Hepatitis C Virus Nonstructural Protein 5A Interacts with Immunomodulatory Kinase IKKε to Negatively Regulate Innate Antiviral Immunity

  • Hepatitis C virus (HCV) infection can lead to chronic hepatitis, liver cirrhosis, and hepatocellular carcinoma. HCV employs diverse strategies to evade host antiviral innate immune responses to mediate a persistent infection. In the present study, we show that nonstructural protein 5A (NS5A) interacts with an NF-κB inhibitor immunomodulatory kinase, IKKε, and subsequently downregulates beta interferon (IFN-β) promoter activity. We further demonstrate that NS5A inhibits DDX3-mediated IKKε and interferon regulatory factor 3 (IRF3) phosphorylation. We also note that hyperphosphorylation of NS5A mediates protein interplay between NS5A and IKKε, thereby contributing to NS5A mediated modulation of IFN-β signaling. Lastly, NS5A inhibits IKKε-dependent p65 phosphorylation and NF-κB activation. Based on these findings, we propose NS5A as a novel regulator of IFN signaling events, specifically by inhibiting IKKε downstream signaling cascades through its interaction with IKKε. Taken together, these data suggest an additional mechanistic means by which HCV modulates host antiviral innate immune responses to promote persistent viral infection.

Immune-mediated Liver Injury in Hepatitis B Virus Infection

  • Hepatitis B virus (HBV) is responsible for approximately 350 million chronic infections worldwide and is a leading cause of broad-spectrum liver diseases such as hepatitis, cirrhosis and liver cancer. Although it has been well established that adaptive immunity plays a critical role in viral clearance, the pathogenetic mechanisms that cause liver damage during acute and chronic HBV infection remain largely known. This review describes our current knowledge of the immune-mediated pathogenesis of HBV infection and the role of immune cells in the liver injury during hepatitis B.

  • Korea is an endemic area of chronic hepatitis in the world. Liver cirrhosis and liver cell carcinoma, presumed to be related to such chronic hepatitis, are the major causes of death in this country. The purpose of this study is disclosing the sources of chronic hepatitis in Korea establishing its histologic characteristics, disclosing the patterns of progression in chronic hepatitis, delineating its prognosis and finally speculating its etiology. The study group was composed of 183 patients with biopsy-proven acute icteric viral hepaticis, 32 patients with biopsy- proven anicteric hepatitis and 260 patients with biopsy- proven chronic hepatitis. These patients submitted to long-term follow-up by means of liver needle biopsy and/or clinicolaboratory evaluation. The period of follow-up ranged from two months to 18 years. The histological features of the initial biopsy specimens of chronic hepatitis permitted a division of the cases cases into the following five types: Type I. Persisting portal hepatitis : so called persisting hepatitis 43 Type II. Chronic inactive hepatitis with incomplete strand septal fibrosis. This type has thin fibrotic septation in addition to Type I with portal sclerosis 38 Type III. Chronic active periportal hepatitis(CAPH) : so called aggressive hepatitis, characterized by marked piecemeal necrosis. This type has been subdivided further into three groups: AB and C on the basis of histologic features. A CAPH without cirrhosis 15 B CAPH with cirrhosis 99 C CAPH with diffuse acinus type parenchymal nodules; characterized by rosette-forming micronodules 21 Type IV. Subacute hepatic necrosis; characterized by multilobular and/or bridging necrosis. 14 Type V. Persisting lobular hepatitis; characterized by spotty necrosis, which looks very similar to acute viral hepatitis. Such histologic changes should be persisted for more than six months 30 In Korea the main source of chronic hepatitis is the anicteric type. Of the chronic hepatitis observed in the hospital, Type IIIb was the most frequent in its incidence and occasionally exhibited development of hepatocellular carcinoma, but the mortality was highest in Type IIIc during the period of follow-up. Histologic characteristics of these five types suggest a spectrum of chronic hepatitis in Korea from an early and mild stage to advanced and fatal cirrhosis, which is occasionally associated with primary hepatic cell carcinoma. It seems that Type IV can be followed by flare-up of various stages of acute and chronic hepatitis with HBsAg and that many cases of liver cirrhosis prevalent in Korea occur through such an active process of Type IV. The etiology is not established, but in Korea it is mainly related to HBsAg.

