Management of failed periodontal surgical intervention for a furcal lesion with a nonsurgical endodontic approach

  • As long as the prognosis of teeth remains a matter of concern, the endodontic-periodontal relationship will be considered a challenge for the clinician. Many etiologic factors, including bacteria, fungi, and viruses, plus other contributing factors, such as trauma, root resorptions/perforations, and dental malformations, play a role in the co-occurrence of endodontic and periodontal lesions. Whatever the cause, a correct diagnosis on which to base the treatment plan is the key to successful maintenance of the tooth. This article reports the successful endodontic management of a furcation lesion in a mandibular molar that was nonresponsive to a previous periodontal surgical graft. The case had presented a diagnostic challenge for the clinicians, and this article reviews the key points that can lead to a correct diagnosis and treatment planning.

요통의 진단과 치료 (Diagnosis and Management of Low Back Pain)

  • Low back pain is a common clinical condition with heterogeneous causes and challenges to manage. High prevalence and numerous assessments result in an enormous socioeconomic burden. Clinician must conduct efficient and stepwise evaluation process to rule out serious spinal pathology, neurologic involvement, and identify risk factors for chronicity. The process can be achieved through the focused history taking and physical examination. Certain factors related to serious spinal pathology include age (>50 years), trauma, unexplained fever, recent urinary or skin infection, unrelenting night or rest pain, unexplained weight loss, osteoporosis, immunosuppression, steroid use, and widespread neurological symptoms. In non-specific low back pain, diagnostic imaging and laboratory studies are often unnecessary and can disturb an appropriate management. For the management of acute low back pain, patient education and medication such as acetaminophen, non-steroidal anti-inflammatory drugs, and muscle relaxants are recommended. For chronic low back pain, behavior therapy, back exercise, and spinal manipulation are beneficial. The evidence based approach could improve success rate of management, result in prevention of acute low back pain from being chronic intractable pain.

한국인의 평생건강관리 프로그램 (Lifetime Health Maintenance Program for Korean)

  • Lifetime Health Maintenance Program(LHMP) for Koreans is the Korean guide to clinical preventive services which include screening for chronic diseases, counseling and immunization. The Korean Task Force on the LHMP vigorously reviews evidence for interventions to prevent over 50 different illnesses and conditions which are important in Korea. The problems addressed in this program are common ones seen every day by primary care physicians in Korea: cardiovascular, infectious diseases, gastrointestinal and endocrine diseases, cancers, alcohol and smoking, and many others. Primary care clinicians have a key role in screening for many of these problems and immunizing against others. Of equal importance, however, is the clinician's role in counseling patients to change unhealthful behaviors related to diet, smoking, exercise, injuries, and sexually transmitted diseases. The recommendations are grouped by age, sex, and other risk factors.

치과용 인상재의 젖음성 및 경화시간 비교 (Comparison of wettability and setting time of dental impression materials)

  • The purpose of this study is to evaluate and compare wettability and setting time of twelve polyvinylsiloxane impression pastes. For comparing the wettability, the contact angle of a water drop on the impression materials was measured. It is important for impression materials to have higher wettability when trying to make impressions of interproximal spaces and gingival crevices. The higher wettability the better the material will flow into these spaces and the more accurate the impression. An ideal impression material will have adequate working time but a fast intraoral setting time. The clinician needs time to inject material into the sulcus, place the impression material into the tray and position it in the mouth, but the material should set rapidly to reduce time in the patient's mouth. It is considered that the results obtained in this study will provide guideline information for the manufacturing of impression materials and for selecting appropriate impression materials.

Review of common conditions associated with periodontal ligament widening

  • Purpose: The aim of this article is to review a group of lesions associated with periodontal ligament (PDL) widening. Materials and Methods: An electronic search was performed using specialized databases such as Google Scholar, PubMed, PubMed Central, Science Direct, and Scopus to find relevant studies by using keywords such as "periodontium", "periodontal ligament", "periodontal ligament space", "widened periodontal ligament", and "periodontal ligament widening". Results: Out of nearly 200 articles, about 60 were broadly relevant to the topic. Ultimately, 47 articles closely related to the topic of interest were reviewed. When the relevant data were compiled, the following 10 entities were identified: occlusal/orthodontic trauma, periodontal disease/periodontitis, pulpo-periapical lesions, osteosarcoma, chondrosarcoma, non-Hodgkin lymphoma, progressive systemic sclerosis, radiation-induced bone defect, bisphosphonate-related osteonecrosis, and osteomyelitis. Conclusion: Although PDL widening may be encountered by many dentists during their routine daily procedures, the clinician should consider some serious related conditions as well.

