The objective of this study was to evaluate the effects of Lactobacillus acidophilus ATCC 43121 and L. fermentum MF27 on biochemical indices in the serum, cholesterol metabolism in the liver and mucin expression in the gallbladder in lithogenic diet (LD)-induced C57BL/6J mice to determine the preventive effects of lactobacilli on gallstone formation. By the end of 4 wk of the experimental period, mice fed on a LD with high-fat and high-cholesterol exhibited higher levels of total and low-density lipoprotein cholesterol in the serum compared to mice fed on control diet or LD with L. acidophilus ATCC 43121 (LD+P1; p<0.05). Cholesterol-lowering effects observed in the LD+P1 and LD with L. fermentum MF27 (LD+P2) groups were associated with reduced expression of 3-hydroxy-3-methylglutaryl coenzyme A reductase in the liver compared to the LD group (p<0.05). Furthermore, expression of the gel-forming mucin, including MUC5AB and MUC5B, was suppressed in the LD+P1 and LD+P2 groups compared to the LD group (p<0.05). Therefore, steady intake of both L. acidophilus ATCC 43121 and L. fermentum MF27 may have the ability to prevent the formation of cholesterol gallstones in LD-induced C57BL/6J mice.
Gallstone diseases appear frequently in surgical patients. One of the etiolic factors in the formation of cholesterol gallstones is the supersaturation of bile with cholesterol. The bile stasis in the gallbladder may also play a role in the formation of cholesterol gallstones by supplying the proper condition to nucleation of cholesterol crystal. Author evaluated gallbladder empting in response to fat meal by ultrasonography in 12 patients with gallstones and normal control group of 12 healthy subjects. Of the 12 patients with gallstones, six had radiolucent stones and the rest of them had radioopaque stones. Additionally, author evaluated gallbladder contraction in both cholesterol gallstones and pigment gallstones. The gallbladder was significantly contracted in control group by the fat meal but not by water. The contraction of gallbladder by fat meal with gallstone patients was significantly decreased compared with those in control group. The time of maximum contraction was 30 minutes in patients with gallstones. At 30 minutes, the gallbladder contraction was more significantly decreased in patients with radiolucent stones than in those with radioopaque stones. In conclusion, the more significant decrease of gallbladder contraction in patients with radiolucent stones may be interpretated as the possible factor of pathologenesis for the formation of the cholesterol stone.
Gallstone composition has changed over the past decades in the Korean with a prominent increase in the prevalence of cholesterol gallstone. This trend is possibly due to the westernization of dietary habits. The purpose of this study was descriptive of GB patient's health related eating behaviors and nutrient consumption patterns. One hundred and six gallstone patients who have had cholecystitis surgery enrolled in this study. Anthropomertic indices, such as height, weight, waist circumference, and hip circumference were measured and calculated BMI and WHR. As the biomarker, TC, TG, LDL-C, HDL-C, FBG level and SBP/DBP were measured and analyzed the relationship with GB stone formation. The structured checklist of health related eating behavior and the semiquantitative food frequency questionnaire after pre-test was used in the face to face interview study. The mean age was $54.9{\pm}13.3$ and gallstone disease was more frequent in the fifties and sixties. Mean BMI was $24.3{\pm}2.8$ in males, and $23.4{\pm}3.9$ in females, the average of waist circumference was $91.1{\pm}7.0cm$ in males and females were $85.4{\pm}9.6cm$. The WHR of men and women was $0.93{\pm}0.0,\;0.90{\pm}0.1$, respectively. The obesity and overweight trend was observed in gallstone patients. The mean blood sugar was researched at $114.5{\pm}35.0mg/dL$. And the ratio of both symptoms DM and gallstone was 26.4%. The rate of dramatic eating repast was significantly higher in the females (p<0.01). Males tended to more frequently consume meat, of high fat content meats and greasy food consumption when eating out. The CPF ratio was 57:16.5:26.5. There was a significant positive correlation between WC and energy (r=0.252, p<0.05), carbohydrate (r=0.255, p<0.05) and niacin (r=0.227, p<0.05). In addition, carbohydrate were significantly correlatied with TC (r=0.230, p<0.05). BMI appeared positive in correlation of protein (r=0.201, p<0.05) and fat (r=0.205, p<0.05). These findings provide a little association that dietary habits are related with cholesterol gallstone formation.
