A Stochastic Study for the Emergency Treatment of Carbon Monoxide Poisoning in Korea (일산화탄소중독(一酸化炭素中毒)의 진료대책(診療對策) 수립(樹立)을 위한 추계학적(推計學的) 연구(硏究))

  • Emergency medical service is an important part of the health care delivery system, and the optimal allocation of resources and their efficient utilization are essentially demanded. Since these conditions are the prerequisite to prompt treatment which, in turn, will be crucial for life saving and in reducing the undesirable sequelae of the event. This study, taking the hyperbaric chamber for carbon monoxide poisoning as an example, is to develop a stochastic approach for solving the problems of optimal allocation of such emergency medical facility in Korea. The hyperbaric chamber, in Korea, is used almost exclusively for the treatment of acute carbon monoxide poisoning, most of which occur at home, since the coal briquette is used as domestic fuel by 69.6 per cent of the Korean population. The annual incidence rate of the comatous and fatal carbon monoxide poisoning is estimated at 45.5 per 10,000 of coal briquette-using population. It offers a serious public health problem and occupies a large portion of the emergency outpatients, especially in the winter season. The requirement of hyperbaric chambers can be calculated by setting the level of the annual queueing rate, which is here defined as the proportion of the annual number of the queued patients among the annual number of the total patients. The rate is determined by the size of the coal briquette-using population which generate a certain number of carbon monoxide poisoning patients in terms of the annual incidence rate, and the number of hyperbaric chambers per hospital to which the patients are sent, assuming that there is no referral of the patients among hospitals. The queueing occurs due to the conflicting events of the 'arrival' of the patients and the 'service' of the hyperbaric chambers. Here, we can assume that the length of the service time of hyperbaric chambers is fixed at sixty minutes, and the service discipline is based on 'first come, first served'. The arrival pattern of the carbon monoxide poisoning is relatively unique, because it usually occurs while the people are in bed. Diurnal variation of the carbon monoxide poisoning can hardly be formulated mathematically, so empirical cumulative distribution of the probability of the hourly arrival of the patients was used for Monte Carlo simulation to calculate the probability of queueing by the number of the patients per day, for the cases of one, two or three hyperbaric chambers assumed to be available per hospital. Incidence of the carbon monoxide poisoning also has strong seasonal variation, because of the four distinctive seasons in Korea. So the number of the patients per day could not be assumed to be distributed according to the Poisson distribution. Testing the fitness of various distributions of rare event, it turned out to be that the daily distribution of the carbon monoxide poisoning fits well to the Polya-Eggenberger distribution. With this model, we could forecast the number of the poisonings per day by the size of the coal-briquette using population. By combining the probability of queueing by the number of patients per day, and the probability of the number of patients per day in a year, we can estimate the number of the queued patients and the number of the patients in a year by the number of hyperbaric chamber per hospital and by the size of coal briquette-using population. Setting 5 per cent as the annual queueing rate, the required number of hyperbaric chambers was calculated for each province and for the whole country, in the cases of 25, 50, 75 and 100 per cent of the treatment rate which stand for the rate of the patients treated by hyperbaric chamber among the patients who are to be treated. Findings of the study were as follows. 1. Probability of the number of patients per day follows Polya-Eggenberger distribution. $$P(X=\gamma)=\frac{\Pi\limits_{k=1}^\gamma[m+(K-1)\times10.86]}{\gamma!}\times11.86^{-{(\frac{m}{10.86}+\gamma)}}$$ when$${\gamma}=1,2,...,n$$$$P(X=0)=11.86^{-(m/10.86)}$$ when $${\gamma}=0$$ Hourly arrival pattern of the patients turned out to be bimodal, the large peak was observed in $7 : 00{\sim}8 : 00$ a.m., and the small peak in $11 : 00{\sim}12 : 00$ p.m. 2. In the cases of only one or two hyperbaric chambers installed per hospital, the annual queueing rate will be at the level of more than 5 per cent. Only in case of three chambers, however, the rate will reach 5 per cent when the average number of the patients per day is 0.481. 3. According to the results above, a hospital equipped with three hyperbaric chambers will be able to serve 166,485, 83,242, 55,495 and 41,620 of population, when the treatmet rate are 25, 50, 75 and 100 per cent. 4. The required number of hyperbaric chambers are estimated at 483, 963, 1,441 and 1,923 when the treatment rate are taken as 25, 50, 75 and 100 per cent. Therefore, the shortage are respectively turned out to be 312, 791. 1,270 and 1,752. The author believes that the methodology developed in this study will also be applicable to the problems of resource allocation for the other kinds of the emergency medical facilities.

