성인병 환자를 위한 유산소성 운동프로그램의 효과 (The Effect of Aerobic Exercise Program for Chronic Disease Patients)

  • The purpose of this study is to determine the effects of exercise programs on the treatment of chronic diseases. For this purpose, nine middle-aged women suffering from such chronic diseases as obesity, hypertension and coronary heart disease(CHD) were sampled to perform a 4-month-long exercise program which required the lactate threshold(LT) level corresponding to 70% of maximal oxygen uptake(VO2max). Thereupon, their physiques, physical fitness, blood components and physiological varibles were compared pre and post of exercise program. The results were summarized as follows ; 1) After the 4-month-long exercise program, %fat was significantly decreased, while such variables as grip strength, side step, trunk flexion and leg balance were significantly increased. 2) Blood components such as TG, TC, HDL-C, LDL-C, hemoglobin and hematocrit did not show and change in after exercise program. 3) Physiological varables such as HR, SBP, DBP, HR/LT, SBP/LT, DBP/LT, RPE/LT, VE/LT, VO2/LT and VO2 max showed significant increase after 4-month exercise program. These results suggest that regular aerobic exercise in a long period might help to decrease % fat and to increase aerobic capacity in middle-aged women suffering from chronic diseases.

만성폐질환 환자의 기능상태에 영향을 미치는 요인 (Factors Influencing Functional Status in People with Chronic Lung Disease)

  • The purpose of this study was to identify factors that influence the functional status of chronic lung disease patients. Method: A descriptive, correlational study design was used. The study was conducted at the outpatient respiratory clinic of the large university hospital in Korea. A convenience sample of 128 chronic lung patients (age = 64.2 yrs; 106 COPD, 17 bronchiectasis, 5 DILD) with mean FEV1 64.4 % predicted. Functional status was measured with SIP. Physical variables (FEV1% predicted, dyspnea, fatigue, pulmonary symptom distress), psychological variables (mood, stress), and situational variable (sleep quality) were examined. Dyspnea was measured by the BDI, fatigue was measured with the MFI. Mood was measured with the modified Korean version of POMS. Sleep quality was measured with the Pittsburgh Sleep Quality Index. Potential independent variables for the regression were age, gender, years since diagnosis, FEV1% predicted, dyspnea, fatigue, pulmonary symptom distress, stress, and sleep quality. Result: In general, functional status was relatively good. In regression analysis, functional status were significantly influenced by dyspnea, mood, age and fatigue. These variables explained 70 % of the variances in functional status. Conclusion: The results suggest that psychophysiologic symptom management should be a focus to enhance the functional status in this group.

『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 백호탕(白虎湯) 투여 후 호전된 불면증 증례 1례 (Insomnia Treated with Baekho-tang Based on the Shanghanlun Disease Pattern Identification Diagnostic System)

  • Objective: This study aimed to report the improvement of a patient with chronic insomnia who was treated with Baekho-tang based on the Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods: The patient's symptoms were checked on days 19, 37, and 52 after taking Baekho-tang. Chronic insomnia was evaluated by the degree of insomnia, hours spent asleep, and the number of times the patient woke up during the night, which were all assessed by a visual analog scale (VAS) with the Leeds Sleep Evaluation Questionnaire. In addition, new clinical implications for the interpretation of the 350th Shanghanlun provision were reviewed. Results: After administering Baekho-tang for 52 days, based on the 350th Shanghanlun provision according to the DPIDS, the VAS score decreased from 10 to 1. Conclusions: The patient recovered from chronic insomnia. This case report suggests that the words "滑," "厥," and "裏" in the 350th provision of Shanghanlun mean physical and psychological causes of insomnia. In addition, Baekho-tang had a therapeutic effect in improving the chief complaint of the patient. Therefore, the interpretation of the 350th Shanghanlun provision requires a new definition that includes the patient's lifestyle, family conflict, and past history of the patient as a cause of chronic insomnia.

