자하거 약침으로 호전된 방아쇠 수지 환자의 증례 보고 (The Case Report of Trigger Finger Improved with Hominis Placenta Pharmacopuncture Treatment)

  • 본원에서 방아쇠 수지 환자 3례를 자하거 약침으로 5-7회 치료한 결과 유의한 효과가 있었다. 그러나 연구자의 치료예가 부족하여 4도 정도로 굴건의 걸림이 심한 경우나, 양방의 보존적 치료 후 재발되는 경우, 증상이 오래된 경우에 대한 연구와 1년 후의 재발율에 대한 추적 조사가 더 필요한 것으로 생각된다. 그리고 방아쇠 수지를 급성기, 결절형의 염증상태와 만성기, 미만형 퇴행상태로 정확하게 구분하여 염증상태에는 봉약침을 사용하고, 퇴행화 상태에서는 자하거 약침을 사용한다면 좋은 효과를 볼 것으로 기대된다. 앞으로 각종 수지관절 및 건의 질환에 대한 치료방법에 대한 연구가 더욱 더 필요할 것으로 생각된다.

  • Science of prescriptions is an important part in the education of Korean Oriental Medicine. In spite of that, there is less agreement on measures for improving the education quality of science of prescriptions. Science of prescriptions can be classified into generalities and particulars. This study sought to present contents that must be incorporated into Introduction to Science of Prescriptions to enhance the quality of education by examining both teaching materials being used in colleges of traditional Chinese medicine and those of Korean oriental medicine and the Introduction part of books related with science of prescriptions. And when this study was carried out, training Korean oriental medicine practitioners and researchers and educators of science of prescriptions was taken into account. It is judged that Introduction to Science of Prescriptions needs to be divided into seven chapters and that each chapter requires containing opinions of ancient doctors and references to lay the basis of learning and revised and practical contents in addition to traditional ones. 정골의학적(Osteopathic) 수기요법 분석 및 경근추나와의 관련성 연구 (Analysis of Osteopathic Manipulation and Study on Relationship with Chuna Manual Therapy for Meridian Sinew System)

  • Objectives : The aim of this study is to analyze the actual investigation and classification of osteopathic manipulation by investigation of the various literature of the inside and outside of the country, and to present the application plan of osteopathy in chuna manual therapy for meridian sinew system. Methods : I referred to the domestic and foreign books about osteopathy and chiropractic. In order to investigate domestic dissertations, I searched 4 Korean medical databases and 4 Korean medical journals of osteopathy. Search terms used were osteopathy, osteopathic, fascia, proprioreceptor, mechanoreceptor, muscle spindle, golgi tendon organ, osteopathic manipulation technics. And I classified all the searched studies into principle and region and etc. In order to investigate foreign dissertations, I search 'NCBI pubmed'. Search terms used were osteopathy, osteopathic technique, osteopathic manipulative technique. Results : 1. Osteopathy do not regard the systems which compose the human body in individual territory, but regard whole. It is diagnosis, prevention and medicine which treats 2. Osteopathic manipulation techniques are classified into direct techniques, indirect techniques, and compound techniques. 3. Osteopathic manipulation techniques are classified into fascia, muscle, ligament-joint in applied region. 4. I could search clinical cases in domestic and foreign study. I found cases about myofascial release technique(MFR), postisometric relaxation(PIR), proprioceptive neuromuscular fascilitation(PNF), muscle energy technique(MET), joint mobilization in domestic studies, and strain-counterstrain technique(SCS), MET, AK in foreign studies. Conclusions : Osteopathic manipulation techniques can be used in diagnosis and treatment of meridian muscle theory, because osteopathy and the oriental medicine have many similarities in theoretical background. So osteopathic manipulation technique can be useful in oriental medicine treatment techniques.

