Incidence and Mortality of Female Breast Cancer in Jiangsu, China

  • Objectives: The aim of this study was to describe and analyze the incidence and mortality of female breast cancer in Jiangsu Province of China. Methods: Incidence and mortality data for female breast cancer and corresponding population statistics from eligible cancer registries in Jiangsu from 2006 to 2010 were collected and analyzed. Crude rates, age-specific rates and age-standardized rates of incidence and mortality were calculated, and annual present changes (APCs) were estimated to describe the time trends. Results: From 2006 to 2010, 11,013 new cases and 3,068 deaths of female breast cancer were identified in selected cancer registry areas of Jiangsu. The annual average crude incidence and age-standardized incidence by world population (ASW) were 25.2/ and 17.9/100,000 respectively. The annual average crude and ASW for mortality rates were 7.03/ and 4.81/100,000. The incidence was higher in urban areas than that in rural areas, and this was consistent in all age groups. No significant difference was observed in mortality between urban and rural areas. Two peaks were observed when looking at age-specific rates, one at 50-59 years and another at over 85 years. During the 5 years, incidence and mortality increased with APCs of 4.47% and 6.89%, respectively. Compared to the national level, Jiangsu is an area with relatively low risk of female breast cancer. Conclusion: Breast cancer has become a main public health problem among Chinese females. More prevention and control activities should be conducted to reduce the burden of this disease, even in relatively low risk areas like Jiangsu.

Pancreatic Cancer Incidence and Mortality Patterns in China, 2009

  • Objective: To estimate the incidence and mortality rates for pancreatic cancer in China. Methods: After checking and reviewing the cancer registry data in 2009 from 72 cancer registry centers, we divided cancer registry areas into urban and rural areas. Incidence/mortality rates, age-specific incidence/mortality rates, age-standardized incidence/mortality rates, proportions, and cumulative incidence/mortality rates for pancreatic cancer were calculated. Results: The total number of newly diagnosed pancreatic cancer cases and deaths in 2009 were 6,220 and 5,650, respectively. The crude incidence rate in all cancer registry areas was 7.28/100,000 (males 8.24, females 6.29). The age-standardized incidence rate by Chinese standard population (ASR) was 3.35/100,000, with ranking at 7th among all cancers. Pancreatic cancer incidence rate was 8.19/100,000 in urban areas whereas it was 5.41/100 000 in rural areas. Cancer mortality rate in all cancer registry areas was 6.61/100,000 (males 7.45; females 5.75), with ranking at 6th among all cancers, and 7.42/100 000 in urban but 4.94/100000 in rural areas. Conclusions: Pancreatic cancer incidence and mortality rates have shown a gradual increase in China. Owing to the difficulty of early diagnosis, identification of high-risk population and modification of risk factors are important to reduce the burden of pancreatic cancer.

Short-course Versus Long-course Preoperative Radiotherapy plus Delayed Surgery in the Treatment of Rectal Cancer: a Meta-analysis

  • Background: Short-course preoperative radiation (SCRT) with delayed surgery was found to increase pathologic complete response (pCR) rates in several trials. However, there was no clear answer on whether SCRT or long-course chemo-radiotherapy (LCRT) is more effective. Therefore we conducted this meta-analysis to evaluate the safety and efficacy of SCRT versus LCRT, both with delayed surgery, for treatment of rectal cancer. Materials and Methods: The literature was searched from PubMed, EMBASE, Web of Science, Cochrane Library and clinicaltrials.gov up to November, 2014. Quality of the randomized controlled trials (RCTs) was evaluated according to the Cochrane's risk of bias tool of RCT. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) system was used to rate the level of evidence. Review Manager 5.3 was employed for statistical analysis. Pooled risk ratios (RRs) and 95% confidence intervals (CIs) were calculated. Results: Three RCTs, with a total of 357 rectal cancer patients, were included in this systematic review. Metaanalysis results demonstrated there were no significantly differences in sphincter preservation rate, local recurrence rate, grade 3~4 acute toxicity, R0 resection rate and downstaging rate. Compared with SCRT, LCRT was associated with significant increase in the pCR rate [RR=0.49, 95%CI (0.31, 0.78), P=0.003]. Conclusions: In terms of sphincter preservation rate, local recurrence rate, grade 3~4 acute toxicity, R0 resection rate and downstaging rate, SCRT with delayed surgery is as effective as LCRT with delayed surgery for management of rectal cancer. LCRT significantly increased pCR rate compared with SCRT. Due to risk of bias and imprecision, further multi-center large sample RCTs were needed to confirm this conclusion.

