Formulation of the Scope and Key Questions of the Guideline Recommendations for Immunosuppressive Treatment in Kidney Transplantation (신장이식 면역억제요법 가이드라인 개발을 위한 권고안 적용 범위 및 핵심질문 선정)

  • Background: Although a growing number of guidelines and clinical researches are available for immunosuppressive treatment of post-transplantation, there is no clinical practice guideline for the care of kidney transplant recipients in Korea. Selection of a researchable question is the most important step in conducting qualified guideline development. Thus, we aimed to formulate key questions for Korean guideline to aid clinical decision-making for immunosuppressive treatment. Methods: Based on previous published guidelines review, a first survey was constructed with 29 questions in the range of immunosuppressive treatments. The experts were asked to rate the clinical importance of the question using a 5-point Likert scale. The questions reached 60% or more from the first survey and additional new questions were included in the second survey. In analyzing the responses to items rated on the 9-point scale, consensus agreement on each question was defined as 75% or more of experts rating 7 to 9. Results: In the first survey, 50 experts were included. Among the 29 questions, 27 were derived to get 60% or more importance and 3 new questions were additionally identified. Through the second survey, 9 questions were selected that experts reached consensus on 75% and over of the options. Finally, we developed key questions using PICO (patient, intervention, comparison, and outcome) methodology. Conclusion: The experts reached a high level of consensus on many of key questions in the survey. Final key questions provide direction for developing clinical practice guideline in the immunosuppressive treatment of transplantation.

Related factors of clinical practice satisfaction and social support in dental hygiene students (일부 치위생과 학생들의 실습 만족도와 사회적 지지 관련 요인)

  • Objectives: The purpose of the study was to investigate the related factors of clinical practice satisfaction and social support in dental hygiene students. Methods: A self-reported questionnaire was filled out by 420 dental hygiene students in Gwangju and Jeonnam from March 8 to April 8, 2014. Except 26 incomplete answers, 394 data were analyzed. The instrument consisted of general characteristics of the subjects(7 questions), social support(7 questions), and satisfaction level with clinical practice(22 questions). The instrument for satisfaction level with clinical practice included self-esteem(1 question), interest and usefulness(2 questions), knowledge application(1 question), place of clinical practice(1 question), and influence by practice leader(1 question). Cronbach alpha was 0.773 in the study. The instrument of social support was adapted from Park and reconstructed. Social support included emotional support(11 questions), self-esteem support(7 questions), and informative support(4 questions). Social support was score by Likert 5 scale and higher score showed the higher social support. Results: The dental hygiene students got a mean of $3.11{\pm}0.55$ in clinical practice satisfaction. They got 3.35 points in satisfaction with major and 3.32 in satisfaction with curricula(p<0.001). They got a mean of $3.68{\pm}0.55$ in social support. They got $3.69{\pm}0.57$ in emotional support; $3.81{\pm}0.59$ in self-esteem support; and $3.53{\pm}0.66$ in informative support. The influencing variables on satisfaction with clinical practice were self-esteem support(B=0.202), satisfaction with major(B=0.234), and satisfaction with curricula (B=0.128). Conclusions: There was a close relationship between satisfaction with clinical practice and social support. In order to enhance the satisfaction with clinical practice, it is necessary to connect self-esteem support with major satisfaction and curricula satisfaction.

Revision of the modified Blood Stasis Questionnaire II (어혈 진단 설문지 개정 연구)

  • Objectives : The objective of this study was to revise the modified Blood Stasis Questionnaire II. Methods : This revision focused on refining the Korean literature expression of Blood Stasis Questionnaire II consisting of 30-question questionnaire. Seven external experts and five researchers of Korean Institute of Oriental Medicine reviewed the questionnaire and its protocol, while the addition or deletion of questions and changes in scoring method were not dealt with in this revision. Results : Among thirty questions, four questions were corrected to appropriate expressions. In case of eight questions, explanations in Korean or Chinese were added. Thirteen questions in the phrase were changed in sentence form to unify the whole questionnaire. Conclusions : This study introduces the revised version of the modified Blood Stasis Questionnaire II. It is expected that clinical demand of this questionnaire will increase and it will be used vigorously in blood stasis research.

