Anterior Mediastinal Teratoma which Complicated Empyema - Report of one case - (농흉을 합병한 전종격동 기형종 -1예 보고-)

  • Many mediastinal masses seen radiologically are clinically asymptomatic. Only 55 to 65 percent of such lesions are asymptomatic, and of these many have only nonspecific symptoms by rupture, infection and pressure of surrounding organs. This report presents one case of a mediastinal teratoma which complicated an empyema. The patient was 7-year-old female and chief complaints were fever, left chest pain and general ache. In her past history, lung decortication and drainage procedure under the diagnosis of an empyema were carried out, 5 years ago, but she had not been improved, And so, explothoracotomy was done. At the time of operation, a mass of adult fist size was placed in the left anterior mediastinum and covered with the mediastinal pleura. Extirpation of the tumor and decortication were carried out and the left lung was remained.

Double Outlet Left Ventricle - One Case Report - (양대동맥 좌심실기시증치험 1례)

  • Origin of both great vessels from morphological left ventricle [DOLV] is a rare cardiac anomaly which embryologic possibility has been explained by differential conal development concept and differential canal absorption concept. Recently we had surgical experience of DOLV in 4 month-age infant weighing 5.7Kg. The chief complaints on admission were cyanosis and anoxic spell during severe crying, and right heart catheterization and right ventriculogram were performed but incorrect diagnosis was made. The operative procedures were ligation of patent ductus arteriosus, patch closure of subaortic VSD aligning aorta and pulmonary artery with left ventricle, suture closure of proximal pulmonary artery and valve and the use of extracardiac valved conduit [Carpentier-Edward l4mm] from right ventricle to distal pulmonary artery. Postoperative course was uneventful and discharged in the good condition.

  • Intracranial schwannomas preferentially arise from the vestibular branch of the eighth nerve, and rarely from the trigeminal nerve, facial nerve, and lower cranial nerves. Anterior cranial fossa schwannomas are extremely uncommon and few details about them have been reported. The patient was a 39-year-old woman whose chief complaints were anosmia and frontal headache for 2 years. The gadolinium (Gd)-enhanced magnetic resonance imaging (MRI) showed an extra-axial mass from ethmoid sinus to right frontal base region near the midline, with solid enhancement in lower portion and multicystic formation in upper portion. The tumor was totally resected via basal subfrontal approach. At operation, the tumor had cystic portion with marginal calcification and the anterior skull base was destructed by the tumor. The olfactory bulb was involved, and the tumor capsule did not contain neoplastic cells. The histopathological diagnosis was schwannoma. We report a rare case of anterior cranial fossa schwannoma with literature review.

Congenital Esophageal Stenosis due to Tracheobronchial Remnants - 3 Case Reports - (기관기관지 잔유조직에 의한 선천성 식도협착증 수술 치험 -3예 보고-)

  • Congenital esophageal stenosis due to tracheobronchial remnants is a rare anomaly, resulting in dysphagia and recurrent pneumonia, We have experienced three cases of csophageal stenosis due to ectopic tracheobronchial remnants and performed operative correction. Two patients were 20 months and five year old male with a chief complaints of swallowing difficulty from birth and the other was a twenty three year old female with a slowly increasing symptom of dysphagia for twenty years. Esophagogram of the patient with tracheobronchial remnants shows abrupt narrow segment at distal esophagus with marked proximal dilatation, and linear barium collections perpendicularly projecting from the stenotic esophagus. All of them were performed surgical correction by esophagectomy of the stenotic portion and esopahgo-gastrostomy with anti-reflux procedures, The resected specimens of these patients showed ectopic tracheobronchial chondroepithelial tissue within the esophageal wall histopathologically. Postoperative course was uneventful and have been in good condition without any problems.