Strategy to Overcome Drug Resistance That Develops during Treatment of Chronic Hepatitis B in Children

  • Development of antiviral resistance to lamivudine is the most important factor for the treatment failure. It is necessary to establish proper guidelines to overcome drug resistance for children with chronic hepatitis B. Primary treatment with lamivudine should be considered if patients are in immune-clearance phase and have persistently elevated ALT levels more than twice the upper limit of normal value. Before initiating the therapy, careful consideration of the patient's status is required to exclude abnormal liver function tests due to other causes. The treatment option should be carefully decided to suppress the viral replication effectively. To obtain good compliance, clinicians should educate patients and their parents. Appropriate monitoring for virologic breakthrough and genotypic resistance is important in deciding to change the treatment plan. Sequential monotherapy should be avoided and a combination of drugs in other categories is recommended. New antiviral agents, such as entecavir and tenofovir, which have high potency and high genetic barrier, are soon expected to be available for use with children.

Hepatitis B Virus DNA Polymerase Displays an Anti-Apoptotic Effect by Interacting with Elongation Factor-1 Alpha-2 in Hepatoma Cells

  • Hepatitis B virus (HBV) genome P-encoded protein HBV DNA polymerase (Pol) has long been known as a reverse transcriptase during HBV replication. In this study, we investigated the impact of HBV Pol on host cellular processes, mainly apoptosis, and the underlying mechanisms. We showed a marked reduction in apoptotic rates in the HBV Pol-expressed HepG2 cells compared to controls. Moreover, a series of assays, i.e., yeast two-hybrid, GST pull-down, co-immunoprecipitation, and confocal laser scanning microscopy, identified the host factor eEF1A2 to be associated with HBV Pol. Furthermore, knockdown of eEF1A2 gene by siRNA abrogated the HBV Pol-mediated anti-apoptotic effect with apoptosis induced by endoplasmatic reticulum (ER) stress-inducer thapsigargin (TG), thus suggesting that the host factor eEF1A2 is essential for HBV Pol's anti-apoptosis properties. Our findings have revealed a novel role for HBV Pol in its modulation of apoptosis through integrating with eEF1A2.

Secondary Fish-Odor Syndrome Can be Acquired by Nitric Oxide-mediated Impairment of Flavin-containing Monooxygenase in Hepatitis B Virus-Infected Patients

  • Primary fish-odor syndrome (FOS) is a genetic disorder caused by defective flavin-containing mono-oxygenase 3 gene (FMO3) with deficient N-oxidation of trimethylamine (TMA), causing trimethylaminuria (TMAU). By contrast, secondary FOS can be acquired by decreased FMO activities in patients with chronic liver diseases, but the underlying mechanisms are unknown. In the present study, we examined plasma NOx concentrations and viral DNA contents as well as in vivo FMO activities and their correlations in chronic viral hepatitis (CVH) patients. Plasma concentration of NOx was significantly increased by 2.1 fold $(56.2{\pm}26.5\;vs.\;26.6{\pm}5.4\;{\mu}M,\;p<0.01)$, and it was positively correlated with plasma hepatitis B virus (HBV) DNA contents $(r^2=0.2838,\;p=0.0107)$. Furthermore, the elevated plasma NOx values were inversely and significantly correlated with in vivo FMO activities detected by ranitidine-challenged test $(8.3%\;vs.\;20.0%,\;r^2=0.2109,\;p=\0.0315)$. TMA N-oxidation activities determined in CVH patients without challenge test were also significantly low (73.6% vs. 95.7%, p< 0.05). In conclusion, these results suggested that secondary FOS could be acquired by the endogenously elevated NO in patients with CVH.