심병증 진단요건의 표준 설정을 위한 연구 (Study on Establishment of criteria for Heart Disease in Oriental Medicine)

  • The objective is to establish the standard of criteria for differential diagnosis of signs and symptoms. This study selected signs and symptoms related to heart which stands for Fire(火) as a kind of five phase(五行). Eleven experts was asked to evaluate the adequateness of criteria which was developed by Korea Institute of Oriental Medicine(KIOM) and to suggest the amendment of them. To implement the study, we used the questionnaire which asks about the diagnosis criteria for an insufficiency of the heart-qi(心氣虛證), deficiency of the heart blood(心血氣證), deficiency of the heart-yin(心陰虛證), insufficiency of the heart-yang(心陽虛證), exuberant fire due to hyperactivity of the heart(心火亢盛證), stagnation of the heart blood(心血瘀阻證), heart disturbed by phlegm-fire(痰火擾心證), attack of the heart by retainedfluid(水氣凌心證). Every criteria consists of primary symptoms, secondary symptoms, tongue findings. and pulse findings. In perspectives of the classification of patterns for signs and symptoms and criteria for diagnosis, the result shows that the previous standard doesn't have so many problem. So many of experts were agree with the criteria which was suggested but the trend is that they use, in their actual practice, less than the criteria. Additionally, they pointed that every element in a criterion should have the different weight value, criteria for the overlapped pattern should be added, and, in future, criteria which are based on clinical investigation should be established.

  • Stroke, also called an attack on the brain, is a complex disease that results from the interaction of many genetic and environmental factors. StrokePortal is a comprehensive resource for information on stroke that integrates and provides essential findings regarding stroke pathology, diagnostics, and treatments, based on Oriental and Western medicine. The stroke information was collected from various sources, such as journal articles, books, websites, and news stories, and it was refined, classified, and stored into a relational database system by automatic classification and manual curation. To provide the stored information effectively to users, a specialized retrieval system, based on web interfaces, was implemented. StrokePortal provides cutting-edge information to experts; interested people, including patients and their families; and investigators to broaden their knowledge of effective treatments for patients and offer many preventive measures. It provides a specialized feature with which users can upload their information and opinions to StrokePortal, which will enrich and mature the content even further. 서양의학 독성학의 기본적 개념 및 한약의 $LD_{50}$ (Basic Concepts of Western Medicine Toxicology and $LD_{50}$ in Herbal Drugs)

  • Today, toxicology is used for many purpose, in many fields. Classification of special toxic effect is related next 4 important principles. 1. The chemical substance must move to target organ or tissue that can induce Biological effect. For this movement, we have to understand the physical-chemical characteristic of substance, and the rout of absorption, metabolism, diffusion and excretion of toxic substance. 2. Every biological effect that induced by chemical substance is not harmful. For example, some specific chemical substance is not harmful in liver enzyme system. 3. The strength of biological effect induced by chemical substance is deep related with dose. Nearly all substance is not effective below the specific dose, and it may toxic to death over the specific dose. It is the 'Dose - response relationship' But carcinogen may toxic whether it is law dose or not. 4. The information that was obtained by experimental animal test, could have to adapt in human biology. Because biological effect of chemical substance could be different in every biological species. In past, drugs was obtained by animal or plants. But in the future, it could be obtained by biochemistry, and genome project. Therefore, in Oriental medicine, research and approach is needed at this time, and have to develop new method of experience in toxic method.

한방병원에 내원한 척추측만증 환자의 특성에 대한 후향적 연구 (Retrospective Study on the Characteristics of Patients with Scoliosis at the Korean Medicine Hospital)

  • Objectives This study was conducted to characterize scoliosis patients visiting Korean medicine hospital and to analyze the demands and factors affecting discomfort. Methods This retrospective study analyzed 33 scoliosis patients who visited Korean medicine hospital from March, 2021 to October, 2021. The data analysis consisted of three factors: (1) demographic characteristics, (2) characteristics of demands on Korean medicine (reasons for choosing Korean medical treatment, preferred treatment methods, most uncomfortable part, treatment priorities) and (3) discomfort factors (treatment experiences, diagnosed age and Cobb's angle). Statistical analyses were performed and a p-value≤0.05 was considered to be statistically significant. Results 43.75% of the patients chose 'effectiveness' for the reason why they preferred Korean medicine treatment. 'Chuna treatment' was the most preferred treatment method. The patients chose 'lower back' for the most uncomfortable part and 'pain' for the highest priority of improvement. The Cobb's angle of included patients was 16.02±7.65° and it is not much differ to average of Cobb's angle in Korean. Discomfort was more severe in the patients with treatment-experienced than treatment-naive. The score of discomfort in appearance and psychological were higher in the patients diagnosed in childhood or adolescent period than who were diagnosed after adult. Classification based on Cobb's angle showed no statistical difference. Conclusions Not only Cobb's angle but also other clinical factors should be considered for effective treatment in scoliosis. Also, It is necessary to pay attention to adult scoliosis patients.

