Estimation of Supply and Demand for Cardiologists in Korea

  • Background and Objectives: The objective of this study was to estimate the supply and demand for cardiologists in Korea and provide evidence for healthcare policy to ensure a stable and adequate workforce for optimal cardiovascular disease management. Methods: Past trends of inflow and outflow of cardiologists were used to make crude projections, which were then adjusted based on demands of services to obtain final projections. Inflow of cardiologists was estimated using second-order polynomial regression and demand for cardiology care was estimated using linear regression. Results: There were 1,139 active cardiologists who were under the age of 65 in clinical practice in Korea. The estimated number of cardiologists from 2022 to 2040 showed that the number of cardiologists would peak at 1,344 in 2032 and gradually decrease thereafter. We also estimated an increase of 947,811 cases of heart-related procedures annually from 2023 to 2032. The number of heart-related procedures per cardiologist would increase 1.4 times from 12,964 in 2023 to 17,862 in 2032. The estimated number of emergency patients per cardiologist under 50 years old would almost double from 544 in 2022 to 987 in 2032. Conclusions: We expect significant shortage of cardiologists in Korea within the next 10 years. The number of emergency patients per cardiologist will increase by nearly 50%, leading to high individual workload for cardiologists. To prevent this imbalance between supply and demand, an organized and collective approach by the specialty of cardiology is imperative to produce a balanced workforce.

The Effect of Job-stress and Self-efficacy on Depression of Clinical Nurses (임상간호사의 직무 스트레스와 자기효능감이 우울에 미치는 영향)

  • Purpose: The purpose was to investigate the relations among job-stress, self-efficacy, and depression of nurses. Methods: The data were collected from a random sample of 213 nurses working in two general hospitals of a local area. A self-reported questionnaire was used to assess the level of job-stress, self-efficacy, and depression. Results: The mean score of job-stress was 49.1 and the score of job demand was the highest. The mean score of self-efficacy was 3.4, and depression was 18.2. The prevalence of depression was very high. The job-stress and depression were negatively correlated with self-efficacy. Hierarchial multiple regression showed that the self-efficacy and the high job demand, lack of reward, and organizational injustice of job-stress explained 53% of the variance for the nurses' depression. Conclusion: The findings indicated that the self-efficacy and job stress, especially job demand, organizational injustice, and lack of reward contributed to the depression. In order to prevent and decrease the depression, the developing programs to improve self-efficacy are needed.

A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization (한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

Development of Training Instruments on Visual functions using HMD type Display and Investigation of its Demand

  • We constructed 3-D images by HMD (Head Mount Display) type and polarization technique through which a training instrument of binocular function based on clinical vision therapy was developed and investigated its demand. To design the instrument that performs Test- Diagnosis - (Prescription) - Training - Treatment we provided a customized training method for the trainers using optical IT instruments of binocular test. Through the contents and the instruments of the vision therapy technology developed in this work we can replace the current vision test instruments to those costing 60% lower price in eye clinics and eye glasses shops. By replacing the existing vision test instruments it is possible to create a new market of the vision therapy in vision training instruments.

  • The demand of cyclotron for PET (positron emission tomography) has rapidly grown as the more use of PET or PET-CT equipment requires the increased amount of radioactive isotopes for clinical positron emission. While research on failure statistics of medical equipment used in medical centers has continued to be done, the analysis and study on failure statistics of cyclotron have rarely been conducted. However, the growing demand shows the urgency of systematical management guideline and countermeasures for device failure to minimize any supply delay of radiopharmaceuticals occurred by such failure and complains from waiting patients for PET-CT diagnosis. Therefore, this study aims to analyze the failure report from Minitrace equipped in cyclotron which is owned by the department of nuclear medicine at Yeungnam University Medical Center and draws on the data to build effective management system for cyclotron.

A Study on Job Stress and Emotional Burnout of Clinical Nurses

  • This study attempts to provide basic data for the development of manpower maintenance programs by checking the degree of job stress and emotional burnout for nurses working in a university hospital and identifying factors affecting emotional burnout. Data were obtained through a structured questionnaire survey conducted on 187 nurses. The average score for job stress of nurses was 2.50 (range 1 to 4) and emotional burnout was 3.29 (range 1 to 5). The factors affecting emotional burnout were occupational climate, job demand, job insecurity, and lack of reward, which accounted for 44% of explanatory power. In order to reduce the emotional burnout of nurses, the management of medical institutions needs administrative and financial support. Further, it is necessary to improve the organizational culture regarding job assignment through job analysis, employment security, and a performance-based reward system.