Cholesterol supersaturation in bile, which causes gallstone formation, is the result of low bile acid secretion or high cholesterol secretion. The quantitative analysis of cholesterol, bile acids and sterols which are precursors of cholesterol have been used to examine the changes in bile component. We described a simple, sensitive and reproducible method for simultaneous determination of cholesterol, five bile acids and seven sterols in human bile juices and gallstones by capillary column gas chromatography-mass spectrometry (GC/MS). Clinical samples were hydrolyzed by alcoholic KOH, extracted twice (pH 14 and 1) and derivatized to trimethylsilyl (TMS) ether with $MSTFA/NH_4I$ (N-methyl-N-trimethylsilyltrifluoroacetamide/ammonium iodide) mixture in order to be detected on the GC/MS. The good quality control data were obtained through within-a-day and day-to-day test (RSD values were 1.72-13.79, 0.68-14.10, respectively) and the recovery range of them was 73.56-96. 95 Using this method, biliary and gallstone compositions in the patients with intrahepatic stones were analyzed. The amounts and its relative distribution of cholesterol, bile acids and sterols showed different pattern in bile juices and gallstones.

A method to discriminate human gallstones by nea. infrared(NIR) spectrometry using a soft independent modeling of class analogy (SIMCA) has been studied. The fifty NIR spectra of gallstones in the wavenumber range from 4500 to 10,000 cm$\^$-1/ were measured. The forty samples were classified to three classes, cholesterol stone, calcium bilirubinate stone and calcium carbonate stone according to the contents of major components in each gallstone. The training set which contained objects of the different known class was constructed using forty NIR spectra and the test set was made with ten different gallstone spectra. The number of important principal components(PCs) to describe the class was determined by cross validation in order to improve the decision criterion of the SIMCA for the training set. The score plots of the class training set whose objects belong to the other classes were inspected. The critical distance of each class was computed using both the Euclidean distance and the Mahalanobis distance at a proper level of significance(${\alpha}$). Two methods were compared with respect to classification and their robustness towards the number of PCs selected to describe different classes.

The reported risk factors for gallstone disease (GD) are old age, female sex, obesity, metabolic syndrome and migrants. Many younger adults tend to live in Jeju City, where transportations are convenient and commercial activities are active. Whereas, older people tend to live in Seogwipo City, because they engaged in fisheries and agriculture. Rates of migrants and old age could affect the prevalence of GD among residents in two regions. Therefore, the purpose of this study was to compare the GD prevalence and analyze risk factors affecting GD including residencies. A total of 13,050 subjects who visited a single health medical check-up center on Jeju Island between 2012 and 2019 were included. We performed univariate and multivariate analysis to identify risk factors for GD. The prevalence of GD among residents were 5.7% in Jeju City and 5.8% in Seogwipo City, respectively. Multivariate analysis revealed that age (P=0.008), body mass index (P=0.044), high-density lipoprotein cholesterol (P=0.006) and gamma-glutamyl transferase (P=0.013) were independent factors affecting GD. The old age, mean higher body mass index, gamma-glutamyl transferase and mean lower high-density lipoprotein cholesterol were independent risk factors affecting GD. However, residencies did not affect the prevalence of GD.

With changes in human dietary patterns, the proportion of high-fat and high-cholesterol foods in the daily diet has increased. As a result, the incidence rate of cholelithiasis is increasing rapidly. Many studies have reported on the crucial role that the intestinal microflora plays in the progression of gallstones. Although the whole herb of Lysimachia christinae, a traditional Chinese medicine, has long been extensively used as a remedy for cholelithiasis in China, its effects on the intestinal microflora remain unknown. Hence, in this study, we investigated the ability of the aqueous extract of L. christinae (LAE) to prevent cholesterol gallstones (CGSs) in model animals by affecting the intestinal microflora. The effects of LAE on body weight, serum lipid profile, visceral organ indexes, and histomorphology were studied in male C57BL/6J mice, which were induced by a lithogenic diet. After the 8-week study, CGSs formation was greatly reduced after LAE treatment. LAE also reduced body weight gain and hyperlipidemia and restored the histomorphological changes. Moreover, the intestinal microflora exhibited significant variation. In the model group fed the lithogenic diet, the abundances of the genera unclassified Porphyromonadaceae, Lactobacillus and Alloprevotella decreased, but in contrast, Akkermansia dramatically increased compared with the control check group, which was fed a normal diet; the administration of LAE reversed these changes. These results imply that L. christinae can be considered an efficient therapy for eliminating CGSs induced by a high-fat and high-cholesterol diet, which may be achieved by influencing the intestinal microflora.