중의체질의학과 치미병(治未病)센터에 대한 보고 (A Report on Traditional Chinese Medicine (TCM) Constitution Theory and Chinese Health Administration Center)

  • 1. Objectives: The objectives of this paper are to report Traditional Chinese Medicine (TCM) Constitution Theory and the present state of Chinese Health Administration Center. 2. Methods: We visited China for "China Technology Survey" in 2009. Our purposes were to research for the study of constitutional medicine in China and to search for the Development Plan of Constitutional Medicine Through the Research of Chinese Constitutional Medicine. This paper is the report of the "China Technology Survey". 3. Results & Conclusions: In China, the concept and diagnosis of TCM constitutions is nine(Balanced Constitution, Qi-deficient Constitution, Yang-deficient Constitution, Yin-deficient Constitution, Phlegm-dampness Constitution, Damp-heat Constitution, Stagnant blood Constitution, Stagnant qi Constitution, Inherited specific Constitution). And Chinese government established the Classification and Diagnosis Standards for the Constitutions of TCM in 2009. TCM Constitution Theory is being widely applied at Health Administration Center that treats 'predisease' patients. Theses Chinese clinical implications of constitutional medicine are a good example for "Sasang Constituional Medicine" Care Service.

설진 시스템 특허동향 분석 (Analysis of patent trends of computerized tongue diagnosis systems)

  • Objectives Tongue diagnosis is an important diagnostic method in traditional Eastern medicine, and it has a high potential to be used in the future healthcare because of easy, quick, and non-contact measuring features. Recently, research and development efforts on computerized tongue diagnosis systems (CTDS) have been active that led to the technical advancements in the field of photographing techniques, image extraction and classification algorithms. In this study, we analyzed the trends in the CTDS patents. Using the WIPS search engine (www.wipsglobal.com), quantitative and qualitative patent analyses were performed through Korea, China, Japan, U.S.A and Europe. Methods For a systematic search and data analysis, we defined patent categories based on the application area and technical details. By applying thus-obtained categorical key words, we obtained 360 relevant patents on photographing techniques, image extraction and classification algorithms for the purpose of diagnosis or security. Results As a result, companies related to image acquisition, medical imaging and mobile devices and research groups of universities in East Asia were major patent applicants. In all the five countries, the number of patents have been increasing since 1980. In particular, technology related to color correction and image segmentation were most actively patented categories, and expected to continue a high application rate.

글루타티온 S-전환효소 다형성과 뇌혈관질환(腦血管疾患) 및 사상체질 사이의 연관성(聯關性)에 관한 연구 (Interrelationships among Glutathione S-Transferase Polymorphisms, Cerebrovascular Disease and Sasang Constitution)

  • Glutathione S-transferase polymorphisms (GST) were examined in 98 cases with cerebrovascular disease (CVD) to test the hypothesis that GST polymorphisms confer a risk to an individual to develop CVD. Tobacco smoke is a major cause of both cancer and vascular disease. We therefore were stratified the subjects with CVD for smoking status, and then examined whether polymorphisms in this detoxification enzyme gene, GST, influence risk of CVD. Neither GSTM1 nor GSTT1 genotypes in the CVD group was significantly different from the control group (n=230), even in smokers. We attempted the combined analyses for GSTM1 and GSTT1 genotypes in CVD for smoking status. No significant association observed between the combined genotypes and CVD. We also classified the subjects and control group into four types according to Sasang Constitutional Medicine, Korean Traditional Oriental Medicine, and investigated the association among GST genotypes, CVD, and Sasang constitutional classification. Our observations do not confirm the effect of the GSTM1 and GSTT1 genotypes as a risk factor for CVD, even in smokers. Furthermore, we first attempted to evaluate the efficacy of Sasang Constitutional Medicine, and to find an association with CVD.
