Changes in Posterior Cerebral Artery Blood flow Velocity Following Head rotation and body Positioning (머리회전과 측정자세에 따른 뒤대뇌동맥의 혈류속도 변화)

  • PURPOSE: Vertebrobasilar insufficiency (VBI) should be carefully assessed in patient for whom manipulation of the cervical spine is to be undertaken. The purpose of this study was to investigate the changes in posterior cerebral artery blood flow velocity following head and body positioning by transcranial doppler ultrasonography (TCD) in healthy subjects. METHODS: Twenty two healthy female (mean age $20.77{\pm}1.30yrs.$) participants volunteered to participate in the study. None of the participants had a history of neck pain or headache within the last 6 months. To evaluate the cerebral blood flow, we measured the mean flow velocity of the posterior cerebral artery unilaterally (right side). The blood flow velocity was measured under 3 different head positions (in a neutral head position, ipsilateral head rotation and contralateral head rotation position) and 2 different body conditions (supine position and sitting position). RESULTS: The mean blood flow velocity of posterior cerebral artery was decreased in body positioning from supine to sitting (p<.05), but the decreased rate of blood flow velocity in posterior cerebral artery did not change significantly between ipsilateral head rotation and contralateral head rotation (p>.05). CONCLUSION: These result of our study show that body positioning (sitting and supine) affect the blood flow velocity in posterior cerebral artery.

A Clinical Survey of Patients of Neuro-Pain Clinic 1 Year Period (신경통증클리닉 환자의 1년간 통계 고찰)

  • 1,666 patients treated by nerve block from September 1994 to August 1995 we statistically analyzed according to sex, age, diseases, and kinds of nerve blocks. Most patients were in the range from 30 to 60 year old, with a distribution of 43.9% male and 56.1% female. Diseases and ailments were as follows: low back pain 30.6%, frozen shoulder 14.0%, facial spasm 10.0%, cervical syndrome 9.7%, headache 7.3%, and hyperhidrosis 7.2%. Most common nerve blocks were stellate ganglion block 30.9%, epidural block 25.6%, trigger point injection 16.1%, and suprascapular nerve block 6.7%. Nerve blocks under fluoroscopic guide were as follows: facet joint block 28.6%, spinal root block 22.9%, thoracic sympathetic ganglion block 21.7%, and lumbar sympathetic ganglion block 15.4%.

Clinical fetures of kawasaki disease in school-aged children (학동기 아동에서의 가와사끼병의 임상 특징)

  • Purpose : Kawasaki disease (KD) rarely occurs in school-aged children. We clarified the characteristics of KD in this age group to provide tips for a high index of suspicion. Methods : Features of 38 patients with KD who were 7 years of age or older were retrospectively reviewed. Results : The incidence of the KD patients ${\geq}7years$ was 4.9 percent. The ratio of male to female was 2.5:1. Of the 38 patients, nine patients (24.0 percent) were diagnosed with typical KD and 29 patients (76.0 percent) with incomplete KD. In incomplete KD patients, cervical lymphadenopathy (69.0 percent) occurred most frequently, followed by conjunctival injection (62.0 percent) and polymorphous rash (45.0 percent). These patients occasionally presented with other additional symptoms including abdominal pain, headache, vomiting and arthralgia. Incomplete KD was initially diagnosed as cervical lymphadenitis (34.0 percent), viral infection (14.0 percent), scarlet fever (7.0 percent), meningitis (7.0 percent), and Kikuchi disease (7.0 percent). Coronary complications were noted in 15 patients (39.0 percent). Of the 37 patients treated with intravenous immunoglobulin, five (14.0 percent) were resistant to the therapy and all had coronary abnormalities. Conclusion : Most patients with KD ${\geq}7years$ of age have incomplete presentations. They tend to have a higher incidence of initial presentations of unilateral neck mass and coronary artery involvement. In school-aged children, fever and cervical lymphadenitis or suspected neck infection unresponsive to intravenous antibiotics should signal the possibility of KD. A high index of suspicion and prompt treatment is essential in this age group of patients.

