19세 미만 인구의 정신장애 진단 분포: 국민건강보험 자료를 중심으로 (The Diagnostic Distribution of Psychiatric Disorders among the Population Under 19 Years Old: Based on the National Insurance Data)

  • Objectives: As a preliminary study on the prevalence of mental disorders in Korean children and adolescents, we investigated the diagnosis distribution of mental disorders in people under 19 years of age using the health insurance data. Methods: From the 2011 health insurance data of 1,375,842 people, the data of 286,465 people under 19 years old were extracted. Among them, we selected 10,778 with psychiatric diagnoses; male 6,453 (59.9%) and female 4,325 (40.1%). The frequency of psychiatric diagnoses in the males was 10,140 (62.7%) cases, which was significantly higher than that (6,031; 37.3%) in the females. We categorized the groups into 3 year intervals. Results: In the males under 3 years old, F80 was the most prevalent diagnosis, followed by F98. F90 and F41 were the most prevalent in the 4-15 and above 16 years old groups, respectively. In the females, F98 was the most prevalent in the children up to 3 years old, while F80 was the second most prevalent. In the 4-6 years of age group, F98 was the most prevalent, followed by F93. F90 and F41 were predominant in the 7-12 and above 12 years old groups, respectively. Conclusion: In the analysis of the frequency of diagnosis of mental disorders in the population under 19 years old, there were significant differences in the distribution based on age and gender.

어린이집 재원 다문화 가정 영아의 양육 현황과 어려움 및 지원요구 (Present Status, Difficulties and Support Needs of Multi-cultural Families Raising Children under 3 Years Old Enrolled at Child Care and Education Centers)

  • Objective: This study intended to analyze parenting characteristics, difficulties and support needs of multi-cultural families that had children under 3 years old, in order to develop their supporting resources. Methods: This study performed semi-structured interviews with 7 mothers from multi-cultural families that had their young children enrolled at child care and education centers in Gyeonggi Do. The interview data transcribed were analyzed using the successive contents analysis, and led to 4 themes and 12 sub-themes. Results: First, the mothers from multi-cultural families were married without understanding the Korean language and culture. Second, the multi-cultural mothers didn't properly respond to their children's needs in interaction and language aspects. As a result, the multi-cultural support centers and child care and education centers gave help to them. Third, in the view of the difficulties facing mothers, parenting problems caused by the lack of information concerning child education, the biased burden of parenting, and the different parenting values of Korean families were all expressed by the mothers. Fourth, regarding support needs, providing individual professional support for children and child-raising support for mothers, customized language and employment support for mothers and parenting education for fathers and family education on multi-culturalism are needed. Conclusion/Implications: Suggestions for decreasing the difficulties facing multi-cultural families that had children under 3 years old were discussed.

Estimation of Under-5 Child Mortality Rates in 52 Low-migration Countries

  • Reduction of the under-5 mortality rate is a target of the Sustainable Development Goals. Therefore, this study aimed to estimate under-5 child mortality rates in 52 low-migration countries using population data. The study utilized population data from the US Census Bureau from 1990 to 2015. The method involved first estimating mortality rates for countries with negligible net migration and then applying these rates to countries with matching mortality profiles, where it is reasonable to assume that migration is negligible for children under the age of 5 years. The highest child mortality was concentrated in the African region, followed by Asia and the Western region. However, steady progress in child mortality trends was concentrated in low-income countries. This simple method demonstrated that child mortality has significantly improved in high-income countries, followed by middle- and low-income countries. To reduce the under-5 mortality rates even further in these 52 countries, there is a need to accelerate equitable plans and policies related to child health to promote children's longevity and survival.

이혼 가족 아동 (Children of Divorced Families)

  • There has been a significant increase in the number of divorced families during the past 10 years in Korea. Divorce imposes a great impact on their children as well as divorcees. As many as 1.41 million children under 20 years-old have experienced their parents' divorce in past 10 years. Children are faced with much difficulty in adjustment after the parental divorce. Issues of the research and policy on the divorced family are discussed in this study. For future studies, a longitudinal research model, father-custody and grandparent-custody families, custody parents's gender, a theoretical model for Korean divorced families should be considered. New legislations have been recently enacted to enhance children's well-being, but further efforts such as the involvement of child development specialists in the divorce process should be followed in the pursuit of the best interest of the children.

Prevalence of child malnutrition in agro-pastoral households in Afar Regional State of Ethiopia

  • Based on data generated from 180 randomly selected households with children age under five years old in Aysaita district of Afar region of Ethiopia, this study explored prevalence of malnutrition and scrutinized household characteristics, maternal characteristics, specifics of the child and economic variables associated with child malnutrition. The height-for-age Z-scores (HAZ), weight-for-height Z-scores (WHZ) and weight-for-age Z-scores (WAZ) were used to measure the extent of stunting, wasting and underweight, respectively. The results revealed that prevalence of long term nutritional imbalance and malnutrition status indicator (i.e. stunting) was 67.8%. The short term measure (wasting) was found to be 12.8% and underweight was found to be 46.1%. Moreover, children in households which are headed by women, and characterized by more dependency ratio, less access to assets, health services and institutions are more likely to be undernourished.

