  • Objectives : This study was undertaken in order to establish clinical application of Taegeuk acupuncture treatment by Sasang(4-type) Constitutional medicine. Methods : 1. Dr. Lee Byung-haeng suggested nine applications of Taegeuk acupuncture treatment of Soyang(lesser Yang) type men. 2. The author summarized the application of Taegeuk acupuncture treatment gathered by clinical experience. 3. The author researched the effect of Taegeuk acupuncture on patients after treatment. Results & Conclusions : 1. Taegeuk acupuncture treatment is effective against psychogenic disease. 2. Taegeuk acupuncture treatment is effective against autonomic nervous system dysfunction(For example, blood circulation disorder, etc.) and has an effect on recovery from sub-health. 3. The author conclude that Taegeuk acupuncture treatment for incurable diseases or diseases of unknown etiology needs a further clinical study in the future.

  • Objectives: The aim of this study is to introduce the Clinical presentation and announce the importance of developing Clinical presentation of Korean medicine and suggest about development direction of Clinical presentation of Korean medicine. Methods: To Investigate the Clinical presentation used in western medicine. I think that Clinical presentation of Korean medicine is a systematic list of Korean medicine symptoms and a standard syndrome differentiation and treatment(辨證論治). So I would like to offer "PangYakHapPyon(方藥合編)" as a basis for developing Clinical presentation of Korean medicine. Results: The clinical presentation term has become widespread in use at Calgary Medical college. Calgary Medical college created a list of 120 clinical presentations In 1991. In Korea, 101 clinical presentations were made in 2016. "PangYakHapPyon(方藥合編)" has been used effectively for over 130 years and widely used in the public. In addition, "PangYakHapPyon(方藥合編)" is summarized in the symptoms and prescriptions that occur frequently in Korea. Conclusions: For the globalization and standardization of Korean medicine, Clinical presentation of Korean medicine should be developed. The overall form of Clinical presentation of Korean medicine uses the form of Clinical presentation of Canada and a standard syndrome differentiation and treatment(辨證論治) for diagnosis and treatment is based "PangYakHapPyon(方藥合編)".

Comprehensive Korean Medicine Treatment for Benign Essential Blepharospasm in a Patient Responding Poorly to Botulinum Toxin Treatment

  • Benign essential blepharospasm (BEB) is a condition/disease which involves involuntary muscle contractions causing the eye to repetitively close, making it impossible for the patient to perform daily activities. This study reports Korean medicine treatment of a rare case BEB in a 34-year-old male patient whose symptoms showed minimal improvement following botulinum toxin injection (the standard treatment). Acupuncture, pharmacopuncture, and herbal medicine treatment for 11 days resulted in clinical improvement for all symptoms as assessed by using the Jankovic Rating Scale, Blepharospasm Disability Index, numerical rating scale, duration of spasms lasting more than 1 hour per day, and number of spasms lasting less than 1 minute per day. This case report suggests that comprehensive Korean medicine treatment could be a treatment option for BEB in patients who do not respond well to botulinum toxin injection.

  • Skeletal anterior open bite is a difficult problem to correct in orthodontic treatment. To treat adult patients who have skeletal anterior open bite, we considered two methods. Combination treatment of orthodontics & surgery and camouflage orthodontic treatment. In adults, treatment of severe skeletal anterior open bite consists mainly of surgically repositioning the maxilla or the mandible. However, camouflage therapy is often the treatment of choice for skeletal open bite patients who have mild to moderate skeletal discrepancies when growth modification is no longer possible. But excellent results generally require careful coordination of the orthodontic and surgical phases of treatment. This is a case report of a skeletal anterior open bite patients who were treated with orthodontic treatment and orthognathic surgery. First case was diagnosed as skeletal class I malocclusion & bimaxillary protrusion with anterior open bite, and finally treatment ended for removal of open bite with orthodontic procedure and bimaxillary anterior segmental osteotomy surgery. Second case was diagnosed as skeletal class II malocclusion with open bite & mandibular retrusion, and was treated with only camouflage orthodontics because she feared to have a surgery. In a regular follow up visit after debonding we proposed to the patient advanced genioplasty, and in her agreement her facial esthetics was improved through the surgery.

  • Chronic inflammatory demyelinating polyneuropathy (CIDP) is an immune-mediated neuropathy with heterogeneous features. Appropriate treatment will produce a favorable outcome, but a poor treatment response and severe disability have also been reported. The roles of the clinical phenotypes and electrophysiological features of CIDP as well as of autoantibodies against nodal and paranodal proteins have been highlighted previously due to their association with the treatment response and long-term prognosis. This review addresses the diverse factors associated with the prognosis of CIDP.