Removal Characteristics of Sulfonamide Antibiotic Compounds in Biological Activated Carbon Process (생물활성탄 공정에서의 Sulfonamide계 항생물질 제거특성)

  • In this study, the effects of three different biological activated carbon (BAC) materials (each coal, coconut and wood based activated carbons), empty bed contact time (EBCT) and water temperature on the removal of sulfonamide 5 species in BAC filters were investigated. Experiments were conducted at three water temperatures (5, 15 and $25^{\circ}C$) and four EBCTs (5, 10, 15 and 20 min). The results indicated that coal based BAC retained more attached bacterial biomass on the surface of the activated carbon than the other BACs, increasing EBCT or increasing water temperature increased the sulfonamide 5 species removal in BAC columns. In the coal-based BAC columns, sulfachloropyridazine (SCP), sulfamethazine (SMT) and sulfathiazole (STZ) removal efficiencies were 30~80% and sulfadimethoxine (SDM), sulfamethoxazole (SMX) removal efficiencies were 18~70% for 5~20 min EBCT at $25^{\circ}C$. The kinetic analysis suggested a first-order reaction model for sulfonamide 5 species removal at various water temperatures (5~$25^{\circ}C$). The pseudo-first-order reaction rate constants and half-lives were also calculated for sulfonamide 5 species removal at 5~$25^{\circ}C$. The reaction rate and half-lives of sulfonamide 5 species ranging from 0.0094~0.0718 $min^{-1}$ and 9.7 to 73.7 min various water temperaturs and EBCTs in this study could be used to assist water utilities in designing and operating BAC filters for sulfonamide antibiotic compounds removal.

Experimental Study on the Agglomeration Characteristics of Coal and Silica Sand by addition of KOH (KOH 첨가에 의한 석탄 및 유동사의 응집특성에 대한 실험적 연구)

  • The agglomeration characteristics of coal and silica sand were investigated under various conditions using mixed samples consisting of coal, silica sand, and potassium hydroxide, which is an agglomeration accelerator. The samples were prepared by either physically mixing or using aqueous solutions. The experiments using the physically mixed powder samples were performed with a two hour reaction time. The results showed that the number of aggregates generated increased as the reaction temperature and the total potassium content increased. The experiments using aqueous solutions were performed at 880 ℃, which is the operating temperature of a fluidized bed boiler, and at 980 ℃, which assumes a local hot spot. The amount of agglomeration generated as the reaction time increased and the total potassium content increased was identified. In the experiment performed at 880 ℃, the amount of aggregate generated clearly increased with the reaction time, and in the experiment performed at 980 ℃, assuming a local hot spot, a large amount of aggregate was generated in a relatively short time. The aggregates became harder as the potassium content increased. When the total potassium content was less than 1.37 wt.%, the aggregates were weak at both temperatures and collapsed even with a slight impact. Additionally, the surface characteristics of the silica sand and ash aggregates were observed by SEM-EDS analysis. The analysis revealed a large amount of potassium at the bonding sites. This result indicates that there is a high possibility of aggregation in the form of a eutectic compound when the alkali component is increased.

