Reorganization of myun health care service is one of the main issues in health care delivery in rural Korea. The fundamenta, concept of the role and function of the myun health subcenter is that it is the basic unit of rural health care service and is to provide comprehensive health care service through the integration of curative and preventive services. The aim of this study is to analyze the patterns of curative activities in the myun health subcenter in terms of the most prevalent types of diseases, necessary diagnostic methods and required equipment, types of treatment, necessary drugs and materials, and finally the cost of curative services. The population on which this study was done was the 1596 patients who visited the two myun health subcenters (Sunwon Myun and Naega Myun) in Kang Wha County, the area of the Yonsei University Community Health Teaching Project, during period from May 1, 1975 to June 10, 1976. For the patient's record in the clinic, problem oriented medical records were used. Decisions regarding the disease classification, the diagnostic methods used and selection of the most appropriate and adequate medical treatment were made by a group of three experienced physicians after reviewing the medical records which had been written by public physicians who were treating patients in the study area. The records were reviewed by resident staff members of the Department of Preventive Medicine, of Yonsei University College of Medicine. A brief summary of results of the study is as follow: 1. 29.9% of the patients who visited the clinics were ages between 0-4. No sex difference was observed among patients less than 20 years of age. However, among patients over 20 years old, females predominated. Thus it is evident that the majority of patients were either children or mothers and grandmothers. 2. The distance from the individual villages to the myun health subcenter was one of important factors in determining the ratio of clinic visits. However, other factors such as the activities of the health workers also affected the rates substantially. 3. The most common 25 diseases comprised 90.2% of all the diseases recorded. Acute respiratory infection (25.5%), Skin (12.7%) , diarrheal diseases (6.8%), neuralgia and back pain (4.9%) and. all other injuries (3.9%) were the five most common diseases. 4. Of all the diseases diagnosed and treated, 9.2% required simple laboratory tests for diagnosis, 6.5% required X-ray examination, and altogether 13.6% required either laboratory test or X-ray examination. 5. Treatment and management of 42.0% of the cases could be accomplished with simple, inexpensive drugs, 12.8% required the use of more expensive drugs (mostly antibiotics) and injections were required in 19.7% of the cases. Minor surgery and referral were necessary in 5% of the cases. 6. The cost for diagnosis and treatment was estimated with a standard which was set by general concensus. The average cost of diagnosis was 144 per case and the cost of treatment was 726 per case, The Total average cost per visit was 870.
This study is for the analysis of the influence that the factors(communication, domestic life, offensive action, economic problem) of marital conflicts of old age and ecological systems factors(individual, family, sociality) have a impact to the divorce of elderly(thoughts and attempts of divorce) and the impulse to commit a crime(violence, egoism, running away from home). The guinea pigs is 350 married people of ages greater than 55 in the capital area. The 338 collected data excluding incomplete 12 data were analyzed. Statistical methods used in this study were factor analysis, frequency analysis, reliability analysis, t-test, ANOVA, Hiearchical regression analysis and Scheffe test with SPSS 18.0. The result are as follows. First, the marital conflict and ecological systems have a severe impact to the divorce of elderly, the more the conflict of the domestic life and the offensive action, the conflict in communicative and economic problem are getting higher. Second, the marital conflict and ecological systems have a severe impact to the impulse to commit a crime, the more the conflict of a domestic life and the conflict caused by spouse's attack, the economic problem, the egotistic impulse are getting higher. Third, there are some statistically similar differences of all the parts(sex, academic ability, job, children coresidence, religion, physical condition, monthly average income) in the marital conflict by characteristic of sociology of population, ecological systems, the divorce of elderly and the impulse to commit a crime.
