Objectives : The aim of this study was to investigate the association of blood pressure levels with the common carotid artery intima-media thickness (CCA-IMT) and carotid plaques. Methods : Data were obtained from 2,635 subjects, aged 50 years and over, who participated in the Community Health Survey (a population-based, cross-sectional study) in Dong-gu, Gwangju city between 2007 and 2008. Participants were categorized into three groups according to blood pressure levels; normotensives (<120/80 mmHg), prehypertensives (120-139/80-89 mmHg), and hypertensives ($\geq$140/90 mmHg). Prehypertensives were further categorized as low prehypertensives (120-129/80-84 mmHg) and high prehypertensives (130-139/85-89 mmHg). Carotid intima-media thickness and plaques were evaluated with a high-resolution B-mode ultrasound. Statistical analyses were performed using chi-square test, ANOVA, and multiple logistic regression. Results : Prehypertensives had significantly greater maximal CCA-IMT values than normotensives, with a multivariate adjusted odds ratio of 1.78 (95% CI=1.36-2.32) for abnormal CCA-IMT (maximal CCA-IMT$\geq$1.0 mm), and 1.45 (95% CI=1.19-1.77) for carotid plaques. The multivariate adjusted odds ratio of low prehypertensives was 1.64 (95% CI=1.21-2.21) for abnormal CCA-IMT, and 1.30 (95% CI=1.04-1.63) for carotid plaques compared with normotensives. Subject with hypertension had higher frequency of abnormal CCA-IMT (odds ratio, 2.18; 95% CI=1.49-3.18), and carotid plaques (odds ratio, 1.98; 95% CI=1.46-2.67) compared with normotensives after adjustment for other cardiovascular risk factors. Conclusions : Our results indicate that there is a significant increase in the prevalence of carotid atherosclerosis in subjects with prehypertension (even in low prehypertensives) compared with normotensive subjects. Further studies are required to confirm the benefits and role of carotid ultrasonography in persons with prehypertension.
Background: We vigorously reviewed patients' operation record who had adhesion of the Denonvilliers' fascia and found out most of these patients had prostatic bleeding after prostatic gland biopsies. We examined the magnitude of prostatic bleeding and frequency after biopsies and the relationship with oncological outcomes. Materials and Methods: A total of 285 patients were selected for the final analyses. Inclusion criteria were as follows: receiving MRI three weeks after biopsiesand laparoscopic radical prostatectomy within 300 days after biopsy. We divided the patients into two groups with (group A) or without (group B) prostatic bleeding. We examined the magnitude of prostatic bleeding after biopsies and the relationship with operation time (OT), positive surgical margin (PSM), biochemical recurrence (BCR) and other factors. Furthermore, we created a logistic-regression model to derive a propensity score for prostatic bleeding after biopsies, which included all patient and hospital characteristics as well as selected interaction terms, and we examined the relationship with PSM and BCR. Results: In all patients, the OT in the group B was shorter than the group A (p < 0.001). Prostatic bleeding was associated with PSM (p=0.000) and BCR (p=0.036). In this propensity-matched cohort, 11 of 116 patients in the group B had PSM as compared with 36 of 116 patients from group A (match-adjusted odds ratio, 4.30; 95%CI confidence interval, 2.06 to 8.96; P=0.000). In addition, eight of 116 patients in group B encountered BCR, as compared with 18 of 116 patients in group A (match-adjusted odds ratio, 2.48; 95%CI, 1.03 to 5.96; P=0.042). Kaplan-Meier analysis in the propensity matching cohort showed a significant biochemical recurrence-free survival advantage for being free of prostate bleeding after biopsies. Conclusions: Our findings in the present cohort should help equip surgeons to pay attention to careful excision especially for those who experienced deferred prostatic bleeding.
Background: Cigarette smoking is a well-established risk factor of pancreatic cancer (PC). Although an association between nicotine dependence phenotype, namely time to first cigarette (TTFC) after waking, and the risk of several smoking-related cancers has been reported, an association between TTFC and PC risk has not been reported. We assessed the impact of smoking behavior, particularly TTFC, on PC risk in a Japanese population. Materials and Methods: We conducted a case-control study using 341 PC and 1,705 non-cancer patients who visited Aichi Cancer Center in Nagoya, Japan. Exposure to risk factors, including smoking behavior, was assessed from the results of a self-administered questionnaire. The impact of smoking on PC risk was assessed with multivariate logistic regression analysis adjusted for potential confounders to estimate odds ratios (ORs) and 95% confidence intervals (CIs). Results: Cigarettes per day (CPD) and/or smoking duration were significantly associated with PC risk, consistent with previous studies. For TTFC and PC risk, we found only a suggestive association: compared with a TTFC of more than 60 minutes, ORs were 1.15 (95%CI, 0.65-2.04) for a TTFC of 30-60 minutes and 1.35 (95%CI, 0.85-2.15) for that of 0-30 minutes (p trend=0.139). After adjustment for CPD or smoking duration, no association was observed between TTFC and PC. Conclusions: In this study, we found no statistically significant association between TTFC and PC risk. Further studies concerning TTFC and PC risk are warranted.

