Cost-Benefit Analysis of Maternal and Child Health Program - Focusing on Lao People's Democratic Republic - (모자보건 증진사업에 대한 비용편익 분석 - 라오스 국가를 중심으로 -)

  • Objectives: The purpose of this analysis was to assess the maternal and child health program in Lao PDR, which was implemented to reduce maternal and child mortality in Laos. Target areas for the project included 2 provinces (Xiengkhuang, Houaphan) with all 18 districts within them. The beneficiaries of this project included 121,000 childbearing women and 62,000 children under 5 years old (LSIS, 2012). Methods: In this study, it was done for efficiency evaluation of the assistance projects of the health sector of maternal and child health promotion program in Laos that took place from 2010 to 2013. We conducted a cost-benefit analysis for the evaluation of the effectiveness of that program was being carried out effectively. Results: This evaluation adopted the Cost-Benefit analysis approach. Key findings of the Cost-Benefit analysis are the following: The Benefit-Cost Ratio of 1.012, Net Present Value of 84,250,000 Korean Won, an estimated Internal Rate of Return of 10.12%. These findings suggest that project activities were efficiently implemented. Conclusion: As a result, maternal and child health project in Laos has been analyzed that there was economic efficiency. Therefore, It is considered necessary and continued support expansion of program in the future. The direction of maternal and child health project in Laos, approach and community policy support must be included. Integrated approach between the program approach and overall health should be performed for healthy life habit.

Moderating Effects of Marital Fondness & Admiration and Father's Child Rearing Involvement on the Association between Parenting Stress and Marital Satisfaction: Comparison of Unemployed and Employed Mothers (어머니의 취업유무에 따른 양육스트레스와 결혼만족도의 관계: 부부간 호감과 존중과 아버지 양육참여의 조절효과)

  • This study investigated the moderating effects of marital fondness and admiration as well as the father's involvement in child rearing on the association between parenting stress and marital satisfaction for a comparison of unemployed and employed mothers. The subjects were 159 unemployed mothers and 103 employed mothers with children under 3 years of age. The major findings of this study were as follows. First, unemployed mothers who had a lower level of marital fondness and admiration were dissatisfied with their marriage when they had higher parenting stress. Marital fondness and admiration are necessary for unemployed mothers in their transition period to parents. Especially, it is important for fathers to understand challenging conditions and encourage and support the spouse. However, there was no moderating effect of the father's involvement in child rearing in the association between parenting stress and marital satisfaction. Second, employed mothers who had a lower level of father's involvement in child rearing were dissatisfied with their marriage when they had higher parenting stress. The father's involvement in child rearing is necessary for employed mothers in their transition period to parents. Furthermore, it is important to implement policies at the national level to support childcare hours for balanced work and life. However, there was no moderating effect of marital fondness and admiration for the association between parenting stress and marital satisfaction.

Cross Validation of Attention-Deficit/Hyperactivity Disorder-After School Checklist

  • Objectives: This study aimed to evaluate the efficacy of the attention-deficit/hyperactivity disorder (ADHD)-After School Checklist (ASK) by comparing the results of the Comprehensive Attention Test (CAT) and Clinical Global Impression-Severity (CGI-S) Scale and then by calculating the area under the receiver operating characteristic (ROC) curve. Methods: We performed correlation analyses on the ASK and CAT results and then the ASK and CGI-S results. We created a ROC curve and evaluated performance on the ASK as a diagnostic tool. We then analyzed the test results of 1348 subjects (male 56.8%), including 1201 subjects in the general population and 147 ADHD subjects, aged 6-15 years, from kindergarten to middle school in Seoul and Gyeonggi province, South Korea. Results: According to the correlation analyses, ASK scores and the Attention Quotient (AQ) of CAT scores showed a significant correlation of -0.20--0.29 (p<0.05). The t-test between ADHD scores and CGI-S also showed a significant correlation (t=-2.55, p<0.05). The area under the ROC curve was calculated as 0.81, indicating good efficacy of the ASK, and the cut-off score was calculated as 15.5. Conclusion: The ASK can be used as a valid tool not only to evaluate functional impairment of ADHD children and adolescents but also to screen ADHD.

