담석증 환자의 담낭수축능에 관한 연구 (The Study on the Gallbladder Empting in Patients with Gallstones)

  • Gallstone diseases appear frequently in surgical patients. One of the etiolic factors in the formation of cholesterol gallstones is the supersaturation of bile with cholesterol. The bile stasis in the gallbladder may also play a role in the formation of cholesterol gallstones by supplying the proper condition to nucleation of cholesterol crystal. Author evaluated gallbladder empting in response to fat meal by ultrasonography in 12 patients with gallstones and normal control group of 12 healthy subjects. Of the 12 patients with gallstones, six had radiolucent stones and the rest of them had radioopaque stones. Additionally, author evaluated gallbladder contraction in both cholesterol gallstones and pigment gallstones. The gallbladder was significantly contracted in control group by the fat meal but not by water. The contraction of gallbladder by fat meal with gallstone patients was significantly decreased compared with those in control group. The time of maximum contraction was 30 minutes in patients with gallstones. At 30 minutes, the gallbladder contraction was more significantly decreased in patients with radiolucent stones than in those with radioopaque stones. In conclusion, the more significant decrease of gallbladder contraction in patients with radiolucent stones may be interpretated as the possible factor of pathologenesis for the formation of the cholesterol stone.

Micellar Solubilization of Cholesterol, Cholesteryl Myristate and Gallstones by Synthetic Surfactants

  • Solubilization of cholesterol, its fatty acid esters and gallstones were undertaken employing nonionic, anionic and cationic surfactants. Cholesterol was effectively solubilized by all of these surfactants. Cationic surfactants were most effective. However, cholesteryl myristate was not solubilized at all. In the dissolution test of gallstones in surfactant solutions, cationic surfactant solutions were exceptionally effective in dissolving gallstones. These results suggest the possible existence of interaction between alkylammonium radical and cholesterol on micellar surface.

  • Cholesterol gallstones are a digestive disease of high prevalence that has many risk factors; for this reason, research has focused mainly on how to prevent it rather than how to treat it. Many molecules of the hepatic, bile and intestinal systems are involved in the pathophysiology of this disease, making it very difficult to find a therapeutic target. The pharmacological treatment is limited, so when gallstones generate symptoms, medical treatment indicates gallbladder removal. Ursodeoxycholic acid is used to dissolve cholesterol stones, and ezetimibe and statins are other drugs with possible applications in the treatment of this disease. Given the small number of drugs that have been developed for treating this disease, the research of natural products becomes of paramount importance. Resources such as black radish, glucosinolates, fenugreek, capsaicin, curcumin, garlic, and onion, have all shown significant effects in the prevention and treatment of cholesterol gallstones. In this review, we made a synthesis of the scientific reports that deal with these natural products and that can serve as antecedents for finding a way to treat the most common disease of the gallbladder.

건강한 성인의 담석 유병률과 위험인자 (Prevalence and Risk Factors of Gallstones in Adult Health Screening Population)

  • 담석은 담도계의 가장 흔한 질환으로 최근 우리나라에서 콜레스테롤 담석의 비율이 증가하고 있으며 그 대표적인 위험인자로는 연령, 여성, 비만 등이 거론되고 있다. 본 연구는 건강검진자를 대상으로 최근 3년간의 담석의 유병률 및 위험인자를 알아보고자 하였다. 2011년 1월에서 2013년 12월까지 대구지역 종합검진전문병원에서 건강검진을 받은 4,696명을 대상으로 성별과 연령별 분표를 조사하고 키와 몸무게, 공복혈당과 간기능 및 기본 지질 검사를 하였고, 초음파 검사를 통해 담낭의 담석을 진단하였다. 체질량 지수에 따라 저체중, 정상체중, 과다체중, 비만 집단으로 나누었고, 공복혈당 수치는 미국 당뇨 협회에서 제시한 당뇨병의 진단 기준에 따라 분류하였다. 간기능 수치는 본원에서 사용하는 간기능 정상치를 참조하여 대상군을 나누고 지질 수치는 NCEP APTIII에 따라 분류하였다. 분석결과 담석이 발견된 경우는 148명(3.15%)으로 남자 84명(1.79%), 여자 64명(1.36%)으로 남자에서 더 높았으나 성별 간 유병률은 차이는 없었다. 연령별 유병률은 40대 이하 1.84%, 40대 3.38%, 50대 이상 4.66%로 담석이 없는 집단에 비해서 유의하게 높았다(p<0.003). 또한 담낭 담석으로 판명된 대상자의 신체검사 상 고지혈분류 항목에서 비정상군은 Total-cholesterol 52명으로 가장 많았고, LDL-cholesterol 39명, Triglyceride가 36명, HDL-cholesterol 19명이였다. 단변량 분석을 시행한 결과 연령, 공복혈당, 체질량지수는 담석의 유병률과 의미 있는 상관관계를 보였고, 다중 로지스틱 회귀분석에서도 연령, 체질량지수가 담석증의 독립적인 위험인자로 선정되었다. 결론적으로 비록 담석이 대사증후군과의 관계에서 나타나지 않았지만 이를 규명하기 위해서는 건강검진 대상자뿐만 아니라 일반 국민을 대상으로 한 추가적인 연구가 필요하다고 사료된다.