Glaucocalyxin A Activates FasL and Induces Apoptosis Through Activation of the JNK Pathway in Human Breast Cancer Cells

  • This study was conducted to analyze the molecular mechanisms responsible for anti-proliferation effects of glaucocalyxin A in cultured MCF-7 and Hs578T breast cancer cells. The concentration that reduced cell viability to 50% (IC50) after 72 h treatment was derived and potential molecular mechanisms of anti-proliferation using the IC50 were investigated as changes in cell cycle arrest and apoptosis. Gene and protein expression changes related to apoptosis were investigated by semi-quantitative RT-PCR and western blotting, respectively. Involvement of phosphorylated mitogen-activated protein kinases and JNK signaling in regulation of these molecules was characterized by western blotting. Cell viability decreased in a concentration-dependent manner and the IC50 was determined as $1{\mu}M$ in MCF-7 and $4{\mu}M$ in Hs578T cell. Subsequently, we demonstrated that the GLA-induced MCF-7 and Hst578T cell death was due to cell cycle arrest at the G2/M transition and was associated with activation of the c-jun N-terminal kinase (JNK) pathway. We conclude that GLA has the potential to inhibit the proliferation of human breast cancer cells through the JNK pathway and suggest its application forthe effective therapy for patients with breast cancer.

Genotype Distribution and Behavioral Risk Factor Analysis of Human Papillomavirus Infection in Uyghur Women

  • We investigated the distribution of HPV genotypes in Uyghur women in Xinjiang region of China, and behavioral factors which could predispose them to HPV infection. In this cross-sectional study, women aged 15-59 years were recruited by cluster sampling method in Yutian region in 2009. Liquid-based cytology samples were analyzed centrally for HPV genotype with a linear array detector. Univariate and multivariate logistic regression analyses were performed to identify behavioral risk factors for HPV infection. A total of 883 Uyghur women were recruited successfully. The prevalence of high-risk HPV and low-risk HPV were 7.25% and 1.58%, respectively; the most common HPVs were HPV16, 51, 31, 39 and 58. We found that age of first sexual intercourse was a strong predictor for HPV infection (odds ratio of 4.01 for ${\leq}15$ years versus ${\geq}25$). Having sexual partners ${\geq}3$ was the second predictor (OR 3.69, 95% CI 2.24-7.16). Cleaning the vagina after sex showed an increased risk of HPV infection (OR 2.72; 95% CI 1.98-5.13); Using the condom showed protective factors for HPV infection (OR 0.36; 95%CI0.12-0.53). HPV16, 51, 31, 39 and 58 were the priority types; the age of first sexual intercourse was identified as a major risk factor for HPV infection. Other notable risks were number of sexual partners and cleaning the vagina after sex. Changing these behavioral risk factors could help to reduce the occurrence of cervical cancer in this population.

Study on Soy Isoflavone Consumption and Risk of Breast Cancer and Survival

  • Aim: Isoflavones in soy foods are part of a larger class of flayonoid compounds that have have been demonstrated to be potent dietary anti-cancer agents, and the effect of soy intake on the survival of ovarian cancer is conflicting. Therefore, we aimed to explore the whether soy intake is related to the risk of death of breast cancer. Methods: A prospective study was conducted. A total of 256 patients included in this study had breast cancer and were recruited between January 2004 and January 2006. All of them were followed up from since January 2011. A univariate Cox's regression analysis was used to assess the association between soy intake and survival. Results: The education level, menopausal status, ER/PR status and TNM stage were significant difference in the survival of breast cancer. The highest soy isoflavone was associated with a decreased death risk of breast cancer (OR=0.25, 95% CI=0.09-0.54). Moreover, the higher consumption of soy protein also presented a trend decreased breast cancer risk, and the highest consumption significantly reduced the cancer risk compared with the lowest consumption (OR=0.38, 95% CI=0.17-0.86). Conclusion: The present study suggests soy intake is associated with a significant reduced death risk of breast cancer in Chinese population. Further large sample studies are warranted to confirm the inverse association of soy consumption and breast cancer survival by menopausal status.

A Literature Study of Samguchup - Based on the Traditional Chinese Medical Journal - (삼구첩(三九貼)에 관한 문헌적 고찰 - 중의학 잡지를 중심으로 -)

  • Objectives: The Samguchup is a plaster therapy in winter to prevent and treat diseases occurring frequently in the winter. Studies on samguchup published in China were analyzed for our study in order to utilize in Korea. Methods: 15 studies with keywords such as 'Sanjiutie' or 'Dongbingdongzhi' published in CAJ (China Academic Journal) until July 31, 2012 were compared, in terms of clinical effects, herbs, acupoints, attatching time, and accompanying treatment. Results: Treatment rate ranged from 65% to 92.65%, in recurrent respiratory infection, cough, allergic rhinitis, asthma, chronic bronchitis, chronic obstructive pulmonary disease, sleep apnea, and rheumatic arthritis. The herbs used more than 5 times were Semen Sinapis Albae, Herba Asari, Ginger juice, Radix Kansui, and Rhizoma Corydalis. The acupoints used more than 5 times were BL13, BL23, GV14, CV17, CV22, and EX-B1. The attaching time were usually 4 hours, but shorter for child. The Sambokchup, Korean medicine, moxibustion on ginger, drug(western medicine), injection, and inhalation were used as accompanying treatment. Conclusions Appropriate use of Samguchup is necessary to treat or prevent diseases in winter.