Strategies for Worksite Health Interventions to Employees with Elevated Risk of Chronic Diseases

  • Chronic disease rates have become more prevalent in the modern American workforce, which has negative implications for workplace productivity and healthcare costs. Offering workplace health interventions is recognized as an effective strategy to reduce chronic disease progression, absenteeism, and healthcare costs as well as improve population health. This review documents intervention and evaluation strategies used for health promotion programs delivered in workplaces. Using predetermined search terms in five online databases, we identified 1,131 published items from 1995 to 2014. Of these items, 27 peer-reviewed articles met the inclusion criteria; reporting data from completed United States-based workplace interventions that recruited at-risk employees based on their disease or disease-related risk factors. A content rubric was developed and used to catalogue these 27 published field studies. Selected workplace interventions targeted obesity (n = 13), cardiovascular diseases (n = 8), and diabetes (n = 6). Intervention strategies included instructional education/counseling (n = 20), workplace environmental change (n = 6), physical activity (n = 10), use of technology (n = 10), and incentives (n = 13). Self-reported data (n = 21), anthropometric measurements (n = 17), and laboratory tests (n = 14) were used most often in studies with outcome evaluation. This is the first literature review to focus on interventions for employees with elevated risk for chronic diseases. The review has the potential to inform future workplace health interventions by presenting strategies related to implementation and evaluation strategies in workplace settings. These strategies can help determine optimal worksite health programs based on the unique characteristics of work settings and the health risk factors of their employee populations.

직장인의 만성질환 위험 요소별로 구성된 영양 교육의 효과 비교 연구 (Effects of Workplace Nutrition Education Program Tailored for the Individual Chronic Disease Risks)

  • The incidence of chronic disease is continuously increasing in Korea. Especially, office workers have higher risk of chronic disease because of their dietary habit and lifestyle. The study aimed to investigate the effect of tailored nutrition counseling on improving chronic disease risk factors. Ninety-nine male workers (age $46.9{\pm}7.0$ yrs) volunteered for 12 weeks of nutrition program containing dietary intake and physical activity adjustment. Five individualized programs were performed with the main theme of weight loss (WL, n = 16), blood pressure lowering (BL, n = 34), normalizing blood glucose (GL, n = 21), lipid lowering (LL, n = 13) and reducing MS risk factors (ML, n = 15). Anthropometric data, blood-pressure, self-reported questionnaire, blood profiles were measured before and after 12weeks of nutrition education. The education program included 5 times of 1:1 interview. Compared to 0 week, anthropometric data (weight, BMI, fat, visceral fat, waist, SBP, DBP) were significantly decreased after 12 weeks (p < 0.001). Fasting blood glucose and total cholesterol were decreased (p < 0.05). The primary outcomes with individually tailored programs showed to be more effective than one general nutrition program.

청소년의 우유 섭취,영양 섭취 및 신체 발달간의 관계에 대한 고찰 (A Review on the Relationship of Milk Consumption, Dietary Nutrient Intakes and Physical Growth of Adolescents)

  • Adolescents grow rapidly and have lots of schoolwork for entrance examination, so good nutrition is very important to them. However, adolescents tend to have a poor dietary behavior including frequent consumption of empty calorie foods and skipping breakfast. In particular, frequency of drinking milk has been low among Korean adolescents, thus their dietary calcium intakes has been low. Undesirable dietary behavior of adolescents is a risk factor for poor nutrition, obesity and chronic disease of them. Milk is a good source for calcium, high quality of protein, vitamin and bioactive compounds including anti-obesity component, thus it has been reported that milk helps improvement of dietary nutrient adequacy and physical growth, increment of bone mineral density, and prevention of obesity and chronic diseases. Therefore, milk consumption of adolescents should be encouraged in order to help their nutritional status, physical growth and health.

우리나라 고령자의 HINT-8 (한국형 건강 관련 삶의 질 측정도구)에 따른 건강습관 및 근력, 만성질환, 정신건강, 영양소 섭취상태 분석: 2019 국민건강영양조사자료 (Factors Associated with HINT-8 Index (Korean Health-Related Quality of Life Instrument with Eight Items), Focusing on Health Habits, Hand Grip Strength, Chronic Diseases, Mental Health, and Nutrient Intake Status in Korean Older Adults: Based on Korea National Health and Nutrition Examination Survey 2019 Data)

  • This study investigated the factors associated with HINT-8 (Korean health-related quality of life instrument with eight items), including health habits, hand grip strength, chronic disease, chewing function, mental health, and nutrient intake. In this study, the health-related quality of life (HRQoL; HINT-8) was assessed using the data from the 2019 Korea National Health and Nutrition Examination Survey (KNHANES VIII) among Koreans aged over 65, with HINT-8 being evaluated based on the general characteristics. Of the participants from KNHANES VIII, 1,455 subjects were analyzed using SPSS statistics complex samples (Windows ver. 27.0). The HINT-8 score was significantly lower in females and in those aged 75 years or older, lowest educational level, lowest income level, and single households. Logistic regression analysis, adjusted for the general characteristics, revealed a significant association between a lower HINT-8 group and various factors: low aerobic physical activity (OR=1.71, 95% CI=1.25~2.35), low hand grip strength (OR=1.34, 95% CI=1.02~1.78), chewing difficulties (OR=2.17, 95% CI=1.62~2.92), poor self-rated health status (OR=6.11, 95% CI=4.42~8.45), stressful mental status (OR=3.57, 95% CI=2.27~5.63) and poor calcium status (OR=1.45, 95% CI=1.11~1.88). On the other hand, this study could not reveal a significant association between a lower HINT-8 group and factors such as smoking, drinking, chronic disease, and nutrient intake status, except for calcium intake. This study suggests that HINT-8 is associated with aerobic physical activity, hand grip strength, chewing function, mental health, and calcium intake, but not with smoking, drinking, chronic disease, and nutrient intake.

