Preserved Superficial Fat Skin Composite Graft for Correction of Burn Scar Contracture of Hand (얕은 지방층을 포함한 피부복합조직이식을 이용한 손화상 반흔구축의 교정)

  • Purpose: Split or full thickness skin graft is generally used to reconstruct the palmar skin and soft tissue defect after release of burn scar flexion contracture of hand. As a way to overcome and improve aesthetic and functional problems, the authors used the preserved superficial fat skin(PSFS) composite graft for correction of burn scar contracture of hand. Methods: From December of 2001 to July of 2007, thirty patients with burn scar contracture of hand were corrected. The palmar skin and soft tissue defect after release of burn scar contracture was reconstructed with the PSFS composite graft harvested from medial foot or below lateral and medial malleolus, with a preserved superficial fat layer. To promote take of the PSFS composite graft, a foam and polyurethane film dressing was used to maintain the moisture environment and Kirschner wire was inserted for immobilization. Before and after the surgery, a range of motion was measured by graduator. Using a chromameter, skin color difference between the PSFS composite graft and surrounding normal skin was measured and compared with full thickness skin graft from groin. Results: In all cases, the PSFS composite graft was well taken without necrosis, although the graft was as big as $330mm^2$(mean $150mm^2$). Contracture of hand was completely corrected without recurrence. The PSFS composite graft showed more correlations and harmonies with surrounding normal skin and less pigmentation than full thickness skin graft. Donor site scar was also obscure. Conclusion: The PSFS composite graft should be considered as a useful option for correction of burn scar flexion contracture of hand.

Feasibility of the Use of RapiGraft and Skin Grafting in Reconstructive Surgery

  • Background Skin grafting is a relatively simple and thus widely used procedure. However, the elastic and structural quality of grafted skin is poor. Recently, various dermal substitutes have been developed to overcome this disadvantage of split-thickness skin grafts. The present study aims to determine the feasibility of RapiGraft as a new dermal substitute. Methods This prospective study included 20 patients with partial- or full-thickness skin defects; the patients were enrolled between January 2013 and March 2014. After skin defect debridement, the wound was divided into two parts by an imaginary line. Split-thickness skin grafting alone was performed on one side (group A), and RapiGraft and split-thickness skin grafting were used on the other side (group B). All patients were evaluated using photographs and self-questionnaires. The Manchester scar scale (MSS), a chromameter, and a durometer were used for the scar evaluation. The average follow-up period was 6 months. Results The skin graft take rates were 93% in group A and 89% in group B, a non-significant difference (P=0.082). Statistically, group B had significantly lower MSS, vascularity, and pigmentation results than group A (P<0.05 for all). However, the groups did not differ significantly in pliability (P=0.155). Conclusions The present study indicates that a simultaneous application of RapiGraft and a split-thickness skin graft is safe and yields improved results. Therefore, we conclude that the use of RapiGraft along with skin grafting will be beneficial for patients requiring reconstructive surgery.

Radiation Therapy Following Total Keloidectomy; A Preliminary Report (켈로이드 전절제와 방사선 치료 병행요법: 예비보고)

  • The authors evaluated 14 patients from July 1999 to February 2004 treated with total keloidectomy followed by postoperative radiation treatment, delivered within 24 hours or 24-36 hours after surgery. The total dose of radiation was 1200 cGy in three fractions for 4-5 days. Among the 14 patients treated, 8 patients were evaluated for following-up. The age range of these patients were from 22 to 44 years old, with the average age of 30 years. The site of keloid lesions consisted; 6 on the ear lobe, 5 on the anterior chest and 1 on the upper arm. The mean follow-up period was 29.9 months. The recurrence was evaluated with photogrammetric analysis and skin color analysis. The photogrammetric analysis was performed with planimetry for the comparison of the ratio of the reduced size to the preoperative size. The mean value of the relative size of reduction was 55% and the ratio of the ear lobes were greater than the ratio of the chests. The skin color analysis was performed with chromameter CR-300 for the analysis of color difference (E) between the surrounding normal skin and the lesion. The larger the recurred size was, similar to the original size, the larger the E value was, so the E value posses the probability of predictable objective tool of recurrence. Although verifying the effectiveness of radiation therapy following keloidectomy need more cases and long term follow-up evaluation, this therapeutic modality seems very effective in reducing the keloid size, especially in the ear lobe.

