Status of Nosocomial Urinary Tract Infections in the ICU: Molecular Epidemiology of Imipenem Resistant P. aeruginosa (중환자실내 병원성 요로감염 실태와 전파경로: Imipenem Resistant P. aeruginosa[IRPA]의 분자역학적 특성을 중심으로)

  • Purpose: This retrospective study was done to evaluate the status of nosocomial urinary tract infections and to determine the risk factors and transmission route of causal IRPA through molecular epidemiology. Method: Two hundred ninety-nine of 423 patients admitted to the internal medicine and surgery ICU at a university hospital incity B had a positiveurine culture. Twelve of the 299 patients who had a urinary tract infection had IRPA strains. The data was collected from November 1, 2004 to January 31, 2005. The following results were obtained after the data was analyzed using percentile and UPGMA. Result: The rate of nosocomial urinary tract infections in the ICU was 10.8%. Therewere 16.8 cases of infection based on the period of hospitalization. There were 16.9 cases of infection based on the use of a foley catheter. The rate of nosocomial urinary tract infection in the ICU and urinary tract infections related to IRPA were higher in patients with the following characteristics: men, old age, admission through the emergency room, longer than seven days admission, severity of admitting causes, disturbance of consciousness, hydration less than 300cc in 24hours, a long course of antibiotics, a long period of foley catheterization and perineal care. Most of the microorganisms that caused the urinary tract infection were gram negative bacilli, among which P. aeruginosa was found in 70 patients (18.5%) and IRPA in 12 (4.0%). Among the 12 IRPA strains that were tested with PFGE, eight showed a dice coefficient higher than 80%, suggesting a genetic relationship. They were related with the period of hospitalization in the same ICU. These patients all received direct care for a urinary tract infection. Conclusion: Through these results, IRPA can be consideredas a contributing factors to urinary tract infections thus, active preventative measures are needed by the medical staff.

Non-destructive quality prediction of truss tomatoes using hyperspectral reflectance imagery (초분광 영상을 이용한 송이토마토의 비파괴 품질 예측)

  • Spectroscopic measurement method based on visible and near-infrared wavelengths was prominent technology for rapid and non-destructive evaluation of internal quality of fruits. Reflectance measurement was performed to evaluate firmness, soluble solid content, and acid content of truss tomatoes by hyperspectral reflectance imaging system. The Vis/NIR reflectance spectra was acquired from truss tomatoes sorted by 6 ripening stages. The multivariable analysis based on partial least square (PLS) was used to develop regression models with several preporcessing methods, such as smoothing, normalization, multiplicative scatter correction (MSC), and standard normal variate (SNV). The best model was selected in terms of coefficient of determination of calibration ($R_c^2$) and full cross validation ($R_{cv}^2$), and root mean standard error of calibration (RMSEC) and full cross validation (RMSECV). The results of selected models were 0.8976 ($R_p^2$), 6.0207 kgf (RMSEP) with gaussian filter of smoothing, 0.8379 ($R_p^2$), $0.2674^{\circ}Bx$ (RMSEP) with the mean of normalization, and 0.7779 ($R_p^2$), 0.1033% (RMSEP) with median filter of smoothing for firmness, soluble solid content (SSC), and acid content, respectively. Results show that Vis / NIR hyperspectral reflectance imaging technique has good potential for the measurement of internal quality of truss tomato.

  • Recently, environmental problems have become an area of growing interests. In-situ monitoring of water quality is fundamental to most environmental applications. The accurate measurement of nitrate concentrations is fundamental to understanding biogeochemistry in aquatic ecosystems. Several studies have reported that one of the most feasible methods to measure nitrate concentration is the use of Ion Selective-electrodes (ISEs). The ISE application to water monitoring has several advantages, such as direct measurement methodology, high sensitivity, wide measurement range, low cost, and portability. However, the ISE methods may yield inconsistent results where there was a difference in temperature between the calibration and measurement solutions, which is associated with the temperature dependence of ionic activity coefficients in solution. In this study, to investigate the potential of using the combination of a temperature sensor and nitrate ISEs for minimizing the effect of temperature on real-time nitrate sensing in natural water, a prototype of on-site water monitoring system was built, mainly consisting of a sensor chamber, an array of 3 ISEs, an waterproof temperature sensor, an automatic sampling system, and an arduino MCU board. The analog signals of ISEs were obtained using the second-order Sallen-key filter for performing voltage following, differential amplification, and low pass filtering. The performance test of the developed water nitrate sensing system was conducted in a monitoring station of drinking water located in Jeongseon, Kangwon. A temperature compensation method based on two-point normalization was proposed, which incorporated the determination of temperature coefficient values using regression equations relating solution temperature and electrode signal determined in our previous studies.