This study was carried out to purpose drawing up the efficient method of rural residents' health management, analyzing factors having influences on the behavioral intention and the behavior of hepatitis B vaccination of rural residents, using the theory of planned behavior(TPB). Surveying the first questionnaire about TPB to 439 people of rural adults over 40-year old who participated to hepatitis B examination program was made from March 14 to April 23 in 2001 at two primary health care posts in a city in Gyeongsangbuk-do and the second survey for hepatitis B vaccination was made to the last subjects of 332 people(75.6%) that had been proved as subjects of inoculation against the hepatitis B as the result of examination after two months of notification. In the behavioral intention rate of hepatitis B vaccination of the subjects within a month, 100%(the top) was 45.2%, 80-90%(the middle) was 21.1% and under 70%(the low) was 33.7%. In simple analysis, vaccination intention was related with significantly health security type and economic status(p<0.05). And vaccination intention was related with significantly all variables of attitude toward behavior and subjective norm, and economic power of utilizing medical facilities and perceived power variable of perceived behavioral control(p<0.01). The vaccination rate of the subjects within a month was 31.3%. In simple analysis, the vaccination rate was higher significantly in the lower the age was, the higher the educational level was(p<0.01), the larger the family was, and subjects having transport(p<0.05). And the vaccination rate was higher in the higher the economic power was and the higher the perceived ability was. As the results of making generalized logit analysis for behavioral intention, the more positive the subjective norm was, the higher was the score of the perceived behavioral control, the intention was higher significantly in the top class on a basis of the low class of behavioral intention. In multiple logistic regression analysis for vaccination(behavior) using intention and perceived behavioral control as dependent variables, it showed that perceived behavioral control variable only influenced significantly. The higher was the score of perceived behavioral control, it showed that they got the more vaccination(p<0.01). And even in the result having analyzed, adding the significant general characteristics variables in the simple analysis to the independent variable, the higher was the score of perceived behavioral control, it showed that they got vaccination more.
Objective: The aim was to evaluate roles of vitamin D3 (VD3) and beta-carotene (BC) in the development of esophageal squamous cell carcinoma (ESCC) in a high-risk area, Huai'an District, Huai'an City, China. Methods: 100 new ESCC diagnosed cases from 2007 to 2008 and 200 residency- age-, and sex-matched healthy controls were recruited. Data were collected from questionnaires, including a food frequency questionnaire (FFQ) to calculate the BC intake, and reversed phase high-performance liquid chromatography (RP-HPLC) was used to measure the serum concentrations of BC and VD3. Odds ratios (OR) and 95% confidence intervals (CI) were calculated in conditional logistic regression models. Results: The average dietary intake of BC was $3322.9{\mu}g$ (2032.4-5734.3) in the case group and $3626.8{\mu}g$ (1961.9-5827.9) in control group per capita per day with no significant difference by Wilcoxon test (p>0.05). However, the levels of VD3 and BC in the case group were significantly lower than in the control group (p<0.05). The OR values of the highest quartile and the lowest quartile of VD3 and BC in serum samples were both 0.13. Conclusion: Our results add to the evidence that high circulating levels of VD3 and BC are associated with a reduced risk of ESCC in this Chinese population.
The objective of this study is to identify the secular trend in age at menarche and to investigate the factors influence the early menarche(<12 years old) among Korean middle and high school girls. We analyzed data from the Korean Youth Risk Behavior Web-based Survey(KYRBWS) 2006-2014. This study was a descriptive study of 216,917 Korean middle and high school girls born between 1988 and 2002. Linear trends test performed to assess the trend age at menarche and percentage of early menarche. Multiple logistic regression analysis was to assess the risk factors influence the early menarche. Mean age at menarche decreased from $12.61{\pm}1.32$ years for middle and high school girls born 1988 to $11.88{\pm}0.75$ years for those born 2002(p for trends<.001). Percentage of early menarche increased from 19.7% to 25.2% between 1988 and 2002(p for trends<.001). Living in city, higher stress level, short sleep duration, and higher body mass index were associated with an early menarche among middle and high school girls(all p<.001). We found that age at menarche is still falling in the Korean adolescents, and it need intervention strategies to control the early menarche.

