evaluate patients' satisfaction with healthcare services in Hawtat Bani Tamim Governorate during the spread of coronavirus. The importance of this study stems from the fact that Covid-19 is challenging countries all over the world. The study adopted a questionnaire for collecting the data. The sample size reached 231 members. The study applied descriptive statistics and inferential statistical methods to obtain the result of the study. The analysis of the data revealed that most of the participants are satisfied with the healthcare services in hospitals and health centers during the Corona pandemic, Health precautions and distancing measures were applied appropriately, Vaccination against corona was available in health centers and easy to access, and rapid response of health care service providers to patients' requests. The participant's opinions showed that the application of Mawid is a suitable method to access healthcare services during Corona, the application is more efficient in providing the required healthcare services and facilitating their access to healthcare services. In addition, patient satisfaction with the "My health" application provided fast and convenient services during the Corona pandemic and provided medical consultations easily.

Public Health Center Service Experiences and Needs among Immigrant Women in South Korea

  • Purpose: To support implementation of comprehensive, person-centered healthcare, this study aimed to explore immigrant women's public health center (PHC) service experiences and needs while considering Photovoice's feasibility for this purpose. Methods: This qualitative study included 15 marriage-based immigrant women. Participants were recruited from churches and multicultural family support centers using purposive and snowball sampling. Data were collected through four focus group interviews and were subjected to inductive content analysis. Results: Five categories of experiences were identified: language barriers, hectic environment, affordable and practical primary healthcare, feeling ignored and discriminated against, and feeling frustrated. In addition, five categories of needs were identified: language assistance services, ease of access, healthcare across the lifespan, expansion of affordable healthcare, and being accepted as they are. This study provides preliminary evidence that the Photovoice approach can facilitate the interview process in a qualitative inquiry involving participants with limited ability to express their perspectives in the researchers' language. Conclusion: Study findings highlight the need to implement institutional policy and procedural changes within PHCs and to provide culturally competent, personcentered care for South Korea's marriage-based immigrant women and other ethnic minority populations. The findings also provide evidence-based direction for PHC service planning.

Assessment of Community Capacity Building Ability of Health Promotion Workers in Public Health Centers

  • Objectives : This study was performed to assess the community capacity building ability of health promotion workers of public health centers and to identify influential factors to the ability. Methods : The subjects were 43 public officers from 16 public health centers in Busan Metropolitan City. Questionnaire was developed based on 'Community Capacity Building Tool' of Public Health Agency of Canada which consists of 9 feathers. Each feather of capacity was assessed in 4 point rating scale. Univariate analysis by characteristics of subjects and multivariate analysis by multiple regression was done. Results : The mean score of the 9 features were 2.35. Among the 9 feathers, 'Obtaining resources' scored 3.0 point which was the highest but Community structure scored 2.1 which was the lowest. The mean score of the feathers was relatively lower than that of Canadian data. The significant influential factors affecting community capacity building ability were 'Service length', 'Heath promotion skill level', 'Existence of an executive department', and 'Cooperative partnership for health promotion'. According to the result of multiple linear regression, the 'Existence of an executive department' had significant influence. Conclusions : Community capacity building ability of subjects showed relatively lower scores in general. Building and activating an executive department and cooperative partnerships for health promotion may be helpful to achieve community capacity building ability.