  • The development of medical devices based on the theory of tranditional Korean medicine is increasing due to population aging and emerging demand for medical services. Various clinical trials are being conducted to ensure the clinical effectiveness of the developed medical devices, and the importance of systematic management of variously collected information in this process is increasing. In this paper, we designed and constructed the database that can systematically manage the medical information of the patient and the pulse measurement information in conducting clinical trials by using pulse analysis system which is a representative diagnostic method of traditional Korean medicine. The constructed database can be efficiently used for the verification of existing algorithms or developing new algorithms through the quality control of various information and measured data on clinical trials. It also has several advantages in the control of clinical data. The results of this study can contribute to the establishment of Korean medical device data standard for the construction of integrated database management system that can share information with different kind of pulse analysis system and other bio-signal measuring devices.

A Study on the Protection of Trial Subjects in Clinical Trials of Investigational New Drug (의약품 임상시험에서 피험자 보호)

  • This study focuses on the protection of trial subjects, who participate in clinical trials for new drug. It takes long time to develop new drugs and the clinical trials are required. Usually, pharmaceutical company, which develop new drug, request a research institution(usually, hospital) to investigate the examination of security and side effects of new drug. The institution recruit trial subject to participate in the trials. The contract for clinical research of investigational new drug is concluded between the pharmaceutical company and the institution. This thesis studies the legal regulations for protection of participants of clinical research for new drug. In this respect the first matter of this study is to seek which relation between pharmaceutical firm and participants of clinical trials. Especially, there is a question which the trial subject is entitled to demand the pharmaceutical company which requested clinical trials the institution to supply the investigational new drug, after the contract for clinical trials had terminated or cancelled. This study take into account the liability of the pharmaceutical company to trial subject. Secondly, it is researched the roles and authority of Institutional Review Board(IRB). IRB is Research Ethics Committee of the institution, in which clinical trials for new drug are conducted. According to the rule of Korea good clinical practice(KGCP), IRB is the mandatory organization which is authorized to approve, secure approval or disapprove the clinical trials for investigational new drug in the institution. The important roles are the review of ethical perspective of trial research and the protection of trial subject. Thirdly, this paper focuses if the participants are to be paid for the participation for clinical research. This is ethical aspect of clinical trials. It is resonable that the participant is reimbursed for expenditure such as travels, and other expenses incurred in participation in trials. It is not allowed that the benefit of clinical trials is paid to trial subject. The payment should not function as financial inducements for participations of trials. Finally, the voluntary consent of the trial subject is required. The institution ought to inform the subject, who would like to participate in trials, and it ought to received informed consent in writing for subject. In this regard, it is matter that trial subject has ability of consent. It is principle that the subject as severely psychogeriatric patient has not ability of consent. However, it is required that not only healthy people but also patients are allowed to take part in clinical trials of new drug, in order to confirm which the investigation new drug is secure. Therefore there are cases, in which the legal representative of subject consent the participation of the trials. In addition, it is very important that the regulations concerning clinical trials of new drug is to be systematically well-modified. The approach of legal and political approach is needed to achieve this purpose.

Connected Factors of Ethical Perception in Dental Hygiene Students (일부 치위생과 학생들의 윤리의식에 미치는 요인에 관한 연구)

  • Ethical perception may be thought of as 'professional skills'. Despite this, however, the dental hygiene curriculums tend to disregard ethics education. This paper describes ethical perception in dental hygiene students. Questionnaire surveys were conducted for 374 in dental hygiene students. For statistical analyses of collected data, the multiple linear regression were adopted. Demand as necessity of dental ethics education, clinical practice exercise, practice of work ethics education were found to have significant major effects on basic ethics perception. The major variables which related to the licensure and health care law and teamwork were to knowledge of revised ethics code of dental hygienist and to demand as necessity of dental ethics education. The major variables influencing the professional protocol were to knowledge of revised ethics code of dental hygienist, to demand as necessity of dental ethics education, role model for learning ethical principles. Revised ethics code of dental hygienist was the most important factor affecting professional protocol. The major variables influencing the decision-making were to demand as necessity of dental ethics education, to knowledge of revised ethics code of dental hygienist, clinical practice exercise, role model for learning ethical principles (church). About 88.4% of the students unawared to knowledge of revised ethics code of dental hygienist and perception of professional protocol of the students awared to knowledge of revised ethics code of dental hygienist was $3.44{\pm}0.23$ (p=0.000).