Trends in Korean Pediatric Poisoning Patients: Retrospective Analysis of National Emergency Department Information System (한국 소아청소년 중독 환자의 경향: 국가응급진료정보망을 이용한 후향적 연구)

  • Purpose: This study reports the clinical features of infant, child, school aged and adolescent patients treated for acute poisoning in nationwide emergency departments (EDs). Methods: We retrospectively analyzed clinical data pertaining to patients under 19 years of age who were treated for acute poisoning in nationwide EDs from 2013 to 2015. The data were collected by the National Emergency Department Information System (NEDIS). All patients were divided into three groups: 'Infant and child group' (0 to 5 years), 'school age group' (6 to 12 years) and 'adolescent group' (13 to 18 years). General characteristics, Korea Standard Classification of Disease $7^{th}$ (KCD-7) codes and results of care were collected. Results: There were 14,500 pediatric poisoning cases during the study period. The distribution of patient age was bimodal with two peaks among infant, child and adolescent group. The proportion of alert mentality at the ED visit of the infant and child group was 99.3%, while that of the adolescent group was 86.4%. The proportion of intentional intoxication was higher in the adolescent group (40.7%) than other age groups. Among children less than 13 years of age, various poisonous substances and therapeutic drugs were common. Conclusion: There were some clinical differences in acute poisoning patients between age groups. It is necessary to establish a preventive plan considering characteristics by age. Since the KCD-7 code has limitations in analyzing the characteristics of poisoning patients, it is necessary to consider the registration system of poisoning patients.

Long-term safety of PEG 4000 in children with chronic functional constipation: A biochemical perspective

  • Purpose: To evaluate the long-term safety of polyethylene glycol (PEG) 4000 in children with constipation, particularly the biochemical aspects of safety. Methods: Medical records were evaluated, and 100 children, who had been taking PEG 4000 for more than 6 months, and who had been under clinical and biochemical monitoring, were enrolled. Ages; $6.11{\pm}3.12$ years, Duration of therapy; $16.93{\pm}7.02$ months, dose of PEG 4000; $0.72{\pm}0.21g/kg/d$. Results: None of the children complained of clinical adverse effect. The first biochemical test was performed at 8.05 months after beginning of PEG 4000. Serum phosphate (SP) value was high in 10 children, and leucopenia was noted in one child. The second test was performed in 44 children at 7.57 months after the first test. The SP value was high in four children, including the three children whose initial SP value was high and one new child. Six out of 10 children with high initial SP value became normal and one was lost. Hypernatremia was noted in one child. The third test was done in 15 children at 7.5 months after the second test. The SP value of the new child from the second test was high, but became normal after finishing treatment. Two out of 3 children with high SP value at the second test became normal and one was lost. The fourth test was done in 2 children few months after the third test. All of the results were normal. There were no relation between duration of therapy and hyperphosphatemia, or between dose of PEG 4000 and hyperphosphatemia. Conclusions: PEG 4000 is safe for long-term therapy in children with constipation with respect to biochemical parameters.

Prevalence and Treatment Patterns of Sleep Disorders in the Under 20 Population: analysis using a national health insurance claims database

  • Objectives: Child and adolescent sleep is an important factor for brain and physical development. Therefore, it is necessary to investigate the prevalence of sleep disorders and nonorganic sleep disorders in children and adolescents and determine the type of utilization of medical institutions. This study analyzed the prevalence and type of medical institutions in Korean children and adolescents with sleep disorders and nonorganic sleep disorders. Methods: This study used data recorded in the Health Insurance Review and Assessment-National Patient Sample (HIRA-NPS) database from 2010 to 2017. Details of medical institution type and patient's sex, age, and treatment type were extracted for patients younger than 20 years with sleep disorders and nonorganic sleep disorders. Results: Among 2,536,478 patients under age 20, we identified 3,772 patients with sleep disorders or nonorganic sleep disorders. From 2010 to 2017, the prevalence of sleep disorders in children and adolescents was 0.07% to 0.09%. The utilization rate of Korean medical institutions was 30.47%. The prevalence of nonorganic sleep disorders and the utilization rate of Korean medical institutions were 0.06% to 0.08% and 45.99%, respectively. Conclusion: The prevalence of sleep disorders and nonorganic sleep disorders in the under-20 population was 0.14% to 0.16%. More than 70% of patients with nonorganic sleep disorder who were younger than 9 years used Korean medical institutions.

