뇌간 종양의 방사선 치료 성적 (Radiotherapy Result of Brain Stem Tumors)

  • 1979년부터 1987년까지 뇌간 종양 환자 25명이 방사선 치료를 받았다. 6예는 조직학적으로, 19예는 이학적 및 신경학적 진찰, CT 및 MRI등에 의하여 임상적으로 진단하였다. 18예는 근치적 목적의 방사선 치료를, 6예는 수술후 방사선 치료를, 1예는 방사선 치료 후 항암제 병용요법 등을 각각 받았다. 방사선 치료는 통상적 분할치료법에 의하여 $50\~55Gy$를 조사하였다. 치료 완료후 두달째 임상적 scoring scale에 따른 완전관해는 없었고, 부분관해 16예 $(64\%)$, 무변화 2예, 종양진행 4예가 각각 관찰되었다. 생명표법에 의한 1, 2, 3년 생존율은 각각 $50\%,\;40\%,\;30\%$였으며, 연령, 진단당시 전신상태, 뇌신경 장애정도, 조영제 투입후 시행한 뇌단층 촬영 소견, scoring scale에 의한 임상적 관해양상 등의 요인이 생존율에 유의한 영 향을 주는 예후인자였다.

Factors that Predict Clinical Benefit of EGFR TKI Therapy in Patients with EGFR Wild-Type Lung Adenocarcinoma

  • Background: Epidermal growth factor receptor (EGFR) mutations in non-small cell lung cancers have emerged as key predictive biomarkers in EGFR tyrosine kinase inhibitor (TKI) treatment. However, a few patients with wild-type EGFR also respond to EGFR TKIs. This study investigated the factors predicting successful EGFR TKI treatment in lung adenocarcinoma patients with wild-type EGFR. Methods: We examined 66 patients diagnosed with lung adenocarcinoma carrying wide-type EGFR who were treated with EGFR TKIs. The EGFR gene copy number was assessed by silver in situ hybridization (SISH). We evaluated the clinical factors and EGFR gene copy numbers that are associated with a favorable clinical response to EGFR TKIs. Results: The objective response rate was 12.1%, while the disease control rate was 40.9%. EGFR SISH analysis was feasible in 23 cases. Twelve patients tested EGFR SISH-positive, and 11 were EGFR SISH-negative, with no significant difference in tumor response and survival between EGFR SISH-positive and -negative patients. The overall median progression-free survival (PFS) and overall survival (OS) of 66 patients were 2.1 months and 9.7 months, respectively. Female sex and Eastern Cooperative Oncology Group (ECOG) performance status (PS) of 0-1 were independent predictors of PFS. ECOG PS 0-1 and a low tumor burden of extrathoracic metastasis were independent predictors of good OS. Conclusion: Factors such as good PS, female sex, and low tumor burden may predict favorable outcomes following EGFR TKI therapy in patients with EGFR wild-type lung adenocarcinoma. However, EGFR gene copy number was not predictive of survival.

Predictive factors associated with successful response to utrasound guided genicular radiofrequency ablation

  • Background: Ultrasound-guided genicular nerve radiofrequency (RF) procedures are of interest in the management of chronic knee pain. A wide variety of demographic, clinical, and procedural characteristics can affect treatment success. This study aimed to determine predictive factors to provide superior treatment outcomes. Methods: The demographic, clinical, and technical data of patients who received genicular nerve RF for knee pain between September 2016 and September 2021 were evaluated. A positive outcome was defined as at least 50% pain relief on a pain score for at least 6 months. Logistic regression analysis was performed to determine the factors associated with a successful response to genicular RF. Results: Among 206 patients who underwent genicular RF, 62% of the patients reported successful outcomes at 6 months. In the multivariate model, targeting 5 nerves (odds ratio [OR], 6.184; 95% confidence interval [CI], 2.291-16.690; P < 0.001) was the most significant predictor of successful outcomes. Multivariable logistic regression analysis showed that prognostic genicular nerve block with a 50% cut-off value (OR, 2.109; 95% CI, 1.038-4.287; P = 0.039), no opioid use (OR, 2.753; 95% CI, 1.405-5.393; P = 0.003), and depression (OR, 0.297; 95% CI, 0.124-0.713; P = 0.007) were the predictive factors significantly associated with response to genicular RF. Conclusions: Clinical and technical factors associated with better treatment outcomes were ultimately targeting more nerves, performing prognostic block, no opioid use, and no depression. These results are expected to be considered when selecting patients for genicular RF.

  • 전립선암에서 뼈는 가장 흔한 전이병소이며, 전립선암의 질병 상태를 파악하고 치료 반응을 평가하기 위해서는 전이성 골병변의 평가가 필수적이다. 거세 저항성 전립선암은 남성호르몬을 거세 수치로 떨어뜨렸음에도 불구하고 암이 진행하는 상태를 의미하며, 이 상태에서는 원격 전이가 빈번하게 발생한다. 거세 저항성 전립선암의 치료 반응을 객관적으로 평가하기 위해 뼈스캔을 기반으로 한 Prostate Cancer Working Group 3 (이하 PCWG3) 가이드라인이 발표되었으나 실제 이를 쉽게 숙지하여 적용하기에는 어려운 점이 있다. 본 종설에서 PCWG3 가이드라인에 준한 거세 저항성 전립선암의 뼈스캔 기반 골병변 반응 평가를 위한 구체적인 영상 획득 방법 및 치료 반응 평가법을 소개하고자 한다.

Hypothesis Proposal about Predictive Factors and Optimal Age for Response to Herbal Medicine Treatment for Height Gain in Children: a Retrospective Review

  • Introduction: We aimed to investigate the predictive factors and optimal age for response to herbal medicine treatment for height gain in children. Methods: This retrospective chart review included 61 children (age range, 5-16 years) treated for height gain between 2011 and 2015. A predictive model was established by multiple linear regression analysis. Dependent variables were defined by the differences in percentile before and after herbal medicine treatment. The optimal cutoff value of patient age was determined by receiver operating curve analysis. Results : The age of initiation of herbal medicine therapy (p = 0.012) and administration of Forsythiae fructus (p = 0.002) were significant variables for treatment response. The adjusted R2 value was 0.231. The mean ages of the responder and non-responder groups were significantly different (p = 0.023). The optimal cutoff value of age for predicting treatment response was 9.75 years. Treatment response was better among children below 9.75 years of age. Conclusions: Patient age and administration of Forsythiae fructus were identified as determinants of response to herbal medicine treatment. Treatment of rhinitis and initiation of height gain treatment at an early age are critical for better response. These findings will provide fundamental data for further research.

  • The goal of radiation therapy is to maximize the tumor dose and to minimize the dose of normal tissue. In order to achieve this goal, the new radiation therapy techniques such as three dimensional conformal therapy or intensity modulated radiation therapy has been developed and tried to clinical application. In recent years, the use of ABRs has gained popularity as one of the methods of choice in determinig hearing thresholds of newborns and infants. We introduce labeling and computational methods of peak identification in ABR.

  • The nutritional status of host has a profound effect on immune response and its ability to defend aganinst invading pathogen. Almost all nutrient dificiencies impaired host defence, and more than recommended levels of certain nutrient enhance immune response beyond that observed with 'adequate'. But high-fat diets have been associated with various types of immune dysfunction in experimental animal models and humans. Also, high linoleic acid suppressed immune function and growth and metastasis of tumor than saturated fatty acids. The present review focused on the effect of dietary lipid on immune fuction, cytokine production and clinical conditions like infection, autoimmune disease and cancer.