Clinical Characteristics of Subacute Necrotizing Lymphadenitis (아급성 괴사성 림프절염의 임상적 고찰)

  • Background and Objectives: Subacute necrotizing lymphadenitis or Kikuchi's disease has now become recognized in many parts of the world as a well-defined clinicopathologic entity. It typically affects cervical lymph nodes of young women, and subsides spontaneously within a few months without recurrences. The purpose of this study is to report the clinicopathologic finding, radiologic finding and many laboratory tests to elaborate the criteria that are useful in distinguishing this entity from inflammatory disease, granulomatous disease, lymphoma, malignant disease. Material and Methods: Authors evaluated 22 patients, who were diagnosed as subacute necrotizing lymphadenitis on excisional biopsy or fine needle aspiration cytology with retrospective chart review. Results: The patient population consisted of 21 women and 1 men. The mean age was 25 years old(range 17 to 42 years). All patient had palpable neck mass and associated fever, headache, weight loss, otalgia, diarrhea, night sweating. and associated disorder is dermatitis, hepatitis B, SLE, Hashimoto's thyroiditis, albinism et al. The multiple bilateral involvement of cervical lymph node was 20 cases(91%) and multiple unilateral 18 cases(82%), multiple bilateral 2 cases(9%). Solitary involvement was 2 cases(9%), mean size of involved lymph node was 2.0cm. In laboratory tests, leukopenia 17 cases(72%), elevated ESR 12 cases(54%), elevated GOT, GPT 3 cases(13%). In CT finding, lymph nodes enhanced in homogeneous fashion, and there was no evidence of central necrosis. Conclusion: Subacute necrotizing lymphadenitis, Kikuchi's disease may be easily confused clinically, pathologically, radiologically with malignant lymphoma, inflammatory disease, granulomatous disease. We should consider open biopsy of lymph node in the patient which had unilateral multiple cervical lymph node and in laboratory tests, were leukopenia, elevated ESR, especially in young women. otherwise, result of fine needle aspiration biopsy is nonspecific, excisional biopsy is recommended in order to differentiate other disease.

A Clinical Study of Spine Distortion and Self-conscious Pain in High School Students (청소년기(靑少年期)에서 척추체형(脊椎體形)과 자각증(自覺症)과의 임상적(臨床的) 고찰(考察))

  • Objectives : The purpose of this study was to investigate the spine distortion and self-conscious pain in high school students. Method : Using the phase-shifting moire developed by KAIST(Korea Advanced Institute of Science and Technology) we measured the back surface distortion, and using questionnaire investigation, we investigated the self-conscious pain in 30 high school students(17 boy students and 13 girl students). Results : This study show that low back pain was most cases, and next headache, upper back pain, abdominal pain, neck pain, shoulder pain, leg pain, chest pain and gluteal pain. The ratio of each self-conscious pain was 90.0%, 86.7%, 83.3%, 83.3%, 80.0%, 76.7%, 66.7%, 33.3%, 33.3%, 30.0%. In remainder value of left and right shoulder, above of 3.0 were three students., above 2.0 to bellow 3.0 were 10 students, bellow 2.0 were 17 students. In remainder value of left and right gluteal, above of 2.0 was one students, above 1.0 to bellow 2.0 were 18 students, bellow 1.0 were 10 students. In remainder value of cervical vertical line to left and right axillae, above 4.0 was 1 student, above 3.0 to bellow 2.0 were 2 students, above 2.0 to bellow 3.0 were 3 students, above 1.0 to bellow 2.0 were 16 students, bellow 1.0 were 7 students. In remainder value of gluteal vertical line to left and right axillae, above 6.0 was 1 student, above 5.0 to bellow 6.0 were 7 students, above 4.0 to bellow 5.0 were 3 students, above 3.0 to bellow 4.0 were 2 students, above 2.0 to bellow 3.0 were 5 students, above 1.0 to bellow 2.0 were 7 students, bellow 1.0 were 5 students. In remainder value of cervical vertical line to gluteal vertical line, above 40 was 1 student, above 3.0 to bellow 4.0 were 4 students, above 3.0 to bellow 3.0 were 6 students, above 1.0 to bellow 2.0 were 8 students, bellow 1.0 were 11 students. Conclusion : It is suggested that the significant correlation exist between the spinal distortion and self-conscious pain. And we can find many cases as spinal distortion in high school students also. We have to study further more about juvenile postural distortion and it's prevention.

