The Clinical study of Su-Gi therapy's Effects on Bell's palsy by observing of DITI (DITI로 관찰한 Bell's palsy에 미치는 수기요법의 영향에 관한 임상적 연구)

  • Objective : The purpose of this study is to investigate the effect of Su-Gi therapy for Bell's palsy by using DITI. Methods : We investigated 16 patients with Bell's palsy who had visited in the H Korean medicine hospital in Gyeonggi Province from December 27th, 2010 to April 8th, 2015. The Su-Gi therapy was done by 1 times daily. We evaluated the change of them by using Digital Infrared Thermographic Imaging and Yanagihara's unweighted grading system. Results : There aren't meaningful differences in values for the meridian points in pretest and posttest which were observed by DITI of abnormal side and normal side. There are meaningful differences in values, for abnormal side and normal side of the meridian points in pretest and posttest of DITI. It was of significance that pretest Y and average ΔT of each the meridian points in the type of hyperthermia but not in the type of hypothermia. In the Correlation analysis of values of pretest and posttest, chaotic aspects of body heat distribution in the pretest change as a relatively consistent aspects in the posttest. It wasn't of significance that Correlation Analysis of Ups and downs in temperature of TE17 and Recovery speed observed by ΔY. In simple regression analysis of posttest's Y-system values against absolute ΔT by subtracting ΔTE17 from ΔST6, we didn't predict in the pretest, but could predict significantly in the posttest(Regression coefficient : -2.11) In the regression analysis result of the meridian points' ||pretest ΔT|-|posttest ΔT||, 陽白(GB14) and 頰車(ST6) are of significance (Total R-Square=0.447). But we couldn't obtain final regression analysis model. Conclusions: These results suggest that Su-Gi therapy may be effective for Bell's palsy.

  • Objectives : This study is designed in order to evaluate oriental medical treatment of deveation of the eye and mouth caused trauma. Methods : The authors observed patient by Yanagihara's unweighted grading system for operated acupuncture treatment, herbal medicine treatment and physiotherapy. Conclusion : 1. Deveation of the eye and mouth is caused by trauma ; intra cranial trauma, intra temporal bone trauma, extra, temporal bone trauma, etc. Cardinal symptom is palsy of Facial muscle, slobbering, articulation disorder, epiphora, ear pain, hyperacusis, laterality hypogeusia. 2. Deveation of the eye and mouth patient by Lt. temporal bone Fx. is seen evaluate of Yanagihara's total score ; from S to 35. 3. Deveation of the eye and mouth patient by facial nerve inhury is seen evaluate of Yanagihara's total score ; from 10 to 30. 4. Traumatic Deveation of the eye and mouth patient evaluate by oriental medical treatmend ; acupuncture treatment, herbal medicine treatment and physiotherapy. This is based on sil(賞) of stomach channel of foot yangming & larhe intestine channel of hand Yangming.

Effect of Acupuncture Complex Therapy on Three Patients with Bell's Palsy During Chemotherapy (항암치료중 구안와사가 발생한 환자 3례에 대한 고찰)

  • Objectives : The aim of this study was to investigate the effect of oriental medical treatment on Bell's palsy patients receiving chemotherapy. Methods : A clinical study was done on 3 patients who were diagnosed and treated as Bell's palsy at the Dep. of Acupuncture and Moxibustion College of Oriental Medicine Daejeon University. The effects of combined treatment of acupuncture, herbal medicine and physiotherapy were evaluated with Yanagihara's unweighted grading tool. Results : 1. There was significant improvement on Yanagihara score after treating with Gunbiikqi(健脾益氣) method, which was based on pattern differentiation as Hur(虛證). 2. Three patients commonly complained heaviness, dyspepsia, anorexia after receiving chemotherapy and oriental medical treatment showed significant improvement on the symptoms. 3. All three cases received herbal medicine treatment and liver function test. Three cases showed normal range of liver function during and after the treatment. Conclusions : Oriental-medicine treatment did not damage in liver function and showed significant improvement on Bell's palsy in patients receiving chemotherapy. But it is considerd that we need to keep limits in getting enough patients.

