A case of Taeumin diagnosed to pleural effusion (흉막삼출증(Pleural effusion)을 진단받은 태음인(太陰人) 환자의 치험(治驗) 1례(例))

  • We managed a Taeumin(太陰人) patient with pleural effusion due to heart failure. We diagnosed as Wiwan-suhan-pyohan-byung(胃完受寒表寒病), and treated with Jowiseungchungtang(調胃升淸湯), going with western medicinal management. His chief complaint was orthopnea, chest discomfort, cough. The state of patient was measured with patient's complaints and chest X-ray. We gained outstanding improvement on patient's chief complaints, but there was no particular change of chest x-ray until thoracentesis. In this case, it seems that cooperations of east-west medicine would be beneficial to treatment of patients.

A Report on the Collecting Status of the Prospective Clinical Data Included the Constitutional Prescription (전향적 체질치료 임상증례 수집현황에 대한 보고)

  • Objectives The purpose of this study is to report on the collecting status of the prospective clinical data, that is designed to find clinical factors for determining the constitutional prescription.Methods We collected the prospective clinical data of 1208 subjects in the 10 oriental hospitals and 2 oriental clinic, from September 2012 to July 2015. Subjects were diagnosed and prescribed by the Sasang Constitution specialist. We counted the usage frequency of each constitutional herbal formula. Besides we check the main chief complaints on each constitutional herbal formula.Results & Conclusions In TE type, Yeoldahanso-tang, Cheongsimnyeonja-tang were used in high frequency. In SE type, Palmulgunja-tang, Gwakhyangjunggi-san were used in high frequency. In SY type, Hyeongbangjihwang-tang, Yanggyeoksanhwa-tang were used in high frequency. The both of high frequency-prescriptions in TE types were for the Interiorheat disease, expecially liver-heat lung-dry symptomatology. The main chief complaint of high frequency-prescriptions was fatigue in every constitution. We hope that the process of constitutional prescription would be standardized through the subsequent researches.

  • Objectives : The purpose of this clinical case is to evaluate the efficacy of Korean Medicine treatment on Syringomyelia with Herniated Intervertebral Disc(HIVD) of C-SPINE. Methods : We used Acupunture, herbal acupuncture, herbal medicine for this patient. This patient received hospital treatment for 15 days. Results : Patient's chief complain were neck pain and trapezius pain. NRS of chief complaint decreased from 6 to 2 and Patient's NDI decreased from 46 to 14. Conclusions : We experienced improvement of symptoms in this case by acupuncture stimulation, herbal medicine. and we hope this study is helpful for the treatment of Syringomyelia with HIVD of C-SPINE.

Four Cases Report of the Female Lower Urinary Tract Infection with Oryeong-san (오령산(五苓散)으로 호전된 여성 하부 요로감염증 환자 치험 4례)

  • Purpose: This paper aims to report the effects of Oryeong-san on lower urinary tract infection. Methods: Four women were admitted to Dunsan Oriental Hospital with chief complaint that was not supposed to be a symptom of urinary tract infection. During the admission, urine test showed pyuria with white blood cell and their chief complain also accompanied. We prescribed Oryeong-san three times a day, and then took follow-up 4 days later. Results: After treatment, symptoms of lower urinary tract infection were decreased and the results of urine test improved also. Conclusions: These cases indicate that Oryeong-san is effective in treatment of the lower urinary tract infection.

CLINICAL AND RADIOGRAPHIC STUDY OF BENIGN ODONTOGENIC TUMORS IN THE JAWS (악골에 발생한 양성 치성종양의 임상 및 방사선학적 연구)

  • The author observed and analyzed the age, sex, chief complaint and radiographic finding of sixty-one cases of benign odontogenic tumors seen in Yonsei Medical Center, for the period of Jan. 1979 to Aug. 1989. The results were as follows: 1. Benign odontogenic tumors of 61 cases included 52 cases (85.3%) of ameloblastoma and odontoma, and 9 cases of other lesions. Radiographically, the border of the lesions were well-defined. 2. Ameloblastoma constituting twenty-seven cases (44.3%) occurred the average age of 31.1 years and had a 3:1 male predominance. The most common complaint was swelling (20 cases, 74.0%) and followed by pain (13 cases, 48.2%). Radiographically, the most common site was mandibular body area (74.0%) and the lesions were mainly multiocular radiolucency; in 17 cases (63.0%) and unilocular radiolucent lesion were seen in 10 cases (37.0%). 16 cases (59.3%) showed the resorption of roots of adjacent teeth. 3. Odontoma constituting twenty-five cases (41.0%) discovered at the average age of 16.9 years and had a 3:2 male predominance. The most common complaint was delayed eruption of tooth (8 cases, 31.0%) and 7 cases (27.0%) detected on a routine radiograph of the area. Radiographically, 17 cases (68.0%) were of compound type and 8 were of complex variety and compound odontomas were common in the anterior maxilla, whereas complex odontomas occurred more frequently in the posterior mandible. 19 cases (76.0%) showed the impaction of adjacent teeth.

