Efficacy and Safety of Valganciclovir in Congenital Cytomegalovirus Infection with Isolated Intrahepatic Cholestasis: A Randomized Controlled Trial

  • Purpose: Cytomegalovirus (CMV) infection affects the hepatic, neurologic, hematopoietic, respiratory, gastrointestinal, and other organs, resulting in a high mortality rate and longterm sequelae. It may cause acute or chronic hepatitis, or even lead to hepatic cirrhosis. Valganciclovir (VGCV) is an effective, safe, and well-tolerated treatment for congenital CMV infection, without any serious adverse effects. This study was conducted to evaluate the clinical, biochemical, and virological profiles of infants with CMV with intrahepatic cholestasis and to determine the outcomes with or without treatment with VGCV. Methods: Twenty infants aged <6 months diagnosed with congenital CMV infection with evidence of intrahepatic cholestasis were included in this study. Randomization was used to divide the study participants into 2 groups. The control group (n=10) was treated with only supportive management, and the intervention group (n=10) was treated with oral VGCV at 16 mg/kg/dose 12 hours a day for 6 weeks plus supportive treatments. Physical examinations and biochemical, serological, and virological tests were performed at the time of diagnosis and at the end of 6 weeks and 6 months. Results: The control and intervention groups were compared in terms of clinical and laboratory parameters such as jaundice, dark urine, pale stool, hepatomegaly, total bilirubin, aminotransferases, gamma-glutamyl transferase, alkaline phosphatase, and CMV polymerase chain reaction load, which showed a significant reduction after treatment in the intervention group (p<0.05) with oral VGCV, with very few side effects, whereas the control group showed no significant changes. Conclusion: Oral VGCV can be used to effectively treat CMV infection with intrahepatic cholestasis without notable side effects.

  • This study was carried out to find contradiction factors on laws for children's activity spaces and facilities and to suggest the rational options to control and manage those spaces and facilities by environmental and landscape planning methods. The results of this study are as follows: 1. The major laws related to the environmental safety for children's activity spaces are "Environmental Health Act (ERA)" for managing the environmental safety of children's activity spaces; "Safety Supervision Law of Children's Play Facilities(SSLCPF)" for the inspection and management for safety of children's play facilities; "Quality Management and Industrial Products Safety Management Law(QMIPSML)" for managing safety certification on children's play equipments. 2. The interior space such as "living room" by the Children's Welfare Law(CWL), "Children Park" by the Act on Urban Parks and Green Spaces(AUPGS), "classroom" on private educational institutes by the Act on Establishment and Operation Private Lesson and Training(AEOPLT) and "nursing room" of child care center smaller than $430m^2$ are needed to be managed as an activity space. 3. In order to reduce industrial burden in the production, establishment, construction, and operation and to minimize unwilling extra burden in the administration effort due to legally double regulate, it is necessary to mitigate the inspections on the equipment certificate from QMIPSML and overlapped or different factors and standards must be unified. With this study, the landscape domain could he enlarged from producing, import of play equipment and establishment, construction and operation of play facilities for a comprehensive range of activity spaces, and the landscape industry such as engineering industry, academic research, management, etc.

  • The purpose of this study is identify factors influenced of children's library use and management through investigating the use patterns and recognition of children's parents on children's libraries. Questionnaire was used as a research method. Data was collected from the parents who entered library entrance with their child and children on four public libraries. Six factors were identified from four public libraries setting. There were the spatial planning based upon library use patterns, the flexibility and mobile furnishings, the activity area for causing children's Interesting, the guarantee of safety Place for children, the quiet area for emotional stability, and functional place for moving around.

Perceptions of the Retailers within Green Food Zone on the enforcement of the Special Act on the Safety Management of Children's Dietary Life (어린이 식생활 안전관리 특별법 시행에 대한 학교주변 판매업자 인식조사)

  • This study is a survey of regulatees' perception on main contents and enforcement effect of the Act, the retailers at food stores within Green Food Zone, for the effective enforcement of "The Special Act on the Safety Management of Children's Dietary Life" legislated in 2008. Percentage, frequency analysis, and T-test are derived from the survey carried out to the 175 retailers at food stores within Green Food Zone, across the country except Jejudo, for the two months March and April of 2011. The survey results are as follows. Over 80% of the respondents are aware of comprehensive policy for child food safety and the enforcement of 'the Special Act' and considering the rate of satisfaction on food safety information provided by government, over 90% of the respondents, virtually most of them, are satisfied. The rate of awareness of Green Food Zone is about 80% and that of outstanding business for children is about 50%, showing little perception of it. The comparison of the survey result of 2011 with that of 2010, which have the same questions to the retailers within Green Food Zone, still indicates a need for enhancement of understanding on absolute standard, though it showed fairly better improvement in general. Several proposals are given in this study based on the survey results, which will contribute to the children food safety and health improvement in the end.