A Clinical Study of 17 Cases of Patients with Big Toe Extensor Muscles Weakness Caused by Lumbar Disc Herniation (요추 추간판 탈출증으로 유발된 족무지 신전 근력약화 환자 17례의 임상적 고찰)

  • Objectives : To investigate the improvement rate of muscle weakness caused by lumbar disc herniation, 17 patients was treated with oriental medicine. Methods : To evaluate weakness of big toe extensor muscles caused by lumbar disc herniation, we measured the active range of motion(ROM) of big toe extension and muscle grading of big toe extensor muscles. We also measured numerical rating scale(NRS), oswestry disability index(ODI) and compared them with the active ROM of big toe extension. Results : 1. Progressive weakness of big toe extensor muscles was observed in 2 patients. Weakness of big toe extensor muscles were improved in 13 patients(76%). 2. Changes in manual muscle testing(MMT) scores corresponded with improvement rate of big toe extension. 3. Numerical rating scale(NRS) and oswestry disability index(ODI) improvement rate did not correspond with improvement rate of big toe extension. Conclusions : Muscle weakness caused by lumbar disc herniation can be treated with oriental medicine and progressive muscle weakness must be observed constantly.

  • Objectives : This study has been designed and performed to identify the effect on Bell's palsy according to the injection of herbal medicine induced from the Hominis Placenta. Methods : We measured the facial palsy changes of the patients who were admitted for Bell's palsy in the Oriental Medical hospital of Dongeui medical center from 03-01-2004 to 07-31-2004. Bell's palsy patients were divided into two groups. One group(A group) was injected with Hominis Placenta herbal acupuncture. The other group(B group) was injected with normal saline. Then effects of these treatment was evaluated by Yanagihara's unweighted grading system. Results : A group was marked more higher than B group in treatment outcome. we discovered that it is significant differences between two groups after 4 week. Conclusions : These results provided that A group is more effective than B group. For clearly proving the effect of Hominis Placenta herbal acupuncture on Bell's palsy, it is need more sample's number and more treatment's duration.

Clinical Outcomes of Splenic Injury (비장 손상의 임상적 치료 결과)

  • Purpose: The management of splenic injuries has shifted from a splenectomy to splenic preservation owing to immunity. The purpose of this study was to assess the kinds of management and outcomes through a review of our experience with splenic injuries. Methods: We retrospectively reviewed 47 patients with traumatic splenic injuries using by electronic medical records from Jan. 2007 and Dec. 2011. Splenic injuries were classified according to the American Association for the Surgery of Trauma (AAST) grading system. Results: There were 11 falls, 11 traffic accidents, 10 motorcylcle accidents, 10 pedestrian accidents and 5 abdominal blunt traumas. Low-grade injured patients (${\leq}$ Grade III) were 29 of 43(61.7%), and High-grade injured patients (${\geq}$ Grade IV) were 18 of 43(38.3%). In 34 patients, non-surgical treatment was performed, and 14 patients underwent a splenectomy. There were relatively more high-grade in older patients, and the high-grade-injury group showed need for a transfusion (p=0.002), more need for a splenectomy (p<0.001), a longer mean hospital stay (p=0.036), a longer ICU stay (p=0.045) and more combined organ injury (p=0.036). Conclusion: Conservative treatment should be considered in low-grade-injury patients (${\leq}$ Grade III). A Splenectomy was performed on 56% of the patients with Grade IV injuries, so a splenectomy should be considered carefully in such patients. In patients with a grade V injury, we think surgical treatment may be needed.

Spontaneous Resolution of Chronic Subdural Hematoma : Close Observation as a Treatment Strategy

  • Objective : Chronic subdural hematoma (cSDH) is common condition in neurosurgical field. It is difficult to select the treatment modality between the surgical method and the conservative method when patients have no or mild symptoms. The purpose of this study is to provide a suggestion that the patients could be cured with conservative treatment modality. Methods : We enrolled 16 patients who had received conservative treatment for cSDH without special medications which could affect hematoma resolution such as mannitol, steroids, tranexamic acid and angiotensin converting enzyme inhibitors. The patients were classified according to the Markwalder's Grading Scale. Results : Among these 16 patients, 13 (81.3%) patients showed spontaneously resolved cSDH and 3 (18.7%) patients received surgery due to symptom aggravation and growing hematoma. They were categorized into two groups based on whether they were cured with conservative treatment or not. The first group was the spontaneous resolution group. The second group was the progression-surgery group. The mean hematoma volume in the spontaneous resolution group was 43.1 mL. The mean degree of midline shift in the spontaneous resolution group was 5.3 mm. The mean hematoma volume in the progression-surgery group was 62.0 mL. The mean degree of midline shift in the second group was 6 mm. Conclusion : We suggest that the treatment modality should be determined according to the patient's symptoms and clinical condition and close observation could be performed in patients who do not have any symptoms or in patients who have mild to moderate headache without neurological deterioration.

