병원 간호사들의 근거활용 경험 및 장애요소와 촉진요소에 대한 탐색 (Hospital Nurses' Uses of Evidence, and Barriers to and Enablers of Evidenced-based Practice)

  • Purpose: The purpose of this study was to explore nurses' experience of evidence-based nursing practice in general hospitals. Methods: Data were collected from 13 nurses through in-depth interviews about their experiences with evidence-based practice. The research questions were "What kind of evidence are you using in your practice?" and "What are the barriers to and enablers of evidence-based practice that you have experienced?" Qualitative data from field and transcribed notes were analyzed using qualitative content analysis methodology. Results: Major themes of using evidence were identified as 'research as primary valid evidence', 'information from local context and internet as realistic evidence', and 'clinical experience as pragmatic evidence'. Patient experience was not used as evidence in solving nursing problems. Barriers to and enablers of evidence-based practice were linked. They included both external, organizational factors and individual factors. Main issues were 'lack of evidence and poor work environment', and major facilitating factors were 'improving knowledge and skills related to evidence-based practice' and 'communicating and sharing evidence'. Conclusion: The study findings provide useful information for understanding nurses' experience of using external and internal evidence along with their meaning. A multidimensional approach is needed to overcome barriers to and implement evidence-based practice.

치위생(학)과 학생들의 임상실습과 전공만족도가 진로준비행동에 미치는 영향 (Effects of clinical practice and major satisfaction of dental hygiene students on career preparation behavior)

  • Objectives: The purpose of this study was to investigate the effects of dental hygiene students' clinical practice stress, satisfaction and major satisfaction on career preparation behaviors. Methods:This study was conducted targeting the dental hygiene students who have clinical practice experience. The final 305 subjects were analyzed using SPSS 22.0 program. Results: The correlation between clinical practice stress and career satisfaction and career preparation behavior decreases with the higher degree of clinical practice and career preparation behavior. Analysis results about the affecting factors on the level of satisfaction with major satisfaction factors showed the highest factor of preparation behavior, followed by clinical practice and clinical practice stress in order. Conclusions: The results of this study showed that the higher the satisfaction of clinical practice and the higher the satisfaction of career, the more positive the career preparation behavior. Therefore, it's recommended to support continuous education programs to be coherent with dental hygiene student's career preparation behavior based on reinforcing individual competency by obtaining self-confidence and satisfaction from clinical practice.

임상실습교육환경이 간호학생의 임상실습 스트레스 및 불안에 미치는 영향 (Effects of Clinical Learning Environment on Clinical Practice Stress and Anxiety in Nursing Students)

  • Purpose: The purpose of this study was to identify factors in the clinical learning environment that affect clinical practice stress and anxiety in nursing students. Methods: Participants were 210 senior nursing students from two universities who had completed their clinical experience. Data were collected from self-report questionnaires and analyzed using t-test, ANOVA, and hierarchical multiple regression analysis with the SPSS/WIN 21.0 Program. Results: The mean scores for the clinical learning environment, the clinical practice stress, and anxiety were $3.16{\pm}0.68$, $2.98{\pm}0.66$, and $43.74{\pm}10.18$ points, respectively. The regression analysis showed that the clinical learning environment contributed to 13% of the stress and 20% of the anxiety in the nursing students with the conditions controlled for general characteristics, department satisfaction and social support. Conclusion: The results of this study suggest that the clinical learning environment should be carefully assessed to reduce nursing students' stress and anxiety from clinical practice. Therefore, collaboration between nursing universities and teaching hospitals is necessary to optimize clinical learning environment.