This study aimed to identify self-worth and self-deprecation trajectories and their associated factors among Korean adolescents. For these purposes, we used latent growth curve modeling involving 2,350 students who participated in the Korea Children and Youth Panel Survey in 2010, 2012, 2014, and 2015. Major findings are as follows: 1) Adolescents' self-worth and self-deprecation increased with time, but the speed gradually changed to a quadratic function model; and 2) the types of predictors affecting self-worth and self-deprecation were different. Specifically, the factors that affected only self-worth were adolescents' relationship with teachers and household income, and the factors that affected only self-deprecation were presence of disease and parental over interference. Factors affecting both self-worth and self-deprecation were child's sex, parental affection, peer trust, and peer alienation. These results suggest that independent intervention is needed for self-worth and self-deprecation. Furthermore, the results can be an important basis for establishing a more focused intervention strategy when intervening in self-worth and self-deprecation in adolescents.

The purpose of this study is to analyze the moderating effect of care service satisfaction, housework sharing, gender equality consciousness, and gender role attitude on the relationship between work family balance and the psychological well-being of working mothers. This study was conducted from March 20, 2013 to March 26, 2019, with a total of 163 questionnaires for working moms having elementary school children. The results demonstrating the positive effect of care service satisfaction provide policy implications for extending the care service system. Notably, it was confirmed that psychological well-being increased in cases of work-family conflict as housework sharing, egalitarian sex role attitude, and gender equality consciousness increased. Therefore, subsequent study will be necessary to discuss further about sharing the spouse 's housework and raising the gender equality consciousness.

Objectives : The aim of the study was to investigate the prevalence by gender and to evaluate associations of depression, anxiety and sleep disturbances in a large sample of adolescents from the general population. Methods : 1422 subjects, aged 14 years students were recruited in the seven middle-school of the local community. The self-report questionnaire(Children's Depression Inventory : CDI, Beck Anxiety Inventory : BAI and Pittsburgh Sleep Quality Index : PSQI) was used for screening depression, anxiety and sleep disturbances in adolescent. CDI, BAI, and PSQI score was categorized into the following quartiles : $\leq$21, 22-25, 26-28, and $\geq$29 ; $\leq$21, 22-26, 27-31, and $\geq$32 ; $\geq$5, <5. The cut-off score is that CDI is 22, BAI is 22 and PSQI is 5. The result was analyzed using the independent t-test, the chi-square test and logistic regression analyses. Results : The mean CDI score($12.52{\pm}8.32$ vs. $10.39{\pm}7.52$ ; p=0.003), BAI($7.77{\pm}7.93$ vs. $9.84{\pm}9.04$ ; p<0.001) and PSQI($4.57{\pm}2.67$ vs. $3.64{\pm}2.30$ ; p=0.013) of girls were significantly higher than for boys. But, boys in the fourth quartile of CDI(CDI$\geq$29) and BAI(BAI$\geq$32) were at significantly elevated risk for sleep disturbances more than for girls after adjustment for sex, history of psychiatric treatment(CDI odd ratio, 14.66 ; 95% CI, 4.17-51.53, BAI odd ratio, 32.99 ; 95% CI, 4.26-255.39). Conclusion : The results suggest that high CDI, BAI score appears to increase the risk for developing sleep disturbances in boys more than girls.

Phthalates, endocrine disrupting chemicals, are similar in structure to sex hormones and mainly show reproductive toxicity and developmental toxicity. In this study, we analyzed 11 phthalates, including 3 kinds of phthalates prohibited in cosmetic use and 8 kinds of phthalates regulated in 'Common standards for children's products safety' and EU cosmetic regulation (EC No. 1223/2009). The phthalate analysis was optimized using GC-MS/MS. In analytical method validation, this method was satisfied in specificity, linearity, recovery rate, accuracy and MQL. Therefore, we used this method to analyze 82 products of Nail cosmetics & polish. Although six phthalates such as DBP, BBP, DEHP, DPP, DIBP and DIDP were detected at concentrations of $1.0{\sim}59.8{\mu}g/g$g, they were suitable to Korean cosmetic standards. DIBP and DBP were detected at concentration of $1.1{\sim}2.6{\mu}g/g$ in artificial nail, DBP and DEHP were $1.4{\sim}2.5{\mu}g/g$ in glue for nails, and DIBP, DBP, and DEHP were $2.5{\sim}33.3{\mu}g/g$ in nail stickers. Although substances such as DBP and DEHP in artificial nail, Glue for nails, and nail stickers were detected, they were suitable to 'Common safety standards for children's products. DIBP is not a regulated substance in Korea but showed the third highest detection rate following DBP (84.6%) and DEHP (63.4%). The concentration of phthalates detected in nail products is considered to be safe in current standards but continuous monitoring and research about non-regulated substances are also needed to be considered.