The perception in regard to the clinical trial of dental hygienists (치과위생사의 임상시험에 대한 인식도)

  • Objectives: The purpose of the study was to investigate the perception of clinical trials of dental hygienist. Methods: A self-reported questionnaire was filled out by 126 dental hygienists in Seoul, Kyonggi and other area from October 1 to November 15, 2012. The instrument of the study was adapted from Jung and reconstructed. The questionnaire consisted of general characteristics of the subjects(9 questions) and recognition for clinical trials(10 questions). Recognition for clinical trials instrument included recognition for clinical trial conception(2 questions), objective recognition(5 questions), and subjective recognition for clinical trial(3 questions). The instrument was score by Likert 5 scale and Cronbach alpha was 0.936 in the study. Data were analyzed by SPSS 20.0 program and set at p<0.05. Independent t-test, chi-square test, one way ANOVA, and post-hoc Duncan test were done. Results: Those who had higher ages and higher education tended to have longer clinical careers(p<0.05). There was a significant difference in clinical trials between the level of education, clinical dental hygienists and professors. The differences in problem perception for activation of clinical trial according to workplace appeared to lack of publicity and inadequacy of facilities(p<0.05). Conclusions: To improve the perception of the clinical trials in dental hygienists, it is necessary to prepare the human resources programs, the additional curricula, continuing education courses and active publicity.

Influencing factors of turnover intention in the clinical dental hygienists (임상치과위생사의 이직의도에 영향을 미치는 요인)

  • Objectives: The objective of the study was to investigate the influencing factors of turnover intention in the clinical dental hygienists. Methods: A self-reported questionnaire was completed by 415 dental hygienists in Gyeonggi, Chungcheong, Jeolla, and Kyongsang province from October 1 to December 31, 2014. Data were analyzed using IBM SPSS statistics 19.0 with a significance level of 0.05. The study instruments included 9 questions of the general characteristics of the subjects, 7 questions of job related characteristics, 16 questions of job stress, 20 questions of job satisfaction, 10 questions of burnout, 15 questions of organizational commitment, 10 questions of happiness index, and 10 questions of turnover intention measured by Likert 5 point scale. Results: Turnover intention was significantly related with job stress, burnout, and organizational commitment. Conclusions: In order to reduce the turnover intention in the hospital workers, it is very important to manage the human resources through continuous development of the competence and performance activation program.

Analysis of Basic Medicine-Related Questions in the Korean Medical Licensing Examination (2016-2018) (우리나라 의사 국가시험 필기시험(2016-2018)의 기초의학 역량 평가 현황의 분석)

  • Basic medical education is important for developing the competencies of medical doctors, and it includes basic biomedical sciences, preventive medicine, medical ethics, and clinical science. This study aimed to reveal the current status of the Korean Medical Licensing Examination (KMLE) regarding its evaluation of competencies in basic biomedical sciences. The basic medicine-related questions were screened and selected from the test forms of the KMLE (2016-2018) by personnel conducting basic biomedical science education, and the selected questions were analyzed by three independent groups of undergraduate students at Chonnam National University Medical School in terms of the learning outcomes of basic medical education. The study scope includes the proportion of basic medicine-related questions, which consist of basic medicine questions and basic medicine-related clinical medicine questions, its annual change, discipline distribution, and associated learning outcomes. The average proportions of basic biomedical sciences, preventive medicine and medical law, and clinical sciences were 2.3%, 5.8%, and 91.9% of all questions, respectively. The proportion of basic medicine-related questions, except those on preventive medicine and medical law, was 22.0% of the total, and questions on pharmacology and microbiology accounted for 83.0% of the basic medicine-related questions. The proportion of sub-enabling learning outcomes linked with basic medicine-related questions comprised 14.0% of the total outcomes for basic biomedical sciences and 30.4% for preventive medicine and medical law. It is concluded that the KMLE questions may not sufficiently cover the essential competencies of basic medical education for medical doctors, and the KMLE may need to be improved with regard to competencies in basic biomedical sciences.

Development of questionnaire for metabolic disease with blood stasis: A Delphi survey

  • Objectives : The aim of this study was to investigate expert opinions on the questions contained in a questionnaire for diagnosing blood stasis accompanying metabolic disorders. Methods : Two rounds of Delphi survey were conducted online targeting on one hundred Korean medicine doctors. Respondents rated the appropriateness of the 30 questions in diagnosing metabolic disorder on a five-point scale, anchored at '5 = very appropriate', '4 = appropriate', '3 = somewhat appropriate', '2 = inappropriate', and '1 = very inappropriate'. Results : The mean score on 30 questions of first and second Delphi survey was 3.26 points and 3.31 points, respectively. The ranking of the top 10 questions that were rated as appropriate for diagnosing blood stasis accompanying metabolic disorder were 'sublingual varices', 'reddish black tongue', 'reddish black gum', 'reddish black lips', ''dark purple palatal mucosa and venous edema', 'night pain, 'ecchymosis of the tongue', 'piercing pain', 'ecchymosis of the skin' and 'prolonged numbness'. Conclusions : The experts agreed that three of the most typical symptoms of blood stasis and the conditions of capillary vessels in the tongue or the oral cavity were highly associated with metabolic disorder, whereas the questions related to abdominal pain lacked an association with metabolic disorders.