A Mineralogical and Gemological Studies for the Enhancement of Tanzania Ruby by Heat Treatment (탄자니아산 루비의 열처리에 의한 보석·광물학적 품질개선 연구)

  • Ruby is one of the most favor colored gem, for beautiful red tone, be high in scarcity value. However, rubies with high quality are produced in restricted regions, such as in Thailand, Sri Lanka, Myanmar, and Tanzania etc., and they have been gradually exhausted by mining for a long period. Therefore, improving qualities of low level rubies with various treatments is arising an alternative way to obtain better rubies. Gemological and mineralogical properties of the natural ruby from Tanzanian were studied with heat treatments. Those characteristics were compared between only heat and adding flux materials under heating. Tanzanian raw rubies were applied a heat treatment ($1,600^{\circ}C$ for 6 hours). However, chromameter and UV-Vis analyses found that a simple heat treatment is inappropriated for the Tanzanian ruby. Although $Cr^{3+}$ containing for red color in the ruby increased with heat treatment, the ruby displays dark medium red because of Fe in the ruby as a form of $Fe_2O_3$. The low transparency after heat treatment is attributed to the recrystallization of $SiO_2$ which has a low melting point. Chromameter confirmed adding Pb-containing flux under heating greatly improves the clarity and color of Tanzanian rubies with micro-fractures and cavities on the surface. EMPA results show that Pb as an additive fills the cavities and cracks on raw Tanzanian rubies during the heat treatment. As a rewult of it, the quality of the Tanzanian ruby raw dramatically improved. These results indicate that the heat treatment with an additive (Pb in this study) is an effective way to obtain better quality of the Tanzanian ruby. Consequently, this study suggests a suitable method to improve the properties of the Tanzanina ruby. The result of this study would provide useful information to upgrade the qualities of similar gem stones such as corundum and sapphire.

The inhibitory Effects of Coenzyme Q10 on Melanogenesis of cultured Human Melanocytes and in vivo Guinea Pig Model (Coenzyme Q10의 멜라닌 생성억제효과)

  • Coenzyme Q10 is found in all tissues including skin and it is the well-known coenzyme for mitochondrial enzymes. The electron and proton transfer functions of the quinone ring are of fundamental importance for the oxidative phosphorylation pathway to generate energy in the cells. Coenzyme Q10 has been studied as a potent antioxidant molecule in the skin. It is involved in the skin's response to UVR irradiation. The concentration of this antioxidant in UVR exposed skin is higher than in non-exposed skin. However, recent studies have also shown that coenzyme Q10 is one of the first antioxidants to be depleted when skin is UVR-irradiated. This indicates that coenzyme Q10 is primarily involved in defense mechanisms of the skin. Therefore, we questioned whether coenzyme Q10 shows reulatory effect of melanogenesis. Here we report that coenzyme Q10 inhibits melanin neosynthesis of normal human melanocytes grown in culture, and lightens UVB-induced hyperpigmentation of the guinea pig skin in vivo. We treated human melanocytes with 0.05mM to 0.5mM of coenzyme Q10 for a total of two days. This inhibited melanin neosynthesis of cultured human melanocytes dose-dependently. The inhibitory effect of coenzyme Q10 was as effective as kojic acid or vitamin C on cultured human melanocytes. CoQ10 didn't have direct inhibitory effect on tyrosinase activity in in vitro tyrosine hydroxylase activity To further clarify the effect of coenzyme Q10 on the melanogenesis, we established UVB-induced hyperpigmentation on the shaved backs of brownish guinea pigs. The UVB intensity was 500mJ/$\textrm{cm}^2$ and the total energy dose was 1,500 mJ/$\textrm{cm}^2$. The animals were exposed to UVB radiation one times a week for three consecutive weeks. Coenzyme Q10, kojic acid, Arbutin, vitamin C(1% in vehicle) or vehicle alone as a control were then topically applied daily to the hyperpigmented areas twelve times per week far four successive weeks. The lightening effect was evaluated by visual scoring, chromameter and immunohistochemistry. Coenzyme Q10 had lightening effect on the UVB-induced hyperpigmentation without any other side effects, whereas another compounds showed weak lightening efficacies. Therefore, these results suggest that coenzyme Q10 may be useful for solving physiological hyperpigmenting problems for cosmetic purposes.