미국이민 중년 여성의 비만 관련요인과 만성질환에 미치는 영향 (Factors Related to Obesity of Mid-year Korean American Women and Their Prevalence of Chronic Diseases)

  • Purpose: The purpose of this study was to investigate factors related to obesity of mid-age women and to examine the relationship between obesity and chronic diseases including diabetes, hypertension, and heart disease. Methods: Using data from Korean American adults living in California, we analyzed the health behavior (diet, exercise, smoking, and binge drinking) and psychological stress of obese women with body mass index ${\geq}25$ by using chi-square test. Logistic regression was used to investigate independent effects of obesity on chronic diseases, after controlling for risk factors. Results: The prevalence of obesity appeared about 16.9%. The Obese group was less likely than the non-obese group to eat vegetables and more likely feel psychological stress. There were no significant differences in smoking, alcohol intake, and physical exercise between the two groups. Obesity was strongly related to chronic diseases (OR=4.642, 95% CI=1.328-16.222). Conclusion: This study suggests that obesity of mid-age women could be reduced by encouraging health behavior such as eating vegetables daily, performing physical exercise regularly, and taking care of emotional stress. Diet and physical activity interventions and emotional supports should be developed for weight loss and prevention of weight gain in mid-age women.

Effects of Social Support and Self-Efficacy on the Quality of Life of Elderly People

  • Objective: This study investigated how social support and self-efficacy affect the quality of life of elderly people with chronic diseases. Design: The study consisted of descriptive survey research. Methods: A questionnaire covering social support, self-efficacy, and quality of life was distributed to 320 elderly people with chronic diseases. Pearson's correlation analysis was performed to examine the correlation between the respondents' social support (family support, friend support, medical support), self-efficacy (confidence, self-regulation efficacy, preference for task difficulty), and quality of life. Multiple regression analysis was also performed to identify the factors affecting the respondents'quality of life. Results: "Friend support" and "quality of life" (r=-636, p<0.001), had a negative correlation, "confidence" and "quality of life" (r=0.827, p<0.001), "self-regulating efficacy" and "quality of life" (r=0.736, p<0.001), and "preference for task difficulty" and "quality of life" (r=0.295, p<0.001)-had positive correlations. Friend support (𝛽=-0.164, p<0.001), confidence (𝛽=0.592, p<0.001), and self-regulation efficacy (𝛽=0.160, p<0.001) were found to affect quality of life. The independent variables showed the following degrees of influence, in order: confidence, friend support, and self-regulation efficacy. Their explanatory power was 73.3% (F=146.844, p<0.001). Conclusions: The quality of life of elderly people with chronic diseases can be improved by formulating health-promotion programs that foster a sense of community.

만성폐쇄성폐질환 환자에서 골격근 기능 이상 (Skeletal Muscle Dysfunction in Patients with Chronic Obstructive Pulmonary Disease)

  • Patients with chronic obstructive pulmonary disease (COPD) frequently complain of dyspnea on exertion and reduced exercise capacity, which has been attributed to an increase in the work of breathing and in impaired of gas exchange. Although COPD primarily affects the pulmonary system, patients with COPD exhibit significant systemic manifestations of disease progression. These manifestations include weight loss, nutritional abnormalities, skeletal muscle dysfunction (SMD), cardiovascular problems, and psychosocial complications. It has been documented that SMD significantly contributes to a reduced exercise capacity in patients with COPD. Ventilatory and limb muscle in these patients show structural and functional alteration, which are influenced by several factors, including physical inactivity, hypoxia, smoking, aging, corticosteroid, malnutrition, systemic inflammation, oxidative stress, apoptosis, and ubiquitin-proteasome pathway activation. This article summarizes briefly the evidence and the clinical consequences of SMD in patients with COPD. In addition, it reviews contributing factors and therapeutic strategies.