  • Fluidized bed combustion is a coal combustion technology that can reduce both SOx and NOx emission; SOx is removed by limestone that is fed into the combustion chamber and the NOx is reduced by low temperature combustion in a fluidized bed combustor and air stepping, but $N_2O$ generation is quite high. $N_2O$ is not only a greenhouse gas but also an agent of ozone destruction in the stratosphere. The calcium oxide(CaO) is known to be a catalyst of $N_2O$ decomposition. This study of $N_2O$ decomposition reaction in fixed bed reactor packed over CaO bed has been conducted. Effects of parameters such as concentration of inlet $N_2O$ gas, reaction temperature, CaO bed height and effect of $CO_2$, NO, $O_2$ gas on the decomposition reaction have been investigated. As a result of the experiment, it has been shown that $N_2O$ decomposition reaction increased with the increasing fixed bed temperature. While conversion of the reaction was decreased with increasing $CO_2$ concentration. Also, under the present of NO, the conversion of $N_2O$ decomposition is decreased. From the result of kinetic study gained the heterogeneous reaction rate on $N_2O$ decomposition. In the case of $N_2O$ decomposition over CaO, heterogeneous reaction rate is. $\frac{d[N_2O]}{dt}=\frac{3.86{\times}10^9{\exp}(-15841/R)K_{N_2O}[N_2O]}{(1+K_{N_2O}[N_2O]+K_{CO_2}[CO_2])}$. In this study, it is found that the calcium oxide is a good catalyst of $N_2O$ decomposition.

Test Bed Studies with Highly Efficient Amine CO2 Solvent (KoSol-4) (고효율 습식 아민 CO2 흡수제(KoSol-4)를 적용한 Test bed 성능시험)

  • Test bed studies with highly efficient amine $CO_2$ solvent (KoSol-4) developed by KEPCO research institute were performed. For the first time in Korea, evaluation of post-combustion $CO_2$ capture technology to capture 2 ton $CO_2$/day from a slipstream of the flue gas from a coal-fired power station was performed. Also the analysis of solvent regeneration energy was conducted to suggest the reliable performance data of the KoSol-4 solvent. For this purpose, we have tested 5 campaigns changing the operating conditions of the solvent flow rate and the stripper pressure. The overall results of these campaigns showed that the $CO_2$ removal rate met the technical guideline ($CO_2$ removal rate: 90%) suggested by IEA-GHG and that the regeneration energy of the KoSol-4 showed about 3.0~3.2 GJ/$tCO_2$ which was, compared to that of the commercial solvent MEA (Monoethanolamine), about 25% reduction of regeneration energy. Based on these results, we could confirm the good performance of the KoSol-4 solvent and the $CO_2$ capture process developed by KEPCO research institute. And also it was expected that the cost of $CO_2$ avoided could be reduced drastically if the KoSol-4 is applied to the commercial scale $CO_2$ capture plant.

0.1 MW Test Bed CO2 Capture Studies with New Absorbent (KoSol-5) (신 흡수제(KoSol-5)를 적용한 0.1 MW급 Test Bed CO2 포집 성능시험)

  • The absorption efficiency of amine $CO_2$ absorbent (KoSol-5) developed by KEPCO research institute was evaluated using a 0.1 MW test bed. The performance of post-combustion technology to capture two tons of $CO_2$ per day from a slipstream of the flue gas from a 500 MW coal-fired power station was first confirmed in Korea. Also the analysis of the absorbent regeneration energy was conducted to suggest the reliable data for the KoSol-5 absorbent performance. And we tested energy reduction effects by improving the absorption tower inter-cooling system. Overall results showed that the $CO_2$ removal rate met the technical guideline ($CO_2$ removal rate : 90%) suggested by IEA-GHG. Also the regeneration energy of the KoSol-5 showed about $3.05GJ/tonCO_2$ which was about 25% reduction in the regeneration energy compared to that of using the commercial absorbent MEA (Monoethanolamine). Based on current experiments, the KoSol-5 absorbent showed high efficiency for $CO_2$ capture. It is expected that the application of KoSol-5 to commercial scale $CO_2$ capture plants could dramatically reduce $CO_2$ capture costs.

Development of the Advanced Manganese-Based Sorbent for Hot Coal Gas Desulfurization (고온 석탄 가스 탈황을 위한 개선된 망간계 탈황제 개발)