Comparison of Pharmacist License Examination between Korea and Canada (우리나라와 캐나다 약사면허 국가시험제도 비교)

  • A newly-structured Korean pharmacist license exam has been launched in 2015, reflecting upon the changes in the pharmacy curriculum from a 4 year program to a 6 year program in 2009. In order to provide new ideas to ensure that the new exam is one of the most effective pharmacist evaluations that have taken place thus far, this study was done to compare the pharmacy exams in Korea and Canada. One of the major differences noted between the two countries' exams is that along with paper based MCQ portion of the exam, Canada's exam also includes a performance-based section, known as OSCE, which the Korean Pharmacy Exam (KPE) does not have. Furthermore, with the MCQ portion of the exam, the Canadian exam asks about 300 questions, with 450 minutes of test time allocated and taken during a period of two consecutive days, the KPE asks 350 questions, with 325 minutes of test time allocated in one day. Although, similarly, many of the questions in both exams place emphasis on clinical or patient care, Canada's exam puts significantly more emphasis (50.5% of exam questions) on these types of questions than Korea (29.7% of exam questions). However, this percentage does not reflect the exact weight placed for the specific areas of knowledge it requires to answer these questions, since the types of questions asked in this section in Canada could be placed in another section on the KPE. Canada's exam also has more questions (10% +150 questions for BC) on the topics of law and ethics compared to the KPE (5.7%). The reason for this may be that the Canadian society puts emphasis on the legal and ethical duties of pharmacists as a leader. However, since each country is unique in their social, economical, and cultural points of view, comparing the KPE to the Canadian licensing exam and applying these differences to the new KPE may not be appropriate. One last thing to consider is that, as WHO/FIP mentioned, in good pharmacy practice, continually updating and developing an appropriate pharmacy exam with consideration of societal changes, is key to success in developing the scope of practice for current and future pharmacists.

The relationship between vocational calling and ethical inclination in clinical dental hygienists (일부 임상 치과위생사의 직업적 소명의식과 윤리적 성향의 관계)

  • Objectives: The purpose of the study is to investigate the relationship between professional vocation and ethical inclination in clinical dental hygienists. Methods: A self-reported questionnaire was filled out by 221 clinical dental hygienist in Busan and Kyeongnam from April 1 to 30, 2014. The questionnaire consisted of structured questions including demographic characteristics(9 questions), vocational calling(9 questions), deontology(12 questions), asceticism(13 questions), and utilitarianism(8 questions). Questionnaire was carried out by Likert scale. Data were analyzed using the statistical package SPSS Window 19.0 for frequency, mean and standard deviation analysis, t-test, one-way ANOVA and linear regression. Results: There was a significantly positive correlation between professional vocation and ethical inclination. The deontology was associated with education, dental service career, duty hours, and number of clients in multivariate analysis after adjusting for socio-demographic characteristics and professional vocation. Asceticism was also correlated with education, and utilitarianism was closely associated with vocational calling, work place, number of clients, and monthly income. Conclusions: It is necessary to develop the curricula including vocational calling and ethical inclination in the dental hygiene education. The dental hygienists should learn the ethical and vocational calling as a professional career.

Proposal of Pharmacy School Objective Structured Clinical Examination (OSCE) Contents and Test Methods (약학대학 실기시험 문항개발에 대한 제언)

  • Competencies of entry-level pharmacists are evaluated by multiple-choice questions (MCQ) examination and objective structured clinical examination (OSCE). OSCE helps to evaluate student's communication skills and attitudes, which are difficult to evaluate by the MCQ exam. In Korea, pharmacy OSCE is not implemented; however, the needs of OSCE is growing. This study proposed OSCE exam contents and exam methods that can be implemented in colleges of pharmacy. Further study is needed to develop OSCE scenarios and questions, validate examination questions and evaluation methods by performing a pilot study, and provide feedback on the curriculum and study methods.