  • Oral leukoplakia is the most common premalignant lesion and malignant transformation has been reported from verrucal lekoplakia. Homogenous, benign leukoplakia develops into a line of squamous cell carcinoma such as verrrucous carcinoma, papillary squamous cell carcinoma and invasive squamous cell carcinoma. Early diagnosis and intervention of premalignant leukoplakia is up-most important to prevent transformation into a oral squamous cell carcinoma. Any change in surface, size and color warrants repeated biopsy. If verrucous carcinoma is evidently derived from the previous leukoplakia, wide surgical excision and periodic follow up is needed. Surgically removed lesion of leukoplakia has the tendency to recur. Follow-up is very important to patient and clinician. Although many therapies have been reported to oral leukoplakia and verrucal carcinoma, accepted treatment principle is not exist so far. But surgical removal is recommended as the treatment of choice.

Long-term survival of retained deciduous mandibular second molars and maxillary canine incorporated into final occlusion

  • Orthodontic treatment of a complex case that involves retained deciduous mandibular second molars with missing permanent successors is challenging. Usually, congenitally missing teeth are manifested with other dental anomalies that further complicate orthodontic treatment, such as retained deciduous teeth, impactions, transpositions and peg-shaped lateral incisors. Even though the long term prognosis of the retained deciduous tooth is not fully predictable, if the teeth are in good condition, the patient and clinician may incline towards a decision to preserve the deciduous teeth as long as possible. This case report demonstrates that deciduous teeth, in this case the mandibular second molars and maxillary canine, can be incorporated into final occlusion with clinically stable long-term results.

  • Impacted teeth are a frequent phenomenon encountered by every clinician. The artificial eruption of embedded teeth is the process of directing an impacted tooth into normal occlusion. This procedure is currently attracting attention, with the aim of finding the best technique to use according to each case. This article presents key information regarding impacted incisors, canines, and premolars. In addition, we describe the most common techniques to use for artificial eruption, the open and closed techniques. We review the literature concerning these techniques and outline how clinicians can manage every type of impacted tooth.

삼차신경절 액조내 순수글리세롤을 이용한 삼차신경통의 치험 -증례보고- (Retrogasserian Gangliolysis with Pure Glycerol for the Treatment of Tic Douloureux -A case report-)

  • Trigeminal neuralgia(tic douloureux) is a common clinical syndrome which is characterized by a painful facial condition. The clinician must be able to identify a patient with tic douloureux because this most severe pain syndrome can almost always be controlled. Gangliolysis is the most recent development in the long history of destructive procedures for tic douloureux. H$\ddot{a}$kason pioneered a technique of placing a needle for injecting glycerol into the trigeminal cistern which he found safe and effective for pain control. In August at 1991, I injected pure sterile glycerol three times into the cistern of trigeminal ganglion using the H$\ddot{a}$rtel approach to relieve a patient from pain. The results were as follows; 1) The response of pain relief to a glycerol gangliolysis was excellent. 2) As a complication, there were mild sensory deficit, transient headache and herpes simplex around the mouth angle.

  • In ancient times, respiration's perceived purpose was only to cool the blood, and nasal mucus was thought to be a discharge from the brain. Later it was demonstrated that fluids cannot travel from the brain to the nose. It appears that credit for pointing out the medical significance of bronchial secretions and the biological value of the exocrine function of the lungs belongs to Laennec. He described the “chronic idiopathic pituitous catarrh” known today as bronchorrhea, which is characterized by paroxysms of expectoration. The importance of all this is that airway secretions, and their alterations, became one of the cardinal signs of many respiratory diseases (1).
