  • The mumps virus is a single-stranded, non-segmented, negative-sense RNA virus belonging to the $Paramyxoviridae$ family. Mumps is characterized by bilateral or unilateral swelling of the parotid gland. Aseptic meningitis is a common complication, and orchitis is also common in adolescents and adult men. Diagnosis is based on clinical findings, but because of high vaccination coverage, clinical findings alone are not sufficient for diagnosis, and laboratory confirmation is needed. Mumps is preventable by vaccination, but despite high vaccination coverage, epidemics occur in several countries, including Korea. Many hypotheses are suggested for these phenomena. In this review, we investigate the reason for the epidemics, optimal methods of diagnosis, and surveillance of immunization status for the prevention of future epidemics.

  • Intestinal malrotation is usually asymptomatic and most often is found during abdominal exploration for other surgical diseases. However, a serious complication of intestinal malrotation, midgut volvulus is a true surgical emergency of childhood. The clinical findings, diagnostic procedures, treatment, and prognosis of midgut volvulus were reviewed by a retrospective study. Between 1980 and 2005, 29 patients with midgut volvulus ranging in age from 1 day and 15 years were treated at HanYang University Hospital. Seventy-nine percent of the patients presented before 1 month of age. Midgut volvulus occurred 2 times more frequently in male. The clinical findings were bilious vomiting (96.6 %), irritability (34.5 %), abdominal distention (13.8 %), abdominal pain (10.3 %), and palpable abdominal mass (6.9 %). The diagnosis was made by abdominal simple x-ray (17.2 %), upper gastro-intestinal contrast study (37.9 %), abdominal sonogram (20.7 %), abdominal CT (3.4 %), and abdominal exploration (20.7 %). Among the 29 patients, 5 patients developed gangrene of small intestine due to strangulation and underwent resection of bowel. Two patients died due to sepsis.

  • This study investigated the differences between adolescents' own perceptions of their psychopathology and perceptions by clinically depressed parents of their adolescents' psychopathology. The study also examined parental characteristics that accounted for discrepancies between parents and adolescents. The clinical sample consisted of 61 adolescents and their parents who were diagnosed with a major depressive disorder. The adolescents and parents evaluated the adolescents' psychopathology in separate interviews with the Child Behavior Checklist (CBCL) and the Youth Self-Report (YSR). Parents reported on current depressive symptoms and parenting practices using questionnaires. The results revealed that parent-adolescent discrepancies were greater in regard to affective and anxiety problems compared to oppositional defiant and conduct problems. Parental rejection was associated with differences in scores for affective problems after controlling for parents' current depressive symptoms and adolescents' age and gender. The findings highlight the importance of considering adolescents' affective and anxiety problems when treating depressed parents. Furthermore, the findings suggest that parental rejection may play a pivotal role when interpreting the discrepancy concerning adolescents' affective problems.

  • Several authors suggested that the clinical characteristics of incomplete presentation of Kawasaki disease are similar to those of complete presentation and that the 2 forms of presentation are not separate entities. Based on this suggestion, a diagnosis of incomplete Kawasaki disease in analogy to the findings of complete presentation is reasonable. Currently, the diagnosis of incomplete Kawasaki disease might be made in cases with fewer classical diagnostic criteria and with several compatible clinical, laboratory or echocardiographic findings on the exclusion of other febrile illness. Definition of incomplete presentation in which coronary artery abnormalities are included as a necessary condition, is restrictive and specific. The validity of the diagnostic criteria of incomplete presentation by the American Heart Association should be thoroughly tested in the immediate future.

  • Central neurocytoma is a rare, well-differentiated neuronal tumor and is usually located in the lateral or third ventricle of young adults. The occurrence of an intraventricular tumor with a characterisitic magnetic resonance image findings including isointense signal in T1-weighted images, the presence of a cystic component, small signal-void areas due to calcification, heterogenous and hyperintense "bubbly" appearance in T2-weighted images in a young patient should suggest preoperatively the diagnosis of central neurocytoma. The typical immunohistochemical finding, positivity for synaptophysin, is the main pathological feature. We experienced two cases of central neurocytomas with typical radiological and histopathological findings. We expect growth arrest of these cases by subtotal removal to avoid postoperative neurologic deficit followed by radiation therapy.

  • Flail arm syndrome (FAS), known as one of the atypical amyotrophic lateral sclerosis (ALS) variants, has a similar clinical course and pathologic findings as ALS. Therefore it is difficult to differentiate between ALS and FAS at a glance. There are few reports involving individual analysis of FAS patients to date. The findings of polysomnography (PSG) in patient with FAS are not well known. We report a male FAS patient with review of literatures and several issues related to the diagnostic process.

  • Hashimoto's encephalopathy (HE) is a heterogeneous encephalopathy with diverse clinical presentations. Here we report on a 69-year-old woman who presented with confusion, aphasia, fever, and focal ictal discharges. Cerebrospinal fluid analysis and a workup for other fever origins revealed no abnormality and a high level of thyroperoxidase antibody was detected, which findings led to a diagnosis of HE. The symptoms subsided after treatment. This study highlights the importance of considering HE in patients presenting with fever and abnormal EEG findings.

  • Objectives : The purpose of this clinical study is to evaluate the clinical efficacy of Pasaekhwalhyeol-Tang on acute headache from acute cerebral infarct. Methods : Pasaekhwalhyeol-Tang was prescribed to 15 acute cerebral infarct patients for two weeks. Symptoms were checked and VAS(Visual Analog Scale) scores were kept every week. Results : After prescription of Pasaekhwalhyeol-Tang, VAS scores on headache and accompanying symptoms decreased. No significant changes were shown in laboratory findings. As for the laboratory findings, ALT showed an upward tendency within a week and increased over normal values in two cases(13%). But two weeks later, ALT decreased within normal values. And the mean serum level was within normal values and the other hepatic enzymes did not increase over normal values in either week nor week 2. Conclusions : Results suggest that Pasaekhwalhyeol-Tang is effective in reducing the severity of acute headache from acute cerebral infarct.

  • The present studies were undertaken in attempt to investigate the therapeutic effect of ethanol in dogs intoxicated with ethylene glycol Three dogs treated with ethylene glycol and other three dogs with ethylene glycol plus 20% ethanol orally were examined on clinical signs, endoscopic views, histopathological findings, and autopsy findings respectively. The results obtained were summarized as fellows : 1. The clinical sings and their severity of dogs intoxicated with ethylene glycol were time related and progressed from vomiting, depression, thirsty, tachycardia, tachypnea, convulsiot ataxia, melena, uremia and coma, but clinical signs of dogs treated with ethylene glycol and ethanol simultaneously only stowed vomiting and thirsty. 2. In the gastroscopic view, the dogs intoxicated with ethyelne glycol showed edematous, hyperemia, errosive and ulcerative lesions in the fundus and body area but the dogs treated with ethylene glycol and ethanol simultanously showed edematous and hyperemic lesions. 3. Oral treatment of ethanol with ethylene glycol simultaneously have reduced the signs of EG intoxications in dogs.