Gallstone is the most common disease of the biliary system. Korean has experienced an increase in the percentage of cholesterol gallstones. The major risk factors associated with cholesterol gallstones are age, gender as well as obesity. This study was designed to determine the prevalence of gallstones in the last three years and evaluate the associated risk factors in the population who underwent health screening. The study population consisted of 2,484 males and 2,212 females who visited the health promotion center in Dalseogu, Daegu in Korea from January 2011 to December 2013. Each participant in the study had their biliary system gallbladder examined using ultrasonography. Classified as underweight, normal weight or overweight using the population of obese according to the body mass index, and classified according to mood diagnosis of diabetes presented by the American Diabetes Association. Fasting blood glucose and number of liver function, the divided the control group by referring to the normal liver function values used herein. The geological map, I was classified as NCEP APT III. A showed of total 148 people were found to have gallstones. The prevalence of sex among 148 patients (3.15%) 84 men (1.79%) and 64 women 1.36%) which shows significantly there is little difference. 1.84% 40 years and below, 3.38% 40's showed age prevalence was 4.66% in 50's and above. In addition, Total-cholesterol was at the most in 52 people, LDL-cholesterol in 398 people, Triglyceride in 36 people, HDL-cholesterol in 19 people. The abnormal group, was created from the total-cholesterol categories from a physical examination of a subject that has been found to be gallstones in the gallbladder. A result of conducting the univariate analysis shows the prevalence of gallstones, a correlation that is meaningful. The logistic regression analysis of multiple ages was chosen to show risk factors age independent cholelithiasis. In spite of the conclusion, gallstones are not displayed in relation to the metabolic syndrome but in order to clarify this, not only the subject of a health examination is needed but, a further study of the general public when possible.
Purpose: We investigated the clinical features and factors affecting the choice of treatment modality and the course of pediatric gallstone (GS) disease. Methods: We retrospectively analyzed the medical records of 65 patients diagnosed with GS using imaging studies between January 2009 and December 2017 were included. Results: This study included 65 patients (33 boys and 32 girls; mean age, 8.5±5.3 years; range, 0.2-18 years) who primarily presented with abdominal pain (34%), jaundice (18%), and vomiting (8%). Idiopathic GS occurred in 36 patients (55.4%). The risk factors for GS included antibiotic use, obesity, hemolytic disease, and chemotherapy in 8 (12.3%), 7 (10.8%), 6 (9.2%), and 4 patients (6.2%), respectively. We observed multiple stones (including sandy stones) in 31 patients (47.7%), a single stone in 17 (26.2%), and several stones in 17 (26.2%). GS with a diameter of <5 mm occurred in 45 patients (69.2%). Comorbidities included hepatitis, choledocholithiasis, cholecystitis, and acute pancreatitis in 20 (30.8%), 11 (16.9%), 11 (16.9%), and 4 patients (6.2%), respectively. Ursodeoxycholic acid (UDCA) was administered to 54 patients (83.1%), leading to stone dissolution in 22 patients (33.8%) within 6 months. Cholecystectomy was performed in 18 patients (27.7%) (mean age, 11.9±5.1 years). Most patients treated surgically had multiple stones (83%) and stones measuring <5 mm in size (89%), and 66.7% of patients had cholesterol stones. Conclusion: Cholecystectomy is feasible in patients with small-sized or large numbers of GS and those with persistent abdominal pain and/or jaundice. UDCA administration with close follow-up is recommended in patients with uncomplicated GS.
The concentration of free fatty acids and fatty acid composition as well as cholesterol supersaturation in bile may be an important factor in the gallstone formation. Therefore, we simultaneously determinded 23 fatty acids in bile by selected ion monitoring (SIM) method of gas chromatography-mass spectrometry (GC-MS). Biliary fatty acids were extracted by aminopropyl column and the extracts with (phospholipid fraction) or without (free fatty acid fraction) alkaline hydrolysis of phospholipid were derivatized with MSTFA/TMCS (N-methyl-N-trimethylsilyl-trifluoroacetamide/trimethylsilylchloride) mixture in order to be detected on the GC-MS. The recovery range of this method was 61.1-99.0% and the RSD value of within-a-day and day-to-day test were 3.1-25.6% and 3.8-27.0%, respectively. Using this method, biliary profile was investigated in the bile of normal controls and patients with gallstones. The amounts and their distribution of free and phospholipid fatty acids showed different pattern between normal subjects and patients.
