한약 및 천연물의 항암 임상연구 방법론에 대한 고찰 (Review of Clinical Cancer Research Methodology of Botanical Agents)

  • Objectives : The cancer incidence and cancer burden is increasing. In addition, the use of botanical agents in cancer care is increasing. This article aims to review a research strategy for botanical agents. Methods : The clinical studies of anticancer botanical agents and the papers about clinical research methodology of botanical agents were reviewed. Results : In phase I study, safety confirmation, optimal dose determination and drug interaction study are important. Most botanical agents have low toxicity and some have non-monotone dose response. Therefore, dose-response curve must be evaluated separately from the dose-toxicity curve to determine optimal dose. Although anticancer botanical agents can't shrink tumor size rapidly, they do extend survival. So, in phase II study, response should be evaluated by the survival. Conclusions : Clinical research of botanical agents in cancer is different from traditional methods and strategies. Considering the characteristics of botanical agents and experimental mechanism is necessary in conducting botanical based clinical trials.

  • Background : I investigated clinical research methodology of Traditional Korean Medicine(TKM) for treatment of lung cancer with Evidence Based Medicine(EBM) approach. Methods : I conducted the internet search of National Central Library, the National Assembly Library, Korea Education & Research Information Service, Korea Institute of Science and Technology Information for Korean theses and papers, and MEDLINE for the outside articles. Conclusion : In addition to outcomes of clinical trial, abundant clinician's experience and patients' preference are also considered. Pragmatic trials and quasi-experimental trials are better than clinical trials to plan clinical design of TKM for lung cancer and appropriate endpoints includes overall survival, disease-free survival, patient reporting outcome.

  • Recently, Despite improvements of therapeutic methods on malignancy, the need of modalities on the treatment of cancer have been increasing nowadays and Traditional Oriental Medicine have been considered as alternatives and the uses of it have continued to increase in cancer therapy. The aims of this paper is to analysis of approach modality of experts for the research and treatment on cancer and study of clinical research methodology in Korean Medicine.

Untold story of human cervical cancers: HPV-negative cervical cancer

  • Cervical cancer is the fourth most common malignancy in women worldwide. Although infection from human papillomavirus (HPV) has been the leading cause of cervical cancer, HPV-negative cervical cancer accounts for approximately 3-8% of all cases. Previous research studies on cervical cancer have focused on HPV-positive cervical cancer due to its prevalence, resulting in HPV-negative cervical cancer receiving considerably less attention. As a result, HPV-negative cervical cancer is poorly understood. Its etiology remains elusive mainly due to limitations in research methodology such as lack of defined markers and model systems. Moreover, false HPV negativity can arise from inaccurate diagnostic methods, which also hinders the progress of research on HPV-negative cervical cancer. Since HPV-negative cervical cancer is associated with worse clinical features, greater attention is required to understand HPV-negative carcinoma. In this review, we provide a summary of knowledge gaps and current limitations of HPV-negative cervical cancer research based on current clinical statistics. We also discuss future directions for understanding the pathogenesis of HPV-independent cervical cancer.

앙상블 기법을 활용한 RNA-Sequencing 데이터의 폐암 예측 연구 (A Study on Predicting Lung Cancer Using RNA-Sequencing Data with Ensemble Learning)

  • In this paper, we explore the application of RNA-sequencing data and ensemble machine learning to predict lung cancer and treatment strategies for lung cancer, a leading cause of cancer mortality worldwide. The research utilizes Random Forest, XGBoost, and LightGBM models to analyze gene expression profiles from extensive datasets, aiming to enhance predictive accuracy for lung cancer prognosis. The methodology focuses on preprocessing RNA-seq data to standardize expression levels across samples and applying ensemble algorithms to maximize prediction stability and reduce model overfitting. Key findings indicate that ensemble models, especially XGBoost, substantially outperform traditional predictive models. Significant genetic markers such as ADGRF5 is identified as crucial for predicting lung cancer outcomes. In conclusion, ensemble learning using RNA-seq data proves highly effective in predicting lung cancer, suggesting a potential shift towards more precise and personalized treatment approaches. The results advocate for further integration of molecular and clinical data to refine diagnostic models and improve clinical outcomes, underscoring the critical role of advanced molecular diagnostics in enhancing patient survival rates and quality of life. This study lays the groundwork for future research in the application of RNA-sequencing data and ensemble machine learning techniques in clinical settings.