임상실습이 치위생 전공학생의 전문직 정체성에 미치는 영향 (Effect of clinical practice of dental hygiene students on professional identity)

  • Objectives : This study aimed to get the data necessary for dental hygiene students to perceive the need for establishing a professional identity as dental hygienists and developing a program which reinforce pride as a preliminary professional. Methods : The subjects were 695 dental hygiene students in Seoul, Gyeonggi and Chungcheong areas. The collected data were analyzed using the SPSS program. Results : 1. The professional identity of dental hygiene students was 3.02 points, and the satisfaction with institutes for practice was 2.96 points. 2. The higher the satisfaction with the major was, the higher the identity was. Also, the identity was high when the frequency of clinical practice was 1 (3.11 points) and when they selected their major which corresponds with his aptitude (3.20 points). 3. The satisfaction with clinical practice was high when they didn't have any experience in dental clinic before clinical practice and when the satisfaction with the major was high. 4. Regarding the correlation between the professional identity and the satisfaction with clinical practice, the higher the satisfaction with clinical practice, the higher the professional identity. 5. The effective variables regarding the professional identity were satisfaction with a major, satisfaction with clinical practice, and frequency of clinical practice. Conclusions : The professional identity of students may be established through school education involving clinical practice. The identity of students may affect the occupational satisfaction, the identity and the work efficiency of dental hygienists in the future. Thus, it is deemed necessary to have a continued collaborative relationship between a school and an institute for clinical practice so that students may establish their correct views and identity.

응급구조과 학생의 성취동기와 임상실습 만족도와의 상관성 연구 (A Correlation Study on the Relationship between Motivation for Achievement and Satisfaction on Clinical Practice in EMT students)

  • Purpose : This study examines satisfaction of students at the department of Emergency Medical Technology with clinical practice and relationship between motivation of achievement and satisfaction with clinical practice and aims to provide basic materials contributing to the improvement of clinical practice education. Method : It selected 237 students in the second and third grade at the dept. of Emergency Medical and having experience in clinical practice as the subjects, collected data from Sept. 1 to 30, 2007 and analyzed 237 questionnaires collected with SPSS 12.0 program. Result : 1. Degree of motivation of achievement scored 3.46 of full marks of five points and they showed motivation of achievement over average score. Students who entered the dept. of Emergency medical after beginning social career(3.78) showed higher motivation of achievement in the period of entrance and group having will to be volunteer(3.66) showed the highest motivation of achievement in selecting this major. The better personal relationship they have, the greater hospital grade and the higher their transcript, the higher motivation of achievement they have. In the desired area of employment after graduation, first-aid staffs at the field of industry group(3.85) showed the highest motivation of achievement. 2. Degree of their satisfaction with clinical practice scored 3.53 of full marks of five and they showed satisfaction with it over average score. They showed the highest satisfaction in the contents of practice(3.64). 3. There was statistically significant positive correlation between motivation of achievement and satisfaction with clinical practice(r = .165, p < .05). Conclusion : There was statistically significant positive correlation between motivation of achievement and satisfaction with clinical practice. Repeated research through sampling based on probability to generalize relevance of motivation of achievement and satisfaction with clinical practice is needed and further researches on developing program to inspire motivation of achievement in theoretical learning as well as in clinical practice are required.

병원 간호사의 중동호흡기증후군 격리 지침에 대한 지식과 수행도 (Knowledge and Practice of Middle East Respiratory Syndrome Isolation Precaution among Hospital Nurses)

  • Purpose: The purposes of the study were to examine the knowledge and practice about Middle East Respiratory Syndrome (MERS) isolation precaution, and to explore influencing factors of the isolation practice among hospital nurses. Methods: A total of 182 nurses were recruited from four general hospitals where MERS patients had been treated. The knowledge and practice of MERS isolation precaution were measured by the scales developed based on the CDC guidelines. The collected data were analyzed by SPSS/WIN 22 with descriptive statistics, T-test, One-way ANOVA, Pearson correlation coefficients, and hierachical multiple regression analysis. Results: The nurses were 30 years old in average, and half of them had less than 5 years of clinical experience. and knowledge on droplet precautions (${\beta}=.171$, p=.019) were the significant predictors, explaining 19.6% of variance in the MERS isolation guideline practice. Clinical experience (${\beta}=.225$, p=.002), working at infection unit (${\beta}=-.203$, p=.011). Conclusion: The knowledge on droplet precaution and general knowledge on MERS were the important modifiable factor to improve the MERS isolation guideline practice among hospital nurses, even after adjusting clinical experience and demographic variables. It is necessary to develop an efficient education program on specific guidelines for prevention and management of infection by improving the knowledge on infectious disease such as MERS as well as droplet precaution which are modifiable factors.