Clinical Comparison Studies on Bell's Palsy Patients by Existence of Postauricural Pain (이후통과 Bell's palsy의 예후와의 상관성 연구)

  • Objectives : This study was designed to evaluate the influence of postauricular pain on Bell's palsy patients. Methods : We investigated 71 cases of patients with Bell's palsy and classified them as existence of Postauricural pain, 71 patients were sequentially interviewed and examined. We evaluated the treatment effect of each group by using Gross Grading System of House-Brackmann(H-B grade) before treatment and after final treatment and we researched differences of sequelae of Bell's palsy, period of treatment, changing point -period from onset of Bell's palsy to the day which the change begins to be seen at the face- and improvement -period which Bell's palsy is improved from onset to H-B gradeII. Results : 1. In age, sex, lesion, duration of disease, we found that two groups have no significant differences. 2. In improvement and period of treatment, we found that two groups have significant differences. In changing-point, we found that two groups had the difference of the average, but they were not statistically significant. 3. As a result of evaluation by using H-B grade, treatment score after final treatment was marked higher than that before treatment within each group. 4. After final treatment, Non-postauricular pain group had significant difference(result) on H-B grade compared with Postauricular pain group. 5. In frequency of sequelae symptoms of Bell's palsy, Postauricular pain group had more higher compared with Non-postauricular pain group. Conclusion : These results suggested that Non postauricular pain group should be get better than Postauricular pain group.

Objective Evaluation of Sublingual Veins Using Color Differences (색차(色差)값을 활용(活用)한 설하락맥(舌下絡脈)의 정량적(定量的) 평가(評價))

  • Background: Although several efforts have been made recently to grade the severity of sublingual veins, these methods still meet some problems: clinicians' subjectivity and diversity of lighting conditions. Objectives: The purpose of our study was to quantify the severity of sublingual veins using CDs (color differences), and to verify clinical validity of the parameters of sublingual veins using CDs. Methods: We photographed the inferior surface of subjects' tongue from 74 subjects. CDs were computed through two sets of mean $L^*a^*b^*$ values extracted from BRC (black reference color) and the inferior surface of the tongue. We assumed that SVI (sublingual veins index) normalized percentage of a reciprocal of CDs between BRC and the inferior surface of the tongue including sublingual veins and BISV (black index of sublingual veins) percentage of a reciprocal of CDs between BRC and the main trunk of sublingual veins could be applied as quantitative parameters of sublingual veins and examined whether there are meaningful correlations between CDs and subjective grading by the clinicians or not. Results: There were high positive correlations of SVI and BISV to clinician evaluation respectively (r=0.715, 0.634). We computed a multiple regression equation which includes SVI and BISV as independent factors $(r^2=0.60);\;Y=0.711+0.046X_1+0.205X_2$ (Y: the severity of sublingual veins, $X_1:\;SVI,\;X_2:\;BISV$). Conclusions: We conclude that the severity of sublingual veins can be quantified through SVI and BISV, minimizing the clinicians' subjectivity and the diversity of lighting conditions.

Values of Three Different Preoperative Regimens in Comprehensive Treatment For Young Patients with Stage Ib2 Cervical Cancer

  • Objective: To compare the clinical efficacy of concurrent chemoradiotherapy, neoadjuvant chemotherapy, and intracavity brachytherapy in comprehensive treatment for young patients with stage Ib2 cervical cancer. Methods: One hundred and twelve young patients with stage Ib2 cervical cancer were enrolled retrospectively in our hospital from January 2003 to June 2005. They were categorized into three groups according to preoperative regimens, including the concurrent chemoradiotherapy group (Group 1, n=38), the neoadjuvant chemotherapy (Group 2, n=49), and the intracavity brachytherapy group (Group 3, n=25). Radical hysterectomy was performed following these regimens. Chemotherapy and radiotherapy were given according to pelvic lymph node metastasis, deep cervical stromal invasion, intravascular cancer emboli, histological grading, vaginal stump and positive surgical margin. Results: The cancer disappearance and superficial muscle invasion rates were statistically significantly better in the concurrent chemoradiotherapy group than in the other two groups (P<0.01). No statistically significant difference was noted in the deep muscle invasion rate, surgical time and intraoperative blood loss among three groups, but significantly more postoperative complications occurred in the concurrent chemoradiotherapy group. The 2-year pelvic recurrence was statistically significantly lower in the concurrent chemoradiotherapy group compared to other two groups, while the 5-year survival was higher. Conclusion: Concurrent chemoradiotherapy is efficacious for young patients with stage Ib2 cervical cancer.