Optimized Serological Isolation of Lung-Cancer-associated Antigens from a Yeast Surface-expressed cDNA Library

  • The technique of serological analysis of antigens by recombinant cDNA expression library (SEREX) uses autologous patient sera as a screening probe to isolate tumor-associated antigens for various tumor types. Isolation of tumor-associated antigens that are specifically reactive with patient sera, but not with normal sera, is important to avoid false-positive and autoimmunogenic antigens for the cancer immunotherapy. Here, we describe a selection methodology to isolate patient sera-specific antigens from a yeast surface-expressed cDNA library constructed from 15 patient lung tissues with non-small cell lung cancer (NSCLC). Several rounds of positive selection using patient sera alone as a screening probe isolated clones exhibiting comparable reactivity with both patient and normal sera. However, the combination of negative selection with allogeneic normal sera to remove antigens reactive with normal sera and subsequent positive selection with patient sera efficiently enriched patient sera-specific antigens. Using the selection methodology described here, we isolated 3 known and 5 unknown proteins, which have not been isolated previously, but and potentially associated with NSCLC.

한의 종양학 연구 분야에서의 Propensity Score Matching Method 적용에 대한 문헌 고찰 (A Literature Review on the Application of the Propensity Score Matching Method in the Field of Asian Oncology)

  • The Randomized Control Trial (RCT) is the most well-established and widely used statistical methodology in clinical research; however, applying thorough RCT to cancer patients presents challenges such as ethical concerns, high costs, short clinical periods, and limitations in collecting various side effects. To address this issue, the propensity score matching method, which takes advantage of the benefits of observational research while compensating for the drawbacks of randomized control trials, is used in a variety of fields. In recent years, 28 studies on the effectiveness of Korean medicine on tumors have been conducted abroad using the Propensity Score Matching Method, but none have been conducted in Korea. The majority of studies have focused on liver cancer, colon cancer, lung cancer, and stomach cancer, with endpoints such as survival time, incidence rate, quality of life, and treatment outcomes revealing statistical differences in how Korean medicine intervention affects treatment outcomes. As a result, well-established studies using the propensity matching score methodology should be useful in evaluating the impact of Korean medicine in oncology treatments.

  • The precaution and treatment for cancer become inevitable with the rising of morbidity and mortality. In this article, a promising new methodology for cancer treatment, sono-dynamic therapy (SDT) was introduced. In addition, we extensively reviewed the molec-ular mechanisms of SDT killing cancer and summarized the classification of sonosensi-tizers. At the same time, research progress of SDT indicates that it is possible to become a developing field for cancer treatment in clinical application.

한약에 대한 N-of-1 임상시험의 현황 - 문헌 고찰 (Current Status of N-of-1 Trials for Herbal Medicine -Literature Review)

  • Objectives: To overcome the limitations of randomized controlled trials, many other trials design is tested. The n-of-1 trial is a promising research method in the field of Korean medicine because of this methodology can examine the optimal treatment for each patient strictly. Therefore, we reviewed the status of N-of-1 studies on herbal medicine. Methods: A systematic literature review was conducted based on the pubmed database. The search term were 'N-of-1 Trial', 'Chinese Medicine', 'Herbal Medicine', 'Kampo'. There was no restriction in year. Results: Four clinical trials have been identified to demonstrate the effectiveness of herbal medicines for Kidney-yin-deficiency syndrome, bronchiectasis and gastric cancer. These studies suggest that the N-of-1 design is a study that encourages patient involvement, demonstrates the effectiveness of herbal medicines and helps reduce unnecessary medication. Conclusion: The N-of-1 clinical trial may be a rigorous methodology suitable for the clinical setting and may help the development of evidence-based Korean medicine. Attention is also needed in this research method in Korea.

Characterizations of Two-step Matrix Application Procedures for Imaging Mass Spectrometry

  • In this paper, I describe the importance of matrix spraying conditions in imaging mass spectrometry (IMS) to obtain successful imaging results. My developed matrix application methodology, which is a "two-step matrix application" sequentially combined with matrix sublimation and spraying matrix solution can provide high reproducibility and high ion yield compared with a conventional direct spraying method. However, insufficient IMS results were obtained occasionally despite the two-step method. Therefore, I wanted to characterize the methodology to continuously provide high quality data. According to my results, the sublimation time was not a strict parameter, and the most important step was the first spraying condition. This means that the extraction conditions from the tissue section and co-crystallization of the matrix were the most important factors.