A Study on the Positive Rates of HBsAg and Anti-HBs from Old People's Home and Casual Ward in Daegu, Korea

  • Serum hepatitis is still recognized as a major public health problem in many countries. The most common etiologies are acute viral hepatitis A and B, and hepatitis B virus (HBV) was known as the pathogen of serum hepatitis. since the HBV causes chronic hepatitis. HBV infection is hyperendemic in Korea. It is known that 8% and 1% of the Korean population is chronic carriers of HBV. The total number of 487 serum specimen (old people's home 190, causal ward-Daegu hope village 297) were collected at the laboratory of Department of Clinical Pathology of Daegu medical center. The laboratory tested the specimen to detect for hepatitis B surface antigen (HBsAg) and anti-HBs using reverse passive hemag-glutination (RPHA) and passive hemagglutination (PHA) method between April, 2011 and June, 2011. In one-step test, HBsAg and anti-HBs of Humasis were used. The positive rate for HBsAg was 6.80% (33/487), and the rate in male was 6.93% (19/274) higher than that of female 6.57% (14/213). In the positive rate of HBsAg by age group, the highest positive rate group was in the group of 40-49 years in male (12.0%), and 50-59 years in female (17.65 %). The overall positive rate for anti-HBs was 42.70% (208/487), showing the higher positive rate of 43.80% in male (120/274) than that of 41.30% (88/213) in female. The highest positive rate of anti-HBs in age group was the 40-49 years group in male (52.00%) and the group of under 29 in female (66.67%).

소아에서 발생한 비-A, B, C형 바이러스성 간염의 임상 고찰 (Clinical Features of Non-A, B, C Viral Hepatitis in Children)

  • 목 적: 소아에서 발생하는 급성 간염 중 많은 빈도를 차지하는 비-A, B, C 바이러스성 간염의 원인과 간염의 임상적 특징 및 경과에 대한 임상 양상에 대해 알아보고자 하였다. 방 법: 2001년 1월부터 2004년 6월까지 부산대학교병원 소아과에서 급성 간염으로 진단된 환자 중 독성, 대사성, 자가 면역성 간염과 신생아 간염은 제외하고 A형, B형, C형의 간염 바이러스 표지자 검사에서 음성 판정을 받은 총 45명의 환아를 대상으로 하여 후향적으로 원인 및 임상적 특징을 분석하였다. 결과: 1) 45명 중 원인 불명의 바이러스성 간염이 26명(57.8%)이었고, CMV 간염이 14명(31.1%), EBV와 HSV 간염이 각각 2명(4.4%), RV 간염이 1명(2.2%)이었다. 2) 남녀비는 1.8 : 1 이었고, 1세 이하가 27명(60.0%)이었다. 3) 여름에 환자 발생이 18명(40.4%)으로 가장 많았고, 동반 증상으로는 발열 13명(28.9%), 오심 또는 구토 12명(26.7%), 황달 10명(22.2%), 설사 8명(17.8%)의 순으로 많았다. 4) 신체 검사에서 간종대가 15명(33.3%), 공막 및 피부 황달 10명(22.2%), 비종대 9명(20.0%)이었다. 5) 임상 경과는 전형적인 급성 간염이 43명(95.6%), 전격성 간염이 2명(4.4%)이었으며, 한 명은 간부전으로 사망했다. 6) 혈청 ALT의 최고치는 $488.7{\pm}771.9IU/L$였고, 원인 불명의 간염이 $606.2{\pm}962.2IU/L$로 비교적 높았다. 7) 혈청 ALT는 2년 동안 경과 추적이 가능했던 38명 중 31명(81.6%)이 6개월 이내에, 4명(10.5%)이 1년 이내에 정상화되었다. 3명(7.9%)이 1년 이상 증가되었지만, 1년 6개월 이내에 모두 정상화되었다. 8) 혈액학적 이상으로 백혈구 증가증이 17명(47.2%)이었고, 재생 불량성 빈혈, 비정형적 림프구증가증, 혈소판 감소증이 각각 1명(2.8%) 있었다. 결 론: 비-A, B, C형 바이러스성 간염은 대부분 예후가 양호하나, 임상적으로 만성 간염의 경과를 보인 경우도 있고 전격성 간염으로 진행한 경우가 있으므로 면밀한 관찰이 필요하다.