Decreasing Effect of Wet-Cupping at Daechu (대추, Dazhui:GVl4)-point on the Increase of Blood Pressure in Patients with Stroke (중풍환자의 혈압 상승에 대한 대추혈 사혈의 혈압 강하 효과)

  • Objectives : Daechu (Dazhui:GV14 (Governor Vessel 14))-point is located between the spinous process of the 7th cervical vertebra and that of the 1st thoracic vertebra. GV14 has been used to treat hypertension, high fever, neck pain, common cold, headache and so on. Hypertension often negatively affects the improvement of stroke patients. We investigated whether wet-cupping at GV14 had any effect of decreasing blood pressure (BP). Methods : In this study, 81 stroke patients were studied, from Oct. 2000 to Sept. 2001. They were composed of a Sample group (n=36) and Control group (n=45). The Sample group (n=36) was divided into Sample-Normal (n=9), Sample-Mild (n=7), Sample-Moderate (n=l1), and Sample-Severe group (n=9). The Control group (n=45) was divided into Control-Moderate (n=34) and Control-Severe group (n=ll). We checked blood pressure 5 times (just before treatment, then after 30,60, 90, and 120min. (2hrs.)) in each sample group and 3 times (baseline, after 30, and after 120min. (2hrs)) in the control group. Results : In a comparison of before-treatment BP and after-treatment BP in the sample group, SBP significantly decreased as time passed, while DBP slightly decreased but not significantly. In comparison of BP differences in each sample subgroup, BP slightly increased in the Sample-Normal group, but significantly decreased in the Sample-Mild, Sample-Moderate, and Sample-Severe groups. In comparison of BP differences between the sample and control groups, SBP and DBP of the sample group definitely decreased more than those of the control group, but not significantly. Conclusions : This study suggests that wet-cupping at GV14 has significant decreasing effectson blood pressure in stroke patients. Accordingly, we hope that this measure will be used more widely as an emergent treatment for increased blood pressure.

Clinical Analysis of Cesarean Sections (제왕절개술환자의 임상적 고찰)

  • One hundred and fifty seven cesarean sectioned patients among 2010 deliveries, were reviewed with the following clinical charts from January, 1973 to December, 1974 at Taegu Presby-terian medical Center. The results are as follows; 1) The section rate was 7.8% 2) In age distribution, the most frequent age was 26~30ys and it~s rate was 37%, and others were as follows : 31~35ys 24.2%, 21~25ys 21%, 36~40ys 10.8%, 41~45ys 7% respective]y- 3) In indication, the cephalopelvic disproportion was the most prevalent and it's rate was 29.3%, and others were as follows : malpresentation 14% previous cesarean section 10.8% uttering dysfunction 9.6%, totemic 4%, elderly primigravida 4%, fetal distress 2.5% post maturity 3.2%, wanted cesarean section 1, 9%, pelvic deformity 1.2% twin 1.9%, abruptio placenta 1.2%, Vaginal stricture 1.2%, and others 2.5%. 4) In type of operation, tower cervical transverse type was 82.1%, classical type 17%, ana cesarean hysterectomy was 0.9% respectively. 5) General Anesthesia was major type and it's rate was 63%, and spinal anesthesia 17.2%, epidermal anesthesia 14% and other combined type was 5.8%. 6) Percental mortality was 11.4%. 7) There was no maternal death. 8) Among the cesarean section complications, wound infection was 8,4%, headache 4.4%, urinary tract infection 1.2% and one case in endometritis. 9) In combined surgery, tubal legation was 31.2%, appenectomy 7.6%, posterior repair 1.2%, salpingoop-horectomy 1.9%, and one case in resection of uttering septum. 10) Cesarean section was performed 61.3% in multipara and 38.2% in primipara. 11) 47 (30%) had received artificial abortions. 12) 28 (17.8%) had previously experienced cesarean section.

Comparison of Clinical and Psychological Characteristics between Self-Reported Bruxism and Clinically Detected Bruxism by Wear Facet on Splint