  • This experiments have been made to develop of manganese-based sorbent for the removal of hydrogen sulfide from hot coal gases. Manganese-based sorbent were tested in an ambient-pressure fixed-bed reactor to determine steady state $H_2S$ concentrations, breakthrough times and feasibility of the sorbent when subjected to cycle sulfidation and regeneration testing. Effects of particle size of sorbent, temperature of sulfidation, regeneration temperature and regeneration characteristics on the $H_2S$ removal efficiency were investigated. Experimental results showed that the $H_2S$ removal efficiency was optimal when the temperature was about $800^{\circ}C$ and the smaller particle size, the better $H_2S$ removal efficiency but in the range of 0.214~0.631mm didn't influence it much. The equilibrium constant(K) is represented as a log(K)=3.396/T-1.1105 and the utilization efficiency of sorbents was about 92% at $800^{\circ}C$. Regeneration in air produced $SO_2$ concentration as high as 8.5% at $800^{\circ}C$, 8.4% at $850^{\circ}C$, and 8.8% at $900^{\circ}C$ and may be used in sulfuric acid production.

Biodegradation Characteristics of Aldehydes using Biological Activated Carbon Process (생물활성탄 공정을 이용한 오존처리 부산물인 aldehyde류의 생분해 특성평가)

  • In this study, the effects of biofilter media type (three different activated carbons and anthracite), empty bed contact time (EBCT) and temperature on the removal of four aldehyde species (formaldehyde, acetaldehyde, glyoxal and methylglyoxal) in BAC filters were investigated. Experiments were conducted at three water temperature (5, 15 and $25^{\circ}C$) and four EBCTs (5, 10, 15, and 20 min). The experimental results indicated that the coal based BAC retained more bacterial biomass on the surface of the activated carbon than the other BACs, and increasing EBCT or increasing water temperature also increased the four aldehyde species removal in BAC filters. To achieve above 80% of removal efficiency for four aldehyde species in a BAC filter, above 15 min EBCT at $5^{\circ}C$ and 10 min EBCT at above $15^{\circ}C$ were required. The kinetic analysis indicated a first-order reaction rate for the biodegradation of four aldehyde species at various water temperatures. Data obtained from the BAC filters at various temperatures were also used to evaluate pseudo first-order rate constants for four aldehyde species. The half-lives evaluated for formaldehyde, acetaldehyde, glyoxal and methylglyoxal in the coal-based BAC ranging from 0.89 to 3.19 min, from 0.75 to 3.35 min, from 2.16 to 4.72 min and from 1.49 to 3.86 min, respectively, could be used to assist water utilities in designing and operating BAC filters.

Evaluation of Domestic CCPs(Coal Combustion Products) Quality by API Test Method (API시험법에 의한 국내 석탄회의 품질 평가)

  • Recently, recycling of industrial by-products and CO2 reduction have been important issues in the world. In this reason, applications and reuse of Fly ash as a binder for concrete, which is generated in thermoelectric power plant, have been one of the effective recycle methods. In order for Fly ash to be applied to concrete, Korean Standard(KS) has selected and managed quality such as $SiO_2$, fineness, specific gravity, ignition loss and activity index. However, there is a limits for activity index, whose test period required is at least 28 days or 91 days. Activity index is the critical indication standard to determine mechanical strength of concrete that contained Fly ash. To complement the disadvantage of test method, this research provided "API test method", which quickly measure Pozzolanic reaction of Fly ash can be considered as a alternative of activity index. Then, the adaptable API test method need to be investigated through comparative analysis with the test result of API, activity index and K-value. The test method can make evaluation of Fly ash quality faster and more accurate. As a result, most Fly ash produced in Korea has not been satisfied in the KS quality standard except water content and specific gravity, and especially fluidized bed boiler ash has its characteristics. Also, API, activity index and K-value have superior interrelationship. The interrelationship between API and activity index and K-value gets increased as the material age gets higher, so API test can be considered as very useful test method for Pozzolanic reaction evaluation of Fly ash.

Synthetic Musk Compounds Removal Using Biological Activated Carbon Process in Drinking Water Treatment (정수처리용 생물활성탄 공정에서의 인공 사향물질의 제거 특성)

The pseudo-first-order biodegradation rate constants and half-lives were also calculated for MK, HHCB and AHTN removal at 5, 15 and $25^{\circ}C$. The pseudo-first-order biodegradation rate constants and half-lives of MK, HHCB and AHTN ranging from 0.0082 $min^{-1}$ to 0.4452 $min^{-1}$ and from 1.56 min to 84.51 min could be used to assist water utilities in designing and operating BAC filters for SMCs removal.