GC/MS에 의한 담석과 담즙내의 Cholesterol, Bile Acids 및 Sterols의 동시 Profiling (Simultaneous Determination of Cholesterol, Bile Acids and Sterols in Human Bile Juices and Gallstones Using GC/MS)

  • 담즙산 분비 저하 및 콜레스테롤의 과도한 분비는 담석 형성의 주된 원인으로 담즙내의 담즙산과 콜레스테롤 및 그 전구 물질인 스테롤의 상대적인 농도는 담즙 성분의 이상을 알아보기 위한 중요한 지표로 알려져 있다. 이에 본 연구에서는 염기를 이용한 가수 분해와 pH 14와 1에서 두 번의 액체-액체 추출을 거친 후, $MSTFA/NH_4I$ 혼합액으로 유도체화 시키는 전처리 방법과 GC/MS를 이용한 새로운 분석 방법을 설정, 담즙산과 스테롤 그리고 콜레스테롤의 동시 분석을 시도하였다. 그 결고 회수율은 73.56-96.95% 이었고 within-a-day 및 day-to-day 분석의 RSD 값은 각각 1.72-13.79%, 0.68-14.10% 이었으며, 이 방법을 간내 담석증 환자에 적용하여 담즙과 담석내에 존재하는 7종의 스테롤과 5종의 담즙산, 그리고 콜레스테롤의 농도 및 그들간의 상대적인 농도 분포를 측정한 결과 담즙고 담석에서 현저히 다른 차이를 보였다.

Aqueous Extract of Lysimachia christinae Hance Prevents Cholesterol Gallstone in Mice by Affecting the Intestinal Microflora

  • With changes in human dietary patterns, the proportion of high-fat and high-cholesterol foods in the daily diet has increased. As a result, the incidence rate of cholelithiasis is increasing rapidly. Many studies have reported on the crucial role that the intestinal microflora plays in the progression of gallstones. Although the whole herb of Lysimachia christinae, a traditional Chinese medicine, has long been extensively used as a remedy for cholelithiasis in China, its effects on the intestinal microflora remain unknown. Hence, in this study, we investigated the ability of the aqueous extract of L. christinae (LAE) to prevent cholesterol gallstones (CGSs) in model animals by affecting the intestinal microflora. The effects of LAE on body weight, serum lipid profile, visceral organ indexes, and histomorphology were studied in male C57BL/6J mice, which were induced by a lithogenic diet. After the 8-week study, CGSs formation was greatly reduced after LAE treatment. LAE also reduced body weight gain and hyperlipidemia and restored the histomorphological changes. Moreover, the intestinal microflora exhibited significant variation. In the model group fed the lithogenic diet, the abundances of the genera unclassified Porphyromonadaceae, Lactobacillus and Alloprevotella decreased, but in contrast, Akkermansia dramatically increased compared with the control check group, which was fed a normal diet; the administration of LAE reversed these changes. These results imply that L. christinae can be considered an efficient therapy for eliminating CGSs induced by a high-fat and high-cholesterol diet, which may be achieved by influencing the intestinal microflora.

Discrimination Analysis of Gallstones by Near Infrared Spectrometry Using a Soft Independent Modeling of Class Analogy

  • A method to discriminate human gallstones by nea. infrared(NIR) spectrometry using a soft independent modeling of class analogy (SIMCA) has been studied. The fifty NIR spectra of gallstones in the wavenumber range from 4500 to 10,000 cm$\^$-1/ were measured. The forty samples were classified to three classes, cholesterol stone, calcium bilirubinate stone and calcium carbonate stone according to the contents of major components in each gallstone. The training set which contained objects of the different known class was constructed using forty NIR spectra and the test set was made with ten different gallstone spectra. The number of important principal components(PCs) to describe the class was determined by cross validation in order to improve the decision criterion of the SIMCA for the training set. The score plots of the class training set whose objects belong to the other classes were inspected. The critical distance of each class was computed using both the Euclidean distance and the Mahalanobis distance at a proper level of significance(${\alpha}$). Two methods were compared with respect to classification and their robustness towards the number of PCs selected to describe different classes.