The purpose of this study was to define the quality of nursing services in relation to a nurse's psychosocial factors (type A behavior pattern, self-esteem, locus of control, and anxiety), job stress, and fatigue. This study targeted 503 nurses currently working at one of six general hospitals in Daejeon city, the data were collected by a self-administered questionnaire, which was surveyed from April 1 to June 30, 2014. According to multivariate logistic regression analysis, the high self-esteem, locus of control, and strong support by supervisor and co-workers decrease the risk of a low career quality. In contrast, high situational fatigue and overall fatigue increase the risk of a low career quality. According to the results of hierarchical multiple regression analysis, the quality of nursing services increased with age, the subjective health condition was satisfying, aptitude corresponded to the jobs, self-esteem was high, anxiety was low, support by co-workers was strong, and the overall fatigue was low. Above results suggest that the quality of nursing services of nurses who work at a general hospital has a strong correlation with the psychosocial factors, job stress, and fatigue.
The purpose of this study is to analyse the convergent factors which influence on the stages of exercise behavior change of middle-aged women. 200 middle-aged women surveyed, 41 to 64years old, is residing D city, Chung-Nam province, who understand the purpose of this study and agree to participate in this study. This study data is analyzed by using frequency, percentage, standard deviation, t-test, $x^2$ test and Logistic regression analysis. The study show that the social support factors(${\beta}$=.998, p=.010) and the motives of exercise(${\beta}$=3.407, p<.001) among middle aged women have an effect on the stages of exercise behavior change meaningfully. That is, the social support of middle aged women who have exercise behavior change is 2.5 times higher than middle aged women who don't have exercise behavior change, and the motives of exercise is 20.1 times higher. Based on theoretical ground, this study suggests that government and community seek programs for promoting the exercise behavior, considering the motives of exercise and the social supports.
Although many efforts to stop speeding or inappropriate speed, it failed to reduce the number of speeding-related crashes. Therefore, it is important to analyze the characteristics of speeding offenders and the cause of speeding at hot spots. In this study, we investigate the level of severity in speeding by the characteristics of speeding offenders using the Ordered Logistics Regression Models(OLRM). For the analysis, we use the speeding data collected by 39 traffic enforcement cameras in city of Cheongju and other related data including the speeding ticket history of drivers during the most recent 3 years, their demographic characteristics, their own vehicles, and road environment factors. The major results of this study are as follows. Firstly, commercial fleet drivers are more likely to repeat the conviction of high-range speeding with more than 30km/h over speed limits than other drivers. Secondly, mid-range speeding are observed as 21,462 frequency which is 76.7% of total speeding frequency and occurred mostly at suburban and rural area. It concludes that contributory factors affecting the severity of speeding at signalized intersection are drivers' speeding offence history, posted speed limits, time of day, gender of driver, and location of the intersection as show by the OLRM developed in this study(McFadden R-square : 0.296).
A case-control study was conducted to investigate the effect of low-level blood lead on the blood pressure. The plasma renin activity(PRA) was measured also to investigate one of the possible mechanisms by which lead nay play a role to influence on the blood pressure. Seventy-two hypertensive and sixty -nine control study subjects were selected from the workers who had no history of b-related lead exposure, in Ulsan city and it's vicinity, Korea. In addition to measuring blood lead levels and PRA, body mass index(BMI), hematocrit, serum sodium, potassium, creatinine, ionized calcium, and cholesterol were measured. Also, the habits of smoking, alcohol drinking and family history of hypertension were checked. The blood lead level of the hypertensive group was $19.8{\pm}5.5{\mu}g/dl$, which was significantly higher than that of the control group, $12.5{\pm}4.7{\mu}g/dl$(p<0.01). On multiple logistic regression analysis, the odds ratio of blood lead level on the occurence of high blood pressure was 1.38, also statistically significant (p<0.01). There was no significant differences between the hypertensive and the control group in the PRA or In (PRA), but there was a marginally significant linear relationship between blood lead and PRA in the hypertensive group(p<0.1). In conclusion, blood lead level which has been known to be within normal limits may be one of the possible risk factors of hypertension and PRA alteration by lead may act as one of the mechanisms.