  • Purpose: Bruxism is commonly considered a major risk factor for temporomandibular disorders (TMD), and the psychosocial factors had been one of the etiologic factor of bruxism. But there are still unsolved issues on the relationship between sleep bruxism and TMD and the etiologic factors of bruxism. This study is aim to evaluate the clinical and psychosocial characteristics according to diagnostic grade of bruxism in TMD patients. Methods: Three hundred subjects were enrolled who were under the stabilization splint therapy for TMD. Recently international consensus proposed a diagnostic grading system of "possible", "probable", and "definite" sleep or awake bruxism for clinical and research purpose. According to their suggestion, we classified these subjects as self-reported bruxism (SRB) and wear facet bruxism (WFB). We investigated the clinical characteristics (sex, age, chief complaint, pain duration, visual analogue scale), sum of tenderness (temporomandibular joint, masticatory muscles, cervical muscles), diagnosis of TMD according to research diagnostic criteria (the Research Diagnostic Criteria for Temporomandibular Disorders, RDC/TMD), headache, subjective sleep quality (Pittsburgh Sleep Quality Index, PSQI), and psychosocial characteristics (Symptom Checklist-90-Revised, SCL-90-R) in enrolled subjects. We compared the clinical and psychosocial characteristics between these bruxism groups. Results: There were no significant correlation between self-reported and WFB (p=0.13). SRB subjects more reported pain as a chief complain than subject who did not report bruxism (p=0.014). The mean score of global PSQI was significantly higher in SRB than in did not report positively subjects (p=0.045). The mean score of anxiety and phobic anxiety was significantly higher in SRB than in did not reported positively subjects (p=0.045, p=0.041). Conclusions: Although bruxism is regarded as risk factor of TMD, this study showed inconsistent result between SRB and clinically detected bruxism by wear facet on slpint. We suggest that the clinician should consider with extreme caution when they assess SRB.

The Influential Factors Associated with Hospitalization Period of Patients from Traffic Accident by Groups: A Korean Medicine Hospital Experience, Respective Chart Review (한방병원에 교통사고로 입원한 환자의 입원기간에 영향을 미치는 요인분석)

  • Objectives This study was designed to find out influential factors associated with Hospitalization of the patients from traffic accident by Groups. Methods Based on the medical charts, we analyzed statistical study of 486 patients putting them into two groups to identify the factors associated with the duration of hospitalization involving age, sex, elapse day (Interval between time of injury and visit to hospital), and the types of accident. Patients in group A were diagnosed with a mere 'sprain', and patients in Group B had other symptoms accompanied by 'sprain'. Results 1. In group A, cervical sprain was the most and male patients with the symptom outnumbered female patients. In group B, headache was most where female patients had the symptom 1.7 times more than male patients. 2. Female patients in group B were found to stay for a longer period of time in the hospital than the male counterparts (p<0.05). 3. Both group A and B have positive correlation between length of hospital stay and age though group B exhibited stronger correlation. 4. Patients who had accident while riding bus in group A and those who were pedestrians in group B were found to stay in the hospital for a significantly longer period of time than those who had any other types of accident (p<0.05). Conclusions We noticed that hospital stay lengthened when the patients were older and when they had accidents while riding bus in Group A whereas in Group B, patients stayed longer when they were females and older as well as when they were pedestrians in accidents.

A Study of the Relationship between Spine Distortion and Pain of Elementary and Middle School Students - through moire measurement and questionnaire investigation - (초.중학생의 척추비틀림과 통증과의 관계고찰 - 경근무늬(모아레) 측정법과 설문지 조사를 통해 -)

  • Objectives : The aim of this study is to investigate the relationship between spinal distortion and pain in elementary and middle school students. Methods : Using the phase-shifting moire developed by KAIST(Korea Advanced Institute of Science and Technology) we measured the back surface distortion, and using questionnaire investigation, we investigated the partial pain in 1008 elementary and middle school students(532 boy students(52.8%), 476 girl students(47.2%)) in Seoul in 1999. And, we statistically analyzed the relationship between spinal distortion and partial body pain Results : This study show that the more age increase, the more the pain in head, neck, shoulder, back and low back Increase considerably among the students aged 9 to 15. The ratio of each partial body pain was 28.4%, 19.7%, 18.2%, 18.7%. 16.0% at the age of 9 and 59.7%, 67.4%, 51.8%, 51.8%, 64.4% at the age of 15 respectively. And, it was shown there was a significant correlation between spine distortion and partial body pain area. When the spinal distortion was found at the upper thoracic and cervical area, the students complained the more pain on head, neck and shoulder(p<0.05) and when the spinal distortion was found at the sacral base, the more headache(p<0.05) Conclusions : It is suggested that the significant correlation exist between the spinal distortion and pain and the pain related to spine disorder get more increase as students grow. We have to study further more about Juvenile postural distortion and it's prevention.