Prevention of Cholesterol Gallstone Formation by Lactobacillus acidophilus ATCC 43121 and Lactobacillus fermentum MF27 in Lithogenic Diet-Induced Mice

  • The objective of this study was to evaluate the effects of Lactobacillus acidophilus ATCC 43121 and L. fermentum MF27 on biochemical indices in the serum, cholesterol metabolism in the liver and mucin expression in the gallbladder in lithogenic diet (LD)-induced C57BL/6J mice to determine the preventive effects of lactobacilli on gallstone formation. By the end of 4 wk of the experimental period, mice fed on a LD with high-fat and high-cholesterol exhibited higher levels of total and low-density lipoprotein cholesterol in the serum compared to mice fed on control diet or LD with L. acidophilus ATCC 43121 (LD+P1; p<0.05). Cholesterol-lowering effects observed in the LD+P1 and LD with L. fermentum MF27 (LD+P2) groups were associated with reduced expression of 3-hydroxy-3-methylglutaryl coenzyme A reductase in the liver compared to the LD group (p<0.05). Furthermore, expression of the gel-forming mucin, including MUC5AB and MUC5B, was suppressed in the LD+P1 and LD+P2 groups compared to the LD group (p<0.05). Therefore, steady intake of both L. acidophilus ATCC 43121 and L. fermentum MF27 may have the ability to prevent the formation of cholesterol gallstones in LD-induced C57BL/6J mice.

이상지질혈증과 치료제 연구개발 경향 (Drug research and development tend to hyperlipidemia)

  • Most of the cholesterol is synthesized by liver in the body while about one of third is taken via dietary. The main functions of cholesterol is to protect membranes in cell surface, avoid the arterial bleeding by hypertension, and prolong the life of erythrocytes, and so on. However, overload of cholesterol leads to arteriosclerosis associated with leading death cause. Lack of physical activity, emotional and environmental stress, and low intake of protein or vitamin E induce the unbalance between HDL- and LDL-cholesterol so become a basis of ischemic disorders in heart, brain and elsewhere in the body. So far, four major classes of medications for hyperlipidemia are HMG-CoA reductase inhibitors (statins), bile acid sequestrants, nicotinic acid, and fibric acids. The statins can lower LDL and levels triglyceride, but may induce myopathy and an elevation of liver enzyme levels. The bile acid sequestrants lower LDL levels and raise HDL levels with no effect on triglyceride levels but side effects of gastrointestinal (GI) distress, constipation, and a decrease in the absorption of other drugs. Nicotinic acid and fibric acids lower LDL and triglyceride levels with showing flushing, hyperglycemia, hyperuricemia, GI distress, and hepatotoxicity dyspepsia, gallstones, myopathy, and unexplained noncardiac death as adverse effects. Above western drugs lower cholesterol by 15 to 30% while all have notable adverse effects. In traditional medicine, hyperlipidemia is regarded as retention of phlegm and fluid disease. Etiology and pathogenesis of hyperlipidemia is basically based on Spleen-Deficiency and Phlegm-Stagnation, accumulation and stasis of -heat, and Qi & blood stagnation induced by Phlegm-damp, water-dampness, and blood stasis. Thereby, strengthening Spleen and dissolving Phlegm, clearing away heat and diuresis, and supplementing Qi and activating blood circulation are commonly used therapeutic methods for hyperlipidemia. The traditional herbal medicine, have been used for patients with CVA, hypertension or hyperlipidemia in Oriental hospital or Oriental clinic. The lock and key theory is used to develop most of western medicine, however many diseases are caused by mixed factors in body-complex system. We expect that Oriental pharmacological theory could be newborn as a novel drug showing high advantage of blood levels of lipidsand QOL of performance without side effects.

Gallbladder polyps: evolving approach to the diagnosis and management

  • Gallbladder (GB) polyp is a mucosal projection into the GB lumen. With increasing health awareness, GB polyps are frequently found using ultrasonography during health screening. The prevalence of GB polyps ranges between 1.3% and 9.5%. Most patients are asymptomatic and have benign characteristics. Of the nonneoplastic polyps, cholesterol polyps are most common, accounting for 60%-70% of lesions. However, a few polyps have malignant potential. Currently, the guidelines recommend laparoscopic cholecystectomy for polyps larger than 1 cm in diameter due to their malignant potential. The treatment algorithm can be influenced by the size, shape, and numbers of polyps, old age (>50 years), the presence of primary sclerosing cholangitis, and gallstones. This review summarizes the commonly recognized concepts on GB polyps from diagnosis to an algorithm of treatment.