  • Objectives : The purpose of this study was to investigate changes in child and adolescent outpatients at a university hospital in Daegu from 2004 to 2013. Methods : The subjects were first-visit patients under 18 years old, who visited Dongsan Medical Center, Keimyung University, from January 1, 2004 to December 31, 2013. Computerized medical records of 2,738 patients (male 1,906, female 832) were reviewed. Results : The ratio of male to female was 2.3 : 1 in the period of study. The most prevalent age category was 7-9 years. The mean age was $10.12{\pm}4.68$ years ($9.84{\pm}4.59$ years in males, $10.76{\pm}4.81$ years in females). The ratio of child and adolescent patients to total outpatients was 27.0%. The most common diagnostic category was the attention-deficit hyperactivity disorder (ADHD) group, followed by the mental retardation (MR) group. In males, the ADHD, MR, communication disorder groups were more prevalent, but in females, the MR, ADHD, depressive disorder groups were more prevalent. Conclusion : The female to male ratio and total mean age were on the rise. The ADHD group was the most prevalent and the depressive disorder group was also on the rise in this period.

Korean Divorced Mothers' Experiences with Child Support from Noncustodial Fathers

  • The purpose of this study was to explore Korean divorced mothers' experiences with child support. The data were collected from 17 Korean divorced mothers who were divorced between the years of 2004 and 2009 and were raising at least one minor child. Data were analyzed based on the inductive data analysis method. Most mothers negotiated to receive child support from noncustodial fathers without experiencing much conflict with the fathers at the time of the divorce, but only five mothers received child support at the time of the interviews. The mothers experienced difficulties in receiving child support from the fathers under the child support policy in Korea that treated child support as a personal issue and provided little systemic support for child support. Regarding required parenting agreements, most of the mothers acknowledged the need to have parenting agreements but they still wanted voluntary payment of the child support from the fathers. The results suggest that it is necessary to expand parenting education for divorced parents to encourage voluntary participation in parenting and to improve the child support policy by introducing child support guidelines and enhancing child support collection systems.

Trends and Characteristics of Mortality Associated with Congenital Anomalies in Korean Children under 5 Years of Age

  • Purpose: Studies have been conducted on the prevalence and infant mortality rate of congenital anomalies; however, studies on child mortality are rare. Therefore, we evaluated the characteristics of deaths associated with congenital anomalies among children born in Korea who died within 5 years of age. Methods: Birth-to-death cohort linked data of children under the age of 5 years from 2010 to 2013, and statistical data on the cause of death by age from 1999 to 2019, both provided by the Korea National Statistical Office's Microdata Integrated Service, were retrospectively investigated. We investigated the trends and characteristics of mortality associated with congenital anomalies. Results: Among 1,858,945 children, 6,510 children who died were under 5 years of age, and among them, 1,229 deaths were associated with congenital anomalies, while 5,281 deaths were due to other causes. Deaths associated with congenital anomalies accounted for 18.9% of all deaths. When comparing congenital anomalies by systems, anomalies of the cardiovascular system (52.6%) were the most common. The mortality rate associated with congenital anomalies and those of other causes showed similar declining trends in 21 years. Conclusion: The mortality rate of congenital anomalies during the first 5 years of life did not increase differently from the prevalence of congenital anomalies but rather decreased. Deaths associated with congenital anomalies accounted for 20.5% of all infant deaths and 12.1% of child deaths, since the major causes of death in infants and children are slightly different, continuous and careful monitoring is required.

양육수당이 자녀 돌봄유형 선택에 미치는 영향 : 이상적인 돌봄유형과 현재 돌봄유형의 일치여부를 중심으로 (The Influence of Home Child Care Allowance on the Choice of Child Care Types : Focusing on the Concordance between an Ideal and Actual Child Care Type)

  • This study aims to examine how home child care allowance influences a mother's choice of child care type. To accomplish this goal, we surveyed 432 mothers who had at least one child under 5 years old. The data were analyzed by frequencies, percentages, means and binomial logistic regressions. The results showed that unemployed mothers and mothers with younger children had a high tendency to choose home child care allowance instead of child care subsidy. Second, mother's employment status, age of first child and an interactive term of mother's employment status and home child care receipt influenced a concordance between an ideal and actual child care type. Unemployed mothers and mothers with younger children were more likely to experience a concordance between their ideal and actual types of child care. By investigating how home child care allowance affects the right of choice in child care type, this study provides empirical information to policy makers and researchers and contributes to develop cash-benefit policies for families with young children.

고 위험 집단의 아동학대에 관한 연구 - 생활보호 대상자를 대상으로 - (Chlid Abuse in High-risk Group)

  • The purpose of this study was to provide basic data on preventive child abuse program development. Method: Data were collected on 105 high-risk families of child abuse intervention was obtained from their main child rearer who raised the child under 18 years, olds during 10 months period from May, 2000 to March 2001. Result: The results were as follows : 1. Child abuse occurred in the subject's home, in the case of mild child abuse, 'throwing an object at the child' had the highest percentage 39.1%, in severe child abuse, 'rod, stick, belt, broom beating or using a variety of objects such as' had the highest percentage 49.5%, and in very severe child abuse, 'hospitalized by belting' had the highest percentage 3.8% 2. The degree of child abuse potential showed high risk child abuse score with a mean of 213.3. The degree of beliefs in corporal punishment showed that subjects perceived corporal punishment of children positively with a mean of 32.2,. 3. With respect to the child abuse potential, there were significant correlations with the subjects' age (r=.294, p=.002), education level (r=-.442, p=.000), and family income (r=-.355, p=.000). Conclusion: From this study not only child abuse occurrence but also child abuse potential were severely increased in poor livelihood families. Therefore to the high risk group, individual preventive approach must be applied.