Review of Clinical Researches about Korean Medicine Treatment on Language Disorder of Cerebral Palsy (뇌성마비 언어장애에 대한 한의 치료 연구 동향)

  • Objectives: The purpose of this study was to obtain some understanding about Korean medicine treatment on language disorder in cerebral palsy for future practice and the research, from the clinical studies. Methods: The literature was searched using the database-China Academic Journals (CAJ). Clinical studies of Korean medicine treatment for language disorder in cerebral palsy, including Randomized controlled trial (RCT), case control study, case series, case report were analyzed. Results: Fifteen Clinical studies met our inclusion criteria; One case study and six case series, one non-randomized controlled trial and seven RCTs. Acupuncture treatment, especially Head acupuncture, was the major treatment for language disorder of cerebral palsy in clinical studies, as it was used in fourteen studies. Acupoint massage, tuina, and acupoint injection were employed as treatment methods in the studies. Acupuncture treatment was used for language disorder of cerebral palsy combined with language therapy and other rehabilitation treatment in many studies. The effectiveness in the treatment groups, regardless of treatment methods, was higher than that of control group in all RCT studies. Conclusions: The results of this study could be used in the practice and the future study about language disorder of cerebral palsy.

  • Since renal cell carcinoma(RCC) has various examination and treatment methods according to clinical stage and histopathological characteristics, it is required to determine accurate and efficient treatment methods in the clinical field. However, the process of collecting and processing RCC medical data is difficult and complex, so there is currently no AI-based clinical decision support system for RCC treatments worldwide. In this study, we propose a clinical decision support system that helps clinicians decide on a precision treatment to each patient. RCC standard big database is built by collecting structured and unstructured data from the standard common data model and electronic medical information system. Based on this, various machine learning classification algorithms are applied to support a better clinical decision making.

Review of Latest Traditional Chinese Medicine Clinical Studies on Pediatric Tic Disorder with Tuina Treatment (추나 치료를 적용한 소아 틱장애에 대한 최신 중의학 임상 연구 고찰)

  • Objectives: The purpose of this review is to investigate the latest traditional Chinese medicine clinical studies on pediatric tic disorder with Tuina treatment and propose the directions of future studies and clinical applications. Methods: To obtain data from CNKI, set the field by '中醫學', '中藥', '中西醫結合' and used keywords '抽动障碍', '抽動穢語綜合', '小兒抽动', '兒童抽动' and '推拿', '導引', '按摩', 'Tuina', 'massage' in cross combination way. Results: 12 clinical studies were selected. These studies were analyzed by author & year, subjects, diagnostic criterion, evaluation criterion, syndrome differentiation, treatment period, methods& results, frequent acupoints and assessment of the quality of studies. All of reports achieved effective therapeutic results on the pediatric tic disorder with Tuina treatment. Conclusion: Tuina treatment has a positive effect and are easily accepted by children who have fear and rejection to traditional Korean Medicine such as acupuncture and Herbal remedy. It is also safe and simple to operate. So the Tuina treatment is worthy to disseminate potential for further development in the treatment of pediatric tic disorders.

  • Objectives : In this article, the application of the theory of Sameumsamyang Gaehapchu to clinical treatment in some important medical books of traditional medicine, for example Jienuelunshu(?虐論疏) etc, is investigated. Methods : The literatures containing the application were searched in the Sikuquanshu(四庫全書) database by key words of Sameumsamyang and Gaehapchu, and then analysis and investigation of significant documents has followed. Results : The application of the theory of Sameumsamyang Gaehapchu to clinical treatment has been mainly done with some characteristics that are expansion to the theory of visceral organs, suggestion of various medicines, and definite explanation of the relationship between Gaehapchu. These characteristics made it easy to predict prognosis of diseases and to determine order of treatment. Conclusions : After the age of Huangdineijing, the theory of Sameumsamyang Gaehapchu to clinical treatment has been developed and realized through application of the theory to clinical treatment.

Prevalence of referral reasons and clinical symptoms for endodontic referrals

  • Objectives: To investigate the prevalence of different primary reasons for endodontic referrals and the clinical symptoms of the referred cases. Materials and Methods: Clinical data of total endodontic treatment cases (1,014 teeth) including endodontic referral cases (224 teeth) between January 1, 2010 and December 31, 2012, at Kangdong Sacred Heart Hospital, were investigated retrospectively. The one major reason for referral, the clinical symptoms, and the resulting treatment procedures of referral cases were recorded. The percentages of clinical symptoms of the endodontic referral cases and the total endodontic treatment cases were compared by ${\chi}^2$ test for each symptom. Results: Persistent pain was the most frequent reason for endodontic referral (29.5%), followed by presence of gingival swelling and sinus tract (24.1%), and apical radiolucency (12.9%). Referrals in cases involving endodontic difficulties such as canal calcification, broken instruments, post, perforation, and resorption were less than 5.0%, respectively. The percentages of four major clinical symptoms of pain, apical radiolucency, previous endodontic treatment, and gingival swelling and sinus tract were significantly higher in the endodontic referral cases than those in the total endodontic cases (p = 0.001). Among the included referral cases, 72.8% were treated with nonsurgical endodontic treatment only. Teeth other than the referred teeth were diagnosed as the origin of the problem in 5.8% of the referrals. Conclusions: The high prevalence of pain, apical radiolucency, previous treatment, and gingival swelling and sinus tract in endodontic referral cases suggest that these symptoms may be what general practitioners consider to be difficult and refer to endodontists.