간호학생의 비판적 사고능력, 의사소통능력, 리더십, 전문직관이 임상실무능력에 미치는 영향 (The Influences of Critical Thinking Ability, Communication Skills, Leadership, and Professionalism on Clinical Practice Ability in Nursing Students)

  • Purpose: The purpose of this study was to identify factors that affect nursing students' clinical practice ability. Methods: The data were collected from 303 nursing students who had more than 500 hours of experience in clinical practicum at 4 universities in Seoul and the metropolitan area. The instruments consisted of 27 items of critical thinking, 19 items of professionalism, 40 items of self reported leadership, 20 items of communication evaluation tool, and 61 items of nursing practice performance evaluation. Results: For the clinical practicum, most difficult for nursing students were cardiopulmonary resuscitation, fecal enema, stoma care, and blood transfusion. Clinical practice abilities were positively correlated with nursing professionalism(r=.26, p<.001), leadership (r=.16, p=.007) and critical thinking(r=.12, p=.031). Professionalism(${\beta}=.32$, p=.001) was the most significant factor influencing the clinical practice ability of nursing students. Critical thinking was the second largest factor but not significant(${\beta}=.16$, p=.058). Conclusion: The findings suggest that the nursing curriculum should include nursing knowledge and nursing skills as well as various case-based or field-based decision making training programs to cultivate professionalism, critical thinking and other abilities for clinical practice.

Text Network Analysis를 이용한 간호관리학 실습경험 분석 (Analysis of Students Experience related of Nursing Management Clinical Practice: Text Network Analysis Method)

  • Purpose: The purpose of this study was to analyze students experiences during clinical practice in nursing management. Methods: Assessing through computerized databases, self-reflection reports of 57 students were analyzed. Text network analysis was applied to examine the research. The keywords from each student's reports were extracted by using the programs, KrKwic and NetMiner. Results: The results of the keyword network analysis of what students learned in the nursing process included 27 words. The keyword network analysis of what students learned from the problem solving process included 23 words and the keyword network analysis of improvements in Clinical Practice of Nursing included 31 words. Conclusion: Studies related to clinical practice have been increasing, and themes of the studies have also become broader. Further research is required to investigate factors affecting clinical practice specifically in nursing management. Further comparative studies are necessary to define differences in clinical practice systems related to improving nursing students competency.

특실병동 간호사의 실무적응과정에서의 의사결정 경험 (Decision Making Experience of VIP Ward Nurses in the Clinical Practice Adjustment Process)

  • Purpose: The purpose of this research was to describe the decision making experience of VIP ward nurses in the clinical practice adjustment process. The research question was about "how nurses adjust their clinical practice to nursing situations and develop decision making process in a VIP ward." Method: The methodology of collecting and analyzing the data was based on the grounded theory of Strauss and Corbin(1998). The data was collected through in-depth interviews with 10 nurses from July to November, 2007. Results: The core category of nursing care decision making process was named as "adjusting with flexibility and deepened insight." The clinical practice adjustment process in nursing care decision making has progressed through four preceding interlocking phases: 1) dependent phase, 2) defensive phase, 3) independent phase, 4) integrative phase. These phases were classified by the level of nurses' dependency, proactivity, presupposition and integration. Conclusion: The result of this study indicated that nurse's decision making depended on their experiences and the nature of social context in which nursing occurs. Therefore, it is important to elaborate an effective training program for nurses to develop the phases of nursing care decision making.