Analysis of Studies on Pyeongwi-san to Establish the Fundament for Evidence Based Medicine (EBM 기반구축을 위한 평위산 전임상 연구 문헌 분석)

  • To establish the fundament for EBM of Traditional Korean Medicine, the papers on Pyeongwi-san (Pingwei-san) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. The papers were classified by the registration of domestic or international journals, the year of publishment, experimental fields and the kinds of studies on biological activities. Of total 518 papers on Pyeongwi-san (Pingwei-san), 32 volumes were selected according to selection creteria. 20 volumes were published in domestic journals, 11 in Chinese journal and 1 in Japanese journal. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Pyeongwi-san (Pingwei-san) using HPLC, GC-MS. The papers on biological activities of Pyeongwi-san (Pingwei-san) showed improvement of gastrointestinal activity and water-electrolyte metabolism, immune regulation, anti-oxidant, anti-inflammatory, analgesic, anti-convulsant activities, hypnotic duration, blood pressure regulation, hepatic protection, congestive heart failure, anti-cancer activities. Further studies including gastrointestinal activity need to be preceeded to establish the fundament for EBM of Pyeongwi-san (Pingwei-san).

Analysis of studies on Bojungikgi-tang(Buzhongyiqi-tang) to establish the fundament for Evidence Based Medicine (EBM) (EBM 기반구축을 위한 보중익기탕 실험 연구 분석)

  • Objective : To establish the fundament for EBM of Traditional Korean Medicine, the papers on Bojungikgitang(Buzhongyiqi-tang) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Bojungikgi-tang(Buzhongyiqi-tang) using HPLC method. The papers on biological activities of Bojungikgi-tang(Buzhongyiqi-tang) showed mainly immune regulation, anti-infection, anti-cancer, gastrointestinal activity, neuropsychiatric regulation, anti-inflammation, hepatoprotection, musculoskeletal regulation. Among biological activities, papers on immume regulation were reported mostly. Conclusions : The efficacy of Bojungikgi-tang(Buzhongyiqi-tang) could be related to immune regulation, gastrointestinal activity, anti-fatigue and further study need to be preceeded to establish the fundament for EBM of Bojungikgitang(Buzhongyiqi-tang).

A Study on the Mitigation of Nitrous Oxide emission with the Horticultural Fertilizer of Containing Urease Inhibitor in Hot Pepper and Chinese Cabbage Field (고추와 배추 재배지에서 요소분해효소 억제제 함유 원예용 비료 시용에 따른 아산화질소 배출 저감 효과)

  • BACKGROUND: About 81% of nitrous oxide ($N_2O$) emissions from agricultural land to the atmosphere is due to nitrogen (N) fertilizer application. Mitigation of $N_2O$ emissions can be more effective in controlling biochemical processes such as nitrification and denitrification in the soil rather than decreasing fertilizer application. The use of urease inhibitors is an effective way to improve N fertilizer efficiency and reduce $N_2O$ emissions. Several compounds act as urease inhibitors, but N-(n-butyl) thiophosphoric triamide (NBPT) has been used worldwide. METHODS AND RESULTS: Hot pepper and chinese cabbage were cultivated in five treatments: standard fertilizer of nitrogen-phosphorus-potassium(N-P-K, $N-P_2O_5-K_2O$: 22.5-11.2-14.9 kg/ha for hot pepper and $N-P_2O_5-K_2O$: 32.0-7.8-19.8 kg/ha for chinese cabbage), no fertilizer, and NBPT-treated fertilizer of 0.5, 1.0, and 2.0 times of nitrogen basal application rate of the standard fertilizer, respectively in Gyeonggi-do Hwaseong-si for 2 years(2015-2016). According to application of NBPT-treated fertilizer in hot pepper and chinese cabbage, $N_2O$ emission decreased by 19-20% compared to that of the standard fertilizer plot. CONCLUSION: NBPT-treated fertilizer proved that $N_2O$ emissions decreased statistically significant in the same growth conditions as the standard fertilization in the hot pepper and chinese cabbage cultivated fields. It means that NBPT-treated fertilizer can be applied for N fertilizer efficiency and $N_2O$ emissions reduction.