  • In this study, we investigated the effect on the increase of In vitro skin permeation experiments and In-vivo skin whitening efficacy using a O/W nanoemulsion produced via PIC(Phase Inversion Composition) with 1,3-di(pentyloxyl)benzene. skin permeation experiments of RS-nanoemulsion formulated with selected condition was evaluated compared to mineral oil containing 1,3-di(pentyloxyl)benzene and normal O/W type RS-emulsion. Compared to mineral oil with 1,3-di(pentyloxyl)benzene and RS-emulsion. RS-Nanoemulsion has a statistically significant high percutaneous absorption in terms of index substance, which is 1,3-di(pentyloxyl)benzene. In vivo test were prepared in the system of O/W cream containing RS-nanoemulsion. There was no adverse reactions in both samples. After 8 weeks, the subjects was evaluated by a dermatologist's scoring and Chromameter. In conclusion, the testing product showed statistically improvement (p<0.05) compared to the controlled product and proved its whitening efficacy.

Comparison of Treatment Effect of the Dried Bovine Amniotic Membrane and the Cultured Allogenic Keratinocytes in the Partial Thickness Burn Management (부분층 화상에 적용한 건조소양막과 동종배양표피세포의 치료효과 비교)

  • Purpose: In the partial thickness burn management, despite of several advantages, the use of human amniotic membrane has been limited. The authors applied dried bovine amniotic membrane(DBAM) to overcome disadvantages of amniotic membrane for partial thickness burn and compared the effectiveness with cultured allogenic keratinocytes(CAK) that have been recently used for the management of burn. Methods: 16 patients with partial thickness burn, the mean age of 38 ranging 12 to 59 years, between August 2007 and May 2008 were assigned to this study. Either DBAM or CAK was applied, and the secondary dressing was removed on the following day. To compare treatment effect, time for epithelization, Vancouver scar scale and chromameteric results were evaluated. Results: The time for epithelization of DBAM was 10.1 days, that of CAK was 9.1 days, and they were shorter than the previous 2 - 3 weeks. At the follow up Vancouver scar scale was 2.8 for DBAM and 3.0 points for CAK and showed good results. The result of chromameter showed that the $L^*$, $a^*$, and $b^*$ values of the area applied DBAM were 60.1, 13.6, and 13.3, respectively, and the values of the area applied CAK were 60.1, 12.4, and 12.4, respectively. It was found that the skin color of the healed area after burn was darker, the redness was higher, and the yellowness was lower. After dressing, significant side effects were not observed, and in the cases of applying CAK, it was inconvenient as the moving area had to be fixed. Conclusion: With CAK, DBAM has several advantages such as the shortening of the epithelization period, reduction of scar and pigmentation, and convenient application, etc. Thus it is an effective method for the partial thickness burn management.

Effect of the Particle Size of Jeju Scoria on Growth Performance, Carcass Characteristics and Fecal Components in Pigs (입자도가 다른 제주 화산암 분말(Scoria) 급여가 돼지의 성장, 도체특성 및 분성분에 미치는 영향)

  • The effect of different particle sizes of scoria inclusion in weanling-growing-finishing diets on growth performance and carcass quality was investigated in this study. Treatments were the control (basal diet), and 3 scoria treatments with different partical sizes (500, 1,000, 1,700 ${\mu}m$, respectively) in which $3\%$ of basal diet was replaced by scoria. This study was carried out for 143 days using 84 pigs. Each treatment has 3 replicates (4 gilts and 3 barrows per pen). 1. During nursery period, all scoria treatment levels showed poor Average daily gain (ADG) and feed efficiency compared to the control. During growing period, The ADG and feed efficiency of pigs fed the $1,000\;{\mu}m$ swine treatment tended to improve. For the whole growth period (d 0-143), $1,000\;{\mu}m$ scoria treatment showed significantly higher (P<0.05) ADG and feed efficiency than the $1,700\;{\mu}m$ scoria treatment. 2. Carcass weight was higher in all of scoria treatment levels Either the control (P<0.05), while backfat thickness was unaffected among treatments. Carcass Grade A or B grade appearance was more evident in all of scoria treatments compared to the control. 3. Hunter L value was numerically higher in the control while there was no significant differences among the control, $1,000\;{\mu}m$ scoria and $1,700\;{\mu}m$. Hunter a value was highest (P<0.05) in $1,000\;{\mu}m$ scoria, while Hunter b values were not influenced by the scoria treatment levels. 4. The different scoria treatment levels reduced N and P excretion. For growing period, $1,000\;{\mu}m$ scoria was most efficient in reducing N excretion, while during the finishing period, $1,700\;{\mu}m$ scoria was the best. The P excretion was affected by scoria particle size during growing-finishing periods rather than weanling period. The above findings indicated that the inclusion of different scoria partical sizes in nursery diet resulted to adverse effects on growth performance in contrast to the growing finishing diets wherein the growth performance and carcass quality were not affected. Finally, scoria regardless of particle size proved to be effective in reducing N and P excretion and can be considered an environment friendly mineral source for growing-finishing hog diets.