Clinical characteristics of adverse reaction to radiocontrast media in children - A single center experience (소아에서 방사선 조영제 유해반응의 임상적 특징)

  • Purpose: Radiocontrast media are widely used in medical imaging to improve diagnostic accuracy. However, studies on the adverse reactions of radiocontrast media in children are limited. We aimed to describe the characteristics of adverse reactions to radiocontrast media among children who had a computed tomography scan or magnetic resonance imaging in a tertiary university hospital. Methods: We retrospectively collected data on adverse reactions to radiocontrast media by the reporting system of a tertiary university hospital. We selected data from children under the age of 19 from July 2011 to December 2017 and analyzed their characteristics. We focused mainly on the characteristics of the index case which is defined by the first adverse event of each subject. Results: During the period, a total of 88,050 radiocontrast media-enhanced imaging studies were performed and 184 cases of adverse reactions were reported. A total of 71 were identified as index cases. Forty-nine (69.0%) were male and the mean age was $12.7{\pm}3.2$ years. The incidence of radiocontrast media-related adverse reactions was 0.09% and severe reactions were 0.002%. The most common clinical feature was skin manifestations (54.9%), followed by gastrointestinal symptoms (40.8%) and neuropsychiatric symptoms (7.4%). Conclusion: Adverse reactions to radiocontrast media rarely occur in children and the incidence of severe reactions is low. Most reactions are mild and are related to the skin and gastrointestinal system. This report would provide good evidence for establishing a management strategy in children scheduled for imaging studies using radiocontrast media.

Assessment for Exposure Levels of Hazardous Substances in Kids Cafes within Some Metropolitan Area (일부 수도권지역에서의 키즈카페 내 유해물질 노출수준 평가)

  • Objectives: In this study, we investigated an assessment for exposure levels of hazardous substances to kids cafes that unregulated by law. Methods: Heavy metals, volatile organic compounds, formaldehyde, pesticides and phthalate were measured at 20 kids cafes in Gyeonggi-do and Incheon. Samplings were conducted from April to July in 2018. Results: Heavy metals were detected over the standard mainly around the floor and walls in the 19 kids cafes. Pesticides were detected in 7 locations with chlorpyrifos, diazinon and cypermethrin, and showed the highest detection level of chlorpyrifos. The concentrations of DEHP in all kids cafes were exceeded with standard. The risk assessment results showed that HCHO as carcinogen had a safety level and DEHP as non-carcinogens had a safety level as assessed to be under than 0.1. Conclusions: Considering the exposure investigation results and the importance of child health protection, kids cafe should be included in the legal management list of children's activities zones.

Descriptive Literature Review on Health Education for Child and Adolescence in Korean School Setting (국내 학교기반 보건교육 연구에 대한 설명적 문헌고찰(1990-2008))

  • Objectives: This literature review was conducted to understand overall research trend and change in school health education during the past 20 years and to identify major outcomes and future directions of research. Methods: Descriptive literature review was conducted for school health education in Korea. A total of 117 peer-reviewed journal articles and thesis published between 1900 and 2008 (for 19 years) conducted in school were finally included in this review process. The research setting should be elementary, middle, or high schools and the major topic of the research should health education for students. Results: Research in elementary school was 38.0% which was greater than research in middle or high school setting. Surveyor intervention research was slightly increased in 2000s. General health and other review were the most frequently employed topics in 1990s' research and general health, smoking, and safety were in 2000's research, which implied that recently research specified its topics more than 1990s' research. Recently research was developed in intervention model comparing with 1990s' research. Research using experimental or quasi -experimental design seems having less intervention effects than the research using nonexperimental design. Conclusions: It is necessary that health education research in school focuses more in students' interest, participation, and factors related to intervention effects from now on.