Purpose : Juvenile dermatomyositis (JDM) is the most common of the idiopathic inflammatory myopathies in children. The purpose of this study is to observe demographic, initial presentations, duration of time between disease onset and diagnosis, clinical manifestations and laboratory findings at diagnosis of patients with JDM. Methods : Forty seven patients identified at Seoul National University Children's Hospital from January 1986 to May 2007. Medical records were reviewed retrospectively focusing on initial presentations, clinical manifestations and laboratory findings at the time of diagnosis of patients with JDM. Results : Male and female patients were 25 and 22, respectively and sex ratio was 1.14:1. The average age at the time of diagnosis was 6.51 years. Skin rash (94%) was the most common symptom, followed by the proximal muscle weakness (89%). The disease activity score was 10.8. The duration between the onset of the skin rash and the muscle weakness and diagnosis was 7.18 and 4.70 months, respectively. The serum muscle enzymes, LDH, AST, CK and aldolase, were elevated in the patient with JDM. Autoimmune antibodies, antinuclear antibody, anti SSA antibody and anti SSB antibody, were negative findings. Electromyography findings were consistent with JDM in 88% of the patients, the muscle biopsy was in 91% and all MRI findings were compatible with those of patients with JDM. The most common symptom besides musculocutaneous lesions was the calcinosis (62.5%). The most common site of calcinosis was the pelvic area and buttocks. Conclusion : This study shows that the major symptoms are proximal muscle weakness and cutaneous lesion, and they are important to diagnose JDM.
The purpose of this study was to evaluate growth changes and skeletal characteristics of Korean children with Class III malocclusions from 10 to 14 years of skeletal age. Radiographs of 60 children with Class III malocclusion and 60 normal controls were assessed. Both groups were subdivided into 6 samples according to sex and skeletal age. Skeletal age was assessed using handwrist X-rays using the Greulich and Pyle norms. The Krogman-Walker plane (occipitale-maxillon) through Sella was used as a reference plane in this study with x-axis perpendicular to the x-axis. Sir Student t-tests were conducted to compare the control group with the Class III group according to each gender a:nd age. The characteristics of Class III malocclusion group compared to the control group included shorter anterior and posterior cranial base, shorter and retrusive maxilla, forger mandible, increased molar-incisor distance, retroclined lower incisors, labially proclined upper incisors, and anteriorly located mandibular molar, smaller upper and middle facial depth, and larger lower facial depth. Landmarks representing facial depth, size of maxilla and mandible, and their AP relationship including anterior facial height indicate that growth characteristic was determined early in life. But growth Pattern of cranial base and some of the dental landmarks showed progressive divergence between Control and Class III groups with age. The position of the posterior border of the mandible was found to be significantly forward in both females and males by the age of 14 and at the anterior border in males and females at all ages. Hyperdivergent mandibular plane, changes in anterior segment of mandible, small anterior cranial base, and decrease in cranial base flexure was also noted.