The Relationship Between Measurements of Color and Water-Holding Capacity in Pork Loin (돈육 등심의 육색과 보수력 측정치 사이의 상관관계에 관한 연구)

  • The objective of this study was to investigate the relationship between measurements of color and water-holding capacity (WHC) in pork loin. A total of 419 pork loins were sampled from crossbred (Landrace$\times$Jeju native black pig) F2 pigs at a commercial slaughter house. Meat color measurements (CIE $L^*$, $a^*$, $b^*$), chroma ($C^*$) and hue angle (h) were measured with the Minolta Chromameter and WHC was measured by filter paper fluid, drip loss and cooking loss. Also pH, moisture content and crude fat content were measured at 24 hr postmortem. CIE $L^*$, $b^*$ and h values had positive correlation with drip loss (r=0.52, 0.42), but CIE $a^*$ and $C^*$ values were not related to drip loss. Results showed that CIE $L^*$, $b^*$, and h color system was better than CIE $L^*$, $a^*$, $b^*$ color system to predict WHC, especially drip loss. pH was negatively correlated to drip loss (-0.42) and CIE $L^*$ (-0.67). Although CIE $L^*$ and pH were correlated to drip loss, the accuracy of their estimates for drip loss was 27% and 17%, respectively. Consequently, it was confirmed that meat color and WHC were not perfectly related and suggested that CIE $L^*$, $b^*$, and h color space should not be used independently to predict WHC of pork loin.

Comparison on accuracy of porcelain color reproducibility using two colorimeters (2종의 측색기에 의한 도재 색조 재현의 정확성 비교)

  • Statement of problem: Although the use of colorimeter to improve the accuracy of tooth color measurement has increased, it is real situation that there is almost no comparative study about the accuracy among various colorimeters. Purpose: The purpose of this study is to compare the accuracy of porcelain color reproducibility using ShadeEye-Ncc dental chroma meter (Shofu Inc., Kyoto, Japan) and Shadepilot$^{TM}$ system (Degudent Inc., Hanau, Germany). Material and methods: Color of 2M2 and 3M2 shade tap of Vitapan 3D Master shade guide were measured using the above two colorimeters. Porcelain specimens were fabricated through this data. The porcelain systems used were Vintage Halo Porcelain system with ShadeEye-Ncc dental chroma meter, and VitaOmega900 Porcelain system with Shadepilot$^{TM}$ system. Colors of shade tap models and porcelain specimen were measured with spectrophotometer, and calculated delta E based on the results. Results: The following results were obtained. 1. Shadepilot$^{TM}$ system showed more suitable porcelain color reproducibility compared to ShadeEye-Ncc dental chroma meter (P <.05). 2. The result showed the difference of porcelain color reproducibility according to shade of Shade tap which is model. More suitable porcelain color reproducibility was shown at 3M2 shade tap than at 2M2 shade tap (P <.05). 3. The average of ${\Delta}E*$ of porcelain specimens which was made by the two colorimeters was 4.44-6.14, so the difference of shade was shown through the naked eye. Conclusion: Shadepilot$^{TM}$ system showed more suitable porcelain color reproducibility. When using ShadeEye-Ncc dental chromameter, it will be necessary to improve accuracy through coloring various parts of the whole tooth.