Development and Preliminary Validation of Mibyeong Questionnaire(MQ) Based on Deficiency-Stagnation pattern (허울(虛鬱) 기반 미병 평가도구 개발 및 예비타당성 검증)

  • Objective : This study aimed to develop and preliminary validate the Mibyeong Questionnaire (MQ) based on Deficiency-Stagnation pattern, which was report of a person's health condition that comes directly from the person. Method : The first phage of developing a MQ was to generate and exhaustive list of all MQ issue that are relevant to the domain of interest, using literature research and expert group discussions. Through those steps, we established MQ with 33 items divided into two parts: 21-item deficiency pattern and 12-item stagnation pattern. The second phage, we examined the preliminary tests of reliability and validity including the 16-item Deficiency of MQ (16D-MQ), with data (n=1,890) already collected on the Korean medicine data center in KIOM. Results : Exploratory factor analysis revealed three factors of the 16D-MQ. These factors were fatigue(Qi, 氣); psychic and physical elements(Shen, 神); and skin and hair(Jing, 精). Cronbach's coefficient alpha was 0.876 and the intraclass correlation coefficients was 0.368-0.538. In support of criteria validity, the 16D-MQ was weakly correlated with EQ-5D and physicians's opinion, but it was acceptable. Conclusion : The MQ shows that it has an appropriate level of internal consistency and validity. We think further study to reveal its reliability and validity, including stagnation pattern as well as deficiency pattern, is needed.

Remaining Fatigue Life Evaluation of Steel Railroad Bridge (강철도교의 잔존피로수명 평가)

  • A systematic procedure to evaluate fatigue damages and to predict remaining fatigue lives is introduced for a steel railway bridge. Fatigue damages are evaluated by using the currently available fatigue damage theory. Fatigue lives with the condition of fatigue crack initiation are estimated by the probabilistic approach based on the reliability theory as well as the simplified procedure. A equivalent deterministic procedure is also suggested to assess the remaining fatigue life under various traffic conditions. Numerical simulations are used to assess dynamic stress histories with correction factors. Loading models are obtained from the passenger volume data. Train coincidences are also considered. Based on the results, the fatigue life is found to be underestimated by without considering the coincidence of trains on the bridge. The simplified method proposed in this study are found to yield approximately the same results as the systematic procedure.

Deep Learning-based Prediction of PM10 Fluctuation from Gwanak-gu Urban Area, Seoul, Korea (서울 관악구 도심지역 미세먼지(PM10) 관측 값을 활용한 딥러닝 기반의 농도변동 예측)

  • Since fine dust (PM10) has a significant influence on soil and groundwater composition during dry and wet deposition processes, it is of a vital importance to understand the fate and transport of aerosol in geological environments. Fine dust is formed after the chemical reaction of several precursors, typically observed in short intervals within a few hours. In this study, deep learning approach was applied to predict the fate of fine dust in an urban area. Deep learning training was performed by combining convolutional neural network (CNN) and recurrent neural network (RNN) techniques. The PM10 concentration after 1 hour was predicted based on three-hour data by setting SO2, CO, O3, NO2, and PM10 as training data. The obtained coefficient of determination value, R2, was 0.8973 between predicted and measured values for the entire concentration range of PM10, suggesting deep learning method can be developed into a reliable and viable tool for prediction of fine dust concentration.

Grain Boundary Trap Levels in ZnO-based Varistor (ZnO계 바리스터의 입계포획준위)

  • The trap levels of ZnO-based varistor are obtained by Isothermal Capacitance Transient Spectroscopy method. Here ICTS measuring system consists of YHP 4192A Impedance Analyzer and a personal computer for the data acquisition. Between $-40^{\circ}C$ and $60^{\circ}C$, the grain boundary trap levels of 0.48 and 0.94eV were detected for $ZnO-Bi_2O_3-MnO$ system. The hole omission spectra are observed in the case of the addition of CoO into the $ZnO-Bi_2O_3$ system, while the electron emission spectra are detected in the case of the addition of MnO. The nonlinear resistance coefficient $\alpha$ increases with the decrease of the dormer concentration. Finally, the trap level density of $ZnO-Bi_2O_3-MnO$ system is found to decrease with the amount of CoO, while $\alpha$ is found to increase with the amount of CoO.