  • A 42-year old male patient was referred to the Department of Oral Medicine, Kyungpook National University Hospital due to the chief complaint of limite mouth opening. Three years ago, the patient was diagnosed as an infarction of both cerefellar hemispheres, acute obstructive hydrocephalus and acute epidural hematoma of frontal lobe at the department of neurosurgery.Both of the infarcted cerevellar hemispheres and the epidural hematoma of frontal lobe were removed with suboccipital and frontal craniectomu. After the brain surgery jaw opening range was decreased progressively and ultimately mouth opening became almost impossible. Spasmodic and rhythmic contractions of the masseter muscles occurred intermittently during daytime as well as sleeping. Food intake was available only through Levin -tube. Actibe jaw opening exercise was prescribed with the aids of tongue blades. A moist hot pack and indomethacin phonophesis were also applied 20 minutes three times a day to decrease discomfort muscle activities. After a month of treatments, the opening range was increased to 5mm at the premolar area and oral food intake was possibel. The L-tube was removed and the patient was discharged.

  • Coronary arterial fistula was first described by Krause in 1865. Since then more than 200 examples of this lesion have been reported In the literature. It is relatively rare disease and it more commonly originate in the right than in the left coronary artery. A 9-year old male was admitted with the chief complaint of cardiac murmur and diagnosed as coronary A-V fistula at middle segment of the right coronary artery by cardiac catheterization and aortography. On the operation field, the right coronary artery was markedly dilated as one cm. in diameter from the aorta to the middle segment at acute angle of the right ventricle. Fistula opening was obliterated by one stitch horizontal mattress suture with pledget. Postoperative course was uneventful and discharged without problem.

  • We had expierienced pulmonary lymphangioleiomyomatosis(LAM) with bilateral chylothorax and chylous ascites. A twenty-one-year-old lass with chief complaint of abdominal pain was admitted through the emergency room. She received emergent pelvicoscopic surgery for the rupture of the right corpus luteum. We aspirated 1000ml of the uncoagulated blood. The bleeding point was cautherized electrically. LAM was diagnosed with tissue from the retroperitoneum. Chylous ascites and bilateral chylothorax were occurred despite of various treatments. On thoracotomy, bullous changed lung and lymphatic leakage from visceral and parietal pleura were observed. She died of respiratory insufficiency and general weakness after 6 months from admission.

  • A 16-year-old female was admitted to Wonkwang dental hospital with a chief complaint of painful ulceration on right buccal mucosa around mandibular 3rd molar area. Computed tomography and magnetic resonance imaging showed relative large soft tissue mass on the infratemporal fossa and masseter muscle region. By the feature of T1-weighted and T2-weighted of MR imaging, we suspected this mass as a kind of myogenic sarcoma. Histopathological and immunohistochemical studies established a definitive diagnosis of embryonal rhabdomyosarcoma. A review of the literature was also presented.

  • A toxoplasmic uveitis case was reported on the focus of impairment of pathological findings and serological antibody titers after chemotherapy. A chief complaint of a 60-year-old male was a decreased and blurred vision in his right eye for 2 weeks after experiencing tremendous stress and fatigue. A steroid therapy for 3 weeks was not effective and the retinal lesion became necrotic. Anti-Toxoplasma gondii antibody titer was checked to be a strong positive by both ELISA and indirect latex agglutination assay (lLA). He was treated with Fansidar F for 8 weeks. His vision improved as the necrotic lesion healed by scarring, but the antibody titers still remained very high without any signs of negative conversion. It is suggested to be a recurrent case of the past asymptomatic infection by presumed immune suppression caused by excessive stress.
