A Case of Cerebral Infarction Patient Complaining Post-Stroke Aggression Accompanying Anxiety Treated with Combined Korean Medical Treatment (뇌졸중 후 불안을 동반한 공격성을 호소하는 뇌경색 환자에 대한 복합 한의치험 1례)

  • Post-stroke aggression refers to aggressive behaviors and languages which are occurred after stroke. In stroke patients, over 15% suffer from the anger and the symptom lowers the quality of life of the patients, family, and caregivers. After diagnosing post-stroke aggression, selective serotonin re-uptake inhibitor(SSRI) and beta-blocker, beta adrenergic agonist treatment usually underwent to treat the anger and anxiety of the patient. In the present case, a 81-year-old female patient with post-stroke aggression underwent Korean medical treatment by Jodeungsan and Ukgansan for 49 days. The effect of the treatment was assessed with State-Trait Anger Expression Inventory(STAXI) per a week, roaming time per a week and roaming frequency per a week. After the treatment, STAXI score, roaming time, and roaming frequecy decreased, and the patient could successfully cease to take beta-blocker, which she started to take for treating her aggression, without any side-effects. This case report suggests that Ukgansan might be an effective option with post-stroke aggression patients taking medication including beta-blocker.

A Case of Soeumin Rectal Cancer Misdiagnosed as a Taeyangin and Mistreated with Mihuedungsikjang-tang (미후등식장탕으로 오치하여 복통(腹痛), 자한(自汗), 오심(惡心)이 발생한 소음인 직장암 환자 1례 증례보고)

  • In this case, we report a case of Soeumin rectal cancer patient who was mistreated with herb medication. This patient was 72 years old female diagnosed rectal cancer stage IV with multiple liver, lung, and LN metastases. She administered our cancer center in Sep. 2011 for cancer treatment, we diagnosed this patient as Taeyangin at first, and treated with Mihuedungsikjang-tang. After 1 week, she complained upper abdominal discomfort, severe abdominal pain, nausea and sweating. Considering several symptoms and conditions, the change of symptoms seemed to a adverse events caused by mistreatment. We re-diagnosed this patient as Soeumin, and treated with Soeumin herbal medication, Baekhaoleejung-tang(白何烏理中湯) and Hyangsayangyi-tang(香砂養胃湯). Several symptoms including abdominal pain were improved 2 weeks later. Therefore, we report this case as a reference that Soeumin misdiagnosed as Taeyangin and the improvement of adverse symptoms.

Review on Clinical Study of Oriental Medical Treatment for Cancer in Korea (국내 한의학 암치료 임상연구 논문에 대한 고찰)

  • Objectives : The aim of this study is to evaluate present clinical evidence of oriental medical treatment for cancer in Korea. Method : A search was performed in the Korean Studies Information, Korea Institute of Science and Technology Information, KoreaMed, Oriental Medicine Advanced Searching Integrated System, National Assembly Library, RISS4u, DBPIA, and Oasis using the keyword 'cancer', 'leukemia', 'malignant tumor', 'lymphoma', 'multiple myeloma', 'melanoma'. The search period spanned between Jan 1, 1980~June 30, 2011 and the results were analyzed and evaluated according to the publication date, journal, method of treatment, type of study, and the primary outcome. Randomized clinical trials (RCTs) and Non randomized clinical trial(NRCT) were reviewed separately. Result : The results yielded 133 trials during the period of Jan 1, 1980~June 30, 2011. Most of the trials were concerned with lung cancer and 94 of the trials were simple case studies or case series studies. 61 trials used herbal Formula/herbal medicine as the primary method of treatment. A total of 4 RCTs existed compared to only one NRCT. Conclusion : The use of oriental medicine for the treatment of cancer is gaining popularity in the field of medical scientific research. However most of the studies that have been published up to date are mostly case studies or case series studies, and RCT/NRCT are rare. In order to provide appropriate evidence regarding the effectiveness of oriental medicine in the treatment of cancer, more rigorous and well-designed studies are warranted.