Factors Influencing Quality of Life in Type II Diabetes Mellitus Patients Registered at Public Health Center (일개 시 보건소에 등록한 제2형 당뇨병 대상자의 삶의 질 영향요인)

  • Purpose: The purpose of this study was to examine the relationships among self-care, self-care agency, self-efficacy, and quality of life in type II diabetic patients registered at a public health center. Method: The study subjects were 128 type II diabetic patients who were living in G city. The data were collected from March 2001 to February 2002. The instruments used for this study were the self-care scale developed by Jeung(1997) and designed by Park (1984) based on the original scale, the self-care agency scale developed by So (1992), the self-efficacy scale developed by a Jeung (1997) and designed by Paek (1996) based on the original scale, and the quality of life scale developed by Ro (1988). The data were analyzed using descriptive statistics, pearson correlation coefficient, and stepwise multiple regression. Results: 1. The relationships among self-care, self-care agency, self-efficacy, and quality of life were significant. Self-care was significantly related to self-care agency (r=.609. p<.01), self-efficacy (r=.763. p<.01), and quality of life (r=.493. p<.01). 2. The stepwise multiple regression analysis was performed to identify factors influencing quality of life of the subjects. The most powerful predictor was self-care agency (48.4%). The combination of self-care, complication status, age, education level, and self-efficacy accounted for 88.7% of the variance of quality of life in type II diabetic patients. Conclusion: The results suggest that self-care, self-care agency, self-efficacy, and quality of life are important variables for development of nursing intervention programs for patients with diabetes.

Effects of Relative Humidity and Temperature on the Transport of Chloride in the Concrete

  • To investigate the role of RH and temperature on the transport of chloride in the concrete, two groups of specimens were configured. For both groups, mix design was based on w/c=0.45, $400kg/m^3$ cement, $794kg/m^3$ fine aggregate and $858kg/m^3$ coarse aggregate. After specimen fabrication these were exposed to four different RH (35, 55, 75 and $95\%$ RH) and temperature (0, 20, 30 and $40^{\circ}C$) conditions. After 3 and 6 months $15\%$ NaCl exposure 5mm cores were taken. These cores were sliced and individual cores were ground to powder. In addition, to evaluate the effect of temperature on the chloride binding some powder samples were leached in the each of four temperature chambers. Chloride titration fur these was performed using FDOT acid titration method. Based upon the resultant data conclusions were reached regarding that 1) effective diffusion coefficient, $D_e$, increased with increasing exposure RH, suggesting that the size and number of water paths increased with elevated moisture content in the specimens, 2) $D_e$ increased with increasing temperature in the range of 0 to $40^{\circ}C$ possibly by elevated thermal activation of chloride ions and reduced chloride binding at higher temperature, 3) water soluble chloride concentration, $[Cl^-]_s$, increased with increasing temperature, and 4) chloride concentration profile for initially dry concrete specimens was higher than for the initially wet ones indicating pronounced capillary suction (sorption) occurred for the dry concrete specimens.

Design of Robust Face Recognition System Realized with the Aid of Automatic Pose Estimation-based Classification and Preprocessing Networks Structure

  • In this study, we propose a robust face recognition system to pose variations based on automatic pose estimation. Radial basis function neural network is applied as one of the functional components of the overall face recognition system. The proposed system consists of preprocessing and recognition modules to provide a solution to pose variation and high-dimensional pattern recognition problems. In the preprocessing part, principal component analysis (PCA) and 2-dimensional 2-directional PCA ($(2D)^2$ PCA) are applied. These functional modules are useful in reducing dimensionality of the feature space. The proposed RBFNNs architecture consists of three functional modules such as condition, conclusion and inference phase realized in terms of fuzzy "if-then" rules. In the condition phase of fuzzy rules, the input space is partitioned with the use of fuzzy clustering realized by the Fuzzy C-Means (FCM) algorithm. In conclusion phase of rules, the connections (weights) are realized through four types of polynomials such as constant, linear, quadratic and modified quadratic. The coefficients of the RBFNNs model are obtained by fuzzy inference method constituting the inference phase of fuzzy rules. The essential design parameters (such as the number of nodes, and fuzzification coefficient) of the networks are optimized with the aid of Particle Swarm Optimization (PSO). Experimental results completed on standard face database -Honda/UCSD, Cambridge Head pose, and IC&CI databases demonstrate the effectiveness and efficiency of face recognition system compared with other studies.