만성 신생아 간염의 임상적 고찰: 비-가족형, 비-대사성, 비-A, B, C형 바이러스성 신생아 간염 (The Clinical Features of Chronic Neonatal Hepatitis: Non-familial, Non-metabolic and Non-A, B, C Viral Hepatitis)

  • 목 적: 비-가족형, 비-대사성, 비-A, B, C형 바이러스성 신생아 간염은 양호한 경과를 취한다고 알려져 있지만, 장기적인 경과 관찰에 대한 연구가 부족하다. 연자들은 이 질환의 임상적 특징을 연구하여 임상 경과 및 예후 예측에 도움을 얻고자 하였다. 방 법: 1998년 1월부터 2004년 1월까지 생후 3개월 이내에 부산대학교병원에서 신생아 간염으로 진단되었던 환자 중 임상 소견 및 생화학적 검사의 이상 소견이 6개월 이상 지속되었던 34명을 대상(A, B, C형 바이러스성, 대사성, 유전성 신생아 간염은 제외)으로 하여 임상적 소견, 검사실 소견 및 조직학적 소견을 병력지와 조직 슬라이드 분석을 통해 후향적으로 연구를 시행하였다. 결 과: 성비는 2.4 : 1로 남아가 많았고, 진단 시 연령은 생후 1~2개월 사이가 가장 많았다. 혈청 ALT의 최고치의 범위는 광범위(100~1,000 IU/L)하였으나, 300 IU/L 미만이 41%였다. 혈청 직접형 빌리루빈의 최고치는 50%에서 1.0~5.0 mg/dL 사이였다. CMV에 대한 IgM 항체 검사 또는 PCR 검사에서 34%가 양성이었다. 추적 기간의 조건을 충족한 29명 중 11명(37.9%)의 환자가 1년 이내에 ALT 수치가 정상화되었고, 1년 이상 ALT 수치가 증가된 환자 13명 중에서는 2년 이내에 정상화된 경우가 9명(69.2%), 2년 이상 지속적으로 상승된 경우가 4명(30.7%)였다. 2년 이상 지속적으로 ALT 수치의 상승이 있었던 경우 간 조직 검사에서 간문맥 주변 섬유화, 간문맥 염증 및 간소엽 염증 등의 변화가 2년 이내에 회복된 경우보다 심했으나 통계학적인 의미는 없었다. 결 론: 비-가족형, 비-대사성, 비-A, B, C형 바이러스성 신생아 간염은 비교적 예후가 양호하지만 혈청 ALT 수치의 상승이 1년 이상 지속 시 간 손상의 정도에 대한 평가와 만성 간질환에 대한 주의 깊은 관찰이 필요하다.

활막 세포에서 HCV Core 단백에 의한 Interleukin-8 발현 유도 (Induction of Interleukin-8 Expression in Synovial Cell by Hepatitis C Virus Core Protein)

  • Background: Rheumatoid arthritis (RA) is a chronic and systemic inflammatory disease that is characterized by invasive synovial hyperplasia, leading to progressive joint destruction. Recent studies have described that RA is caused by virus, bacteria or outside material. Approximately 2 to 20% of RA cases arc reported to be associated with infected hepatitis C virus (HCV). However, the mechanisms underlying virus-induced RA are still unknown. Moreover, few molecular studies have addressed the inflammatory aspects of HCV-associated autoimmune RA. In this study, we aimed to determine whe ther or not another HCV core protein transactivates the IL-8 gene expression, prototypic chemokine, in synovial cell. Methods: To establish the HCV core expressing stable synovial cell line, pCI-neo-core, a plasmid encoding HCV core protein, were transfected to HIG-82 cell line that is an established cell line from rabbit periaricular soft tissue. We examined the morphological changes and cell cycle distribution of HIG-82 cells with expression of HCV core protein by inverted microscopy and flow cytometry analysis, respectively. Also, we determined the mRNA levels of Interleukin (IL)-6 and IL-8 related to the inflammation by RT-PCR and then analyzed regulation of IL-8 expression by the NF-${\kappa}B$ pathway. Results: Our study showed no significant differences in morphology and cell cycle between HIG-82 control cell line and HIG-82 expressing HCV core protein. However, expression of HCV core protein induces the IL-8 mRNA expression in HIG-82 core cells via activated NF-${\kappa}B$ pathway. Conclusion: These results suggest that HCV core protein can lead to enhanced IL-8 expression. Such a proinflammatory role may contribute to the etiologic pathogenesis in RA patients with HCV infection.