Clinical Remission of Crohn's Disease Induced by Herbal Decoction (Jinmoo-tang): A Case Report (진무탕으로 임상적 관해가 유도된 크론병 환자: 증례보고)

  • Crohn's disease (CD) is an idiopathic inflammatory bowel disease (IBD) with unknown etiopathogenesis. Currently there are no definitive treatment modalities for CD. This case report presents clinical remission and its maintenance of CD induced by Korean herbal medicine treatment. A 56-year-old male patient diagnosed as CD by capsule endoscopy presented to a Korean Medicine clinic with severe abdominal pain as a chief complaint. Despite taking 5-aminosalicylic acid and immunosuppressant for 2 years, his abdominal pain was worsening at the time of visit. Treatment was initiated with Jinmoo-tang based on pattern identification and continued for approximately a year with some modifications according to patient's symptom changes. He was mainly given Jinmoo-Tang for a year and clinical remission was achieved; he could stop taking western medications. For 6 years since then, he took Korean medicine from time to time when he felt his condition went down and he is still maintaining remission. There were no adverse events reported. This case report suggests that individualized Korean herbal medicine treatment may have the potential for induction and maintenance of clinical remission of CD. Further research is warranted to establish an evidence-base for such approach.

A Case Report of a Patient with Parkinson's Disease Treated with Acupuncture and Exercise Therapy (침 및 운동 치료로 호전된 파킨슨병 환자 1례에 대한 증례보고)

  • Objectives: Parkinson's disease is characterized by progressive, irreversible damage to dopamine neurons in the substantia nigra pars compacta, as well as motor and non-motor symptoms. This disease currently has no dependable disease-modifying treatment. In this paper, we describe the treatment of a 67-year-old female with Parkinson's disease using acupuncture and exercise therapy. Case Presentation: Clinical symptoms and the United Kingdom Parkinson's Disease Society Brain Bank Diagnostic Criteria were used to diagnose the patient with Parkinson's disease. Over a 12-week period, the patient visited a Korean medicine hospital 18 times and was treated with acupuncture and exercise therapy in addition to anti-Parkinson's drugs. Before and after treatment, clinical examinations were performed using tools such as the Unified Parkinson's Disease Rating Scale, Fall Efficacy Scale, Parkinson's Disease Questionnaire, Berg Balance Scale, and Non-Motor Symptoms Scale. Furthermore, functional near-infrared spectroscopy was used to assess cortical hemodynamics. All clinical examination results improved after 12 weeks of intervention. In particular, improvements on the Total Unified Parkinson's Disease Rating Scale and Part III of this scale demonstrated large, clinically important differences. Conclusion: This case suggests that combining acupuncture and exercise therapy could produce an effective treatment for Parkinson's disease patients.

The Review on the Domestic Studies of Moxibustion (최근 10년간 국내 뜸관련 연구에 대한 고찰)

  • Objectives : This study is to research the trend of the Moxibustion-related studies and to establish the hereafter direction for the study on Moxibustion. Methods : We reviewed the domestic articles published last ten years(2000~2009). Results : We have searched 45 articles in 4 journals of Korean Acupuncture & Moxibusion, Korean Oriental Medicine, Korean Meridian & Acupoint, Korean Oriental Internal Medicine. 1. There were the most research done 2003, 2008. Both of them have 8 articles. In other years there were 2 or more articles. 2. The pattern of study was as follow: the clinical studies were 23, studies of phisical Characteristics and new moxibustion method were 9, the philological study were 5, the reviewed studies were 3, the experimental studies were 2 and the others were 5. The rate of clinical studies was increased. 3. In the clinical studies, the case-control study and non case-control study were more than case study. The disease after stroke were the most predominant. The rates of direct moxibustion and indirect moxibusion were similar. 4. After beginning 2000s, there were more studies of thermodynamic Characteristics. In the mid 2000s, there were more studies of new moxibustion method appearing. Conclusions : We need more studies about various kinds of diseases, side effect and aftereffects. There should be convenient and useful methods of moxibustion.