A Case Report on the Treatment of Cyclosporin-A Induced Gingival Enlargement in Renal Transplant Patient (신장이식 환자에 있어서 Cyclosporin-A에 의한 치은비대의 치험례)

  • The authors treated a 58-year old female patient who had come to the Department of Oral Medicine, KNUH due to the chief complaints of gingival enlargement and bleeding on the upper and lower jaw. The lesions were diagnosed as cyclosporin-A induced gingival enlargement by patient's history and clinical examination. The patient was treated with gingivectomy using pulsed Nd:YAG laser. After gingivectomy the wound was compressed with 0.1% chlorhexidine-soaked gauze to prevent relapse of the lesion. Good healing process was observed and there were no recurrences until 3-month follow-up visit. From the results of this clinical trial it was suggested that a pulsed Nd:YAG laser gingivectomy would be helpful for the treatment of cyclosporin-A induced gingival enlargement in renal transplant patients.

Primary Rhabdomyosarcoma of the Right Atrium -A Case Report- (원발성 우심방 횡문 근육종 -1례 보고-)

  • Primary cardiac tumors are extremely rare and about 20-25% of primary cardiac tumors are malignant. Recently we have experienced a case of primary cardiac rhabdomyosarcoma originated on the right atrial wall. Patient was 41 years-old women and chief complaints were pitting edema and exertional dyspnea. Emergency operation was performed to relieve symptoms by pericardial effusion that hac been seen on the echocardiogram and large soft mass in right atrial cavity on the MRI. About 700ml amounts of bloody pericardial effusion was collected into the pericardial cavity. 4X5X5 cm sized ingrowing soft mass in the right atrial cavity and two small sized outgrowing masses were inspected. A tumor was invaded into the SA node and superior limbic portion of fossa ovalis. And then mass & right atrial free wall was resected but areas of SA node and fossa ovalis were not resected. Defect of right atrial wall was reconstruced with bovine patch. She was discharged after 2 weeks without any problems and she has been treated with chemotherapy.

  • Dental care is becoming more available on the NHI(National Health Insurance) in Korea. Especially, complete denture, partial denture, dental scaling, and dental implant has been applied by NHI from 2012 to 2014. Although, the entire nation is not eligible for the benefit now, the more dental coverage of NHI is extended, the more regulaition is tightened. Essential documents for proof of correctness of dental treatment covered by NHI are dental records and the receipt book. Summary of regal regulation about them is as follows 1. Chief complaints of patients, diagnosis, progress, and act of treatment, drugs and materials of treatment, doctor's sign, date and hour should be placed accurately on dental record 2. Dental clinic should collect patients sharing of the dental cost covered by NHI. 3. Dental clinic should keep the receipt as proof of purchase of dental drugs or materials.

  • 1. Objective: This study is about a Soyangin patient with vertigo after subarachnoid hemorrhage. In this study, we report the effects of Sasang constitutional treatment to this patient. 2. Methods: This patient was treated by Soyangin's constitutional medications and acupunture according to the result of Sasang constitutional diagnosis. 3. Results and Conclusions: This patient's chief complaints were vertigo and gait disturbance. By using Yangkyuksanhwa-tang mainly, she showed positive response about her symptom. This study shows that Yangkyuksanhwa-tang has effect to vertigo in Soyangin.

  • Objective: To describe the effects of Korean medical treatment on a 51-year-old male patient with polyneuropathy and chief complaints of lower limb paralysis and dysarthria. Methods: The Toronto Clinical Neuropathy Scoring System (TCNSS) was used to evaluate the function of the lower limb and the symptoms of the patient were observed daily. We applied herbal medication and acupuncture daily, depending on the patient’s symptoms. Results: After the treatment, the TCNSS score was improved. Conclusions: In this case, Korean medical therapy was effective in improving lower limb function.

Surgical Treatment of Coarctation of the Aorta: A Case Report (대동맥 교약증 수술 1례)

  • A 16 years old Korean boy was admitted with chief complaints of exertional dyspnea, precordial pain, and headache. His brachial and femoral arterial pressure were 180/100 mmHg and 120/95 mmHg respectively. Aortogram by Seldinger technique revealed postductal adult type coarctation and marked dilation of the poststenotic aortic portion. With arfonad 40mg infusion during clamping, resection of the coarctation segment, and end-to-end anastomosis of the aorta maximal brachial arterial pressure was maintained below 180 mmHg. Postopertive course was uneventful and the patient was discharged two weeks after operation. Precordial pain and headache diasappeared. Brachial and fernoral arterial pressure were 140/80 mmHg and 110/90 mmHg at the time of discharge.