Defining the Core Competencies of the Nurses in A Tertiary Hospital and Comparing Different Units based on Their Respective Characteristics (일 종합전문요양기관 간호사의 핵심역량 도출 및 근무지 특성별 중요도 인식 비교)

  • Purpose: The purpose of this study was to define the core components required of nurses to provide quality nursing care to patients and analyze the significance of these components in the hospital's various units to maintain a high level of competence among nurses. Method: The study evaluated 3 categories included 35 subcategories with 148 core components that were derived from literature review and interviews of nursing professionals. The nursing professional category included 18 subcategories with 98 components, the organizational culture category included 4 subcategories with 16 components, and the temperament-attitude category included 13 subcategories with 34 components. The study included 335 nurses with more than one year of hospital experience and measured disparities among different hospital units. The data was analyzed with SPSS-Win 10.0, differing perceptions of the importance of general traits among the participants were measured using standard deviation, and differing perceptions of the importance of professional traits among the participants were assessed using ANOVA and subsequently with the Bonferroni Test. The reliability of the aforementioned research tools were evaluated using the Cronbach's ${\alpha}$. Result: The results of the study were as follows: 1. Among the three categories, temperament-attitude category was perceived to be the most important, followed by nursing professional category and organizational culture category. Among the ten most important subcategories within the three categories, safety and infection prevention as well as responsibility were perceived to be the most important, followed by promotion of physiologic adaptation, document management and presentation, self-control, ethics, observance law, coping with emergency, humanity, and medication. 2. The relative importance of category associated with the core competencies within the hospital unit were as follows : 1) The units that rated nursing professional category as being the most significant were: internal medicine, surgical unit, mother-child unit, emergency room, intensive care unit, and operating room, in that order. 2) The units that rated the organizational culture competencies as being the most significant were: mother-child unit, internal medicine, surgical unit, emergency room, operating room, and intensive care unit, in that order. 3) The units that rated temperament-attitude category as being the most significant were: internal medicine, surgical unit, emergency room, mother-child unit, operating room, and intensive care unit, in that order.

Comparative Study of Institutional Approaches to Children's Playgrounds for Ensuring the Right to Play (놀이권 보장 측면에서 본 국내외 어린이놀이터 관련 법제 비교 연구)

  • The importance of play from a Children's Rights perspective has been continuously emphasized, and the need for play facilities and spaces has been reiterated. Meanwhile, in South Korea, despite the continuous increase in the number of play facilities, efforts to improve children's play spaces are required. Therefore, this study reevaluated the current regulations and policies concerning play and play spaces from the perspective of securing the right to play. An analysis of the legal concept of domestic playgrounds, related laws, construction standards, and regulations was conducted. The systems and characteristics of the related regulations in the UK, Canada, and Germany were analyzed. Based on the results of the comparative analysis, our directions for improving the system of playgrounds to ensure the children's right to play were proposed. First, a legal definition of the concept and scope of playgrounds should be established. Second, beyond safety-focused facility regulations, there is a need for institutional enhancements to ensure the right to play. Third, comprehensive management of playgrounds is required as a spatial concept rather than as individual facilities. Fourth, qualitative management is needed through specific guidelines and guidelines related to the establishment of playgrounds. These results can serve as a basis for establishing systems and promoting policy projects to ensure children's right to play in various fields, including urban planning, child welfare, and education in the future.

Comparison between Japan, China, and Korea on How Parents with a Toddler Feel about Food Safety (일본, 중국, 한국 영유아 부모의 식품 안전에 대한 인식 비교)

  • Purpose : The aim of this research was to clarify the difference in awareness of food safety and what they feel as dangerous among parents in Japan, China, and Korea and how they are different among the nations. Methods : Research subjects were 250 parents in Japan, 181 parents in China, 166 parents in Korea whose children attend a child institution or a day care center. We conducted a questionnaire survey that was self-written and anonymous. Results : Parents in Japan feel safer than Korea and China, and parents in Korea feel safer than China about food that are sold generally. Especially, in China, the average of 5 level scale was lower than the median so it shows that people do not trust food that are sold in the market. All 3 countries tend to have the awareness that "pesticide that is over the standard amount is detected from vegetables in China" and "vegetable grown without pesticides and organic produce are safe" and showed no significant difference. Also, there was a significant difference statistically for the awareness that "it makes it easier to get cancer if you eat food that contain radiation" and "there is a possibility that seafood caught near Japan contain radiation". Parents in Japan emphasize that "the price is low" the most and it was significantly higher than in China and Korea. Also, similar to parents in Japan, parents in Korea tend to emphasize that it is "domestic". On the other hand, it verified that parents in China emphasized "the meat or fish is grown without antibiotics" and "it uses vegetable grown without pesticides".