Present study is to investigate clinical characteristics, treatment method and outcome of 23 children who were diagnosed as selective mutism by DSM-IV criteria at the child psychiatry ouptatient department of SNUH. The results were as follows : 1) The Sex ratio was 1: 4.8, female dominant Mear age of onset was 33 years old and mean age of first referral was 7.7 years old. 2) 22% of subjects had perinatal problem such as low birth weight, preterm birth, 26% of the subjects have history of delayed language development. There are subjects who had been separated with mam caretaker before 3 years old(26%) and who experienced physical or psychological trauma before 3 years old(26%). A few subjects had enurests(30%) and encoprests(4%). 3) Many subjects(65%) had symbiotic relationship with their mother. These families consist of dominant, verbally aggressive mother and passive father. Parents of 39% of all subjects were judged to have definite psychopathology(social phobic, depression, hysterical trait or alcohol problem) 26% of all subject, were reported physically abused. 4) The personality trait of the subjects were frequently described as follows(in order of frequency) ; Shy(100%), anxious(83%), stubborn(83%)m rigid and tense posture(78%), immature(65%) overdependent(65%), irritable(52%), manipulative(39%), depressive(39%). 5) The mean performance IQ of 16 subjects by KEDI-WISC was 88.3 Among them, the subjects with IQ below 69 were seven and those with IQ above 70 were nine. When comparing these two group(Mental retardation group vs Normal IQ group), we could find some difference in language development, personality trait, family dynamics and treatment outcome. 6) Among several treatment methods for selective mutism, play therapy was the most frequently used method(65%). Other commonly used treatment methods were pharmacotherapy(21%), behavioral therapy(8%), combined therapy(play therapy+pharmacotherapy+family therapy+behavioral therapy)(12%), 7) Regarding the outcome of treatment 8.6% was evaluated as Excellent, 30.4% as Good, 52% as Fair, 8.7% as Poor at the tinic of treatment. At follow up interview 21.7% was evaluated Excellent, 13% as Good, 21.7% as Fair, 34.8% as Poor. 8) We classified all subjects by Havden's 4 subtype. Symbiotic mutism was most common(65%) and other subtypes are Speech phobic mutism(8.6), Reactive mutism(13%) and Passive-aggressive mutism(30%).

Objectives : Child and adolescent onset obsessive compulsive disorder(C-OCD) is known to be associated with poor drug response, high comorbid rate and strong genetic tendencies. Till now studies for C-OCD have been very rare in Korea. We conducted this study to investigate the informations about clinical features, familial psychiatric loading, treatment profiles and course of server C-OCD from the retrospective analysis of inpatient data of Seoul National University Children's Hospital. Method : Retrospective chart review and data analysis was performed. Twenty(male 16 : female 4) patients with final C-OCD diagnosis by DSM-IV at discharge from 1994 to 2002 were found and their medical chart, psychological data, family interview data and nursing reports were collected and analyzed. Results : 1) The sex ratio of C-OCD was male dominant(4:1). 2) Phenomenological, most common obsession was pathologic doubt, contamination fear, followed by aggressive obsession, need for symmetry, sexual obsession, most common compulsion was checking and washing, followed by breathing, movement, symmetry, repetitive asking, hoarding, mental compulsion. 3) Most common comorbid diagnosis was depression. Other axis-I diagnosis associated OCD were anxiety disorder, tic disorder, conduct and oppositional defiant disorder and psychosis. 4) Regarding psychiatric familial loadings, 17 patients(85%) had relatives with psychiatric disorders, OC-spectrum disorders(OCD or OCPD) were found in 9 patients(45%). 5) The majority of patients(75%) have received SSRI and antipsychotics treatment. The response rate above 'moderate improved" by CGI was 75%. 6) During follow-up period in outpatient clinic, five patients(25%) showed continuous complete remission, 10 patients (50%) did residual symptoms with chronic course. Conclusion : This seems to be the first systemic investigation of severe pediatric OCD patients in Korea. The children & adolescents with severe OCD in inpatient-setting showed the high comorbid rate, familial psychiatric loading, and combined pharmacotherapy with antipsychotics, As for symptoms, high rate of aggressive-sexual obsession and atypical compulsions like breathing and moving was reported in this study. Severe pediatric OCD patients, however, responded well to the combined SSRI and antipsychotics regimen.