Efficacy and tolerability of adjunctive perampanel treatment in children under 12 years of age with refractory epilepsy

  • Purpose: There is limited data on the use of perampanel in children under 12 years of age. We evaluated the efficacy and tolerability of adjunctive perampanel treatment in children under 12 years of age with refractory epilepsy. Methods: This retrospective observational study was performed in Kyungpook National University Hospital from July 2016 to March 2018. A responder was defined as a patient with ${\geq}50%$ reduction in monthly seizure frequency compared with the baseline. Adverse events and discontinuation data were obtained to evaluate tolerability. Results: Twenty-two patients (8 males, 14 females) aged 3.1-11.4 years (mean, $8.0{\pm}2.5years$) were included in this study. After an average of 9.2 months (range, 0.5-19 months) of follow-up, 15 patients (68%) showed a reduction in seizure frequency, including 5 patients (23%) with seizure freedom. The age at epilepsy onset was significantly lower (P=0.048), and the duration of epilepsy was significantly longer (P=0.019) in responders than in nonresponders. Nine patients (41%) experienced adverse events, including somnolence (23%), respiratory depression (9%), violence (4.5%), and seizure aggravation (4.5%). The most serious adverse event was respiratory depression, which required mechanical ventilation in 2 patients (9%). Eight patients (36%) discontinued perampanel due to lack of efficacy or adverse events. Three out of 4 patients (75%) who discontinued perampanel due to adverse events had an underlying medical condition. Conclusion: Perampanel offers a treatment option for refractory epilepsy in children. Adjunctive treatment with perampanel requires special consideration in those with underlying medical conditions to prevent serious adverse events.

The Factors related to Mothers' Intention to Vaccinate against Hepatitis A: Applying the Theory of Planned Behavior

  • Purpose: This study was conducted using the theory of planned behavior to analyze factors influencing mothers' intention to vaccinate their children against hepatitis A (HA). Methods: This descriptive study used a questionnaire. The participants were 100 mothers with children under 19 years. The collected data were analyzed by the t-test, analysis of variance, and the Mann-Whitney U test. The Pearson's correlation was used to test the correlations among variables. Stepwise multiple regression was used to identify factors related to HA vaccination intention. Results: The attitude of mothers with children under 19 to HA vaccination were quite positive (6.2 of 7), and their perceived behavioral control (5.73 of 7), subjective norm (5.54 of 7), and vaccination intention (5.96 of 7) were relatively high. Attitude toward HA vaccination was the strongest influencing factor, followed by subjective norm. The explanatory power of attitude and subjective norm for HA vaccination was 84%. Conclusion: HA vaccination is very important for preventing HA. Mothers' positive attitudes toward HA vaccination were an important factor influencing their children's HA vaccination. Providing education on the benefits of HA vaccination and the vaccination schedule may foster positive attitude toward vaccination.

The Meaning of Teachers as they Manifest themselves in the Emotional Regulation of 2 Year Old Infants (2세 영아의 정서조절 측면에서 나타나는 교사의 의미)

  • The purpose of this research was to investigate meaning of teachers as they manifest themselves in the emotional regulation of 2year old infants in a daycare center. In addition, the research attempts to provide basic research data that can be used as a guideline for teacher's awareness, roles, attitudes, and classroom management for infant's emotional education in the future. To achieve this goal, participatory observation was conducted in a child care center class for infants under 2 years old from January 17 to January 29, 2012. The teacher is defined as follow : Teachers are authority figures whose image is that of absolute authority, and coupled with their dual role of passive caretakers. Additionally, they function as guides who guided infants through the process of emotional socialization, and played the central role of emotional contagion from whose expression speech and atmosphere the said infants receive much influence. Such results seemed to indicate that teachers today do not fully comprehend the importance of their roles in influencing the emotion regulation of infants.

A Survey of Smartphone-Based Health Education Needs for Parents (아동 부모의 스마트폰 기반 건강교육에 대한 요구도 조사)

  • Purpose: The purpose of this study was to investigate the use of smartphones to address parents' educational needs. Methods: The participants were parents of children under 18 years old, Participants were 347 parents (222 mothers, 125 fathers). Data were collected from August, 15 to September, 2, 2016 using self-report questionnaire. Data were analyzed using descriptive statistics, t-test and ANOVA with the SPSS WIN 21.0 program. Results: Parents had an average score of 3.30 (${\pm}.41$) for the need to have smartphone health education. Among the educational subcategories, 'child and family' showed the highest average for educational needs (3.40, ${\pm}57$). According to the demographic characteristics of the participants, there were significant difference in parents' age (t=4.354, p=.014), and parents' who have received health education versus parents who have not (t=2.483, p=.014). Conclusion: Findings indicate that the smartphone may be an effective educational method for parents.