A Study on the Space Area Guideline of Clinical Laboratory in Korea - Focused on Laboratory Case Studies (국내 종합병원 진단검사의학과의 면적산정 방법에 대한 연구 - 검사실 사례조사를 중심으로)

  • Purpose: Clinical laboratory of hospital has been demanded to extension or relocation regarding with the test number increase and analyzer development. Space area criteria per test numbers, lab functions, hospital bed and lab staffs are needed for draft space programing. So, the purpose of this study is to provide the space area guideline of clinical laboratory for space calculation in planning and design the spatial environment. Methods: Literature review has been used in checking the standards and guidelines. And questionnaire surveys to laboratory supervisors in hospitals have been conducted for the data collection. 60 answers have been analysed statistically by MS Excel program. Results: The result of this study can be summarized into three way calculations. The first one shows that the basic standard workspace and distance is applied in lab design. The second one shows that average space area criteria resulted from case studies is applied by 19㎡ per one staff, 0.9~1.0㎡ per one bed, and lastly linear length calculation of workbenches and analyzers on the bench top and floor mount, is multiplying it by the sum of the counter depth plus aisle width. Implications: In updating the space area calculation guidelines, it is necessary to cooperate with medical staffs and designers.

Anaphylaxis Induced by Surgical Prophylactic Cefotetan and The Application of World Allergy Organization Guide: A Case Report (사례보고: 수술예방적 항생제 Cefotetan에 의한 아나필락시스 보고 및 World Allergy Organization 가이드라인활용)

  • The definition of anaphylaxis is 'a serious, life-threatening generalized or systemic hypersensitivity reaction' and is considered as the life threatening adverse drug reaction. We experienced a case of cefotetan induced anaphylaxis with negative pre-skin test, used for surgical prophylaxis. A 82-year-old female was scheduled for total knee replacement therapy. She had no previous history of allergy and her skin test results were also negative. On her right knee surgery, she underwent cefotetan therapy as a surgical prophylaxis for a week with no problems identified. Next left knee surgery, she also received the prophylaxis of intravenous cefotetan. However, a few minutes later, anaphylactic reaction developed with vomiting, severe hypotension, bronchospasm, and dyspnea. After immediate intensive care treatment, she recovered without significant complications. Though commonly used laboratory data in case reports, such as the specific IgE, tryptase, histamine, or allergic skin prick test were limited, we successfully confirmed anaphylaxis based on clinical criteria for diagnosing anaphylaxis based on WAO 2011 guideline with through concurrent patient°Øs medical history review and the process of identifying the causes.

A Study on Problem-based Learning Model of Orthopedic Manual Physical Therapy (정형도수물리치료의 문제중심학습 모형에 관한 고찰)

  • Background: The purpose of this study was to develop a problem-based learning model for orthopedic manual physical therapy. A problem-based learning (PBL) model for orthopedic manual physical therapy developed from PBL module of Jeju C university (Halla-Newcastle PBL Center). A summary of this study is as follows: 1) PBL model is comprised of a class of 30 students, operated small group as of 4~5 students. 2) PBL is suggested a scenario of clinical case, induced variety reaction through group discussion and presentation. 3) PBL is occurred wide variety learning through group work activity and self-directed learning. 4) The tutor as a facilitator is played a guide for group discussion, work activity and team learning. 5) The evaluation for PBL is performed such as student self-evaluation, group activity evaluation, individual presentation, and practice. This model is considered wide variety learning through team learning and self-directed learning by clinical reasoning and problem solving for musculoskeletal clinical case. We suggest problem based learning for the education of orthopedic manual physical therapy in which the learners are very interested in and has the effective outcome.
